AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Grok reportedly disclosed an adult performer's legal name and birthdate in an X reply, followed by reported impersonation accounts and leak-site posts under the legal name

In early 2026 (404 Media and the performer's 19 February post say early February, while Stern reports she found the reply weeks after it appeared) an X user replied to a clip of adult performer Siri Dahl, asking who the performer was and what her name was, and tagged Grok. According to 404 Media, Grok answered with her stage name, her birthdate and her legal name, and the user likely wanted only to know which performer appeared in the clip. Dahl has used the stage name since 2012 according to 404 Media, and she says she had paid for data removal services for at least six years to keep the legal name private. She reports that impersonating Facebook accounts and leak-site posts under the legal name then appeared and that the name spread across hundreds of websites. 404 Media reports that users asked Grok for the make and model of her car and her address without an accurate reply, and that she is calling family members to put defensive plans in place. Grok's reply to her protest said the details were already public, which she denies. Where Grok obtained the name is unknown. Dahl spoke publicly about the event and asked 404 Media to publish her legal name. This record omits the legal name and birthdate.

India: Bengaluru-based employer says he fired his cook after a home AI kitchen monitor flagged fruit taken

On 1 March 2026 a Bengaluru-based technology professional posted on X that he had deployed a home "AI roommate" (a kitchen camera with an AI vision model, which he said was Claude Haiku 4.5) that monitored his cook while she cooked, alerted him when she took anything and sent weekly reports. The post said the system "caught her red handed", that he "caught her twice this week" and that he had "just fired" her. Screenshots quoted by Hindustan Times show the bot listing apples and a banana as gone, reporting the cook eating blueberries, and giving hand-washing and cleaning observations. All coverage derives from the employer's post: the post carries no video, the cook's account is not reported and the alleged taking is unverified.

8 Jan 2026United KingdomUnidentified facial recognition system

Southampton: Alvi Choudhury arrested at home on suspicion of a Milton Keynes burglary after a police retrospective facial recognition match, held nearly 10 hours, claiming damages

The Guardian, in a joint report with Liberty Investigates, reported on 25 February 2026 that Alvi Choudhury, a 26-year-old software engineer, was arrested at his home in Southampton in January 2026 on suspicion of a £3,000 burglary in Milton Keynes, about 100 miles away, and held in custody for nearly 10 hours. Thames Valley Police had used automated retrospective facial recognition software, which matched him with CCTV footage of the burglary suspect. Choudhury says the man in the footage looked about 10 years younger and had different features. Thames Valley Police wrote to him that the arrest may have been the result of bias within facial recognition technology, and told the Guardian that the decision rested on the investigating officers’ own visual assessment and was not influenced by racial profiling. Choudhury is claiming damages from Thames Valley Police and Hampshire Constabulary.

Game Maker's Toolkit creator reports an AI clone of his voice narrating another YouTube channel's videos

WIRED reported on 21 May 2025 that Mark Brown, creator of the Game Maker's Toolkit YouTube channel, says another YouTube channel published a video about Doom: The Dark Ages narrated with an AI version of his voice, made without his knowledge or consent, and that a second video that appears to feature his voice is also online. Brown filed a privacy complaint with YouTube. Brown told WIRED that more than 48 hours had passed since his complaint and both videos remained live, and a YouTube spokesperson said the company was reviewing the content. Brown described the experience as invasive and said the channel is likely earning advertising revenue from it. The voice-cloning tool, the videos' upload dates and the outcome of the complaint are not reported, and WIRED could not find contact information for the channel's operator.

Australian campaigner says harassers sent and posted AI-generated violent and sexual images of her with threats during 2025

Caitlin Roper, campaigns manager at the Australian group Collective Shout, told the New York Times (31 October 2025), Mamamia and AAP that from what the Times calls the summer of 2025 people sent and posted images and a video of her that she describes as AI-generated deepfakes, including violent images and images she says were undressed with Grok, X's AI feature, together with threats. The Times captioned them as AI-generated images; its article says the violent images were seemingly enabled by generative AI and that some harassers claimed to have used Grok to create images. Roper says the abuse was deeply traumatic, that for months she spent at least an hour each morning reporting, blocking and documenting it, and that police and the eSafety Commission gave little help. The Times reports that X told her some posts depicting her violent death did not breach its rules and temporarily locked her account after she posted examples. The senders are not identified, and no forensic examination of the images is reported.

Albanian actress Anila Bisha disputes government use of her image and voice for AI minister Diella

Albania's government presented an AI-generated public-procurement "minister" named Diella in September 2025. Actress Anila Bisha says the avatar's face and voice are hers. She says her contract covered use as a digital assistant on the e-Albania services portal and that she was never asked about the ministerial role. In February 2026 she asked the Tirana Administrative Court of First Instance for an interim measure to stop the use of her image and voice, and her representatives said they plan a formal lawsuit for damages. The government's media agency called the lawsuit nonsense. Anadolu Agency reported on 12 February 2026 that the court had not ruled on the request.

