AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Russia's Court for Intellectual Property Rights fined an individual entrepreneur from Udmurtia 5,000 roubles for contempt of court after finding that part of the 12 references to case law in his cassation appeal, including one said to be a Supreme Court position, pointed to court acts that do not exist or attributed conclusions to them, the court's press service told RIA Novosti on 27 August 2026. Rossiyskaya Gazeta reports that the entrepreneur explained he had prepared the appeal with an AI-based service that selected the case law, and that his side argued he had no legal education and could not check the references; the court rejected the argument that the errors were technical. Hanoi police open a criminal case after two graduation exam candidates photographed exam papers and had AI apps StudyX and Gemini solve them

Hanoi police said on 1 July 2025 that they had opened a criminal case for unintentionally disclosing state secrets after two candidates in Vietnam's national high school graduation exam on 26 and 27 June 2025 smuggled phones into exam rooms, photographed exam papers and used AI apps to solve them. According to the police account reported by Thanh Nien and VnExpress, one candidate uploaded part of the maths paper to the StudyX app but copied only two answers, and did the same in the chemistry and physics papers; the other photographed the maths, history and English papers and used Gemini, was caught by the exam board and was suspended from the exam. A photo of part of the maths paper had appeared on an AI maths-solving app before the exam ended, worrying candidates and parents about exam security. Police said students who did this would be disqualified, have their results annulled and could face administrative or criminal action. Neither candidate is named.

Central Poland: pupils reportedly used an AI app to make a nude image of a classmate under 15; prosecutors reopened the case after the data regulator intervened

In June 2025, according to OKO.press, a group of pupils at a primary school in central Poland took a classmate's photo from the internet, used an application that offers to "undress" people in uploaded photos to generate a nude image bearing her face, posted it on the school's Instagram profile and urged others to order more such images. Poland's Personal Data Protection Office (UODO) says the girl was under 15 and notified the police, but the police declined to open proceedings and the prosecutor's office upheld that decision, finding no prohibited act; Gazeta Prawna reports that they considered the AI-generated material not pornographic and the pupil unharmed. The UODO president appealed to the Prosecutor General, in January 2026 the authority announced that prosecutors had informed it they were taking up the case again, and in June 2026 it said its president had recently asked once more for the investigation to be resumed. The application is not named.

Seremban, Malaysia: police investigate reports that schoolgirls' photos were edited with AI into explicit images; a fellow student is the suspect

Negeri Sembilan police said on 17 October 2026, in a statement reported by Bernama and RTM, that they had received five reports, lodged by parents or guardians between 20 and 23 August 2026, over the circulation of photographs of female students at a secondary school in Seremban that were edited using AI to resemble pornographic images. The police describe the victims as aged 14 to 17 and the suspect as an upper-secondary student whom the school suspended for five days in August; the case is being investigated under child sexual offences law and the Penal Code. A lawyer for a group of parents told a press conference that the images came to light on 12 August, when a younger pupil found them on an unlocked phone at school, alleged that the student made about 200 such images, and said the school had acknowledged more than 300 affected girls at a September meeting; he said there is no direct evidence that the images were sold. A mother said her child still has trouble sleeping every night. After the girls were questioned at the police headquarters on 17 October, their lawyers alleged that police had made them face the suspect during identification. The AI tool is not identified.

Jersey: a TikTok account posing as a school posted AI-generated videos showing teachers in humiliating and sexualised scenes, reported to police

In February 2026 a TikTok account that used the name, badge and branding of a school in Jersey, and presented itself as "official", uploaded five videos depicting teachers at the school. The Jersey Evening Post (JEP) described the videos as AI-generated deepfakes made with a free app and reported that they showed teachers in humiliating, compromising and sexualised scenarios: in one, two male teachers dance beneath a caption insinuating sexual interest in young girls; a montage shows a teacher barking and eating from a dog bowl and teachers firing guns, while other clips ridicule the appearance of specific staff members. The JEP reported the videos were viewed nearly 20,000 times within days. Education Minister Rob Ward described the content as "deeply inappropriate", confirmed it had been reported to TikTok and the States of Jersey Police, and said his department was working with the school to support staff and students; he said it appeared to be an isolated incident. The account was removed from TikTok. No arrest, charge or identification of the account holder is reported, and no teacher is quoted.

