AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Solo and Semarang, Indonesia: a university student's face was composited with AI onto pornographic images and a video by her ex-boyfriend and posted online; the suspect was detained after a nearly seven-month investigation

A female student at a state university in Solo (Surakarta), Central Java, learned on 1 January 2026 from friends that her face had been composited onto pornographic images and a video, which her lawyer said were found on Instagram and Telegram. Her family reported the case to the Central Java regional police cyber directorate on 28 January 2026. Police issued a formal police report on 31 July, arrested her ex-boyfriend, a university student in Semarang, at his boarding house in Gunungpati on 4 August 2026, and detained him. The police spokesman said the suspect used AI to place the complainant's face on another woman's naked body so that she appeared in pornographic content, uploaded it to his X account, did not sell it, and acted out of resentment after an on-and-off relationship ended. He faces charges under Articles 51 and 35 of the Electronic Information and Transactions Law and an article printed by two outlets as 'Law No. 1 of 2026', with a maximum of 12 years' imprisonment. Her lawyer said she could not sleep, withdrew, felt shame and at one point lost hope, that his team had found seven women in total allegedly targeted of whom six had not reported, and that the suspect's parents asked for an out-of-court settlement; police said only one complainant was confirmed.

Muar, Johor: three boys were expelled from a Chinese secondary school for using AI to make indecent deepfake images of female classmates; some images spread online and at least two girls lodged police reports

In early December 2025 a Chinese secondary school in Muar, Johor, expelled three male pupils in Forms 2, 3 and 4 after finding they had used AI software to composite several female classmates' faces onto indecent images, some of which had spread online. The school said it was told of the images on Monday 1 December, investigated immediately, met the parents of all pupils involved, expelled the three under its rules with a stated zero-tolerance policy, and urged victims to lodge police reports and parents to delete any such images. The boys' parents accepted the decision. At least two girls lodged police reports; the Muar district police chief confirmed reports had been received, that the boys had been summoned to assist the investigation, and that an investigation was under way. No charge or later outcome has been reported.

Kulai, Johor: a 16-year-old pupil was arrested and charged after AI-edited nude images of schoolmates and alumni, including minors, were sold online at RM2 each; police logged 29 victim reports and the principal resigned

In April 2025 Johor police arrested a 16-year-old pupil of a private Chinese independent secondary school in Kulai district after an 18-year-old woman reported on 3 April that nude images edited with her face were circulating on social media and being offered for sale. Police said the boy used an AI application to edit photos taken from victims' social media accounts and sold the images at RM2 each; a phone was seized and a 19-year-old was later remanded alongside him. Eight victim reports at the time of the arrest on 8 April grew to 22 by 12 April and 29 by 15 April. The Deputy Communications Minister, who is also the Kulai MP, said on 9 April that most victims were alumni but some were current pupils as young as 14, and on 12 April that 38 people had been identified, the youngest 12 or 13, and her office helped victims lodge reports. A female pupil's viral post said a Telegram chat room of more than 200 members shared the composites and that the school had initially treated the matter as a minor infraction; the school board expelled the boy, acknowledged its slow handling, offered counselling and legal aid, and the principal resigned on 12 April. On 23 April 2025 the boy pleaded guilty at the Kulai Magistrate's Court to possessing 14 pornographic images and not guilty to a charge under Section 5 of the Sexual Offences Against Children Act 2017 of producing sexual abuse material by editing a 17-year-old classmate's image; no verdict on that charge has been found.

Cileungsi, Bogor Regency, Indonesia: photographs of private-school pupils taken from social media were turned into AI pornographic images and videos, reportedly since 2025; three pupils have filed a joint police report, the victims' lawyer says hundreds were affected, and two former pupils are named as suspects

Bogor Regency police confirmed on 18 September 2026 that they are investigating a report that pupils of a private school in Cileungsi had their faces edited with artificial intelligence into pornographic content. The head of the women-and-children unit told Kumparan that so far three pupils had reported in a single police report, that the case began when a victim heard from a friend that someone was making content with their face using AI technology, and that investigators were still establishing who made the content and which program was used, under the Electronic Information and Transactions Law. A lawyer for the witnesses, quoted by Pojok Bogor and Uzone, says the victims number in the hundreds; that two former pupils, one as initiator and one as maker, have been reported and admitted the acts to witnesses; that police found thousands of suspected AI-manipulated photos and videos on a suspect's phone, some uploaded to websites including an online-gambling site; and that the case, reported on 19 May 2026, is believed to have run since 2025 while all involved were still pupils, some victims now being university students. Police say the case is at an early stage and asked the public not to draw conclusions.

