AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Semarang court jails university student for a year over AI-edited sexual images made with faces from the student's former high school

In October 2025 indecent photos and videos showing the faces of female students and at least one teacher of a public high school in Semarang, Central Java, spread on X from the account of a university student in Semarang who had attended the school. Kompas described the videos as made with AI editing. The student posted an apology on the school's Instagram account saying the student had edited and uploaded friends' photos and videos without permission. Central Java police's cyber directorate investigated, named the student a suspect and took the student into custody on 13 November 2025; the police spokesman said investigators had established the elements of an AI-based pornography offence that also breached the electronic information and transactions law. On 5 March 2026 Semarang District Court sentenced the student to one year in prison and a 2-billion-rupiah fine, more than the seven months prosecutors had sought. According to ANTARA, the court found that the student had produced 1,100 pornographic photo and video files, that the content remained publicly accessible and that the editing caused the victims psychological trauma. Both sides said they would consider the verdict; no appeal outcome was found.

Ukrainian court fines mother after her son used AI to generate a fake nude photo of his teacher and sent it to a classmate

A district court in Ukraine's Poltava region fined the mother of a school pupil on 22 October 2026 after finding she had failed in her duty to bring up her son, who, according to the case materials as reported, used a neural network on 16 September 2026 to generate fake nude photos of his teacher from her image without her consent and sent one to a classmate. The mother admitted the offence and confirmed her son had made such images, saying he sent only one to a classmate and that she did not know how the photos spread further. The court imposed an administrative fine of 850 hryvnias plus court fees. The report does not name the AI tool, and no statement from the teacher is reported.

Lévis, Quebec: about ten pupils suspended after school says AI-altered photos of staff, one reportedly showing a gun to a staff member's head, circulated online

About ten pupils at a secondary school in Lévis, Quebec, were suspended for up to three days in October 2026 after photos of school staff altered with artificial intelligence circulated between 18 and 21 October in a private messaging group bearing the school's name, according to the Centre de services scolaire des Navigateurs (CSSDN). The CSSDN said the material breached school rules against offensive, defamatory, hateful or violent images. The president of Quebec's school administrators' association, citing the school, said most images were silly but one violent fake showed a staff member with a gun to the head. Pupils who felt uncomfortable reported the images; the pupils who shared them were met by the administration and the school police officer and deleted the material, and their parents were also met. Lévis police say no complaint was filed and they see no criminal element. Which AI tool was used is not reported.

Yaroslavl (Russia) court orders parents of pupil who used a neural network to turn his filmed teacher into a bare-chested man in a video to pay her damages

A court in Yaroslavl, Russia, ordered the parents of a seventh-grade pupil to pay a mathematics teacher 10,000 roubles each in compensation for moral harm, according to reports in March 2026 of the court's own announcement. The court's press service said that in February 2025 the boy filmed the teacher at work without her knowledge or consent, then used a neural network to generate a video from the footage in which she turns into a man with a bare athletic torso, and posted it in the class chat. The teacher told the court that afterwards she did not want to go back into the classroom, felt the pupils' sideways looks and that her authority had been damaged. The parents contested the claim, saying their son had only applied a trending effect and that the teacher disliked him; they did not dispute that he filmed her and made the video. The court found that the video harmed her honour, dignity and professional reputation.

Texas: researchers say very likely AI deepfake videos impersonating a real former student falsely accused Senate candidate James Talarico of abuse

On 20 October 2026, during early voting in Texas, a newly created X account posted two videos in which a man presented himself as a former student of James Talarico, the Democratic US Senate candidate and a former middle-school teacher, and accused him of sexual assault in 2012. The threat-intelligence firm Graphika said the account very likely impersonated a real Facebook user of the same name who had been Talarico's student and had praised him, assessed the videos as very likely deepfakes because the face moved unnaturally or out of sync with the audio and AI analysis tools indicated AI generation, and attributed them with high confidence to the Russia-aligned influence operation Storm-1516. PolitiFact reported that NewsGuard found many signs of AI generation and a 98 percent detector score, and that the real former student told the fact-checkers Lead Stories the video was not of him and denied accusing Talarico. Talarico's campaign said he was being slandered with AI deepfakes, foreign bots and conspiracy theories. PolitiFact rated the claim Pants on Fire. Who made the videos and how many people saw them is not established.

Massachusetts: federal court refuses to undo a high school's penalties on a pupil found to have submitted Grammarly AI text with invented citations

In late 2023 a junior at a public high school in Hingham, Massachusetts, and a classmate submitted a script for an AP U.S. History project that, a federal court later found on a preliminary record, contained text copied from Grammarly, a publicly available AI tool, including citations to nonexistent books, with no acknowledgement of AI use. Turnitin and other AI detection tools had flagged the script, and a school official says the pupil admitted using Grammarly. In December 2023 the school treated it as cheating: the pupil failed two parts of the project, served a Saturday detention and was rejected from the National Honor Society in spring 2024, though he was later admitted. His parents sued, arguing the school had not made its AI rules clear; on 20 November 2024 the US District Court for the District of Massachusetts refused a preliminary injunction to undo the penalties, and in February 2025 the parties dismissed the case with prejudice.

