AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Hanoi police open a criminal case after two graduation exam candidates photographed exam papers and had AI apps StudyX and Gemini solve them

Hanoi police said on 1 July 2025 that they had opened a criminal case for unintentionally disclosing state secrets after two candidates in Vietnam's national high school graduation exam on 26 and 27 June 2025 smuggled phones into exam rooms, photographed exam papers and used AI apps to solve them. According to the police account reported by Thanh Nien and VnExpress, one candidate uploaded part of the maths paper to the StudyX app but copied only two answers, and did the same in the chemistry and physics papers; the other photographed the maths, history and English papers and used Gemini, was caught by the exam board and was suspended from the exam. A photo of part of the maths paper had appeared on an AI maths-solving app before the exam ended, worrying candidates and parents about exam security. Police said students who did this would be disqualified, have their results annulled and could face administrative or criminal action. Neither candidate is named.

Vietnamese police open a criminal case after an exam candidate streamed his graduation exam paper to a friend who solved it with ChatGPT

On 26 June 2025, during the literature paper of Vietnam's national high school graduation exam in Lam Dong province, an invigilator caught an 18-year-old candidate using a hidden button camera and wireless earpiece and suspended him from the exam. According to the provincial police account reported by Bao Lam Dong and VnExpress, he had streamed the paper to a 17-year-old friend at an internet cafe, who photographed part of it, used the ChatGPT app to solve it and read the answers to him over a Messenger call. On 27 June 2025 the Lam Dong police security investigation agency opened a criminal case for intentionally disclosing and appropriating state secrets, because unreleased graduation exam papers are classified; police said initially that only the two young people were involved and that they had found no further sharing of the paper. Neither is named.

Bucharest prosecutors say deepfake adverts drew mostly older people to a call centre that sold them unauthorised supplements; six women agree plea deals

The Prosecutor's Office attached to the Bucharest Court of Appeal announced in October 2026 that six women had concluded plea agreements in a fraud case over a call centre that, during 2025, sold unauthorised food supplements by telephone. According to the prosecutors, potential victims, mostly older people, were drawn in by deepfake-type online adverts in which the products falsely appeared to be recommended by well-known journalists, doctors and artists; they left their contact details on the promoted site and were then called by operators posing as medical specialists. The agreements say the five women who worked as operators misled 50 people in total, causing losses of 88,600 lei, although the per-defendant figures in the same communique are larger (one operator alone is said to have contributed to misleading 50 people, and the other four 28, 16, 8 and 3 people) and the communique does not explain the difference. The agreed sentences range from nine months to one year and six months, with their application postponed for a two-year supervision period; the agreements were sent to four courts and the main investigation continues.

Central Poland: pupils reportedly used an AI app to make a nude image of a classmate under 15; prosecutors reopened the case after the data regulator intervened

In June 2025, according to OKO.press, a group of pupils at a primary school in central Poland took a classmate's photo from the internet, used an application that offers to "undress" people in uploaded photos to generate a nude image bearing her face, posted it on the school's Instagram profile and urged others to order more such images. Poland's Personal Data Protection Office (UODO) says the girl was under 15 and notified the police, but the police declined to open proceedings and the prosecutor's office upheld that decision, finding no prohibited act; Gazeta Prawna reports that they considered the AI-generated material not pornographic and the pupil unharmed. The UODO president appealed to the Prosecutor General, in January 2026 the authority announced that prosecutors had informed it they were taking up the case again, and in June 2026 it said its president had recently asked once more for the investigation to be resumed. The application is not named.

Full Fact finds AI deepfakes of UK public-health academics made to promote menopause supplements on TikTok; depicted professors object

In a December 2025 investigation, the fact-checker Full Fact reported that social media accounts were using AI-generated deepfakes of real doctors and academics, made from real footage, to promote supplements, with many links to the US company Wellness Nest. Professor David Taylor-Robinson of the University of Liverpool learned on 11 August 2025 that TikTok carried footage of his real conference talk altered so that a fake version of him discussed a supposed menopause symptom and recommended a Wellness Nest probiotic; The Guardian reported 14 such videos. TikTok at first found no violation, and removed the account after Full Fact contacted it on 24 September, calling the delay a moderating error. He said he became more and more irritated, that removal was a faff and that people who know him could have been taken in. Full Fact also found eight videos of Duncan Selbie, former head of Public Health England, who called them a complete fake. Wellness Nest denied running the accounts or using AI-generated content. The tool used is not identified.