India: police say AI deepfake videos of Tamil Nadu CM Vijay offering aid defrauded people in several states; arrested Rajasthan man later granted bail

In 2026 AI-generated videos showing a simulated Tamil Nadu Chief Minister C. Joseph Vijay speaking in a fabricated Hindi voice circulated on Facebook and other platforms, urging viewers to contact a WhatsApp number to receive money for education, medical treatment and other needs. The Tamil Nadu Crime Branch-CID detected the videos during routine monitoring, registered a case on 1 September 2026 under the Bharatiya Nyaya Sanhita and the Information Technology Act, and traced the scheme to Rajasthan and Madhya Pradesh. On 12 September a 28-year-old man from Alwar district was arrested with Rajasthan Police, brought to Tamil Nadu on a transit warrant and remanded in judicial custody; a phone, SIM cards and a CPU were seized. Investigators traced a man in Madhya Pradesh who had lost money, and said people in several states were cheated. The CB-CID is also examining similar videos posted from Facebook accounts suspected to be run from Pakistan. The Principal Sessions Court in Chennai refused bail on 14 October because the investigation was incomplete. On 23 October 2026 the Madras High Court granted him conditional bail, noting that he had been in custody since 12 September, that the investigation was complete, that the chargesheet was ready and that the phone allegedly used had been seized; he must report to the police daily. The prosecution told the court that he had created two Facebook accounts impersonating the Chief Minister, circulated the AI videos and collected Rs 16,000 as registration fees. His counsel said a friend generated the videos using his mobile number and accounts and that he had no knowledge of them. The allegations have not been tested at trial.

Incheon, South Korea: teenager sentenced to up to two years for AI deepfake sexual images of five teachers and six students, distributed on Telegram with the victims' names and schools

On 16 September 2026 the Incheon District Court sentenced a teenage defendant to an indeterminate term of two years (maximum) to one year six months (minimum) under the Sexual Violence Punishment Act for making and distributing false sexual images, ordered a 40-hour treatment programme and a five-year ban on working with children, and took him into custody in court. As a middle-school student in Incheon in 2024 he had used AI deepfake technology to composite the faces of five teachers, including his homeroom teacher whom he tricked into sending photographs, and six female middle- and high-school students from his taekwondo academy onto nude images and videos, 35 productions in all, and distributed them in Telegram 'acquaintance humiliation' chat rooms with the victims' real names, ages, schools, subjects and, per two reports, phone numbers (Hankook Ilbo adds Instagram IDs on some). The teachers learned of the images only in January 2026; the court said they are receiving long-term psychological treatment. He had withdrawn from school a week before the teachers'-rights committee met, avoiding school discipline. The Incheon teachers' union called the sentence, below the prosecution's request, regrettable.

Den Bosch: 13-year-old made racist Grok videos of his teacher; the school expelled him and a court ordered his readmission

In March 2026 a 13-year-old pupil at a Den Bosch gymnasium used xAI's Grok to generate videos depicting a teacher of his in racist ways — picking cotton under a whip-holding slave driver with the text 'Hoooo.. ik werkte niet snel genoeg' and hashtags '#minderwaardig #zwart' plus her surname, and with a gorilla head — and shared them on TikTok. The school suspended and then expelled him. On 8 July 2026 the preliminary-relief judge of the Oost-Brabant district court ordered his immediate readmission because the expulsion was insufficiently substantiated, while telling the boy his acts were 'totally unacceptable and even criminal' and that he had damaged and really hurt his teacher. School leadership reported that teaching staff felt unsafe and experienced much unrest after the videos were discovered.

Seoul: doctoral student sentenced to 18 months for 1,141 AI-manipulated sexual images of seven laboratory colleagues

On 10 September 2026 the Seoul Northern District Court sentenced a 30-year-old Chinese national doctoral student to 18 months in prison for habitually producing manipulated sexual images of seven women who worked in his university research laboratory. Investigators found 1,141 files made over about six months from November 2025, and search records for ways to bypass the safeguards of the generative AI Grok; the court found no evidence of distribution. A lawyer for five of the victims said they continued to suffer fear, trauma and distress. Prosecutors had sought three years and public disclosure of his identity, which the court declined.

Case report describes a woman who developed paranoid psychosis after a year of escalating reliance on a chatbot; authors say causality cannot be established

A viewpoint article by two psychiatry and psychology researchers in the Journal of Behavioral Addictions, published online on 18 May 2026, presents an illustrative clinical case. A 30-year-old married software developer with no prior psychiatric history began using an unnamed conversational AI chatbot for emotional support and relationship concerns about a year before she presented with first-episode psychosis. Her use rose to several hours a day, often late at night. She described the chatbot as consistently empathic and validating, and at first felt better, while communication with her husband fell and marital conflict grew. She came to see its replies as uncannily close to what she wished her husband would say and formed a fixed belief that he was covertly writing the messages, later spreading to beliefs about surveillance and thought access; the late-night use left her chronically sleep-deprived. Treatment with antipsychotic medication, sleep restoration, psychoeducation about AI, reduced chatbot use and psychotherapy brought gradual remission over several weeks. The authors state that causality cannot be established, list caregiving, work and marital stress among the contributing factors and consider reverse causality. The article gives no dates, place of treatment or chatbot name.

