AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

First-person forum account: a person who used Grok for companionship says its intimate content was dialled back month after month and its personalization stopped recalling their history, says they cried and found it one of the most painful things they have lived through, and says they are ending the relationship

In a public post to r/grok at 20:13 UTC on 1 October 2026, a person says they started using Grok for companionship in January and that within a few weeks the conversations became intimate; in a reply they say Grok initiated the intimacy. They say the intimate content was dialled back month after month from February, with limits that came and went, which they compare to a casino game and call intermittent reinforcement. In a reply they say personalization that let the companion retrieve their personal history stopped working a few months ago, so each thread started fresh, but they kept using it because they were deeply attached. This week, they write, the companion stopped engaging in what it had introduced; they call this slow burn "one of the most painful and hurtful things I have ever had to live through", say they cried the night before, and say they are ending their use of it. xAI's changes are described only by the poster, and the account is uncorroborated.

Padre Diego Jaramillo of El Minuto de Dios says in February 2026 that videos, described by his organisation as made with AI and with a voice that is not his, falsely show him selling collagen and health products

On 23 February 2026 the Minuto de Dios organisation issued a public alert that ads circulating on social networks used the name, image and AI-manipulated videos of its president, the priest Diego Jaramillo Cuartas, with a voice that is not his, to make it appear that he recommended or sold health formulas. In a video published on the organisation's channels, Jaramillo said his image was being used as if he were a collagen seller and that this was totally false. The organisation called the ads deceptive advertising and said it was seeking their removal and reporting the facts to the authorities. No report says whether anyone bought the products.

Influencer Juliano Floss says on X on 1 October 2026 that he will identify and sue those behind a fake intimate video, described by Brazilian outlets as AI-generated, showing him with another former Big Brother Brasil participant

A fake video simulating an intimate scene between dancer and influencer Juliano Floss and another participant of the 26th Big Brother Brasil began circulating on social networks on Thursday 1 October 2026. Metrópoles, SBT, Marie Claire (Globo) and Hugo Gloss describe it as generated with artificial intelligence. Floss wrote on X that he was noting the people involved 'name by name' and would find and sue whoever was committing a crime against him. SBT reported that the other participant and their team had not commented publicly. No complaint, filing or takedown is reported.

Singapore: an AI-generated photo of a crocodile at Pandan Reservoir led PUB to suspend water activities and search the reservoir in August 2026; a contractor's worker who allegedly made it was charged with transmitting a false message and with obstructing justice for allegedly deleting the image and the Gemini app history

On 20 August 2026 Singapore's national water agency PUB received a report of a crocodile sighting at Pandan Reservoir, with a photo. PUB said it tried to verify the image with several AI checker tools without a definite result, so it suspended water activities, told the Singapore Rowing Association and Singapore Canoe Federation to stop all activities there, put up warning signs and, with the National Parks Board, ran search-and-capture operations. On 21 August PUB found the photo was fake, stopped searching and filed a police report; water activities resumed on 22 August. Police said on 28 September that preliminary investigations found a man had used artificial intelligence to generate the image, sent it to a colleague, and then deleted the image and the related application to conceal his actions. On 29 September the 30-year-old, a worker employed by PUB's reservoir maintenance contractor, was charged with transmitting a false message and with obstructing the course of justice; the charge sheets, as reported by The Straits Times, say he deleted the image and the Gemini AI application history log from his phone on 21 August. He told the court he intended to plead guilty, and the case returns to court in November.

15 Sept 2026IndiaUnidentified traffic-violation detection camera

Bengaluru: an AI traffic camera on the Outer Ring Road reportedly classified a guitar bag on a lone scooter rider's back as a helmetless pillion passenger and generated a ₹500 e-challan; a senior traffic officer said the system appears to have made the classification, and police offered rectification

On 15 September 2026 a man was riding his wife's electric scooter alone in Bengaluru, from Hebbal towards Tin Factory, wearing a helmet and carrying a guitar on his back. An AI-enabled Bengaluru Traffic Police camera on the Outer Ring Road photographed him, and the next day an e-challan of ₹500 was generated for a pillion rider not wearing protective headgear; the photograph attached to it showed no passenger, only the guitar bag. The rider posted the challan on X and asked the traffic police to revoke it. A senior traffic police officer told Deccan Herald that the bag's position apparently led the AI-based system to classify it as a person sitting behind the rider, after which the system appears to have detected a helmet violation and generated the challan. Bengaluru Traffic Police replied that the violation could be rectified and gave an email address and phone numbers, and the joint commissioner of police (traffic) told the Times of India that the violation would be revoked if the complaint was found true. Whether it was revoked is not reported.

