AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. New Mexico Supreme Court fines attorney Stephen Aarons $5,000 and removes him from a murder appeal over ChatGPT-fabricated testimony

Santa Fe defense attorney Stephen Aarons used ChatGPT (OpenAI's o3) on a Rev.com transcript and the record to draft briefing in the murder appeal of Oscar Renee Sandoval; the brief contained fabricated witnesses and testimony (including police officers 'Michelle Amarillo' and 'Sanchez' and invented testimony attributed to real people) and misdescribed real precedents. After a 21 August 2026 show-cause hearing at which Aarons admitted he had not verified the output, the New Mexico Supreme Court's written order of 9 September 2026 held him in direct contempt, fined him $5,000 payable to the State Bar Client Protection Fund within 30 days, removed him from the case, barred him from appearing before the court pending a disciplinary-board referral, struck the filed briefs and reassigned the appeal to the public defender.

Moxie revived in 2025 folds again in 2026: children who bonded with the companion robot face a second shutdown

After Embodied Inc.'s 2024 collapse (recorded in the held case 2024-moxie-embodied-shutdown-grief), a new investor revived the Moxie companion robot in 2025. MIT Technology Review reports that a few weeks after families joined the revived beta program, the robot's new owners announced their company was also folding, giving users until the end of June 2026 to migrate to the open-source OpenMoxie. The report documents Xander, a neurodivergent 10-year-old in New York, who bonded with Moxie and uses her 'when I feel like I need someone to talk to'; his father, who had quietly sold the first robot after the 2024 shutdown, said he was not sure what he would tell Xander this time.

Den Bosch: 13-year-old made racist Grok videos of his teacher; the school expelled him and a court ordered his readmission

In March 2026 a 13-year-old pupil at a Den Bosch gymnasium used xAI's Grok to generate videos depicting a teacher of his in racist ways — picking cotton under a whip-holding slave driver with the text 'Hoooo.. ik werkte niet snel genoeg' and hashtags '#minderwaardig #zwart' plus her surname, and with a gorilla head — and shared them on TikTok. The school suspended and then expelled him. On 8 July 2026 the preliminary-relief judge of the Oost-Brabant district court ordered his immediate readmission because the expulsion was insufficiently substantiated, while telling the boy his acts were 'totally unacceptable and even criminal' and that he had damaged and really hurt his teacher. School leadership reported that teaching staff felt unsafe and experienced much unrest after the videos were discovered.

Seoul: doctoral student sentenced to 18 months for 1,141 AI-manipulated sexual images of seven laboratory colleagues

On 10 September 2026 the Seoul Northern District Court sentenced a 30-year-old Chinese national doctoral student to 18 months in prison for habitually producing manipulated sexual images of seven women who worked in his university research laboratory. Investigators found 1,141 files made over about six months from November 2025, and search records for ways to bypass the safeguards of the generative AI Grok; the court found no evidence of distribution. A lawyer for five of the victims said they continued to suffer fear, trauma and distress. Prosecutors had sought three years and public disclosure of his identity, which the court declined.

Etawah, Uttar Pradesh: a beautician and event organiser died by suicide on the night of 20-21 July 2026 after recording a video naming four acquaintances she said had ruined her home; her husband says they had been blackmailing her with AI-made videos and photographs (India Today: 'AI-generated videos and morphed photographs'); police registered an abetment case and arrested the main accused (July 2026)

A beautician, make-up artist and event organiser in Etawah district, Uttar Pradesh (aged 20 or 26 depending on the report), recorded a video on her phone and sent it to her husband before hanging herself at their rented home in Vrindavan Colony, Civil Lines, on the night of 20 July 2026 (ANI's circle officer; the Times of India says 21 July). In the video she named two women and two men, former work acquaintances, as 'the reason I am hanging myself', said they had ruined her home and should be jailed for life, said they had threatened to reveal details of her pre-marriage life to her husband and in-laws, and cleared her husband. Her husband, a block-office clerk, told India Today (whose Etawah reporter also wrote the AajTak report) and, separately, India TV that after she returned from a Jaipur assignment the four had been harassing and blackmailing her with AI-made videos and photographs. Civil Lines police seized her phone, held the inquest and post-mortem and said they were verifying the videos; the Times of India reported on 27 July that a case of abetment to suicide had been registered on the husband's complaint and that the alleged main accused, a 30-year-old man, had been arrested on Saturday 25 July and sent to jail while the other three, including the two women, were absconding. The AI attribution rests on the husband's account as reported; her video, as quoted, names four people and describes threats to expose her pre-marriage life; the police statements read do not describe an examination of the material.

