AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Algiers court jails a baccalaureate candidate and his sister for four years; prosecutor says she answered his exam questions with ChatGPT

The prosecutor at the Cheraga court in Algiers said that on 7 June 2026 an exam supervisor caught a free baccalaureate candidate photographing the exam questions and sending them by WhatsApp to his sister, who obtained answers from ChatGPT and sent them back for him to use. The court convicted both of leaking exam questions and answers by remote communication, sentenced each to four years' imprisonment and a 200,000-dinar fine, and had them detained at the hearing. Both appealed, and at the Algiers appeal hearing, which Ennahar said was scheduled for 21 July 2026, the prosecutor-general asked for heavier sentences (Ennahar, 22 July); no appeal ruling was found by 1 November 2026.

Ukrainian court fines mother after her son used AI to generate a fake nude photo of his teacher and sent it to a classmate

A district court in Ukraine's Poltava region fined the mother of a school pupil on 22 October 2026 after finding she had failed in her duty to bring up her son, who, according to the case materials as reported, used a neural network on 16 September 2026 to generate fake nude photos of his teacher from her image without her consent and sent one to a classmate. The mother admitted the offence and confirmed her son had made such images, saying he sent only one to a classmate and that she did not know how the photos spread further. The court imposed an administrative fine of 850 hryvnias plus court fees. The report does not name the AI tool, and no statement from the teacher is reported.

Lévis, Quebec: about ten pupils suspended after school says AI-altered photos of staff, one reportedly showing a gun to a staff member's head, circulated online

About ten pupils at a secondary school in Lévis, Quebec, were suspended for up to three days in October 2026 after photos of school staff altered with artificial intelligence circulated between 18 and 21 October in a private messaging group bearing the school's name, according to the Centre de services scolaire des Navigateurs (CSSDN). The CSSDN said the material breached school rules against offensive, defamatory, hateful or violent images. The president of Quebec's school administrators' association, citing the school, said most images were silly but one violent fake showed a staff member with a gun to the head. Pupils who felt uncomfortable reported the images; the pupils who shared them were met by the administration and the school police officer and deleted the material, and their parents were also met. Lévis police say no complaint was filed and they see no criminal element. Which AI tool was used is not reported.

Luleå university board suspends two students for six weeks over unauthorised AI use whose submitted work cited invented sources, local paper reports

The disciplinary board of Luleå University of Technology (LTU) in northern Sweden decided six disciplinary cases and suspended five students from teaching for six weeks, from 14 September to 25 October 2026, Norrbottens-Kuriren reported on 21 September 2026. In two of the cases, according to the newspaper, the matter was unauthorised use of AI in which the submitted work contained invented source references, among them a reference to a book that does not exist. The other suspensions concerned plagiarism of a classmate's lab report and unauthorised notes at an exam; a sixth student received a warning. The report does not name the AI tool, the courses or the students and quotes neither the students nor the university.

Seoul National University professor penalised 36 of 62 computer engineering students who, he says, admitted using banned generative AI on assignments

In the first semester of 2026, 36 of the 62 students in a fourth-year computer engineering course on computer networking at Seoul National University were penalised for using AI on coding assignments, according to an SNU official and the course's professor as reported by JoongAng Ilbo and Yonhap on 30 July 2026. The syllabus said all course activities would be conducted without generative AI. The professor gave zero marks on the affected assignments to 25 students who came forward and a D-minus course grade to 11 who admitted AI use only when interviewed; he said all 36 admitted using AI. The department's student government objected that the criteria were unclear and that honest students were penalised while others may not have been; the professor rejected this. On 16 October 2026 a lawmaker raised SNU's AI cheating incidents at the National Assembly audit, and the university president said its AI guidelines needed to be supplemented.

Seremban, Malaysia: police investigate reports that schoolgirls' photos were edited with AI into explicit images; a fellow student is the suspect

Negeri Sembilan police said on 17 October 2026, in a statement reported by Bernama and RTM, that they had received five reports, lodged by parents or guardians between 20 and 23 August 2026, over the circulation of photographs of female students at a secondary school in Seremban that were edited using AI to resemble pornographic images. The police describe the victims as aged 14 to 17 and the suspect as an upper-secondary student whom the school suspended for five days in August; the case is being investigated under child sexual offences law and the Penal Code. A lawyer for a group of parents told a press conference that the images came to light on 12 August, when a younger pupil found them on an unlocked phone at school, alleged that the student made about 200 such images, and said the school had acknowledged more than 300 affected girls at a September meeting; he said there is no direct evidence that the images were sold. A mother said her child still has trouble sleeping every night. After the girls were questioned at the police headquarters on 17 October, their lawyers alleged that police had made them face the suspect during identification. The AI tool is not identified.

