AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Perth woman tells PerthNow men at a Scarborough pub covertly photographed her and a friend and used an AI program to make sexualised images shown to the pair

A Perth woman told PerthNow (report carried by The West Australian and The Nightly, 7 October 2026) that on a Friday night at a Scarborough venue a group of eight to ten men sitting nearby began bantering with her and a friend, and that one of them photographed her without her knowledge, put the photo into an AI program and altered it into sexualised content showing her kissing several of the men. According to her account the men passed the image around and showed it to the two women while laughing, and had also made sexual content from a photo of her friend. The women reported the men to the venue, which banned them, and to Western Australia Police; in messages seen by PerthNow a detective wrote that there was at that point no law covering AI-created photos or media, while a WA Police spokesperson said state legislation criminalises non-consensual distribution of intimate images including AI-altered ones and that a statutory review is considering possession. The man who allegedly made the image later messaged her that he had made it as a joke and apologised. The images are not reported to have been distributed online; the tool is not identified; the account is the woman's own, with the man's private messages and the police correspondence as described by the newspaper.

Premium Times reports deepfake videos of Nigerian public figures ran for months as paid Facebook investment-scam ads; Meta removed them after the paper asked

Premium Times reported on 8 October 2026 that deepfake videos of Nigerian public figures were used in sponsored Facebook posts to promote fraudulent investment schemes. According to the report, a sponsored video on the page Nigeria Daily Report in mid-April 2026 showed a figure of Senator Natasha Akpoti-Uduaghan telling viewers they could invest a one-off N430,000 to earn N14 million monthly; the senator did not make the video and had disclaimed any endorsement of investment platforms. In May 2026 a deepfake video of presidential candidate Peter Obi on the page Nigeria Daily Bulletin asked users to invest N350,000 to earn N4.5 million, linking to a website that cloned Channels Television. Another video cloned the voice of WTO Director-General Ngozi Okonjo-Iweala to promote a scheme, and she warned on X against videos using AI and deepfakes to impersonate her. The newspaper reports that Meta confirmed the ads and videos violated its policy, that the posts stayed on the platform for months, and that they were removed in July 2026, one of them in the first week of July, after the newspaper contacted Meta. No deceived investor is identified, the tool and the creators are not identified, and the deepfake descriptions are the newspaper's and the depicted people's.

New Hampshire Senate Republicans ran Meta ads with an AI-altered image of Black state Senate candidate Leonard Bell amid cash and gold chains, outlet reports

Granite Post reported on 5 October 2026 that the New Hampshire Senate Republicans paid Meta to show Facebook and Instagram users an image of Leonard Bell, the Democratic candidate for state Senate District 18 and the only Black candidate for state Senate in New Hampshire this year, laughing in a leather armchair buried in cash and wearing gold chains, a large ring and a watch. According to the outlet, Meta labelled the ad "Digitally created" and its detail page states that the media was created or edited with AI; Bell’s face appears to have been taken from a family snapshot. Bell told the outlet that everything in the image is false other than his face. Citing Meta’s Ad Library, the outlet says three ads from 7 September put the image on New Hampshire screens 39,000 to 50,000 times. The outlet describes the image as a racial stereotype, and a Democratic co-sponsor of New Hampshire’s 2024 deepfake law called it a racist caricature. The Senate Republicans did not respond to questions.

Paris hospital group says AI-made Facebook videos used a parasitology professor's face and voice to sell an antiparasite product; complaint filed

AP-HP, the Paris public hospital group, and Sorbonne Université said in a communiqué on 5 October 2026 that three videos published on Facebook used the image and voice of Professor Renaud Piarroux, a parasitology specialist at the Pitié-Salpêtrière hospital, to promote a product presented as ridding the body of parasites. The institutions said the videos were unquestionably a montage made with artificial intelligence and were put online without his consent. AP-HP filed a complaint and asked Meta to remove them; Le Parisien reports that the videos were removed and that similar ones later reappeared. AP-HP warned that such content can lead people to use products with no scientific support. Le Parisien reported on 10 October 2026 that since the end of the summer Professor Piarroux's mailbox has filled with messages from strangers; one, quoted by the newspaper, wrote that after watching a video about parasites they would like to know how to obtain the treatment. Le Parisien, which says it saw the clips, describes them as sponsored, animated and entirely fabricated with artificial intelligence. No source reports that anyone bought the product.