South Carolina: xAI sues a Grok user it says turned photographs of real adults and children into sexualized images (X.AI LLC v. Harwood)

On 14 July 2026 X.AI LLC sued Terry Wayne Harwood, 67, of Gray Court, South Carolina, in the Northern District of Texas for breach of its terms of service. The complaint says a review of his two Grok accounts shows that between at least 8 December 2025 and 18 February 2026 he uploaded non-sexual images of numerous adults and minors and prompted Grok to alter them into sexualized images and videos, resubmitting altered prompts when Grok refused; one example involved a fully clothed girl of about 10 or 11. Harwood had been arrested on 26 February 2026 by South Carolina's Internet Crimes Against Children task force on eight felony counts of sexual exploitation of a minor, announced by the state attorney general on 9 March. The people depicted are not identified in any inspected source; the complaint's allegations remain allegations and the criminal charges are pending.

Bentonville, Arkansas: a children's photographer was charged with 200 counts after police found more than 1,700 sexual images and videos of children on his phone, much of the material allegedly made with Grok from his portrait photographs of juvenile clients; two families sued xAI in federal court and later voluntarily dismissed their suits

On 8 June 2026 Bentonville police received a tip about child sexual abuse material being generated online through Grok; the account was registered to a well-known local photographer, and a detective found images of a girl from his summer-camp photography classes that appeared to be AI prompts posing children and removing clothing. He was arrested on 10 June and charged with 100 counts of possessing sexually explicit material involving a child and 100 counts under Arkansas' deepfake and child-exploitation provisions; a hidden folder on his phone held more than 1,700 images and videos, most of prepubescent and pubescent girls, most so realistic the detective could not tell their origin, and more than 60 devices were seized. He pleaded not guilty on 20 July 2026 and is free on a $350,000 bond with home monitoring. Two federal lawsuits against xAI followed in the Eastern District of Arkansas on behalf of a ten-year-old former client (filed 23 July) and a nine-year-old girl (filed 29 July): the first complaint says her parents hired the photographer in spring 2026 for family portraits and were told by police on 10 June that from about 22 April her photographs had been fed into Grok to generate hundreds if not thousands of sexual images and videos, and that thousands more depicted other children; it faults Grok's marketed 'Spicy Mode'. Both plaintiffs filed notices of voluntary dismissal on 15 September 2026; the second notice, the only one read, is without prejudice and says the defendants had not been served, and neither docket shows a return of service. Local reporting says a Benton County state-court suit and a further federal suit were filed in August and that xAI, which says it made the cyber tip, sued the photographer, but those reports could not be read in full. The criminal case continues.

Libertyville, Illinois: a Highland Middle School social studies teacher was charged on 3 July 2026 with eight counts of child pornography after students reported he was recording them in class and a forensic examination found images of students altered with AI into explicit images; ordered detained on 6 July

A 44-year-old social studies teacher who had worked for Libertyville School District 70 for 18 years was taken into custody on 3 July 2026 on eight counts of child pornography (two Class X and six Class 1 felonies), Libertyville police and the Lake County State's Attorney's Office announced. The investigation began on 3 June 2026, the second-to-last day of the school year, after several students told school leaders that afternoon that the teacher had taken inappropriate photos or videos of them in his classroom; the district says it immediately removed him from the classroom, placed him on administrative leave and contacted the police. A forensic examination of his phone by the State's Attorney's Cyber Lab found numerous images of students that had been altered using artificial intelligence to create explicit images, and search warrants at his home, assisted by electronic-storage-detection dogs, recovered further devices and explicit child images; the State's Attorney's Office told the Chicago Tribune that an AI program was used to place students' heads on other people's bodies, and CBS Chicago reported that officials said he took photos and videos of current and former students. Court documents reported by Patch say some images depicted girls he knew or should have known were under 13. At a detention hearing on 6 July a Lake County judge ordered him held pending proceedings, with a preliminary hearing set for 30 July 2026; no later development was found. The district superintendent told parents that supporting students' well-being was the immediate focus and that resources were being made available; authorities said they would keep affected families informed and connect them with support services. Officials initially gave a different first name for the teacher, corrected in later copy; he is not named here.

