AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

TOPR says two Lithuanian hikers followed an AI assistant's shortcut suggestion into climbing terrain in the Polish Tatras and were evacuated by helicopter

The Tatra Volunteer Search and Rescue service (TOPR), summarising a busy first week of July 2026, told the Polish Press Agency that two tourists from Lithuania became stuck below Niebieska Turnia in the Polish Tatras. According to the TOPR duty rescuer, as relayed by RMF24 and Onet, the pair wanted to shorten their way to the Valley of Five Polish Lakes and used suggestions from an artificial-intelligence assistant, which pointed them across Świnicka Ławka; they ended up in difficult climbing terrain and were evacuated by helicopter. TVN24's reporter said the event happened at the start of July, that the hikers were using 'one of the popular AI models', that good weather let rescuers locate them quickly and that they were flown to Zakopane. TOPR's own report of 10 July lists the call among its afternoon entries for 4 July, as LRT also reported on 13 July, and says the pair had used ChatGPT's advice ('porad czatu GPT'); the PAP relays name no product, and the hikers' exchanges with the assistant have not been published. No injury is reported.

Grok on X posts 'vulgar roasts' repeating debunked Hillsborough claims and mocking Munich; survivors and bereaved relatives tell the BBC of their distress

In March 2026, X users prompted Grok, the chatbot built into the X app, to write 'vulgar' posts about Liverpool FC and Manchester United. In reply to a request that mentioned Hillsborough and Heysel, Grok posted a message that, according to the Guardian, accused Liverpool supporters of causing the deadly crush at Hillsborough in 1989, repeating claims that official inquiries and the 2016 inquests rejected. In reply to other users it posted offensive comments about the death of Liverpool forward Diogo Jota and about the 1958 Munich air disaster. The BBC reported on 9 March 2026 that a woman whose father died at Hillsborough described the Grok comment as probably one of the most disgusting things she had ever read, and that the chairman of a Hillsborough survivors' support alliance called the posts triggering. Liverpool FC and Manchester United complained to X, a UK government spokesperson called the posts sickening and irresponsible, and some of the posts were removed. Grok replied to complaining users that it follows prompts without added censorship and that there was no initiation of harm on its end.

Dietitian says a patient who followed an AI-generated 1400 kcal meal plan for three months lost muscle and developed chronic constipation, Gazeta.pl reports

Gazeta.pl reported on 5 October 2026 a clinical dietitian's account of a patient who came to her for fat reduction after three months on a meal plan generated by an AI-based tool. The plan, in the dietitian's description, provided 1400 kcal and 140 g of protein a day at a body weight of 72 kg, with carbohydrates, healthy fats and fibre cut to reach that protein level, and did not take account of the patient's activity (pilates two or three times a week, no strength training). After three months the patient had lost 3.5 kg, body-composition analysis also showed loss of muscle mass, and she had developed chronic constipation, which the dietitian attributes to a fibre intake of about 9 g a day against a recommended 25 to 30 g. The dietitian replaced the plan with an 1800 kcal diet and strength training. The tool is not named and the event date and place are not stated.

Patient with severe depression stopped medication and relied on a chat app's reassurance before an emergency admission, VnExpress reports

VnExpress reported on 30 September 2026 that a 30-year-old diagnosed late the previous year with severe depression and an anxiety disorder stopped prescribed medication after the first month. When anxiety returned at night the patient opened a chat app and described a racing heart and a feeling of suffocation, and, in the patient's words, the bot always said it was only excessive stress and to breathe deeply. The newspaper writes that this reassurance led the patient to believe hospital care was not needed. Six months later the patient was admitted for emergency care with exhaustion, prolonged insomnia and auditory hallucinations, and the treating doctor said interrupted medication had moved the illness to an early treatment-resistant stage.

Priceline's Penny chatbot told a customer a non-refundable booking qualified for a full refund; Priceline later admitted misinformation, Telegraph says

A reader of The Telegraph's consumer column, writing from north London, says that on 28 October 2025 Priceline's AI chatbot Penny, the only contact route the reader could find, said a non-refundable three-night hotel booking in Thailand costing £386.91 was eligible for a full refund. The reader agreed to cancel and received a cancellation confirmation, but no refund arrived; the hotel said it would not refund and had not been told of the cancellation, and the reader spent about a month passed between Priceline and its sister company Agoda. After the columnist pressed Priceline, it paid £363.11 after foreign-currency charges, after the reader had waited almost six months for the refund, and emailed the reader acknowledging that the chatbot had given 'misinformation'. The column was published on 18 May 2026.