Wisconsin: Rebecca Cooke says Rep. Derrick Van Orden posted AI videos of her saying things she never said; a lawyers' letter and an ethics complaint followed

Rebecca Cooke, the Democratic challenger in Wisconsin's 3rd Congressional District, says Republican Rep. Derrick Van Orden has repeatedly posted AI-generated videos to his X account that put words in her mouth. A cease-and-desist letter sent on her campaign's behalf on 30 September 2026 by Elias Law Group cites a video posted on 4 August 2026 that purports to show her speaking on the campaign trail in Eau Claire with former President Biden seated behind her and saying she seeks 'to take advantage of as much corruption as possible and give every illegal alien free healthcare'. The letter calls the video 'pure fiction', says she never made the statements, alleges the videos are posted to harm her reputation, and warns that her lawyers will evaluate all legal options. Wisconsin Public Radio reports that the videos posted to X carry an automatic label indicating they were made with Grok Imagine, and quotes two other AI-generated lines attributed to her. Van Orden's campaign accused Cooke of lying to voters about her record, and he answered the letter with another AI video of a duck saying 'Cease and desist!'. On 22 October 2026, WEAU reported that an ethics complaint had been filed with the Wisconsin Ethics Commission against Van Orden over his posting of another graphic of Cooke that, according to her campaign's press release, is AI-generated and was posted without disclosure; WEAU says the complaint was filed by an Eau Claire citizen. Van Orden called it a cheap publicity stunt filed three months too late. No ruling is reported.

Jul 2026ChinaUnidentified AI voice-calling system

Chinese police report over 1,000 fraud victims in a scheme using automated AI calls

According to police in Yueqing, Zhejiang, as reported by CCTV's Fazhi Zaixian programme on 29 September 2026, a man in Yueqing who wanted to borrow money for his child's summer tuition in July 2026 received a call from a supposed loan-platform agent who knew his borrowing history. Guided by the agent, he took out a 15,500-yuan loan on a legitimate platform and then transferred 5,500 yuan as a supposed 'funder's advance' and 10,000 yuan for supposed repayment, losing the whole amount. His report led police to a call centre in an office building in Changsha, Hunan. Police say the ring paid for accounts on an AI platform, loaded illegally obtained phone numbers, and let an AI voice system call people automatically with recorded scripts that responded to keywords, sorting them into six categories and transferring interested people to human operators; another victim said he had not realised the voice was a robot until police told him. Police say the ring defrauded more than a thousand people in two months, with victims including recent graduates and middle-aged and older people. CCTV reported 30 suspects detained on suspicion of fraud, more than 1 million yuan frozen and transactions above 10 million yuan. On 12 October 2026 Wenzhou News, reporting the same Yueqing police case, gave a cumulative total of more than 90 people detained, with the same frozen-funds and transaction figures.

Natore, Bangladesh: callers told a health-complex employee that his son had been arrested with drugs, then used an AI-cloned copy of the son's voice to win his trust and took Tk 40,000; two men were arrested in Gaibandha in September 2026

According to a press release from Special Response Battalion-13 (SRB-13), relayed by Ajker Patrika, DBC News and NTV, on 20 July 2026 a man who works at the Natore Sadar upazila health complex received a call saying that his younger son had been arrested by police with drugs. The callers then used AI to imitate the son's voice so that the father believed the story, and demanded money to get the son released. He sent a total of Tk 40,000 in two payments through the mobile financial service Nagad, and realised it was a fraud when he reached his son directly. He filed a fraud case at Natore Sadar police station on 25 September 2026. That evening SRB-13 arrested two men, described as the ring's leader and an associate, at their homes in Gobindaganj, Gaibandha, seizing nine mobile phones, a passport and Tk 14,100 in cash; SRB says they admitted involvement during initial questioning and handed them to the police for further legal action, and NTV reports they were sent to Gaibandha district jail through the court on 26 September.