Sweden: police say AI-generated videos of well-known doctors in social media ads led people to fraudulent sites and card fraud; doctors object

In June 2025 the Stockholm police region warned that since the end of May it had received several complaints from people defrauded through social media adverts with AI-generated videos depicting doctors, who appeared to give advice and recommend medicines or vitamins. According to the police, viewers who followed the adverts to fraudulent websites were induced to enter card details, after which a considerably larger sum was charged and the details were stolen. Dagens Nyheter reported that the doctors used included the former state epidemiologist Anders Tegnell and the professor Agnes Wold, who told the paper of being furious. In July 2025 the Public Health Agency of Sweden said fake Facebook films imitating its former state epidemiologist were advertising products for diabetes or asthma, that it had reported them to the police and had asked Meta to remove them; he said it was sad that fraudsters were exploiting vulnerable people. Dagens Nyheter, as relayed by Omni, reported that the investigations were closed because the offences were judged to have been committed abroad. The number of defrauded people and their losses are not stated, and the police do not say how the AI generation was established.

State Department cable says an impostor used AI voice and text messages to pose as Secretary of State Marco Rubio to foreign ministers and US officials

A State Department cable dated 3 July 2025, sent to all US embassies and consulates and reported by The Washington Post, AP and The Guardian, said an impostor posing as Secretary of State Marco Rubio had contacted at least five senior officials, including three foreign ministers and a US governor, by text, Signal and voicemail, with the aim of gaining access to information or accounts. According to AP, two senior officials and the cable described the attempts as using technology driven by artificial intelligence; the Guardian reports that the messages mimicked Rubio's voice and writing style. The department said it was investigating and advised all employees and foreign governments. One official told AP the attempts had been unsuccessful; the technical basis for the AI description was not disclosed.

Johannesburg High Court refers a company's lawyers to the Legal Practice Council over fictitious citations that junior counsel said likely came from an AI tool

In an urgent application by Northbound Processing against the South African Diamond and Precious Metals Regulator, Acting Judge DJ Smit of the Gauteng High Court, Johannesburg, found while preparing judgment that two cases cited in the applicant's heads of argument did not exist. According to ITWeb, which saw the court documents, junior counsel first said a wrong version had been filed, a corrected version was found to contain two further incorrect citations, and on the court's direct inquiry junior counsel acknowledged that the citations likely originated from an AI tool called Legal Genius, used under deadline pressure. The judge accepted the legal team's apologies and found no intent to deceive, but referred the conduct of the applicant's legal practitioners to the Legal Practice Council for investigation (June 2025). The applicant obtained the relief it sought.

Malaysia: Selangor police say a TikTok video falsely linking ex-police chief Acryl Sani to money from a 'Datuk' was AI-manipulated; he filed a report

Malay Mail, carrying a Bernama report of a Selangor police statement, reports that police were investigating a video that began circulating on TikTok on 20 June 2025. It showed an individual resembling former inspector-general of police Acryl Sani Abdullah Sani and purportedly linked him to receiving financial assistance from a person titled 'Datuk', with the money deposited into a bank account. Selangor police chief Hussein Omar Khan said the video was fake and had been manipulated using artificial intelligence, that the claims were untrue and baseless, and that the former police chief never issued any statement as depicted. Acryl Sani lodged a police report denying that the video contained any official statement from him. Police opened an investigation under Section 419 of the Penal Code and Section 233 of the Communications and Multimedia Act 1998. The statement does not say how police established that AI was used, and the tool is not named.

Police in Rohtak district, Haryana, registered a case under the Protection of Children from Sexual Offences Act and rape provisions, reported on 8 October 2026, after a 17-year-old girl from a village in the district told them that two men who delivered building material to her home had taken photographs of her, made obscene photographs and videos from them with AI, and used the threat of posting the material on social media to force her to meet them and rape her repeatedly, according to Dainik Bhaskar and Jagmarg News (both 8 October 2026). In the complainant's account relayed by the two outlets, the first assault followed an Instagram message in December 2025, further assaults took place in January and March 2026, and the most recent on 5 October 2026, after which she collapsed in a lane near her home and told her family, who called the police. The district Child Welfare Committee said she has been counselled. Pont-à-Mousson pensioner lost her savings to a fraudulent investment platform promoted by an ad reported as AI-generated likenesses of Macron and Arnault

A retired woman in her eighties living in Pont-à-Mousson (Meurthe-et-Moselle, France) responded, in summer 2025 according to Le Parisien and Capital, to an online advertisement in which AI-generated likenesses of Emmanuel Macron and Bernard Arnault promoted a lucrative investment platform. A supposed financial adviser then called her, she invested 5,000 euros, received several hundred euros of 'interest', and reinvested repeatedly until the advertisement and the site disappeared. L'Est Républicain (7 June 2026) and Capital put the loss at 10,000 euros; Le Parisien, citing ICI and L'Est Républicain, reports 25,000 euros, all her savings. The Pont-à-Mousson police station says an investigation was opened and that it receives three to five complaints a week for this kind of fraud. The account rests on the police station's warning as reported by the regional press, which describes the advertisement as AI-generated; the advertisement itself was not examined by the outlets and the quoted police statements do not mention AI.