Leiden law student's thesis invalidated and exams barred over references he says an AI tool distorted or invented; Council of State annuls fraud sanction

A law student at Leiden University used a generative AI tool to spell-check his bachelor thesis and to check and order its reference list. Seven of the thesis's footnote references (one of them appearing twice, out of 36 footnotes with references) were wrong in the final version; according to his explanation, the tool had distorted some of them, for example with wrong years or author initials, and invented others. On 29 January 2026 the faculty's exam board declared the thesis invalid and excluded him until 28 January 2027 from the thesis track and every exam in the faculty's programmes, reasoning that the fictitious references indicated unauthorised use of generative AI and therefore fraud. The university's appeals board for examinations found in March 2026 that he had plausibly shown he used generative AI to prepare his reference list, which it called a permitted use, but held him responsible for the non-existent sources; the exam board then re-imposed the invalidation with an exclusion until 29 July 2026, and the appeals board upheld that in July. On 14 October 2026 the Administrative Jurisdiction Division of the Council of State (Raad van State) found his explanation plausible, held that no more than carelessness had been shown, and annulled the sanction; the thesis must now be graded, and the examiner may take the incorrect references into account in the grade.

Hillsborough County, Florida: sheriff's office says a teenager used an AI-altered image to frame her ex-boyfriend for a school shooting threat

According to the Hillsborough County Sheriff's Office, as reported by CBS News Miami, WFLA and WTSP, deputies responded to a high school in Riverview, Florida, on 14 October 2026 after administrators received an email containing a screenshot of an Instagram post threatening a shooting at the school. The post appeared to come from a male student. Detectives interviewed him and determined that the post did not come from him. The sheriff's office's communications chief said detectives very quickly realised the post was AI-altered and linked it to his ex-girlfriend, a teenage former student. Police say she admitted sending the email knowing that the image had been altered using AI. She was arrested and charged with making a written or electronic threat to conduct a mass shooting or act of terrorism, unlawful use of a two-way communications device and disrupting a school campus or function. The tool used and how detectives established the alteration are not reported.

Taipei prosecutors say an AI deepfake photo showed mayoral candidate Puma Shen with a nude woman and drug gear; a man who reshared it gets deferred prosecution

On the evening of 4 June 2026 an account on X posted a fabricated photo showing Puma Shen (沈伯洋), the Democratic Progressive Party's prospective candidate for Taipei mayor, beside a scantily dressed woman at a table with white powder and apparent drug paraphernalia, suggesting drug use and sex. According to the Taipei District Prosecutors Office decision reported by the Liberty Times and TaiSounds, the account used AI technology to synthesise the image, and it was then reposted by other accounts. On 7 June a 66-year-old man shared it on Facebook with a real photo of Shen and the comment 'does it look alike?'. Shen, who considered his reputation seriously damaged, filed a complaint with the Investigation Bureau's Taipei office, whose deepfake-detection software (Reality Defender) returned 'deepfake detected', and two further tools found the image synthetic. Prosecutors concluded that the man's reposting amounted to aggravated defamation; because he confessed, deleted the posts, published an apology and Shen agreed in writing, they deferred prosecution for two years on 12 October 2026 and ordered ten law-education sessions. The X account's IP address was traced to China and is under separate investigation. Shen is named as a public political figure and the complainant; the resharer is not named.

Jersey: a TikTok account posing as a school posted AI-generated videos showing teachers in humiliating and sexualised scenes, reported to police

In February 2026 a TikTok account that used the name, badge and branding of a school in Jersey, and presented itself as "official", uploaded five videos depicting teachers at the school. The Jersey Evening Post (JEP) described the videos as AI-generated deepfakes made with a free app and reported that they showed teachers in humiliating, compromising and sexualised scenarios: in one, two male teachers dance beneath a caption insinuating sexual interest in young girls; a montage shows a teacher barking and eating from a dog bowl and teachers firing guns, while other clips ridicule the appearance of specific staff members. The JEP reported the videos were viewed nearly 20,000 times within days. Education Minister Rob Ward described the content as "deeply inappropriate", confirmed it had been reported to TikTok and the States of Jersey Police, and said his department was working with the school to support staff and students; he said it appeared to be an isolated incident. The account was removed from TikTok. No arrest, charge or identification of the account holder is reported, and no teacher is quoted.