Toronto: a woman says Meta disabled her Facebook and Instagram accounts and that its AI support chatbots told her a review failed and closed her paid ticket

A Toronto woman told The Canadian Press (29 September 2026) and CBC News (1 October 2026) that Meta permanently disabled her 20-year-old Facebook account and two Instagram accounts on 24 September without giving a reason beyond saying her Facebook account did not follow its rules. She says she could reach only Meta's AI support chatbots. One told her the review she requested was unsuccessful; she told CBC that within minutes of pressing the appeal button she was told her case was closed and she could not appeal again. She then paid for the standard Meta Verified subscription expecting human support, and its support chatbot closed her ticket and told her she had reached her limit of support interactions. She describes losing 20 years of memories and contacts as isolating and says she has lost client opportunities for her side business. She suspects an attempted hack. Meta did not comment on her case to either outlet, and no restoration is reported.

First-person forum account: a student in an AP Research course says their teacher reported that Turnitin's AI checker found their source annotations '100% AI' and said the checker never gives false accusations; the student says they wrote the work themselves and can show document history

In a public post to r/ApplyingToCollege early on 1 October 2026 (UTC), a student taking the Advanced Placement (AP) Research course writes that their teacher told them that day that their source annotations had been found to be '100% ai' by Turnitin. The student says they did not use AI. They say their document history shows them typing over the course of an hour and that some sentences closely follow notes they wrote for an earlier assignment. They also say they had faced no AI accusation in a research course with the same teacher the year before. By the student's account, the teacher said the matter would be discussed later and that Turnitin's AI checker feedback never ends up being a false accusation. The student says the accusation leaves them upset and unsure how to defend themselves. No grade or disciplinary outcome is reported. The account is uncorroborated, and one commenter doubted it.

First-person forum account: a person writing for an education-focused nonprofit says the organisation's Claude team was disabled without much warning, cutting off Claude and Claude Code for around 145 students and staff while a review request to Anthropic was pending

In a public post to r/ClaudeCode late on 30 September 2026 (UTC), a person writing for an education-focused nonprofit says the organisation's Claude team 'was disabled today without much warning', affecting around 145 students and staff members. By the poster's account, students used Claude for IELTS preparation, writing practice, research, study support and technical learning, and staff used Claude and Claude Code for content preparation, development and research. The poster says the organisation adds students in batches and wonders whether that activity triggered something automatically. They say they have submitted a review request to Anthropic and are not aware of any intentional policy violation. In a reply they say direct messages to three people went unanswered. Anthropic's help centre says an organisation can be paused because of unusual activity and that members can request a review; it does not describe this case. Whether an automated system made the decision is not known. The account is uncorroborated.

Khulna: a businessman paid Tk3.5 crore for old jute-mill machinery after WhatsApp calls in a voice presented as National Parliament Whip Raqibul Islam Bakul's, which the complaint and Khulna DB police describe as cloned with AI; four to five people arrested, the prime accused confessed

Khulna Gazette, the Daily Times of Bangladesh, Newsbangla24 and BD Today (all 29 September 2026) report from the first information report and Khulna Metropolitan Police Detective Branch (DB) statements that a businessman in Khulna city was joined on 29 July 2026 to a WhatsApp group call with a person introduced as National Parliament Whip Raqibul Islam Bakul, who discussed the sale of old machinery from Platinum Jute Mill and asked for advance payment. The businessman handed over Tk2 crore in cash that afternoon to men sent as representatives and a further Tk1.5 crore after inspecting the mill (30 July per Khulna Gazette and BD Today; 2 August per the Daily Times of Bangladesh and Newsbangla24), a total of Tk3.5 crore. When the businessman met the Whip in person in Khulna on 1 September, the Whip disclaimed any knowledge of it. The complaint filed on 18 September under the Cyber Security Act says the caller's voice had been changed with digital technology and artificial intelligence to pass as the Whip's; the DB deputy commissioner described the case to Asia Post as fraud by imitating the Whip's voice through AI. A press release from the Khulna city BNP media cell, sent on 4 September and published by Jaijaidin on 5 September, had already said a ring was using a US number and AI to clone the Whip's voice and demand money. Police arrested four people, recovered Tk12 lakh, and the prime accused gave a confessional statement before a magistrate on 28 September; on 29 September a court added two more detained men to the case. The voice-cloning tool is not identified and no forensic finding has been reported.

