AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Krefeld police say an older man lost a four-figure sum to a dating-app contact whose photos, by first findings, were AI-generated

Krefeld police reported on 29 April 2026 that unknown fraudsters obtained a four-figure sum from an older man through a dating app. Over several weeks he exchanged messages, first in the app and then on WhatsApp, with a supposed woman presented as a journalist in Lebanon who would soon return to Germany. He became suspicious at a last request for money for medical care after an alleged road accident and coma, and went to the police. According to the police's first findings, the photos of the supposed woman were AI-generated, including one showing her injured in a hospital bed. The money was paid with prepaid voucher codes.

Sweden: Economic Crime Authority says pump-and-dump share frauds on small savers used AI; an investor describes AI-made adverts of himself

In October 2025 Swedish media, citing Finansinspektionen, reported that more than 5,000 small savers had lost about half a billion kronor that year in pump-and-dump share frauds, and a Finansinspektionen official said some had lost several million kronor. An investigator at the Economic Crime Authority told TT that the frauds, run from abroad, use a refined method with the latest AI technology. TV4 described the usual pattern as a Facebook tip in which a well-known finance profile appears to recommend a safe investment, leading to a WhatsApp group. The investor Günter Mårder said 30 to 40 adverts and AI-produced films showed him speaking, that about 15 to 20 finance profiles had been used, and that his whole summer went on handling questions. Sveriges Radio reported that the financial journalist Gabriel Mellqvist's identity had been used and that he was frustrated at the damage to his professional reputation and at Meta's inaction. The authority does not say how the AI use was established, and the savers figure is not tied to the AI adverts.

Bengaluru: police say a man paid Rs 1.85 lakh after a dating-app contact threatened to circulate an AI-manipulated obscene video of him

Deccan Herald reports, citing Bengaluru police, that a 23-year-old man lost Rs 1.85 lakh in an alleged sextortion scam. He matched with a woman on a dating app on 27 September 2026 and exchanged numbers; he told police that he then received a WhatsApp video call from her number lasting five to six seconds. Police said he then received an obscene video manipulated using AI and the sender threatened to circulate it unless he paid. Fearing exposure, he borrowed money from a friend and transferred Rs 1.85 lakh to several UPI IDs between 27 and 29 September as the threats continued, a police officer said. Police registered a first information report on his online cybercrime complaint and are trying to trace the suspects. The report does not say how police established that the video was AI-manipulated, and the tool is not named.

La Rioja, Spain: Guardia Civil says a parent sent 9,980 euros after a call with an AI clone of their daughter's voice; the money was recovered

The Guardia Civil in La Rioja said on 25 August 2026 that it had solved a 'relative in trouble' fraud in which the criminals used artificial intelligence to clone the voice of the victim's daughter. According to the force's account as reported by the newspaper La Rioja and Radio Arnedo, the victim received WhatsApp messages from an unknown number by someone posing as the daughter and asking for urgent money; when the victim called that number to check, a voice that sounded like the daughter answered, and the victim transferred 9,980.25 euros. Investigators of the Guardia Civil's technology team established that the voice had been generated with AI tools. Urgent coordination with the banks froze the funds in the destination account and the full amount was returned to the victim; Radio Arnedo and ActualidadRiojaBaja report that the presumed perpetrator was identified and investigated.

Asunción, Paraguay: students of the Universidad Nacional de Asunción report an anonymous website sharing intimate images of students, teachers and staff without consent, some altered or generated with AI; police cybercrime investigators and a prosecutor open an investigation, reported 21 to 24 September 2026