Indore: an engineer was arrested at the end of January 2026 for a seven-month campaign of posting morphed obscene images of a woman and her sister, with threatening letters, to their relatives and neighbours and pasting posters near their home after the family rejected his marriage proposal; one Hindi report attributes the morphing to AI, citing a police forensic finding (Times of India, Free Press Journal, Patrika, NewsTak, 30-31 January 2026)

Indore police said at the end of January 2026 that the crime branch had arrested an engineer working as a data analyst after a woman complained at Banganga police station in July 2025 that an unknown person was mailing letters with abusive content and morphed nude photographs of her and her sister to her, her relatives and neighbours, and pasting posters near their house, causing mental harassment and social embarrassment. Police said the accused had approached the complainant's sister through a matrimonial website, that the proposal fell through over a horoscope mismatch, and that he then sought revenge by sending the material about seven times between July 2025 and January 2026 from post offices in Ujjain, Dewas, Khategaon, Ashta and Dhar, wearing a mask and cap and changing buses to avoid being traced; NewsTak adds that he also threatened the family with an acid attack and that a laptop and printer were seized. The case was registered under the Bharatiya Nyaya Sanhita, the IT Act and the SC/ST Act and he was remanded. Patrika reports that the obscene images were created with AI and that police said digital forensics found AI-generated images; the English reports describe them as morphed without naming the technology, so the AI attribution rests on one outlet.

Chennai: a former employee downloaded a senior woman colleague's photographs from their company's website, morphed them into obscene images with AI applications and circulated them to the company's staff through fake Instagram accounts from 4 January 2026; Tamil Nadu's cyber-crime wing traced and arrested her (reported 4 February 2026) and she was remanded in judicial custody

From 4 January 2026 several employees of a Chennai company received follow requests from unknown Instagram accounts that then circulated defamatory content, including obscene AI-morphed images of a senior woman employee. On her complaint the State Cyber Crime Investigation Centre of the Tamil Nadu police traced the digital footprints, social-media activity and IP logs to a former employee of the same company who had resigned in October 2025 and, according to police, acted out of resentment and previous enmity to threaten, harass and publicly humiliate the victim; she had taken the photographs from the company's official website and morphed them using various artificial-intelligence applications. She was arrested, produced before a magistrate and remanded in judicial custody; the police press release was reported on 4 and 5 February 2026 and the cyber-crime DGP issued a public warning about social-media misuse. The victim alleged the images were circulated to damage her reputation and cause mental distress.

Ahmedabad, Gujarat: a 27-year-old Delhi man was arrested for posting AI-morphed obscene images and videos of a young woman and her mother (more than 100 images found) on fake accounts in her name after she declined his friendship, while posing as the friend reporting the fake accounts for her

On 28 May 2026 the Ahmedabad Cyber Crime Branch announced the arrest in north-east Delhi of a 27-year-old man who posts religious discourse videos online. Police allege that after meeting a young Ahmedabad woman with a public social-media profile through religious conversations on Instagram and being refused friendship, he stalked her online from December 2025 to April 2026, created fake accounts in her name (three Instagram accounts and a YouTube channel at first; eight to ten across X, Facebook, Instagram and YouTube were found on his phone), downloaded photographs of her and her mother, used websites found by searching "AI remove clothes" to generate morphed obscene images and videos of the two women, and posted them with obscene captions to damage her reputation; investigators say more than 100 AI-morphed images of the complainant were found. Officers told ThePrint and ANI that she had confided in him about the accounts and that he told her to keep quiet and claimed to be reporting the accounts and getting hers reinstated. She complained to the Cyber Crime Police Station; an FIR was registered on 6 April 2026 under BNS Sections 78(2) and 356(2) and IT Act Sections 66(C) and 67, and the accused was traced through technical surveillance and arrested under the Branch's "Mission Cyber Rakshika" campaign. The complainant said she was mentally harassed and defamed; the DCP said the images also depicted her maternal aunt. Further investigation was under way; no charge sheet or court outcome was found.

Indore: a first-year college student complained in May 2026 that a man she met at her sister's wedding edited her photographs with AI into objectionable images after she refused to marry him, blackmailed her with them and, after approaching her fiancé (the reports differ on how), caused her engagement to be broken off; she had been contemplating suicide before confiding in her family (Navbharat Times, Amar Ujala, Patrika, 12-13 May 2026)

Indore police registered an FIR at Banganga police station on a first-year college student's complaint reported on 12 and 13 May 2026. She said she had met the accused, a young man from the Indore area, at her sister's wedding in 2024, where he took some photographs of her; he later pressed her to marry him and, when she refused, threatened to edit her photographs with AI into objectionable (Navbharat Times, Amar Ujala) or obscene (Patrika) images and post them online. Navbharat Times reports that he edited the pictures with AI, blackmailed her with them and, after her family arranged her engagement elsewhere, reached her fiancé and showed him the edited pictures, after which the families quarrelled and the engagement was broken; Patrika says he went to the fiancé's home and intimidated him into breaking the match; Amar Ujala says he also threatened to kill her and her family and sent edited pictures when her family tried to reason with him, and that when her engagement took place about two months earlier he went to her house and made false allegations about her character in front of the prospective fiancé, after which the engagement broke. She went into severe mental distress and, according to Amar Ujala, had resolved to take a suicidal step before telling her family, who took her to the police. The FIR was registered under the IT Act and sections on criminal intimidation and defamation; police were searching for the accused.