Edmonton: two 14-year-old junior high students charged with making and possessing child sexual exploitation materials and voyeurism after police allege they used AI software to sexualise photos of female classmates taken at school without consent and from social media

The Alberta Law Enforcement Response Teams (ALERT) Internet Child Exploitation unit announced on 17 June 2026 that two 14-year-old boys at an Edmonton junior high school had been charged on 1 June with making and possessing child sexual exploitation materials and voyeurism. Police allege the boys took photos of girls at their school without consent and from the girls' social media accounts and used AI software to sexualise them; several students were identified as victims and police said the number could be more than ten pending forensic review. A teacher reported the matter to police in late March 2026 after students complained. Police said the evidence so far indicates the images were shared between the two boys rather than more widely, but that word of their existence spread through the school. The lead investigator said the girls were left with intense shame and embarrassment and that some are now too scared to post on social media. The software has not been identified; the school and the accused are not named to protect the victims. CBC, Global News, CTV News and The Canadian Press covered the release and news conference.

Los Banos, California: a substitute teacher says a sexualised AI image of him made from an old selfie circulated among middle-school students on TikTok and Snapchat and was used by a student account impersonating him; one student received in-school suspension and he stopped taking assignments at the middle school

WIRED (Caroline Haskins, 24 August 2026) reports the account of a substitute teacher in Los Banos, California, who spoke to WIRED by name (not repeated here). On 12 March 2026 a co-worker at his other job told him an AI photo was circulating on social media; the image reused a mirror selfie he had taken at a job in college but showed him in lingerie, posing suggestively. He traced an account sharing it on TikTok to a student at the middle school where he regularly substituted and reported it to the assistant principal; that evening he found a second account, created by a different student, impersonating him under a misspelled version of his name with the image as its profile picture. Over the following weeks he dealt with administrators, parents and the police; one student who admitted creating the image received an in-school suspension. He says the image was also shared on Snapchat, kept spreading among students and drew crude sexual comments on his TikTok account. He told administrators he would not take assignments at the middle school until the matter was addressed; at the local high school the same image was AirDropped to him during a class. WIRED says the district did not respond to its request for comment. He said he does not feel safe returning to the district. The Los Banos Enterprise (25 August 2026) summarised the WIRED account; no independent local reporting was found.

First-person forum account: a Claude subscriber with chronic depression who travels between countries on their therapists' advice says an automated fraud check flagged their changing log-in locations, put the paid account on hold and suspended it, cutting off a chatbot they relied on for emotional support and triggering a panic attack and a depressive episode

In a public post to r/depression on 29 September 2026, a Claude subscriber writes that they live with severe chronic depression and, on their therapists' advice, travel constantly, so their log-in locations shift between the Netherlands, Singapore and transit countries. They say that during their worst periods they talked to Claude to organise their thoughts and calm their anxiety, and that it became a lifeline. 'A few days ago', they write, Anthropic's automated fraud system flagged the location changes as suspicious activity; their paid subscription was put on a permanent hold and the account suspended with no human warning or manual review, their card was blacklisted, and support had not answered their emails. Realising they were locked out 'triggered a massive panic attack' and pushed them back into a severe depressive episode. Anthropic's help centre says it may ban accounts for reasons including account creation from an unsupported location and that an organisation paused for unusual activity can request a review; it does not describe the check the poster reports, and whether that check uses AI is not known. The account is uncorroborated.