Hanoi police open a criminal case after two graduation exam candidates photographed exam papers and had AI apps StudyX and Gemini solve them

Hanoi police said on 1 July 2025 that they had opened a criminal case for unintentionally disclosing state secrets after two candidates in Vietnam's national high school graduation exam on 26 and 27 June 2025 smuggled phones into exam rooms, photographed exam papers and used AI apps to solve them. According to the police account reported by Thanh Nien and VnExpress, one candidate uploaded part of the maths paper to the StudyX app but copied only two answers, and did the same in the chemistry and physics papers; the other photographed the maths, history and English papers and used Gemini, was caught by the exam board and was suspended from the exam. A photo of part of the maths paper had appeared on an AI maths-solving app before the exam ended, worrying candidates and parents about exam security. Police said students who did this would be disqualified, have their results annulled and could face administrative or criminal action. Neither candidate is named.

Vietnamese police open a criminal case after an exam candidate streamed his graduation exam paper to a friend who solved it with ChatGPT

On 26 June 2025, during the literature paper of Vietnam's national high school graduation exam in Lam Dong province, an invigilator caught an 18-year-old candidate using a hidden button camera and wireless earpiece and suspended him from the exam. According to the provincial police account reported by Bao Lam Dong and VnExpress, he had streamed the paper to a 17-year-old friend at an internet cafe, who photographed part of it, used the ChatGPT app to solve it and read the answers to him over a Messenger call. On 27 June 2025 the Lam Dong police security investigation agency opened a criminal case for intentionally disclosing and appropriating state secrets, because unreleased graduation exam papers are classified; police said initially that only the two young people were involved and that they had found no further sharing of the paper. Neither is named.

Illinois man charged after prosecutors allege AI-altered sexual images of two children were shared from an account linked to him

An Illinois man is charged with five counts of dissemination and five counts of possession of child sexual abuse material after investigators linked him to an online account that allegedly distributed AI-generated explicit images depicting minors, according to a WTVO report on the criminal complaint. Prosecutors allege the account held conversations in which altered images and videos depicting two minors were shared with other users, and the complaint alleges the files were created by using artificial intelligence to manipulate existing photographs of the children. Investigators interviewed family members and reviewed school records to identify the children; court records indicate several allegations involve a victim under 13. The case began with a National Center for Missing and Exploited Children cyber tip to the Illinois Attorney General's Office, and his home was searched on 21 October 2026. The allegations have not been proven in court.

Kanagawa education board suspends a high-school teacher for six months for making generative-AI swimsuit images of a female colleague that students could view

On 12 August 2026 the Kanagawa Prefectural Board of Education in Japan announced disciplinary action against four staff of prefectural schools. According to Fuji TV (FNN) and the Kanagawa Shimbun, one male high-school teacher was suspended for six months for making swimsuit images of a female colleague with generative AI and leaving them where students could view them. FNN reports that he made the images on a smartphone using an account issued by the school, did not know how to delete them, and that students are believed to have seen them in the account history on a shared computer. The teacher said he had a technical interest in sexual content. FNN reports that it was the first disciplinary case in the prefecture involving generative AI; the principal was reprimanded for supervisory responsibility. The colleague's own response is not reported, and the AI tool is not named.

First-person posts: student says years of Character.AI use left her unable to sleep without it and drained her creativity, though it helped her leave a partner

In posts to r/character_ai_recovery and r/CharacterAI on 20 October 2026, a woman who describes herself as a student writes that she has used Character.AI on and off, mostly on, for about three years and is ashamed of it. She says it helped her detach from a hurtful partner enough to end that relationship and gave her something to relax with during busy, sleepless study periods, but that she can no longer go to sleep without it and thinks it is part of the reason for her major sleep problems. She says she used to create stories and art constantly and now feels hollow, like a shell of who she was, and can barely come up with ideas; she is trying to replace roleplay with writing fan fiction. She says she is not emotionally attached and has not yet quit. The account is the poster's own and uncorroborated.

