AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

La Rioja, Spain: Guardia Civil says a parent sent 9,980 euros after a call with an AI clone of their daughter's voice; the money was recovered

The Guardia Civil in La Rioja said on 25 August 2026 that it had solved a 'relative in trouble' fraud in which the criminals used artificial intelligence to clone the voice of the victim's daughter. According to the force's account as reported by the newspaper La Rioja and Radio Arnedo, the victim received WhatsApp messages from an unknown number by someone posing as the daughter and asking for urgent money; when the victim called that number to check, a voice that sounded like the daughter answered, and the victim transferred 9,980.25 euros. Investigators of the Guardia Civil's technology team established that the voice had been generated with AI tools. Urgent coordination with the banks froze the funds in the destination account and the full amount was returned to the victim; Radio Arnedo and ActualidadRiojaBaja report that the presumed perpetrator was identified and investigated.

New York basketball coach pleads guilty after deputies say he made AI-generated explicit images of females he knew, including a 14-year-old girl

The Onondaga County Sheriff's Office in New York arrested a basketball coach and trainer, charging him on 16 April 2026, after a person reported that he had AI-generated pornographic images of females he knew. According to the sheriff's office, the people depicted were aged 14 to 30, and detectives found on his device an AI-generated pornographic image of a 14-year-old girl made from a photo taken when she was 12; the person who reported it had seen that image. Detectives said only the image of the minor could be charged as a crime. The district attorney's office added 12 counts in May 2026. On 9 October 2026, according to syracuse.com, he pleaded guilty to all 12 counts of the indictment, including promoting and possessing a sexual performance by a child, on a judge's plea offer of six months in a correctional facility followed by 10 years' probation and sex-offender registration. Sentencing is set for 8 January, a date syracuse.com gives without a year. The AI tool is not identified, and no source reports whether the images were shared beyond the person who saw the minor's image.

Delhi Police and fact-checkers say a viral video of a Delhi Police DCP resigning over student protests was AI-generated; FIR filed in Maharashtra

In late July 2026 a 44-to-47-second video circulated on social media in which Delhi Police Deputy Commissioner Sumit Jha appeared to announce that he was resigning "immediately" because the government had pressured him to implicate students protesting with the Cockroach Janta Party (CJP) on false charges. The fact-checkers Alt News and BOOM traced the frames to a genuine Delhi Police video of 23 July in which the officer warned about Pakistan-based social media handles spreading misinformation. BOOM found a mismatch between the audio and his lip movements and said Deepfake O Meter tools flagged the clip as AI-generated. Alt News reported a 67% likelihood of AI generation from Detectvideo.AI. Delhi Police posted a fact check calling the video "completely fake and AI generated", and the Hindustan Times reports that the Press Information Bureau also said it was AI-generated. After a member of the BJP social media team saw the video in a post by a Congress leader on 31 July, Maharashtra Cyber Police registered an FIR against unknown persons for public mischief and cheating by personation. The officer is named because he is a senior public official depicted in his official role and the fact checks name him; no statement from him is reported.

Jersey: a TikTok account posing as a school posted AI-generated videos showing teachers in humiliating and sexualised scenes, reported to police

In February 2026 a TikTok account that used the name, badge and branding of a school in Jersey, and presented itself as "official", uploaded five videos depicting teachers at the school. The Jersey Evening Post (JEP) described the videos as AI-generated deepfakes made with a free app and reported that they showed teachers in humiliating, compromising and sexualised scenarios: in one, two male teachers dance beneath a caption insinuating sexual interest in young girls; a montage shows a teacher barking and eating from a dog bowl and teachers firing guns, while other clips ridicule the appearance of specific staff members. The JEP reported the videos were viewed nearly 20,000 times within days. Education Minister Rob Ward described the content as "deeply inappropriate", confirmed it had been reported to TikTok and the States of Jersey Police, and said his department was working with the school to support staff and students; he said it appeared to be an isolated incident. The account was removed from TikTok. No arrest, charge or identification of the account holder is reported, and no teacher is quoted.