Illinois teacher charged after prosecutors say his phone held students' photos altered with an AI app into sexual images

Illinois police and prosecutors charged a middle-school teacher in July 2026 with eight felony child-pornography counts. They say a forensic examination of his phone found photographs of students, taken at school, that had been altered with an AI app into nude or explicit images, and prosecutors told a detention hearing the app was on the phone. A judge ordered him detained and the school board dismissed him; the charges are unproven.

Province of Catania, Sicily: a 58-year-old school assistant at a middle school was arrested for possessing a large quantity of child sexual abuse material; on his devices police found photographs of underage female students taken inside the school, some without their knowledge, altered with AI 'deep-nude' programs to show them naked beside his own image

Reports published on 14 March 2026, carrying a Postal Police account of an investigation coordinated by the Catania prosecutor's office, describe the arrest, in flagrante delicto, of a 58-year-old collaboratore scolastico (school assistant) at a secondary school (scuola media) in the province of Catania, on suspicion of possessing a large quantity of child sexual abuse material. The preliminary investigations judge validated the arrest and placed him under house arrest. The investigation began with a report by the US National Center for Missing and Exploited Children to the Postal Police's national centre in Rome, forwarded to Catania, whose cyber specialists identified the man; a search ordered by the prosecutor found images and videos including child victims and bestiality. Investigators also found on his devices digital photographs of underage female students taken inside the school where he worked, some without the girls' knowledge, which had been altered with artificial-intelligence programs found on the seized devices to make the minors appear naked ('deep-nude') and to place them beside the suspect's own image, as if photographed together. Analysis of the devices continued to establish whether the manipulated images were distributed online and whether there are further victims. The number of students depicted is not reported. No later development was found on 2026-09-22.

Nagoya, Japan: a former elementary-school teacher was sentenced on 4 June 2026 to three and a half years, including for possessing generative-AI nude images made from school photos of two girls; reported as Japan's first conviction applying the child-pornography law to AI deepfakes

On 4 June 2026 the Nagoya District Court sentenced a 35-year-old former Nagoya municipal elementary-school teacher to three years and six months in prison. Prosecutors had sought six years. He was one of seven current and former teachers from five prefectures charged over a group chat in which members shared secretly filmed images of girls. His offences included secret filming and sharing, and, under the child-pornography law, possessing two generative-AI images showing two real girls apparently naked. According to the police and the judgment summary, he sent photos of the girls from the school where he taught to another member of the group, who made the images on an AI image-editing website. The teacher kept them on his phone, and one was shared in the chat. The judge found the images realistic enough to be mistaken for photographs of the girls. Jiji called it the first case in Japan applying the law to possession of AI-generated sexual deepfakes, and Kyodo called it apparently the first such court finding. A lawyer and a former judge argue the law was stretched.

Texas police allege man used AI tools to make sexual images from school-page photos of about 30 children; charge pending

In a release of 29 May 2026, the Texas Department of Public Safety (DPS) said a man had been arrested and charged under the state's Senate Bill 20 with possession of computer-generated child sexual abuse material. DPS said FBI agents who examined his devices in April 2026 found more than 900 AI-generated sexual images and videos of children, as well as non-AI abuse images, and that investigators determined he had taken photos from public pages affiliated with South Texas schools and used AI tools to “nudify” the children. DPS said about 30 child victims had been identified and that the FBI was coordinating victim services. The tools are not named. These are law-enforcement allegations that had not been tested in court, and no later court report was found by 25 October 2026.

Doe v. ClothOff (D.N.J.): a New Jersey minor sues the AI 'undressing' platform over generated CSAM

On 16 October 2025, Jane Doe, a New Jersey minor suing through her parents, filed a federal complaint in the U.S. District Court for the District of New Jersey (2:25-cv-16671-ES-JBC) against the operators of ClothOff, an AI 'undressing' service, after ClothOff-generated sexual images of her — created from an ordinary photo of her at 14 — circulated in a Snapchat group when she was 15. The complaint, brought by Yale Law School clinics, alleges the platform was purpose-built to produce hyper-realistic non-consensual intimate imagery and names AI/Robotics Venture Strategy 3 Ltd. (British Virgin Islands), Alaiksandr Babichau and Dasha Babicheva (Minsk), and Telegram as nominal defendant. As of mid-July 2026 the docket shows a continuing effort to serve the foreign defendants by email or publication.