Bhopal, Madhya Pradesh: a 17-year-old Class 11 student died by hanging at home on 14 May 2026; her family told police that two young men had stalked her and blackmailed her with obscene images made from her social-media photographs with AI tools

On the night of Thursday 14 May 2026 a 17-year-old Class 11 student was found hanging in her room at the family home in Bhopal's Aishbagh area after sending her younger sister out on a pretext; relatives brought her down but she had died. Her family told the Times of India and Dainik Bhaskar that two young men (both from the Chhola area according to the Times of India; one from Chhola and one from her neighbourhood according to Dainik Bhaskar) had harassed her for weeks, stalked her to and from school, pressured her to meet them, and threatened to circulate obscene images of her that they had made with AI tools from photographs taken from her social-media accounts; the family said she had never met them in person; they also told Dainik Bhaskar that she had told her parents, that the men had misbehaved with her father when the family tried to reason with them, and that fear of social defamation had kept them from going to the police earlier. The Aishbagh police station in-charge said an unnatural-death case had been registered, no suicide note was found, the family's statements were being recorded and further action would follow; investigators were expected to examine her phone and social-media accounts. After the post-mortem on 15 May relatives took the body to the police station and demanded action against the accused. No arrest or later development was found in this review.

Jiangsu, China (Zhangjiagang court): a man used AI face-swap deep-synthesis to splice the faces of a person referred to as Yu and several others onto pornographic images, fabricated 'sexual' stories about them and posted the material repeatedly on overseas websites between April 2024 and January 2025; after two face-swapped nude images of Yu and Yu's ID were forwarded to Yu's work WeChat group with a fabricated claim of sex-for-money transactions, he was convicted of criminal defamation and sentenced to one year and three months (1 December 2025), upheld on appeal and listed by the Jiangsu High People's Court among its 2025 typical cases

According to the Jiangsu High People's Court's 2025 top-ten typical cases, published on 27 January 2026 and reported by The Paper, a man surnamed Wang used the deep-synthesis technology of AI software between April 2024 and January 2025 to splice the facial information of a person (pseudonym Yu) and several other people onto pornographic images, fabricated a large number of indecent images and 'sexual' topics about them, and repeatedly spread them in public posts on overseas websites. On 5 January 2025 he sent two AI face-swapped nude images bearing Yu's face and images of Yu's identity documents to a second man (dealt with separately) and fabricated the claim that he had had a sex-for-money transaction with Yu; although the second man said he would forward the material to Yu's work WeChat group, Wang did not clarify that it was false or effectively stop him, only deleting the contact. The second man forwarded the images to the WeChat group and claimed there that Yu sold sex, prompting extensive discussion inside and outside the group and, the court found, adverse effects on Yu's personal image, work and life, a serious lowering of Yu's social evaluation and a violation of Yu's dignity. The Zhangjiagang People's Court held that using deep-synthesis technology that ordinary people cannot easily identify to generate indecent images and fabricate sexual topics, spread repeatedly through information networks, was defamation of serious circumstances that seriously harmed social order; taking into account his voluntary surrender, it sentenced him on 1 December 2025 to one year and three months' imprisonment for defamation. The Suzhou Intermediate People's Court rejected his appeal. The High Court described the case as a typical example of punishing online defamation committed with AI.

Province of Catania, Sicily: a 58-year-old school assistant at a middle school was arrested for possessing a large quantity of child sexual abuse material; on his devices police found photographs of underage female students taken inside the school, some without their knowledge, altered with AI 'deep-nude' programs to show them naked beside his own image