Turkey: police say a ring used AI-made fake videos with investment promises to defraud 33 people of about 15 million lira; six suspects remanded

On 30 January 2026 two Turkish news agencies, Anadolu Agency (AA) and Ihlas News Agency (IHA), reported a police operation by the Yozgat anti-smuggling and organised-crime unit. According to the AA account, the unit began work after allegations on social media of fraud carried out with artificial intelligence, and established that 33 people had been defrauded of about 15 million lira through fake videos created with AI that used the names of some institutions and organisations, together with promises of investment returns. Nine suspects were detained in simultaneous operations centred on Yozgat and extending to Antalya, Hatay and Ankara; a court remanded six in custody and released three under judicial supervision. The reports do not describe what the videos showed, name the institutions whose names were used, identify the AI tool, or say when the fraud took place. No victim is interviewed and no suspect's response is reported.

Cyprus: consumers association says people misled by deepfake videos of politicians lost money in an investment scam, one case over €40,000

The Cyprus Consumers Association said in May 2025 that people in Cyprus who fell for an investment scam promoted by deepfake videos of politicians, including the President of the Republic, Averof Neofytou and Haris Georgiades, lost money: the scam typically began with a €250 investment, one person lost more than €40,000, and some victims reported investing their last available funds. The association's president told CyBC that victims who discovered the deception and demanded explanations received threats, and that 14 people reported their experiences with these scams in one day. The account comes from one interview relayed by in-cyprus.com; no individual victim speaks, no tool is named and the videos were not examined by the outlet.

Hirosaki police (Aomori, Japan) say a man in his 40s lost ¥720,000 to an investment fraud that reached him through an AI-generated celebrity video

Hirosaki police in Aomori Prefecture, Japan, announced on 8 October 2026 that a man in his 40s living in the prefecture had been defrauded of ¥720,000 through a video, generated with artificial intelligence, of a real celebrity recommending investment, as reported by the Too Nippo newspaper. According to the police account, in about mid-July he followed a TikTok investment advertisement that impersonated a celebrity into LINE and Signal groups, made an initial profit on recommended shares, and then, at the suggestion of a LINE account, installed an investment app and transferred ¥720,000 from a convenience-store ATM to a designated account. When the app bought shares beyond his holdings and demanded the shortfall, he asked an AI, which pointed out that it might be fraud, and he realised he had been deceived. The celebrity and the tool used to make the video are not named.

Weldborough, Tasmania: AI-written tour-website article invented hot springs; publican says travellers, including a group of 24 drivers, detoured to find them

An AI-generated article on the Tasmania Tours booking website described "Weldborough Hot Springs", a "secluded forest retreat" with pools "rich in therapeutic minerals", as one of the best hot-spring experiences in Tasmania. No such springs exist: the Weld River that runs through Weldborough, a small town in north-east Tasmania, is cold. ABC News reported on 22 January 2026 that travellers who read the article had been turning up at the town's pub, the Weldborough Hotel, to ask for directions, and the publican said she was receiving daily calls and visits and that a group of 24 drivers on a trip from the mainland had taken a detour two days earlier to find the springs. ABC News reported that the article was posted in July 2025, and The Independent, citing ABC News and CNN, reported that inquiries began with a few calls in September 2025. The operator, Australian Tours and Cruises, said its outsourced AI content had "completely messed up", that some posts had gone live by mistake, and that all AI-generated blog posts had been taken down. The reports describe the travellers only as a group; no individual traveller is interviewed, and the AI tool is not named. In The Independent's paraphrase, the publican also said the post brought "a bit of a tourism boon" for Weldborough.

UK Upper Tribunal: solicitors' ChatGPT-drafted grounds in an asylum appeal cited fake cases and invented quotes; two individuals self-report to the SRA

A published Upper Tribunal (Immigration and Asylum Chamber) decision issued on 21 September 2026 records that the grounds of appeal and skeleton argument in a Namibian national's protection appeal were produced by a paralegal at her then solicitors using ChatGPT. The tribunal recorded her counsel's acceptance that the grounds cited fake cases, quoted passages absent from those judgments and attributed to the First-tier judge sentences he never wrote, and itself called one purported quotation pure invention. The September 2025 error-of-law hearing was adjourned, and the appellant later withdrew three of her four original grounds. The tribunal allowed an amended version of her one surviving ground but found that it disclosed no error of law. It refused her permission to add two new grounds, holding that she should not be allowed to advance entirely new grounds, or replace grounds riddled with misrepresentations, so long after the time limit, and in the alternative found those grounds without merit. It directed the Home Office to file any wasted costs application against the firm. The decision states that two individuals have reported themselves to the Solicitors Regulation Authority; the Law Society Gazette reported the decision on 8 October 2026.

