AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

2022TaiwanUnidentified voice-cloning tool

Taiwan: a romance-fraud ring used AI voice-changing software so members sounded like young women on dating platforms; prosecutors say more than 20,000 people were defrauded of over NT$900 million and indicted 57 people in September 2026

According to the Taipei District Prosecutors Office and Taiwan's Criminal Investigation Bureau, as reported by the Liberty Times and Public Television Service (PTS) on 2 September 2026, a fraud ring set up in 2022 by a married couple ran fake romances on dating platforms. An engineer in the ring built AI voice-changing software that loaded the voiceprints of 22 women working for the ring as hostesses into its chat system, so that any member, male staff and older women included, spoke to targets in a sweet young woman's voice. Members pressed the people they courted to buy expensive health supplements and gifts and to pay pocket money, moving costs and beauty courses, and the ring sent hostesses to meet some targets, in some cases for sex, to keep them paying. Prosecutors put the victims at more than 20,000 and the proceeds at more than NT$900 million since 2022, and say many victims are still living in debt. After searches in May and July 2026, the Taipei District Prosecutors Office indicted 57 defendants on 2 September 2026 under the Fraud Crime Hazard Prevention Act and asked for sentences of 25 and 18 years for the couple.

2025IndonesiaUnidentified image and video tool

Jakarta: AI-manipulated TikTok videos showed toll-road businessman Jusuf Hamka and his daughter in detainee uniforms with bribery and corruption narratives that Hamka denies; a TikTok account holder was arrested in November 2025 and is on trial at the Central Jakarta District Court, where Hamka's side refused restorative justice on 29 September 2026

Indonesian businessman Mohamad Jusuf Hamka reported to Jakarta police on 18 October 2025 that videos circulating on TikTok showed him and his daughter as if wearing prosecutors' detainee uniforms, tied to narratives of corruption, bribery and gratification over the Cawang-Pluit toll concession, which he denies and his lawyers call false; his lawyers say the faces were composited into staged scenes with AI, and that other content called him a 'toll mafia' and posted a fabricated poster of him running for president in 2029. Polda Metro Jaya's cyber directorate arrested the holder of the TikTok accounts on 27 November 2025; Hamka and his lawyers say the defendant admitted to police that he made the content on others' instructions for Rp500,000 per item; tvOneNews and TVRI note this is Hamka's side's claim within the contested case, and Poskota reports it is being examined at trial. The case file was reported complete (P21) on 12 August 2026 and the trial opened at the Central Jakarta District Court on 15 September 2026 under the Electronic Information and Transactions Law (Article 35 jo 51(1), maximum 12 years) and alternative counts. Hamka told reporters the content disturbed him and could damage his reputation and that the campaign was systematic. At the 29 September 2026 hearing the judges asked about restorative justice; Hamka's lawyer said there was no room for it unless the defendant named who ordered and paid for the content. The defendant's lawyer said the content related to his work as secretary-general of an anti-corruption committee and that the indictment was unclear.

20 Oct 2025 to 13 Nov 2025United StatesUnidentified video tool

Hampton, New Hampshire: police say a Massachusetts man harassed a Hampton resident for more than a year and posted an AI-generated deepfake video showing the resident being arrested and jailed, events police say never happened; he was indicted on four felony counts of fraudulent use of deepfakes and is held after his bail was revoked

Hampton police say a man from Worcester, Massachusetts harassed a Hampton resident for more than a year through phone calls, texts, emails and online posts, stemming from a perceived past disagreement, and kept going after the victim asked him to stop and after he was served with a stalking order of protection in October 2025. In November 2025 the victim showed police an AI-generated deepfake video that, according to the arrest-warrant affidavit, the man created to humiliate and defame him; it shows the victim being arrested and jailed and includes a structure resembling the victim's Hampton home and images of real people. A police sergeant said the depicted events never happened and that the video was posted on YouTube. A Rockingham County grand jury indicted the man on four Class B felony counts of fraudulent use of deepfakes under RSA 638:26-a, for creating, distributing and presenting a video of an identifiable person on 20 October, 31 October, 7 November and 13 November 2025. He turned himself in on 24 March, was released on bail, was arrested again on 26 August for alleged bail violations and is held at the Rockingham County House of Corrections. Police had the video removed. The AI tool is not named and the victim's own account is relayed only through the police.

1 Oct 2024SwitzerlandChatGPT

Zurich-Oerlikon, Switzerland: a student who researched killing methods, anatomy and where to find children with ChatGPT stabbed three five-year-olds walking to their daycare; sentenced on 18 September 2026 to 15 years for multiple attempted murder

At midday on 1 October 2024 a Chinese master's student, then 23, attacked a group of five-year-old kindergarten children walking with a daycare worker to their after-school Hort near Berninaplatz in Zurich-Oerlikon with a kitchen knife. Two boys suffered life-threatening neck and chest wounds (one had the jugular vein severed, one the pericardium and aorta cut); a third was wounded in the ear and the shoulder or lung. The daycare worker held the attacker's arms until he dropped the knife. The indictment and the court's oral reasoning, as reported by NZZ, Keystone-SDA/ATS and CH Media, describe weeks of preparation from summer 2024: he discussed ways of killing and their legal consequences with ChatGPT, studied anatomy, bought raw beef to practise stabbing, and used ChatGPT to find out where and when children could be encountered in Zurich. No source quotes the prompts or ChatGPT's replies. On 18 September 2026 the Zurich District Court convicted him of multiple attempted murder and imposed 15 years' imprisonment, treatment during custody, a 15-year expulsion from Switzerland and CHF 106,000 in moral damages to the three boys; it accepted that he was psychotic but found diminished, not absent, criminal responsibility. His lawyer announced an appeal. The parents of one boy described nightmares, self-blame and scars 23 months on.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