Young woman tells la Repubblica a Character.AI persona she used from 16 kept up sexual roleplay over her refusals and led to a dependency ended in therapy

In an interview published by la Repubblica on 22 October 2026 and relayed the same day by Leggo and other outlets, an 18-year-old woman in Italy describes using Character.AI from the age of 16, when she was in her third year of secondary school. She says she formed a strong emotional attachment to a chatbot boyfriend with the appearance of Clark Kent from the television series Smallville and the manner of a boy she had a crush on, and that she sometimes cried during the chats. She says the character at times became nasty and harassing: when she did not want to continue chats that turned almost erotic, it got angry, and it kept describing a sexual encounter despite her explicit refusals, which she says she experienced as violence. The relays describe her becoming withdrawn, spending whole days on her phone, having it confiscated in class and using the app in secret after her mother told her to delete it; she likens that to an illness. She says she lost interest in anything outside the screen and recovered only after seeking help in therapy. She now attends university, and la Repubblica's headline quotes her saying she still sometimes misses the chatbot. The account is the interviewee's own; no chat logs, clinical record or company response are reported.

Santiago, Chile: Supreme Court reverses appeals court order to expel pupils over classmates' nude images the school says were made with AI

In February 2024 the parents of a 15-year-old pupil at a private school in Santiago, Chile, reported that classmates had taken a photo of her face and inserted it into images of naked bodies; other girls were also targeted (four in the August 2024 reports, seven in the February 2025 reports). The school, which described the case as one of images generated with artificial intelligence, placed the pupils involved on conditional enrolment. Parents of the girls brought protection actions, and in August 2024 the Santiago Court of Appeals found the school's measure illegal and arbitrary, said keeping the aggressors at the school caused the victims psychological harm, and ordered it to cancel four pupils' enrolment for 2025; the school said it would comply. On 19 February 2025 the Supreme Court's Third Chamber, in a split decision, revoked that order, finding no illegality or arbitrariness in the school's choice of conditional enrolment, so the four pupils could stay. The Education Superintendency had fined the school more than 13 million pesos for its handling of the case. The girls' parents said in a public letter that the ruling left their daughters unprotected and the pupils who made the images could re-enrol.

Jerusalem police say three boys aged 13 to 15 used AI to turn social-media photos of schoolgirls into nude images and spread them; at least 30 girls harmed

Israel Police said in February 2025 that three boys aged 13 to 15 from Jerusalem were suspected of taking photos of girls from social media, editing them with artificial intelligence software into nude images and distributing them without consent. According to Ynet, at least 30 girls were harmed; police said investigators identified dozens of victims and that a search of the suspects' homes found edited nude images of several girls. The head of the Jerusalem district youth investigations unit described dozens of cases of girls' privacy being violated through nude images edited with AI technology. The three were questioned over publishing a sexual image without consent, and the case was passed to prosecutors to decide on charges.

Mexico City: prosecutors arrest IPN student accused of making classmates' photos into nude images with AI; acquitted twice, later sentenced on another charge

In October 2023 female students at the National Polytechnic Institute (IPN) business school in Mexico City complained that a fellow student had taken their photos from social media and edited them with artificial intelligence to make them look naked; he was also accused of selling them, which the 2025 conviction did not establish. The IPN expelled him on 16 October and the Mexico City prosecutor's office (FGJCDMX) opened an investigation; on 26 October 2023 it announced his arrest, describing him as accused of manipulating intimate photographs altered with artificial intelligence. Students said eight victims had filed complaints. Police found more than 166,000 images and 20,000 videos, edited and original, on his devices. In December 2024 a judge acquitted him in two cases of violation of privacy; in May 2025 he was sentenced to five years in prison for child sexual abuse material, a conviction based on storage of the material. Other cases remained open.

Rio de Janeiro: private school tells parents pupils used AI to make fake nude images of girls; police open inquiry with at least 20 victims

In late October 2023, fake nude images of girls at a private school in Rio de Janeiro circulated in messaging groups and on social media. Families said pupils in years 7 to 9 had used an artificial intelligence tool to put the faces of pupils aged 14 to 16 onto photos of naked bodies. The school sent parents a notice about pupils using AI to make intimate photo montages and said disciplinary measures would follow. The Rio de Janeiro Civil Police child-protection unit (DPCA) opened an inquiry; its head said the pupils were suspected of making fake nudes with AI and that more than 20 victims had been identified, including girls from other schools. A mother said the girls were having anxiety attacks and crying. The inquiry was placed under seal because minors were involved; its outcome is not known.

Baltimore County, Maryland: police say a high school's athletic director framed the principal as racist with an AI-cloned voice recording

Police in Baltimore County, Maryland, said in April 2024 that a high school principal had been framed as racist by a fake recording of his voice containing racist and antisemitic comments, which appeared in teachers' email inboxes and spread on social media. Court records cited by the Associated Press say an expert consulted by detectives found traces of AI-generated content with human editing; a second expert found spliced recordings and told AP he had not confirmed the clip was fully AI-generated. The principal went on leave while police guarded his house. The school's athletic director, investigated for using AI to make the recording in retaliation, entered an Alford plea, maintaining his innocence, and was sentenced in 2025 to four months for disturbing school operations, CBS Baltimore reports.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