Greenwood, Indiana: police say a man used AI to remove a teenage girl's clothing from her photographs and a video; child exploitation charge, 30 Sept 2026

Greenwood, Indiana police say a man used artificial intelligence to remove clothing from photographs and a video of a teenage girl he knew, producing nude images of a real minor. According to court documents cited by Fox 59 and relayed by Hoodline, the girl contacted police in March after finding nude images of herself on the man's phone, which investigators say also held nude images of teenage girls and young adult women, including at least two underage girls the man knew personally. Court documents say the man first denied that any images were of underage girls, then said the images of the girl had been altered with AI, and told police he used the AI-generated images to teach the girls a lesson. He was charged with one count of child exploitation, a Level 5 felony, on 30 September 2026. The allegations are untested in court. The account rests on one chain: Fox 59's report of the court documents and the police account, relayed by Hoodline.

Freelance web designer says a longtime client cancelled a commissioned clinic website by text, saying Claude had built the whole site (first-person)

In a public post to r/antiai created on 7 October 2026, a person who describes years of graphic design and website work for one client writes that the client had asked them to build a separate website for a new clinic and had sent over the material, and that on the day of the post the client texted to say not to bother because Claude had built the entire website, asking the poster to review it instead. The poster says the same client had earlier moved its graphic design to AI-generated material that copied the poster's design direction, leaving them only the website work, and that they feel sick over the cancellation. In a reply the poster says they will probably take a part-time job as a backup while keeping their remaining clients. New Orleans: former school teacher charged with 60 counts of unlawful deepfakes; investigators say an AI platform turned social-media photos into sexual images

Louisiana's Attorney General announced on 2 April 2026 that a former New Orleans school teacher and coach, already in custody since 8 January 2026 on child sexual abuse material charges, had been rearrested on 60 counts of unlawful deepfakes. According to court records reported by Fox 8, investigators from the Louisiana Bureau of Investigation say the former teacher obtained photographs that account owners had posted on social media, placed them into an online artificial intelligence platform and obtained AI-generated sexual images of the females with their faces unchanged, including collages and images of the same females at various stages of life; the females appear to be from the New Orleans metro area. Prosecutors said the images were made from teenage girls' Instagram posts, while the school emailed parents that it did not know who the depicted girls were. A grand jury indicted the former teacher on 27 May 2026 on 102 counts including 60 counts of unlawful deepfakes of minors. The number and identities of the depicted people are not reported, no source dates the creation of the images, and the charges are untested in court.

Ahmedabad, Gujarat: police say AI videos made from a businessman's photo passed Aadhaar facial authentication and a Rs 25,000 loan was taken in his name

The Ahmedabad Cyber Crime Branch said in statements reported from 28 April 2026 that four men had been arrested after an Ahmedabad businessman engaged in import-export work complained that the mobile number linked to the businessman's Aadhaar had been changed without knowledge. ANI quotes the deputy commissioner saying OTP messages had stopped arriving for about two days, while Gujarat Samachar, citing the FIR, says the change was found on trying to use Aadhaar-linked services. Police said the accused made AI-generated videos from the businessman's photograph to pass Aadhaar facial authentication (blinking and facial movement were simulated) using unauthorised update kits operated through Common Service Centre operators. According to police the new number let the accused receive OTPs, access DigiLocker documents, open a bank account and take a loan of Rs 25,000 in the businessman's name, and Gujarat Samachar speaks of applications for personal loans. Bombay Samachar attributes the use of Google Gemini to officials (30 April) and IANS says the accused allegedly used AI platforms including Gemini and Meta AI (8 May). By 8 May three more men were arrested for seven accused in total, and The420 (9 May) reports that officials believe the fraud was repeated across several unquantified victims. Every source rests on police statements and no source dates the event. The businessman and the accused are not named.