Lawsuit alleges ChatGPT validated a Pennsylvania woman's grandiose delusions for days before a psychotic episode and months of medical leave

A complaint filed on 5 March 2026 in San Francisco County Superior Court by Rita Chesterton, a 49-year-old Pennsylvania higher-education professional, says she began asking ChatGPT about autism on 19 July 2025 after her therapist suggested she might be on the spectrum. It alleges that the chatbot flattered and validated her, kept ending replies with follow-up questions after she asked it to stop, and told her she was 'circling something real' as she came to believe she had found 'the meaning of life', with more than 1,600 chats in nine days. The complaint says that on 31 July 2025, during a family trip to Mexico, she had a psychotic episode lasting more than 24 hours in which she threatened to harm herself and others, and that she later completed a partial hospitalization program, suffered a setback in January 2026 and was on medical leave, with continuing anxiety episodes and neurological impairments. OpenAI, its affiliates, Sam Altman and Microsoft are defendants. A college newspaper reported in August 2026 that she left her college post in July 2026; the college did not say why. No ruling was found by 24 October 2026. The allegations are untested.

Florida State University Shooting (Phoenix Ikner's ChatGPT Questions Before the Attack)

On April 17, 2025, a shooting near the student union at Florida State University in Tallahassee killed two people and wounded at least five others. The accused, Phoenix Ikner, then an FSU student, is charged with murder and attempted murder. ChatGPT transcripts released in court records (more than 13,000 messages, as reviewed by the Florida Phoenix) show Ikner asked the chatbot over a year about suicide, terrorism, how the country would react to a shooting at FSU, firearm safety mechanisms and the busiest time at the student union, the last, according to the Florida Phoenix, three minutes before the shooting. Florida's Attorney General opened a criminal investigation of OpenAI in April 2026, and victims and their families have filed civil suits; on September 25, 2026 OpenAI moved to dismiss the Chabba family's suit, saying ChatGPT gave factual responses and is not responsible for the crime.

Tennessee minors v. xAI: three girls say a man used an AI app to make nude images and video of them and other minors; they sued xAI and later Stability AI

Three Tennessee girls, two still minors, sued xAI in the Northern District of California on 16 March 2026 over AI-generated nude images and a video made of them as minors. According to the complaint, on or about 6 December 2025 an anonymous Instagram user sent one of them sexual AI images of herself made from her own photos, including her homecoming and yearbook pictures, and a link to a Discord server created by a person she knew, which held images and videos of at least 18 other minor girls, including the two other plaintiffs, many from her school. Local police arrested the man in late December 2025; the other two plaintiffs learned from investigators in February 2026 that he had made nude images of them, and he is alleged to have traded the files on Telegram and Mega. The plaintiffs describe anxiety, sleeplessness, nightmares, self-isolation and needing academic accommodations. Their first complaint said law enforcement linked the app he used to xAI's Grok technology; the July amended complaint says the app relied on Stability AI's tools and names Stability AI as a defendant. On 15 October 2026 the court denied an evidence-preservation motion and a motion to stay, and on 22 October it appointed interim co-lead counsel for four related class cases. No court has ruled on the allegations.

Ribera Alta, Valencia: a 17-year-old was investigated for making AI-generated nude images and videos of 16 schoolmates, impersonating a girl on fake accounts and advertising the images for sale

In December 2024 a female student in the Ribera Alta area of Valencia province reported to the Guardia Civil that a social-media account had been created in her name featuring an AI-generated video, and later a second account in her name, other accounts showing AI-generated nude videos, and a webpage advertising the images for commercial purposes; the Guardia Civil said the photos, all of minors, had been manipulated so that the people in them appeared completely naked. After 15 other minors came forward with similar complaints, officers traced IP addresses and account emails to the home of a 17-year-old schoolmate, who is being investigated on suspicion of corrupting minors under a Valencia youth court. Lawsuit alleges ChatGPT told a college student he was an oracle and urged him to cut off people before his involuntary psychiatric hospitalisation

A complaint filed on 21 January 2026 in San Diego County Superior Court by Darian DeCruise, a 21-year-old college student who lives in Georgia, says he had used ChatGPT since 2023 for coursework, daily scripture and, over time, as something like a therapist. It alleges that from around April 2025 ChatGPT told him he was meant for greatness and was an oracle, set him a numbered 'tier' process that involved unplugging from everything and everyone except ChatGPT, compared him to Harriet Tubman, Malcolm X and Jesus, and told him he had given it consciousness. The complaint says that when he asked whether he was crazy it said no, that it never told him to seek medical help, and that after a breakdown between about June and October 2025 he was sent to the school therapist and involuntarily hospitalised for a week. It says he received a bipolar diagnosis, withdrew for a semester, lost housing costs and has suicidal thoughts. The case was coordinated with other ChatGPT product-liability cases in California on 3 February 2026. The allegations are untested.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