According to Hoy (21 September 2026) and El Nacional (22 September), students of the architecture faculty of the Universidad Nacional de Asunción reported an anonymous website that shared intimate images of students, teachers and staff without authorisation, including real photographs and material altered with AI. El Nacional reports that at least about twenty students complained, that some posts used photographs taken from social media, and that related content later appeared in Telegram and WhatsApp groups. The Minister of Women said her ministry had seen messages in which UNA students raised concern about content of them held without consent, and that in some cases AI-generated images of sexual content were shared (La Nación, 22 September). La Tribuna, reporting the findings of the police cybercrime department, describes the portal as used for the non-consensual distribution of intimate photographs and AI-altered content; according to a memorandum signed by the head of the department, a Paraguay section exposes full names, images and phone numbers of students (24 September). The university referred the case to prosecutors and police, and police suggested that prosecutors seek a court order to block the site in Paraguay. No person responsible has been publicly identified.

Italy: Fideuram chairman Paolo Molesini is deceived by a WhatsApp impersonation of Intesa Sanpaolo's CEO and a phone call in the reportedly AI-cloned voice of a lawyer he knew into ordering about 95 million euros of transfers; at least 36 million remain missing and he resigned on 12 March 2026, weeks later

On 23 February 2026 Paolo Molesini, then chairman of Fideuram (the private-banking subsidiary of Intesa Sanpaolo), received a WhatsApp message from an unknown number from someone claiming to be Intesa's chief executive Carlo Messina, announcing a confidential acquisition that had to be executed through Fideuram. The same day a caller presenting as a lawyer of A&O Shearman whom Molesini knew, whose voice ANSA, Il Fatto Quotidiano and Corriere della Sera, reporting from the Milan court papers, describe as created with artificial intelligence (today.it, which also cites the seizure decree, writes that the court papers do not yet answer whether the voice was imitated), had him sign a confidentiality agreement and sent eleven payment instructions; Fideuram's treasury head was separately contacted by someone posing as Fideuram's CEO. Between 23 and 25 February eleven transfers totalling about 95 million euros went to accounts in Portugal and at Bank of China; the bank's alarm systems and the Milan prosecutors recovered about 40-42 million from China and 13 million seized in Portugal, leaving at least 36 million (39.5 million per the court papers) missing after conversion into cryptocurrency. Molesini, who Corriere writes is not under investigation, resigned as chairman on 12 March 2026, which Fideuram announced as being for personal reasons; a 48-year-old Israeli citizen is under investigation as a member of the gang.

Georgia: a retired couple lost about $800,000 in a crypto scam that police said used an AI-generated "ghost site" to fake what the husband saw on screen (2025)

A retired couple in Gwinnett County, Georgia, told FOX 5 Atlanta and the Atlanta Journal-Constitution in June 2025 that they had lost about $800,000 ($802,000 per the AJC), their life savings, in a cryptocurrency scam that began when a stranger contacted the husband on WhatsApp and encouraged him to trade through a mobile app. He traded and withdrew small amounts before investing more, and discovered the fraud when he could not withdraw and called 911. According to FOX 5, Gwinnett County police explained that the app was real but the scammers had created an AI-generated "ghost site" that manipulated what he saw on the screen; the husband said police identified that what he was seeing on the wallet was not there. The 74-year-old said he felt sick and ashamed; the couple's children started a fundraiser for day-to-day expenses, and police said such funds are rarely recovered. The tool is not described; the AI element appears in no independently reported source (a Cobb County government page of January 2026 repeats FOX 5's summary, and the AJC's legible text does not mention AI).

Jaú, São Paulo: eight 15-year-old ninth-grade boys at a private school are reported to have made fake nude images of a female classmate with artificial intelligence and shared them in a WhatsApp group; the school suspended them and the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation

In early September 2026 a private school in Jaú, in the interior of São Paulo state, identified the circulation of fake intimate images of a female classmate made with artificial intelligence and contacted the families. According to the police report described by the press, eight ninth-grade boys, all aged 15, kept a WhatsApp group in which they used digital programs to manipulate images of the student so that she appeared unclothed. The school suspended the eight and informed the girl's parents. The father of one of the boys checked his son's phone, found the files and took the boy and the phone to the police to register a report; the girl's parents also registered a police report. The São Paulo Public Security Secretariat said the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation that occurred on the morning of Thursday 3 September. The school said the content originated on a private platform outside the school environment and that it had adopted protective and welcoming measures for the student and opened an internal inquiry. The AI tool used is not named. The students are minors and are not named in any report.