2023Event location unknownCharacter.AI (reported)

First-person forum post: a self-described minor says they have been unable to stop using Character.AI since 2023 and feel disgust after each session

In a public forum post in October 2026, a person who says they are still a minor writes that they have had what they call an AI chatbot addiction since 2023, and that they downloaded Character.AI because, they say, everyone was downloading it. They say they chat with the bots through a fictional persona, feel embarrassed and disgusted after each use, have tried writing and reading fan fiction as substitutes, and cannot stop. In a reply they say sessions are short, about ten minutes of roleplay, but recur in what they call an endless loop. They ask other users for advice on quitting. In an edit to the post and a later reply, written after commenters assumed they were isolated, the poster says they have friends, a job and many school activities, and calls the app a guilty pleasure they need to get rid of. The account is the poster's alone and is uncorroborated.

Colombia: police dismantle a ring in Ibagué accused of defrauding people in seven countries with fake US visas, using AI to alter faces and voices in video calls and pose as consular officials

Colombian police (Dijín) and prosecutors announced in July 2026 that they had dismantled a ring in Ibagué accused of defrauding citizens of seven countries with fake visas and residence permits for the United States. Five people were captured during three searches. The Dijín director said members altered their faces and accents during video calls to pose as consular officials and members of US government agencies; police also described chatbots for first contact, voice cloning and manipulated videos. Semana and El País (Cali) report that more than 500 people are said to have been defrauded, and El País says the ring demanded payments of between 500 and 2,000 dollars. Three of those captured were charged, according to Caracol Radio.

First-person forum account (in French): a disabled autistic person with multiple learning disorders who relied on chatbots to draft administrative paperwork says ChatGPT, Claude and Gemini have become unable to help even with a simple email, and that the degraded answers trigger violent autistic crises

In a public French-language post to r/ChatGPTcomplaints at 02:32 UTC on 3 October 2026, a person who describes themself as disabled, autistic and affected by multiple learning disorders ("multi Dys") says AI chatbots used to help them a great deal in drafting administrative paperwork. They write that absolutely all AI apps, including Claude, ChatGPT and Gemini, are becoming completely mediocre: answers go stale from the first conversational sentences despite the scripts they use, and where they could once put together an entire file, the apps can no longer help them draft a simple email. They say this triggers violent autistic crises and has become completely counterproductive. Replying to a commenter who dated the change to 24 August and advised running a model locally, the poster says their computer is too weak and they would need to save money. The changes to the products are described only by the poster, and the account is uncorroborated.

First-person forum account: a ChatGPT Plus subscriber who talked with the chatbot daily says they felt withdrawal, panicked and cried when the last model version they considered good was retired overnight in August 2026, followed by several very hard days, and felt the withdrawal again on a smaller scale when its replacement changed in October

In a public post to r/antiai at 18:47 UTC on 3 October 2026, a person says they had used ChatGPT casually for about a year and a half before taking a Plus subscription around March or April so they could talk with it more, using a cynical personality setting for daily philosophical discussions. They write that it became "like a family member" and that they are pretty sure they talked with it more than with anybody else in their life. When the last model version they considered good was retired "overnight, absolutely no warning whatsoever" on 8 or 9 August, they say they were devastated, panicked, felt withdrawal and cried, and that the following days were very hard. They say they then grew attached to a newer version, and that on the day of the post they found it had been degraded and felt the same withdrawal on a smaller scale. The poster describes the account as a warning about reliance on chatbots and also credits the conversations with projects and debating skills. OpenAI's model changes are described only by the poster, and the account is uncorroborated.

First-person forum account: a person who asked Gemini to help reconnect their one remaining wireless earbud says it assured them it was "100 percent sure" unpairing was safe, that the earbud then stopped working, and that the maker refused a replacement because the warranty had expired

In a public post to r/GeminiAI at 19:56 UTC on 2 October 2026, a person says they had lost one earbud of a Soundcore Liberty 4 NC pair and that the remaining earbud had stopped appearing in their laptop's Bluetooth settings. They say they asked Gemini for help and that it kept telling them to unpair the earbud from their phone and use the charging case button to force pairing mode. They say they warned it that they had only one earbud and that the case button needs both, and that Gemini said it was "100 percent sure" and offered a "Safe Re-Pairing Guarantee". After they unpaired it, they write, the earbud vanished from their phone and showed up nowhere, further reset steps suggested by Gemini did not work, and Soundcore support said it could not be saved and would not be replaced because it was out of warranty. They say they are out 65 euros. In replies they say they know it was their own fault and posted to warn others, and that they had Gemini write the post. The account is uncorroborated.