Phagwara, Punjab: after unrest at Lovely Professional University, a video of the local Superintendent of Police circulates with an AI-made voice saying a relative of the Union Home Minister was behind the violence; Punjab Police call it fake (CyberPeace dates a Kapurthala Police post to 29 September 2026) and a fact-checker finds the original footage

India TV reported on 30 September 2026 that, after violence at Lovely Professional University in Punjab, a video of Phagwara's Superintendent of Police went viral in which the officer's voice claims that a relative of the Union Home Minister was behind the violence and that a police team had gone to Delhi to find him. Professor Nissaf Ben Alaya publicly denies a video circulating on social media that uses the professor's image and an attributed voice to promote a supposed prostate cancer treatment, and attributes it to AI; statement published by Kapitalis on 28 September 2026

Kapitalis reported on 28 September 2026 that Professor Nissaf Ben Alaya had issued a categorical denial of a fraudulent video circulating on social media, which the outlet describes as a deepfake generated with AI. In the statement, which Kapitalis reproduces, the professor says the video uses the professor's image and a voice attributed to the professor to promote a medicine presented as a treatment for prostate cancer. The statement says the video appears to reuse old footage, including from the COVID-19 period, with a voice or words artificially added, a method the professor attributes to AI. The professor says no permission was given, that the professor has never advertised a health product, and asks platforms and the competent bodies to identify and remove the content. The statement warns that such videos can harm the reputation of the people shown and put the public's health at risk. The report does not say who made the video or whether it has been removed.

Jhansi police arrested a 27-year-old woman in Prayagraj in September 2026 on charges of editing photos with AI, blackmail and extortion, after the wife of a senior railway officer living in Jhansi filed an FIR on 2 August 2026. According to the complaint as relayed by Aaj Tak, UP Tak, Dainik Bhaskar and Amar Ujala, the woman threatened to circulate objectionable pictures of women in the family, demanded ₹15 lakh and threatened to kill the family, and out of fear the family paid her ₹40,000 in total on different dates between January and June 2026. On 31 July she sent an objectionable image of the officer to his wife's phone, an image police say she made with AI. Aaj Tak places the ₹15 lakh demand after that image; Dainik Bhaskar reports the complaint as dating the demand and a death threat to the woman's visit to the family's government house on 2 June. Police recovered two phones and a laptop, and Amar Ujala reports that police will have the images examined forensically. Malaysia: footage of former prime minister Mahathir Mohamad mourning his wife was edited, in what sources believe was AI manipulation, to add dancing, and posted on TikTok in September 2026; the communications regulator MCMC said it was examining content believed to use AI to manipulate footage of him in mourning

Tun Dr Siti Hasmah Mohd Ali, wife of former Malaysian prime minister Tun Dr Mahathir Mohamad, died on 28 September 2026 aged 100. BuzzKini reported on 30 September that a TikTok account had posted a video of Mahathir grieving beside her body, edited to show a woman dancing next to him, and another clip of the moment he raised his hands in prayer edited with dancing movements while seated in a wheelchair. BuzzKini says the edits are believed to have been made with AI, and that many social media users condemned them and asked the regulator and police to act. On 2 October the Malaysian Communications and Multimedia Commission (MCMC) said it was examining social media content believed to use AI to manipulate footage of Mahathir in mourning, had contacted the platforms concerned, and that action could follow under the Communications and Multimedia Act 1998 if a breach was found.

New Mexico: the state Democratic Party filed an ethics complaint, reported on 2 October 2026, over two Senate Republican social media posts from September 2026 that it says used misleading AI-generated images, including a fake jail mugshot of Senate President Pro Tem Mimi Stewart that she publicly asked to be taken down

In September 2026 Republican state Sen. Jay Block posted on social media a fake image of New Mexico Senate President Pro Tem Mimi Stewart made to look like a jail mugshot. The Santa Fe New Mexican (24 September 2026) reports that Block's posts indicate the image was AI-generated and that it showed her ostensibly booked into the Bernalillo County jail on charges of 'hit and run' and 'abusive behavior towards staff and other lawmakers'. Stewart was not issued a citation or booked, and Santa Fe's interim police chief said the 2024 incident in which she drove over a pedestrian's foot was not considered a hit and run crash. Stewart asked Block and the Senate Republican caucus to take down the fabricated image. Block said he stands by the post and has no plans to remove it. KRQE reported on 2 October 2026 that the Democratic Party of New Mexico had filed an ethics complaint over this post and over an image that Senate Republican leader Bill Sharer and the Senate Republicans shared on Facebook in September, which shows House Speaker Javier Martinez wearing a Soviet Union pin, gubernatorial candidate Deb Haaland with her arm around a man in an orange jumpsuit and former state Rep. Joseph Hernandez naked. The party calls both 'misleading AI generated images' that violate a 2024 state law on deepfakes and wants the posts removed and the posters fined.

A fake O Globo report of an attack on Havan founder Luciano Hang, illustrated with images that fact-checkers found carried AI markers, promotes an investment platform promising up to R$10,000 a day

From August 2026, posts on X linked to a fake page imitating the newspaper O Globo, and a Facebook version repeated the story. The page reported an attack on businessman Luciano Hang, founder of the Havan stores, and directed readers to a supposed investment platform promising R$3,000 to R$10,000 a day. The fact-checkers Lupa and Comprova found that no attack happened. Lupa reports that an OpenAI verification tool found a watermark indicating OpenAI image generation in the montage on the page; Comprova reports that SynthID indicated that the images it analysed, showing Hang injured, were made with Google's AI tool. Hang said in a note that criminals use his image, including AI manipulations, to promote fake investments. São Paulo's regional electoral court ordered X to remove posts falsely linking candidate Fernando Haddad to the fake attack. No investor loss is reported.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