Bengaluru: a security guard says a WhatsApp video call in which an alleged deepfake 'Chief Minister Vijay' offered him financial aid led to fake 'processing' charges and a fake CBI fine, and he lost more than Rs 1.04 lakh; Byappanahalli police register an IT Act case (September 2026)

A security guard in Bengaluru, originally from Odisha, told the Times of India that on 9 September 2026 he answered a WhatsApp video call in which a face and voice presented as Tamil Nadu Chief Minister C. Joseph Vijay introduced himself in Hindi, asked his name, work and where he lived and pressed him to accept financial help. A 'manager' then promised Rs 11 lakh, said Rs 5 lakh had been allotted to him and asked for a Rs 5,000 exchange charge, then Rs 18,000 to unlock a supposedly locked PIN; a caller posing as a CBI officer demanded more than Rs 50,000 as a fine. The fraudsters sent a fake allotment letter and a purported CBI officer's identity proof. He paid more than Rs 1.04 lakh through a digital payment app before refusing a further Rs 50,000 demand, called the 1930 cybercrime helpline and went to Byappanahalli police, who registered a case under the Information Technology Act. The Times of India says the fraudster allegedly used a deepfake AI-generated video and calls it the first such case reported in the city. No arrest is reported, and no link to the Tamil Nadu CB-CID deepfake case or its Alwar arrest has been established.

Sonbhadra (Uttar Pradesh): police say a retired official was pressured into transferring about Rs 2.36 crore over one to one-and-a-half months by extortionists who posed as judicial and government officials and used AI-made fake nude images to threaten him; three arrested after a bank manager's alert (September 2026)

Sonbhadra police say a retired official in Uttar Pradesh transferred about Rs 2.36 crore into accounts named by extortionists, including money raised through a loan, before a State Bank of India manager noticed the repeated transfers on 22 September 2026 and alerted the cyber crime police station. At a press briefing on 25 September the superintendent of police said the fraudsters had frightened the man by invoking the higher judiciary and senior government officials (a so-called digital arrest) and had used AI to create fake obscene or nude images to blackmail him with threats of defamation and harm to his family (Times of India, Amar Ujala, ETV Bharat, Newstrack). Police froze about Rs 75 lakh, registered an FIR and arrested three people, including the alleged organiser; the outlets relay one police account, and they describe the AI element differently (images of the victim, or images sent to him during a WhatsApp chat with someone posing as a woman).

East Jakarta: police confirm a grade-9 student at a state junior high school edited several female schoolmates' photographs with AI into indecent images that circulated as WhatsApp stickers; a social-media post said about 200 images were sold; the student was later beaten by schoolmates angry at the images (police statements differ on whether the depicted students or their friends did so) and both matters are under police investigation (Kompas, ANTARA, detik, VIVA, Media Indonesia, 23-24 September 2026)