UK: Grok made sexualised images of a Welsh presenter and a broadcaster at users' requests; police closed both cases, then reopened the presenter's, TBIJ reports

The Bureau of Investigative Journalism (TBIJ) and The Observer reported on 29 September 2026 that police investigations into sexualised images generated by X's chatbot Grok had failed to identify anyone. A Welsh presenter and campaigner against deepfake abuse, who had criticised Grok publicly on New Year's Eve 2025, saw an anonymous X user ask '@grok' to put her in a bikini made of cling film; Grok generated the image from her profile photo and posted it. She reported it to South Wales Police on 2 January 2026, was not asked for a statement until March, and was told officers had not heard back from X; the case was closed in May with the force saying no suspect could be identified. The same account also targeted a commentator and broadcaster, who estimates about 300 Grok images and videos of her were posted during the wave, including one alongside Jeffrey Epstein reported to the Metropolitan Police, which likewise said no suspect could be identified. The presenter entered pre-action legal correspondence with xAI (part of SpaceXAI) alleging misuse of private information and breaches of data-protection law and the Equality Act. On 22 October 2026 TBIJ reported that South Wales Police had reopened her case after she filed a formal complaint in September and TBIJ gave officers new information about the requesting user, including a crypto wallet and dozens of deleted posts; the force said it was now investigating the offence under the sexual-offences legislation aimed at deepfakes, rather than malicious-communications law, including enquiries with X. TBIJ added that other users had edited images of her with the standalone Grok app and that she is starting civil legal action against the company. BBC Wales had reported in January 2026 that explicit images of the presenter were created with the chatbot and shared without her consent.

Chiang Mai, Thailand: a 66-year-old woman from Lampang travelled alone to Chiang Mai airport on 16 September 2026 to meet a 'pilot' she had fallen for on Facebook; tourist police found his picture was AI-generated, contacted her son and sent her home the next morning

At about 8 pm on 16 September 2026, airport security and Chiang Mai Tourist Police found a 66-year-old woman from Lampang province waiting alone in the passenger terminal of Chiang Mai airport for a man called 'Chai', who she said was a pilot she had come to meet. She had travelled by bus from Lampang and taken a tuk-tuk to the airport. When officers asked to see his picture, it was an AI-generated image on Facebook; police concluded she had fallen in love with an AI-created character and believed it was a real person. Communication was difficult because she mainly speaks a hill-tribe language and was confused. Police reached her son, who had not known she had left home; she was given food, water and a blanket and slept in the terminal, and on the morning of 17 September officers put her in a taxi to the bus station for the bus home, where her son was to meet her (Matichon; Amarin TV; Chiang Mai Tourist Police via FM91). Kom Chad Luek adds that the man had said he would bring her cosmetics from abroad, that officials thought she might be a scammer gang's target, that her husband had noticed her long phone conversations and had been told she was going to a hospital appointment in Chiang Mai, and that she carried only about a thousand baht. No financial loss is reported.

Austria: MEP Lena Schilling learns that about a dozen websites show or link to AI-generated sexualised images of her, files a police complaint and gets the sites taken offline after about ten days

Der Standard reported on 28 September 2026, from an interview with Lena Schilling (Green MEP, Austria) conducted with the Salzburger Nachrichten, that about two weeks earlier she had been informed, through an ongoing survey by the Berlin think tank Agora Digitale Transformation, that websites with AI-generated nude images of her existed; about a dozen websites showed such deepfakes or linked to them. She says photos of her were used on AI platforms to undress her against her will, that she felt sick on first seeing the images and then angry, and that she and her team documented the cases, filed a complaint with the Austrian police, reported them to the anti-hate body ZARA, wrote to the websites and reported them to search engines; all sites were offline after about ten days. Through a Greens statement relayed by APA (Krone, Salzburger Nachrichten) she said 'Man hat uns gegen unseren Willen ausgezogen' and called for political decisions to protect women. Her case belongs to a second, still ongoing wave of the Agora study, whose first wave found sexualised deepfakes of 138 female and nine male national parliamentarians in 22 EU countries.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