Pont-à-Mousson pensioner lost her savings to a fraudulent investment platform promoted by an ad reported as AI-generated likenesses of Macron and Arnault

A retired woman in her eighties living in Pont-à-Mousson (Meurthe-et-Moselle, France) responded, in summer 2025 according to Le Parisien and Capital, to an online advertisement in which AI-generated likenesses of Emmanuel Macron and Bernard Arnault promoted a lucrative investment platform. A supposed financial adviser then called her, she invested 5,000 euros, received several hundred euros of 'interest', and reinvested repeatedly until the advertisement and the site disappeared. L'Est Républicain (7 June 2026) and Capital put the loss at 10,000 euros; Le Parisien, citing ICI and L'Est Républicain, reports 25,000 euros, all her savings. The Pont-à-Mousson police station says an investigation was opened and that it receives three to five complaints a week for this kind of fraud. The account rests on the police station's warning as reported by the regional press, which describes the advertisement as AI-generated; the advertisement itself was not examined by the outlets and the quoted police statements do not mention AI.

Shawnee, Oklahoma police say a man used an AI app to make child sexual abuse images from photos of local children, one of a neighbour; five felony counts filed

Police in Shawnee, Oklahoma, say a local man used an artificial-intelligence app to create child sexual abuse images from photographs of children in his community, and Pottawatomie County prosecutors have charged him with five felony counts of possessing and creating child pornography. According to News On 6 (28 September 2026), the Oklahoma State Bureau of Investigation's Internet Crimes Against Children task force was tipped through a national database in August, Shawnee police executed a search warrant, and police say they found more than 200 child sexual abuse images and videos on two phones, about 100 of which appeared to have been created through an AI app. Court documents cited by the outlet say police identified some of the young victims through selfies they had posted on social media, and investigators believe one photo used was of a neighbour, taken from the man's front porch. KFOR reported on 6 October 2026 that court documents say he admitted using an app called "Desire AI", that a juvenile's online selfie was turned into sexually explicit AI images, and that a photo he took of a mother and her young daughter was altered to make both appear naked. The account rests on the police and OSBI statements and the charging documents as reported; the total number of children depicted is not stated.

Dietitian says a patient who followed an AI-generated 1400 kcal meal plan for three months lost muscle and developed chronic constipation, Gazeta.pl reports

Gazeta.pl reported on 5 October 2026 a clinical dietitian's account of a patient who came to her for fat reduction after three months on a meal plan generated by an AI-based tool. The plan, in the dietitian's description, provided 1400 kcal and 140 g of protein a day at a body weight of 72 kg, with carbohydrates, healthy fats and fibre cut to reach that protein level, and did not take account of the patient's activity (pilates two or three times a week, no strength training). After three months the patient had lost 3.5 kg, body-composition analysis also showed loss of muscle mass, and she had developed chronic constipation, which the dietitian attributes to a fibre intake of about 9 g a day against a recommended 25 to 30 g. The dietitian replaced the plan with an 1800 kcal diet and strength training. The tool is not named and the event date and place are not stated.

Tasmania parole media ban quashed after state conceded reliance on likely AI-hallucinated case law

In December 2024 the Parole Board of Tasmania added a condition to the parole of Susan Neill-Fraser, released in 2022 after serving 13 years for murder and who maintains her innocence, barring her from communicating with media to claim innocence or wrongful conviction; a broader condition replaced it in May 2025. On 15 September 2026 the Supreme Court of Tasmania declared the conditions invalid. Her counsel told the court that the board's secretary had used artificial intelligence to prepare a rationale document justifying the condition and that it cited case law that did not exist. The Human Rights Law Centre, which represented her, says the State conceded that the board's decision-making was affected by a document containing legal authorities that were likely AI-generated hallucinations, and the ABC reports the court found the board relied on that document. The board had withdrawn the condition in August 2026. Tasmania's Attorney-General announced an independent investigation of the board's professional standards over its use of unverified AI, and the Justice Department announced a review of AI use in past parole decisions.

Man who used AI to make fake nude images of female relatives, alongside secret shower recordings, jailed 27 months, outlet reports

Stomp reported on 4 October 2026, citing Shin Min Daily News, that a man was sentenced to 27 months’ jail after pleading guilty to four of ten charges involving the taking, distribution and possession of intimate images. According to the report, he secretly filmed female family members showering from 2024 and shared images with other men, and investigations also found that he used AI to create fake nude images of female relatives. He shared some of the AI images with a male cousin, who was also involved in creating and distributing them. AI-generated nude images and secret recordings were found on another phone he had borrowed after his release on bail, and a family member made a further police report. One female relative said the incident left her depressed and unable to eat or sleep properly. The court barred the media from publishing information that could identify the victims, including the man’s name.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