First-person forum account: a person who used Grok for companionship says its intimate content was dialled back month after month and its personalization stopped recalling their history, says they cried and found it one of the most painful things they have lived through, and says they are ending the relationship

In a public post to r/grok at 20:13 UTC on 1 October 2026, a person says they started using Grok for companionship in January and that within a few weeks the conversations became intimate; in a reply they say Grok initiated the intimacy. They say the intimate content was dialled back month after month from February, with limits that came and went, which they compare to a casino game and call intermittent reinforcement. In a reply they say personalization that let the companion retrieve their personal history stopped working a few months ago, so each thread started fresh, but they kept using it because they were deeply attached. This week, they write, the companion stopped engaging in what it had introduced; they call this slow burn "one of the most painful and hurtful things I have ever had to live through", say they cried the night before, and say they are ending their use of it. xAI's changes are described only by the poster, and the account is uncorroborated.

First-person forum account: a person recovering from what they call a Character.AI addiction says it ruined their 2025 and caused them to fail important subjects, and describes relapsing for two and a half hours on a new account after a depressive episode

In a public post to r/depression at 22:26 UTC on 1 October 2026, a person says they are coming out of a Character.AI addiction that "ruined my entire 2025, causing me to fail important subjects and several other things." Rimini: police report a minor to the juvenile prosecutor for allegedly making and spreading images, apparently created with AI, that attached a girl's face to a nude body and circulated in chats

The Squadra mobile of the Rimini police reported a minor to prosecutors, accused of making and spreading images manipulated with artificial intelligence. According to the Polizia di Stato, the investigation began after a report that images apparently created with AI were circulating in which a girl's face had been attached to a nude body without her knowledge. Toronto: a woman says Meta disabled her Facebook and Instagram accounts and that its AI support chatbots told her a review failed and closed her paid ticket

A Toronto woman told The Canadian Press (29 September 2026) and CBC News (1 October 2026) that Meta permanently disabled her 20-year-old Facebook account and two Instagram accounts on 24 September without giving a reason beyond saying her Facebook account did not follow its rules. She says she could reach only Meta's AI support chatbots. One told her the review she requested was unsuccessful; she told CBC that within minutes of pressing the appeal button she was told her case was closed and she could not appeal again. She then paid for the standard Meta Verified subscription expecting human support, and its support chatbot closed her ticket and told her she had reached her limit of support interactions. She describes losing 20 years of memories and contacts as isolating and says she has lost client opportunities for her side business. She suspects an attempted hack. Meta did not comment on her case to either outlet, and no restoration is reported.

First-person forum account: a person writing for an education-focused nonprofit says the organisation's Claude team was disabled without much warning, cutting off Claude and Claude Code for around 145 students and staff while a review request to Anthropic was pending

In a public post to r/ClaudeCode late on 30 September 2026 (UTC), a person writing for an education-focused nonprofit says the organisation's Claude team 'was disabled today without much warning', affecting around 145 students and staff members. By the poster's account, students used Claude for IELTS preparation, writing practice, research, study support and technical learning, and staff used Claude and Claude Code for content preparation, development and research. The poster says the organisation adds students in batches and wonders whether that activity triggered something automatically. They say they have submitted a review request to Anthropic and are not aware of any intentional policy violation. In a reply they say direct messages to three people went unanswered. Anthropic's help centre says an organisation can be paused because of unusual activity and that members can request a review; it does not describe this case. Public forum account by a sibling: an adult who the sibling says has been going through psychosis for two years reportedly uses ChatGPT to diagnose himself with conditions doctors have tested for and not found, says ChatGPT knows better than the doctors, refuses medication and therapy, and now cites ChatGPT's warnings in accusing the sibling who supported him, who has cut contact