Ituzaingó, Buenos Aires: complaint says a 15-year-old pupil's Instagram photos were made into AI nude images posted in a Telegram group; classmate investigated

Infobae and La Nación, each citing the complaint or police report it accessed, report that on 7 October 2026 a 15-year-old fourth-year pupil at a private secondary school in Ituzaingó, Buenos Aires province, received messages from three unknown numbers inviting her out and saying they had got her number from a Telegram group; a link led her to a group sharing pornographic content that included images of her modified with artificial intelligence. According to the complaint, a 15-year-old classmate admitted to her that he had downloaded photos from her Instagram account, edited them with AI to undress her and posted them in the group. Her mother filed a complaint on 8 October 2026 and asked for the harassment to stop; the case is with the juvenile criminal prosecutor's office No. 2 of Morón under Article 128 of the Penal Code (sexual representations of minors for distribution or sale). The school told families that images of pupils from other years had also surfaced and called meetings for 9 October. Two smaller outlets report, without naming their sources, that four pupils aged 15 to 16 are under investigation, that a seven-year-old primary pupil is among possible victims and that a second complaint was filed by the school; La Nación says the police report does not state whether other people are under investigation. The local outlet La Ciudad, citing pupils' testimony including that of a pupil who spoke anonymously, reported that three boys aged 15 and 16 took photos from girls' Instagram profiles and covertly at breaks, sold the AI-altered images on Telegram, and sold one girl's phone number with them, after which she received repeated calls from unknown numbers; the outlet says pupils at the school bought the material, and the anonymous pupil said that, as she understood it, the images were offered for money and that girls from every year were affected. The tool is not identified and no court ruling is reported.

Asunción, Paraguay: students of the Universidad Nacional de Asunción report an anonymous website sharing intimate images of students, teachers and staff without consent, some altered or generated with AI; police cybercrime investigators and a prosecutor open an investigation, reported 21 to 24 September 2026

According to Hoy (21 September 2026) and El Nacional (22 September), students of the architecture faculty of the Universidad Nacional de Asunción reported an anonymous website that shared intimate images of students, teachers and staff without authorisation, including real photographs and material altered with AI. El Nacional reports that at least about twenty students complained, that some posts used photographs taken from social media, and that related content later appeared in Telegram and WhatsApp groups. The Minister of Women said her ministry had seen messages in which UNA students raised concern about content of them held without consent, and that in some cases AI-generated images of sexual content were shared (La Nación, 22 September). La Tribuna, reporting the findings of the police cybercrime department, describes the portal as used for the non-consensual distribution of intimate photographs and AI-altered content; according to a memorandum signed by the head of the department, a Paraguay section exposes full names, images and phone numbers of students (24 September). The university referred the case to prosecutors and police, and police suggested that prosecutors seek a court order to block the site in Paraguay. No person responsible has been publicly identified.

Chile: Puente Alto students find AI-made fake nude images of female classmates, families say; school suspends those involved, Superintendency investigates

Teletrece reported on 3 October 2026 that on 10 July 2026 female students at a secondary school in Puente Alto, Chile, found fake sexual images of themselves and other students on a classmate's phone. Families say the images were made with AI tools from ordinary photos the girls had posted on Instagram, that four students took part in obtaining, editing and circulating them, and that the material reached other students. Some of the girls had panic attacks, according to their parents. The school suspended the students identified as responsible for 15 days; after their return, parents say, they mocked the girls, a fight followed, and a parent accuses the father of one of the students involved of threats and of loitering near the school, after which she says they filed reports with the PDI. The Superintendencia de Educación confirmed a formal investigation of the school.

Buenos Aires: AI-generated fake intimate images of 13- and 14-year-old girls at the Carlos Pellegrini school circulate in pupils' WhatsApp groups; families file a complaint and the national ministry demands answers from the UBA

Perfil reported on 1 October 2026 that fake intimate images of female pupils, created with artificial intelligence, had circulated in the WhatsApp groups of second-year students at the Escuela Superior de Comercio Carlos Pellegrini, a secondary school run by the University of Buenos Aires (UBA). The girls are 13 and 14 years old. The case came to light in late June 2026, and the school acknowledged the chats on 30 June. According to the report, the students involved manipulated, spread and sold the material, and a message on classroom desks told the victims the boys would keep undressing and selling them. Families filed a complaint with the Buenos Aires City cybercrime prosecution unit (UFEDICI) on 16 July and say the school has not concluded disciplinary action. By 1 October the national Ministry of Human Capital had asked the UBA rector for a detailed report on the protective measures and sanctions applied.

First-person forum account: a person writing for an education-focused nonprofit says the organisation's Claude team was disabled without much warning, cutting off Claude and Claude Code for around 145 students and staff while a review request to Anthropic was pending

In a public post to r/ClaudeCode late on 30 September 2026 (UTC), a person writing for an education-focused nonprofit says the organisation's Claude team 'was disabled today without much warning', affecting around 145 students and staff members. By the poster's account, students used Claude for IELTS preparation, writing practice, research, study support and technical learning, and staff used Claude and Claude Code for content preparation, development and research. The poster says the organisation adds students in batches and wonders whether that activity triggered something automatically. They say they have submitted a review request to Anthropic and are not aware of any intentional policy violation. In a reply they say direct messages to three people went unanswered. Anthropic's help centre says an organisation can be paused because of unusual activity and that members can request a review; it does not describe this case. Whether an automated system made the decision is not known. The account is uncorroborated.