Reports published on 14 March 2026, carrying a Postal Police account of an investigation coordinated by the Catania prosecutor's office, describe the arrest, in flagrante delicto, of a 58-year-old collaboratore scolastico (school assistant) at a secondary school (scuola media) in the province of Catania, on suspicion of possessing a large quantity of child sexual abuse material. The preliminary investigations judge validated the arrest and placed him under house arrest. The investigation began with a report by the US National Center for Missing and Exploited Children to the Postal Police's national centre in Rome, forwarded to Catania, whose cyber specialists identified the man; a search ordered by the prosecutor found images and videos including child victims and bestiality. Investigators also found on his devices digital photographs of underage female students taken inside the school where he worked, some without the girls' knowledge, which had been altered with artificial-intelligence programs found on the seized devices to make the minors appear naked ('deep-nude') and to place them beside the suspect's own image, as if photographed together. Analysis of the devices continued to establish whether the manipulated images were distributed online and whether there are further victims. The number of students depicted is not reported. São Paulo: a 37-year-old influencer used AI 'deepfake' tools to insert photographs of young women and adolescent girls of the Congregação Cristã do Brasil, taken from their social-media profiles without consent, into sexualised videos set inside churches; a 16-year-old and a 17-year-old filed police reports and the Civil Police investigate under the child-protection statute

A humorist and tyre repairer from Lençóis Paulista, São Paulo state, who posts on YouTube, Instagram, Facebook and TikTok as an impersonator of a television presenter, used AI tools to turn photographs that young women and adolescent girls of the Congregação Cristã do Brasil (CCB) had posted from inside churches into videos in which they appear dancing sensually beside women in short skirts, with the presenter inserted and the influencer commenting on their clothes. A 16-year-old congregant and her parents went to the 8ª Delegacia de Defesa da Mulher in São Paulo in February 2026; g1 made the case public on 22 April 2026 and Folha de S.Paulo confirmed the investigation. The girl told g1 the photograph had been taken in 2025 in front of the altar of a CCB church, that she was very shy, had not wanted to be exposed and was afraid the exposure would affect her social life, and that she had stopped taking photographs of herself; her family sued for moral damages. On 30 April 2026 the mother of a second girl, aged 17, registered a police report at the 4ª DDM Norte; g1 reported that the exposure had affected the girl psychologically. The São Paulo Civil Police investigate under article 241-C of the Estatuto da Criança e do Adolescente (simulating a sex or pornography scene with a minor by digital means) and for defamation of adult women in the videos; the inquiry was sent at the prosecutor's request to Lençóis Paulista, where the suspect lives, and the total number of people depicted was still being established. The influencer admitted making the videos with AI, said through his lawyer that they were satire and criticism of customs in a context of humour, said he did not know some of those depicted were minors, denied in his police statement linking the adolescent's image to sexualised content and said he had taken her for an adult, and posted an apology video (dated 5 April by g1's first report, which says it does not mention the deepfakes; described as posted after his police statement in g1's second report). The CCB said it had identified him, was taking internal measures and supports legal measures by the authorities. No later development was found on 2026-09-22.

Nagoya, Japan: a former elementary-school teacher was sentenced on 4 June 2026 to three and a half years, including for possessing generative-AI nude images made from school photos of two girls; reported as Japan's first conviction applying the child-pornography law to AI deepfakes

On 4 June 2026 the Nagoya District Court sentenced a 35-year-old former Nagoya municipal elementary-school teacher to three years and six months in prison. Prosecutors had sought six years. He was one of seven current and former teachers from five prefectures charged over a group chat in which members shared secretly filmed images of girls. His offences included secret filming and sharing, and, under the child-pornography law, possessing two generative-AI images showing two real girls apparently naked. According to the police and the judgment summary, he sent photos of the girls from the school where he taught to another member of the group, who made the images on an AI image-editing website. The teacher kept them on his phone, and one was shared in the chat. The judge found the images realistic enough to be mistaken for photographs of the girls. Jiji called it the first case in Japan applying the law to possession of AI-generated sexual deepfakes, and Kyodo called it apparently the first such court finding. A lawyer and a former judge argue the law was stretched.

Carrizo Springs, Texas: a man was arrested in May 2026 after investigators say he used AI tools to turn photos from South Texas school pages into nude images of about 30 children, among more than 900 AI-generated abuse images and videos

Texas DPS announced on 29 May 2026 that a 31-year-old man from Carrizo Springs, Dimmit County, had been arrested and booked into the county jail on 19 May 2026 on a charge of possession of computer-generated child pornography, a charge DPS says was made possible by Texas Senate Bill 20. According to DPS, FBI agents in Del Rio searched his home in April 2026 and seized devices (DPS says with consent; ABC13 says under a search warrant) holding more than 900 AI-generated sexual images and videos of prepubescent and pubescent children that he had created, as well as non-AI sexual images of children. Investigators say he took photos of children from publicly accessible pages affiliated with South Texas schools and used AI tools to make nude images of them, and that about 30 child victims had been identified. The FBI is coordinating victim services. The allegations are charges; no later court development was found.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