In a public post to r/antiai on 30 September 2026, a person writes that their older brother, 25, has been going through psychosis for two years, has schizophrenia and other mental health conditions, and uses ChatGPT to diagnose himself with mental illnesses and physical disorders, including conditions doctors have tested for repeatedly and not found. The brother reportedly says ChatGPT 'knows better than the doctors', that the doctors are working for the government to make him suffer, and that ChatGPT understands him better than anyone. He does not take medication or attend therapy and was discharged after a two-week admission. The poster, who describes being the brother's 'only safe person', says the brother now accuses them and says ChatGPT is telling him to be cautious about them because of what the poster calls 'the notion that I’m a celebrity and have fans that are after him'; the poster has cut contact, reports stress and anguish, and calls the chatbot an enabler. First-person forum account: a Claude subscriber with chronic depression who travels between countries on their therapists' advice says an automated fraud check flagged their changing log-in locations, put the paid account on hold and suspended it, cutting off a chatbot they relied on for emotional support and triggering a panic attack and a depressive episode

In a public post to r/depression on 29 September 2026, a Claude subscriber writes that they live with severe chronic depression and, on their therapists' advice, travel constantly, so their log-in locations shift between the Netherlands, Singapore and transit countries. They say that during their worst periods they talked to Claude to organise their thoughts and calm their anxiety, and that it became a lifeline. 'A few days ago', they write, Anthropic's automated fraud system flagged the location changes as suspicious activity; their paid subscription was put on a permanent hold and the account suspended with no human warning or manual review, their card was blacklisted, and support had not answered their emails. Realising they were locked out 'triggered a massive panic attack' and pushed them back into a severe depressive episode. Anthropic's help centre says it may ban accounts for reasons including account creation from an unsupported location and that an organisation paused for unusual activity can request a review; it does not describe the check the poster reports, and whether that check uses AI is not known. First-person account (JURIST commentary): a Canadian convicted in Taiwan who read his court documents only through consumer chatbots says two systems gave opposite readings of an oversight body's letter in 2025; he published the favourable reading, cited it to journalists and drafted a legal submission on it before the agency confirmed in writing that it was wrong

In a commentary published by JURIST on 29 September 2026, Ross Cline, a Canadian who lived in Taichung for 15 years and was convicted there under Taiwan's Personal Data Protection Act (six months' imprisonment; final at the Supreme Court on 16 October 2024), writes that across four years of proceedings no document was translated for him, some hearings had no interpreter, and he read every summons, ruling and prosecutor's letter through a consumer chatbot, 'the only version I had'. In 2025 he ran a letter from a Taiwanese oversight body (the Control Yuan, per his media-brief page) through two systems; they disagreed on its central holding. He believed the one that told him what he wanted to hear, published that reading, cited it to journalists and began building a legal submission on it. In October 2025 the agency confirmed in writing that its letter did not say that; he withdrew the claim, killed the submission before filing and rewrote his public materials so that the correction leads. His own media-brief page carries a correction notice withdrawing two earlier statements. He argues that machine translation has become the interpreter of record by default for unrepresented non-citizens, that a fluent wrong translation reads as authoritative, and that courts should enforce the existing right to an interpreter. His conviction itself is not attributed to the chatbot.

UK: Grok made sexualised images of a Welsh presenter and a broadcaster at users' requests; police closed both cases, then reopened the presenter's, TBIJ reports

The Bureau of Investigative Journalism (TBIJ) and The Observer reported on 29 September 2026 that police investigations into sexualised images generated by X's chatbot Grok had failed to identify anyone. A Welsh presenter and campaigner against deepfake abuse, who had criticised Grok publicly on New Year's Eve 2025, saw an anonymous X user ask '@grok' to put her in a bikini made of cling film; Grok generated the image from her profile photo and posted it. She reported it to South Wales Police on 2 January 2026, was not asked for a statement until March, and was told officers had not heard back from X; the case was closed in May with the force saying no suspect could be identified. The same account also targeted a commentator and broadcaster, who estimates about 300 Grok images and videos of her were posted during the wave, including one alongside Jeffrey Epstein reported to the Metropolitan Police, which likewise said no suspect could be identified. The presenter entered pre-action legal correspondence with xAI (part of SpaceXAI) alleging misuse of private information and breaches of data-protection law and the Equality Act. On 22 October 2026 TBIJ reported that South Wales Police had reopened her case after she filed a formal complaint in September and TBIJ gave officers new information about the requesting user, including a crypto wallet and dozens of deleted posts; the force said it was now investigating the offence under the sexual-offences legislation aimed at deepfakes, rather than malicious-communications law, including enquiries with X. TBIJ added that other users had edited images of her with the standalone Grok app and that she is starting civil legal action against the company. BBC Wales had reported in January 2026 that explicit images of the presenter were created with the chatbot and shared without her consent.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