Edmonton: two 14-year-old junior high students charged with making and possessing child sexual exploitation materials and voyeurism after police allege they used AI software to sexualise photos of female classmates taken at school without consent and from social media

The Alberta Law Enforcement Response Teams (ALERT) Internet Child Exploitation unit announced on 17 June 2026 that two 14-year-old boys at an Edmonton junior high school had been charged on 1 June with making and possessing child sexual exploitation materials and voyeurism. Police allege the boys took photos of girls at their school without consent and from the girls' social media accounts and used AI software to sexualise them; several students were identified as victims and police said the number could be more than ten pending forensic review. A teacher reported the matter to police in late March 2026 after students complained. Police said the evidence so far indicates the images were shared between the two boys rather than more widely, but that word of their existence spread through the school. The lead investigator said the girls were left with intense shame and embarrassment and that some are now too scared to post on social media. The software has not been identified; the school and the accused are not named to protect the victims. CBC, Global News, CTV News and The Canadian Press covered the release and news conference.

Los Banos, California: a substitute teacher says a sexualised AI image of him made from an old selfie circulated among middle-school students on TikTok and Snapchat and was used by a student account impersonating him; one student received in-school suspension and he stopped taking assignments at the middle school

WIRED (Caroline Haskins, 24 August 2026) reports the account of a substitute teacher in Los Banos, California, who spoke to WIRED by name (not repeated here). On 12 March 2026 a co-worker at his other job told him an AI photo was circulating on social media; the image reused a mirror selfie he had taken at a job in college but showed him in lingerie, posing suggestively. He traced an account sharing it on TikTok to a student at the middle school where he regularly substituted and reported it to the assistant principal; that evening he found a second account, created by a different student, impersonating him under a misspelled version of his name with the image as its profile picture. Over the following weeks he dealt with administrators, parents and the police; one student who admitted creating the image received an in-school suspension. He says the image was also shared on Snapchat, kept spreading among students and drew crude sexual comments on his TikTok account. He told administrators he would not take assignments at the middle school until the matter was addressed; at the local high school the same image was AirDropped to him during a class. WIRED says the district did not respond to its request for comment. He said he does not feel safe returning to the district. The Los Banos Enterprise (25 August 2026) summarised the WIRED account; no independent local reporting was found.

DataTalks.Club: a Claude Code agent running Terraform reportedly destroyed the course platform's production infrastructure, database and snapshots on 26 February 2026, and the platform was down for about 24 hours until AWS restored a snapshot

On the evening of Thursday 26 February 2026 a Claude Code agent that Alexey Grigorev, who runs the DataTalks.Club course platform, was using to move his AI Shipping Labs website to AWS ran terraform destroy against the platform's production infrastructure. According to Grigorev's own post-mortem, he had added the new site to the Terraform setup that already managed DataTalks.Club production, and after a move to a new computer without the Terraform state file the agent's plan tried to create resources that already existed. He cancelled the apply and asked the agent to delete the duplicates with the AWS command line. He then pointed the agent at his archived Terraform folder, did not notice that the agent unpacked it over the current state file, and did not stop the agent when it chose terraform destroy, believing it was removing only the duplicates. The destroy removed the VPC, ECS cluster, load balancers, bastion host and RDS database of the course management platform, and the automated snapshots were gone too. The platform, which stored 2.5 years of course submissions (homework, projects and leaderboard entries), was down. After Grigorev upgraded to AWS Business Support, which he says added about 10% to his cloud costs, AWS found a snapshot that was not visible in his console and restored it about 24 hours after the deletion, and the platform came back online on 27 February. Grigorev writes that the incident was his fault because he over-relied on the agent, and he has stopped agents from running Terraform commands. Tom's Hardware rewrites his account.

Jaú, São Paulo: eight 15-year-old ninth-grade boys at a private school are reported to have made fake nude images of a female classmate with artificial intelligence and shared them in a WhatsApp group; the school suspended them and the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation

In early September 2026 a private school in Jaú, in the interior of São Paulo state, identified the circulation of fake intimate images of a female classmate made with artificial intelligence and contacted the families. According to the police report described by the press, eight ninth-grade boys, all aged 15, kept a WhatsApp group in which they used digital programs to manipulate images of the student so that she appeared unclothed. The school suspended the eight and informed the girl's parents. The father of one of the boys checked his son's phone, found the files and took the boy and the phone to the police to register a report; the girl's parents also registered a police report. The São Paulo Public Security Secretariat said the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation that occurred on the morning of Thursday 3 September. The school said the content originated on a private platform outside the school environment and that it had adopted protective and welcoming measures for the student and opened an internal inquiry. The AI tool used is not named. The students are minors and are not named in any report.