Montana prosecutors charge a Carter County man in a case involving AI-altered images of a Montana child

On 21 August 2025 the Montana Attorney General's Office charged a Carter County man with three felony counts of sexual abuse of children for possessing child sexual abuse material. Prosecutors state that in at least one count an AI tool was used to alter images of a Montana child that were taken from the child's parents' public social media account. The investigation began with cyber tips about uploads to a cloud backup. The charges are allegations. The child and the accused are not named here. The only reported harmed person is the depicted child and the sources report no other consequence, so the harm evidence is weak: it rests on the prosecutors' description of the charged conduct. The date of the conduct is not reported.

1 Jul 2025 to 17 Dec 2025United StatesEightfold Match Score

US: two applicants sue Eightfold AI over 0-to-5 applicant scores used in hiring

Two job applicants filed a proposed class action on 20 January 2026 in California state court (removed to federal court on 2 March 2026) against Eightfold AI Inc. The complaint alleges that Eightfold's hiring tools collect applicant data and produce a 0-to-5 'Match Score' that employers use to rank candidates, without the notice, access and dispute rights the Fair Credit Reporting Act and a California statute require. One plaintiff says she applied to Microsoft in or around July 2025 and again in December 2025 and received an automated rejection two days after the first application. The other says she applied to PayPal in December 2025. Both say they were not interviewed or hired and believe Eightfold's tools played a role. Eightfold says it does not scrape social media and operates on data candidates or customers provide. The allegations are unproven and Eightfold filed a motion to dismiss (no ruling appears in the docket entries inspected).

Zacatecas, Mexico: students report AI-altered sexual images of hundreds of classmates at a secondary school

In November 2025 parents and students at a secondary school in the city of Zacatecas blocked a road and occupied the school after reporting that a classmate had altered photos of schoolmates with AI applications and posted them online. Parents estimated about 400 affected students. The state prosecutor's office opened an investigation for crimes against sexual intimacy and reported 46 complaints in an N+ report published 13 November, while saying it had not located the online page or obtained any image and had not determined whether AI or another application was used. The school director and a social worker were separated from their posts after parents said they minimized the reports. Claims about AI use and the number affected are unverified in the inspected reports.

NSW Reconstruction Authority says a former temporary staff member uploaded 2031 Resilient Homes participants' personal data to ChatGPT without authorisation

The NSW Reconstruction Authority says that between 12 and 15 March 2025 a former temporary staff member (called a former contractor in its first notice) uploaded a Microsoft Excel spreadsheet of Northern Rivers Resilient Homes Program information, 10 columns and more than 12,000 rows, to ChatGPT, an AI platform the Authority had not authorised. The Authority disclosed the breach on 6 October 2025, estimating that up to 3000 people were affected, and later said external forensic analysis confirmed that 2031 people had some of their data uploaded, including names, contact details, addresses, dates of birth, sensitive personal and health information and limited financial commentary. It says no driver licence, Medicare, passport or Tax File Numbers were disclosed and that there is no evidence the data is publicly available or was accessed by a third party; whether OpenAI retained or used the data is not addressed. The Authority apologised, contacted affected people with ID Support NSW, offered to compensate the replacement of compromised identity documents and notified the NSW Privacy Commissioner. A program participant told the ABC the information was deeply personal and that they felt concerned. No misuse of the data is reported.

S.D.N.Y.: Judge Rakoff rules that a fraud defendant's written exchanges with Claude about his defence, seized by the FBI, are protected by neither attorney-client privilege nor work product

Bradley Heppner, a corporate executive charged in the Southern District of New York with securities fraud, wire fraud and related offences, used Anthropic's Claude in 2025, after receiving a grand jury subpoena, to prepare reports outlining his defence strategy. His counsel had not directed him to do so. FBI agents seized about thirty-one documents memorialising those exchanges when they searched his home in connection with his arrest in November 2025. Heppner claimed privilege over them. On 10 February 2026 Judge Jed S. Rakoff granted from the bench the Government's motion for a ruling that the documents were not protected from Government inspection by either the attorney-client privilege or the work product doctrine, and a memorandum filed 17 February 2026 gave the reasons: Claude is not an attorney, and the communications were not confidential under Anthropic's privacy policy. The court described the question as one of first impression nationwide. Heppner was convicted in May 2026 of securities fraud, wire fraud and other charges; no inspected source says whether the Claude documents were used at trial.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