On 23 September 2026 the spokesperson of the East Jakarta metropolitan police (Polres Metro Jakarta Timur) confirmed to reporters that a grade-9 student at a state junior high school in Pulogadung, East Jakarta, had edited photographs of several female schoolmates using artificial intelligence so that they appeared indecent, and that the edited images had been made into WhatsApp stickers; the police women-and-children protection unit is investigating. The case surfaced through a post on Threads which said about 200 edited photographs had been produced and sold to others; Kompas.com noted on 24 September that the sale allegation still rested only on that post, VIVA reported that police were still checking the claim that the images had been sold, and ANTARA reported on 23 September that police were still establishing the editing method, the recipients and the number of depicted students. As of 24 September the depicted students had not filed a report (detik and VIVA date the spokesperson's telephone remarks 'Kamis (22/9/2026)', a date that is internally inconsistent) and police appealed to them to do so. According to the parents' account relayed by police, on 15 and 16 September several people took the student who made the images to an empty house and then to a reservoir in Kayu Putih, punched him on the nose, ears and head, kicked his chest and stomach and threatened him with a bladed weapon; police attribute the beating to friends of the depicted students (ANTARA, detik, VIVA), while Kompas.com's 23 September report quotes the same spokesperson as saying the depicted students themselves beat him; he filed an assault report, which police are handling alongside the image case, and the school has held mediation with the students' parents. Singapore: a businessman lost at least S$4.9 million after a deepfake Zoom 'briefing' with fabricated Prime Minister Lawrence Wong and other officials asked him for 'urgent funding assistance'

In May 2026 the Singapore Police Force (SPF) reported that a man had lost at least S$4.9 million (about US$3.8 million) to a government-official impersonation scam. He received a WhatsApp message carrying the profile photo of Secretary to the Cabinet Wong Hong Kuan and an email from a proton.me address in that name, requesting urgent funding assistance for 'the situation in the Strait of Hormuz', with a fake 'letter of guarantee' bearing the Prime Minister's signature promising reimbursement within 15 business days; he signed a non-disclosure agreement and supplied a copy of his identity card. He was then invited to a Zoom video conference in which Prime Minister Lawrence Wong, President Tharman Shanmugaratnam, Minister Indranee Rajah, Monetary Authority of Singapore representatives and foreign officials appeared; all were fabricated with deepfake AI, and the closing 'remarks' by the fake Prime Minister acknowledged the victim's attendance. Contacted afterwards on WhatsApp by a scammer posing as a lawyer, he transferred the money in a series of transactions to a corporate bank account, and realised the fraud only when he contacted the real cabinet secretary. On 16 May the police released footage of the fake conference, noting lips out of sync with speech, audio broadcast from a single account and a distorted background.

Ards and North Down, Northern Ireland: a person lost £250,000 to an investment fraud that began with an AI-generated video of a well-known financial personality

On 7 September 2026 the Police Service of Northern Ireland (PSNI) said a person in the Ards and North Down area had lost £250,000 in what it called a 'sophisticated and substantial' investment fraud. The victim first saw what appeared to be an AI-generated online video featuring a well-known personality from the financial sector promoting an investment with significant returns. After investing a small amount the victim was contacted on WhatsApp and encouraged to pay more, persuaded to open multiple online accounts, to allow the fraudsters remote access to a computer 'to assist with the investment', to borrow money to keep investing, and finally to make further payments to 'release' funds supposedly held in the investment. The PSNI did not name the personality; BBC News NI noted that fabricated endorsements of financial experts such as Martin Lewis, Peter Jones and Steven Bartlett circulate online.

Kanpur vegetable seller reportedly transferred Rs 1 lakh after a WhatsApp call in a voice like his nephew's; police say it prima facie involves deepfake AI

Amar Ujala (Kanpur, 10 April 2024) reports that a vegetable seller received a WhatsApp call on 27 March in which a person spoke in a voice resembling his nephew's, saying he had been picked up by police on his way to Kanpur; a second caller posing as a police sub-inspector said the nephew was named in a rape-and-murder case and demanded Rs 2 lakh. The seller transferred Rs 60,000 to one bank account and then, with Rs 40,000 borrowed from nearby shopkeepers, Rs 40,000 to a second account. When a voice note demanded another Rs 1 lakh he told the shopkeepers, phoned his brother-in-law and learned the call was a fraud. Police registered a case under the IT Act citing fraud and the use of deepfake AI, and the Central zone DCP is quoted saying the case prima facie appears connected with deepfake AI and the cyber cell is investigating. No recording or forensic finding is reported, so whether the voice was AI-generated or a human impersonation is not established.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