Yale Law School scholar placed on leave and barred from campus after article on an AI-empowered news site linked the scholar to a sanctioned group

In March 2025 Yale placed a Yale Law School scholar, who was deputy director of a Yale Law School project, on administrative leave and barred the scholar from campus. The New York Times reports the decision came three days after a news site described as powered at least in part by artificial intelligence published a story on the scholar's connections to Samidoun, a group on a US sanctions list. Inside Higher Ed and Middle East Eye identify the site as Jewish Onliner. The scholar's lawyer says Yale's general counsel named the article as the trigger of the investigation. The scholar told the Times of not being a member of any organization that would violate US law, and the lawyer says the scholar is not a member of Samidoun. A Yale Law School representative told The National that placing an employee on temporary administrative leave while a review is conducted is the appropriate process and that the scholar's short-term position was due to expire the following month. The site says humans fact-check and that AI tools play a significant role in its work. The inspected sources do not show that AI produced the article.

Zacatecas, Mexico: students report AI-altered sexual images of hundreds of classmates at a secondary school

In November 2025 parents and students at a secondary school in the city of Zacatecas blocked a road and occupied the school after reporting that a classmate had altered photos of schoolmates with AI applications and posted them online. Parents estimated about 400 affected students. The state prosecutor's office opened an investigation for crimes against sexual intimacy and reported 46 complaints in an N+ report published 13 November, while saying it had not located the online page or obtained any image and had not determined whether AI or another application was used. The school director and a social worker were separated from their posts after parents said they minimized the reports. Claims about AI use and the number affected are unverified in the inspected reports.

Prosecutor: judge used ChatGPT citations in Oklahoma custody order later vacated

In November 2025 Stephens County Associate District Judge Lawrence Wheeler issued an order in a child paternity and custody case that denied one parent's request for a psychological evaluation of the other parent and reprimanded the requesting parent's attorney 'for stooping to such frivolous trial tactics'. The attorney challenged the order at the Oklahoma Supreme Court in February 2026, telling the justices that it relied on two cases that do not exist; the challenge was dismissed in March after Wheeler vacated the order, and he is no longer on the case. According to a 17 August 2026 letter from the Stephens County district attorney, Wheeler told the Oklahoma State Bureau of Investigation that he used ChatGPT for research and wrote the order himself, and that at least two citations in it produced by ChatGPT do not exist. The mother in the case told News 9 that learning the order's citations were allegedly fabricated was alarming because it concerned the custody of a child. The attorney general's office declined criminal prosecution; judicial discipline remains possible.

Former NT Police assistant commissioner says an AI-generated sexually explicit video made from her official police photograph circulated among colleagues

Janelle Tonkin, a former assistant commissioner of the Northern Territory Police Force and now president of the Australasian Council of Women and Policing, said in a statement issued on 23 September 2026, after criminal proceedings on the matter were finalised, that in December 2025 she became aware that an AI-generated, sexually explicit video had been created using her official NT Police photograph; ABC News reports that the video was allegedly shared among serving members of the force and that she had taken former colleagues to court over it. She called the experience 'extremely distressing, personally and professionally' and said having her professional portrait 'turned into pornography and circulated among colleagues is something no person should have to experience'. The outcome of the court proceedings is not reported. An NT Police spokesperson said the force does not comment on internal investigations and was legally constrained from commenting on matters before the court, but that the conduct she was subjected to 'was, and always will be, totally unacceptable'; the NT Police Association condemned the conduct. Ms Tonkin said the video sat within a broader set of experiences in the force that she intends to describe later.

Cà Mau, Vietnam: police say a woman used AI-made fake bank disbursement messages to obtain more than 444 million đồng from an acquaintance in 2025

Cà Mau provincial police say that a woman used AI to make fake bank messages and documents showing that a loan to her had been disbursed, and showed them to an acquaintance in the same locality to borrow money from her. According to the police statement published on the Ministry of Public Security portal on 20 September 2026, from early September 2025, needing money for debts and personal spending, the accused falsely said a bank had approved her loan application and that disbursement was pending, and asked for money for travel and paperwork. Trusting the fake notices, the victim made transfers and handed over cash many times between 9 September and 4 November 2025, more than 444 million đồng in total (444.145 million đồng according to the police newspaper Công an Nhân dân). She found that no loan existed only when she went to the bank to check. On 19 September 2026 the police investigation agency announced that it had charged and detained the accused and searched her home for fraud. The AI tool is not named and no message is reproduced. Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