Ituzaingó, Buenos Aires: complaint says a 15-year-old pupil's Instagram photos were made into AI nude images posted in a Telegram group; classmate investigated

Infobae and La Nación, each citing the complaint or police report it accessed, report that on 7 October 2026 a 15-year-old fourth-year pupil at a private secondary school in Ituzaingó, Buenos Aires province, received messages from three unknown numbers inviting her out and saying they had got her number from a Telegram group; a link led her to a group sharing pornographic content that included images of her modified with artificial intelligence. According to the complaint, a 15-year-old classmate admitted to her that he had downloaded photos from her Instagram account, edited them with AI to undress her and posted them in the group. Her mother filed a complaint on 8 October 2026 and asked for the harassment to stop; the case is with the juvenile criminal prosecutor's office No. 2 of Morón under Article 128 of the Penal Code (sexual representations of minors for distribution or sale). The school told families that images of pupils from other years had also surfaced and called meetings for 9 October. Two smaller outlets report, without naming their sources, that four pupils aged 15 to 16 are under investigation, that a seven-year-old primary pupil is among possible victims and that a second complaint was filed by the school; La Nación says the police report does not state whether other people are under investigation. The local outlet La Ciudad, citing pupils' testimony including that of a pupil who spoke anonymously, reported that three boys aged 15 and 16 took photos from girls' Instagram profiles and covertly at breaks, sold the AI-altered images on Telegram, and sold one girl's phone number with them, after which she received repeated calls from unknown numbers; the outlet says pupils at the school bought the material, and the anonymous pupil said that, as she understood it, the images were offered for money and that girls from every year were affected. The tool is not identified and no court ruling is reported.

Asunción, Paraguay: students of the Universidad Nacional de Asunción report an anonymous website sharing intimate images of students, teachers and staff without consent, some altered or generated with AI; police cybercrime investigators and a prosecutor open an investigation, reported 21 to 24 September 2026

According to Hoy (21 September 2026) and El Nacional (22 September), students of the architecture faculty of the Universidad Nacional de Asunción reported an anonymous website that shared intimate images of students, teachers and staff without authorisation, including real photographs and material altered with AI. El Nacional reports that at least about twenty students complained, that some posts used photographs taken from social media, and that related content later appeared in Telegram and WhatsApp groups. The Minister of Women said her ministry had seen messages in which UNA students raised concern about content of them held without consent, and that in some cases AI-generated images of sexual content were shared (La Nación, 22 September). La Tribuna, reporting the findings of the police cybercrime department, describes the portal as used for the non-consensual distribution of intimate photographs and AI-altered content; according to a memorandum signed by the head of the department, a Paraguay section exposes full names, images and phone numbers of students (24 September). The university referred the case to prosecutors and police, and police suggested that prosecutors seek a court order to block the site in Paraguay. Portland, Oregon: an anonymous social-media account posted 17 racist and anti-immigrant images targeting David Douglas High School football players and their coach, some digitally altered including with AI, from 22 September 2026; the school reported content to Portland police

David Douglas High School in Portland warned families in a statement reported on 28 September 2026 about social-media posts targeting its students, athletes, coaches and staff, saying some images and videos had been digitally altered, including with AI, and did not accurately represent those depicted; it reported some of the content to the Portland Police Bureau. KATU, which reviewed the account, reports 17 posts using racist, anti-immigrant and religious stereotypes against football players: one image shows a Latino player in a sombrero being detained by people labelled as ICE agents, another shows a player in a head covering with what appears to be a fake explosive vest, and a Black player is depicted beneath a caption referring to George Floyd. The head coach said students were hurt, and that one student texted him over the weekend after seeing one of the posts: 'Coach, this is terrible. What do I do?' The account references Rex Putnam High School, whose team David Douglas played on the Friday before the report; which along with its district says it has no connection to it. Who runs the account and which images were AI-generated are not reported.
