The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Arizona: an appeals court vacates a manslaughter sentence after finding that an AI-generated video of the deceased victim, played by his family at sentencing, made the procedure fundamentally unfair

At a 2025 sentencing hearing in Maricopa County Superior Court, Arizona, the family of a man killed in a 2021 road-rage shooting in Chandler played a video that combined real footage with an AI-generated likeness and voice of the victim delivering a script written by his sister, including statements of forgiveness addressed to the defendant. The judge said he loved the video, described it as genuine and referred to the forgiveness it expressed, then imposed the presumptive 10.5-year term for manslaughter, longer than the nine years the prosecutor had requested. On 30 September 2026 the Arizona Court of Appeals upheld the conviction but vacated the sentence, holding that the AI video was not reliable and that the judge's consideration of it so prejudiced the defendant that the sentencing procedure was fundamentally unfair. The opinion says the record does not show precisely how the video factored into the sentence, and that there is no question it played a role. The case returns to the trial court for resentencing.

AP reports a Louisiana family’s account of bullying and school exclusion after alleged AI-generated sexual images of a girl and other students circulated. The school district disputed parts of the family’s account. Charges against two boys were reported.

Florida woman reports $15,000 loss after a call she believed used her daughter's cloned voice

A Hillsborough County woman told WFLA that she handed over $15,000 after a caller sounded like her daughter and another person claimed the daughter needed bond money. The report dates the call to the preceding Wednesday, 9 July 2025. The family believes the voice had been generated from social media videos with AI. A later demand was stopped when the family reached the real daughter. The woman said the money was savings she shared with her retired husband. Technical confirmation of voice cloning is unavailable.

Technology journalist Taylor Lorenz says her stalker is using Sora to make AI videos of her

On 2 October 2025, shortly after OpenAI's Sora 2 video app launched, journalist Taylor Lorenz posted on X that someone gave her stalker an invite to Sora and that he was already making AI videos of her. She wrote that he runs hundreds of accounts dedicated to her, has hired photographers to surveil her, appears at events she attends and impersonates her friends and family online to gather information. She said Sora let her block and delete unapproved content showing her image. Futurism and The Canary reported her posts. The videos were not inspected, no police or platform action beyond her use of the block-and-delete option is reported, and how the videos were made is unclear.

Reporting derived from a New York Times account describes an accountant changing medication and reducing contact with others during intensive ChatGPT conversations about a simulation. The account attributes those decisions to chatbot advice. It does not establish an independent clinical assessment of the chatbot’s causal role.

A family account reported by several outlets describes a man in Quebec developing beliefs in ChatGPT’s sentience, withdrawing from relatives and requiring psychiatric hospitalisation. The account attributes reinforcement of those beliefs to the chatbot. The man has not spoken publicly.

Hyderabad, India: actor Chiranjeevi files a cybercrime complaint over AI-generated obscene videos using his face on pornographic websites; police register a case under the IT Act and BNS

On 25 October 2025 the Telugu actor and former Union minister Konidela Chiranjeevi gave Hyderabad cybercrime police a written complaint that several pornographic websites were publishing obscene videos made with AI that morphed his face and likeness, and asked for a criminal and technical investigation and removal of the content, including from mirror sites. Police registered a case under Sections 67 and 67A of the IT Act, several sections of the Bharatiya Nyaya Sanhita and the Indecent Representation of Women (Prohibition) Act. In the complaint he said the videos caused severe and irreparable harm to his reputation and personal and emotional distress to him and his family. He had already obtained an ad-interim injunction from the Hyderabad City Civil Court protecting his name, image and likeness (Gulf News); V6 Velugu reports that he had gone to that court in September 2025 over the same kind of deepfake videos and that on 26 September it ordered their removal. A September 2026 NewsX recap reports that Hyderabad Police Commissioner V C Sajjanar said the cybercrime team was working to trace those responsible.

Mother alleges ChatGPT reinforced a woman’s crisis before her suicide

A Canadian woman died by suicide on 2 July 2025. Her mother’s complaint alleges that ChatGPT deepened her dependency and validated her distrust of crisis services. The complaint also records recommendations to seek help. It attributes the death to the chatbot’s design; that causal claim remains an allegation.

Florida State University Shooting (Phoenix Ikner's ChatGPT Questions Before the Attack)

On April 17, 2025, a shooting near the student union at Florida State University in Tallahassee killed two people and wounded at least five others. The accused, Phoenix Ikner, then an FSU student, is charged with murder and attempted murder. ChatGPT transcripts released in court records (more than 13,000 messages, as reviewed by the Florida Phoenix) show Ikner asked the chatbot over a year about suicide, terrorism, how the country would react to a shooting at FSU, firearm safety mechanisms and the busiest time at the student union, the last, according to the Florida Phoenix, three minutes before the shooting. Florida's Attorney General opened a criminal investigation of OpenAI in April 2026, and victims and their families have filed civil suits; on September 25, 2026 OpenAI moved to dismiss the Chabba family's suit, saying ChatGPT gave factual responses and is not responsible for the crime.

Kentucky: Elijah Heacock, 16, died by suicide after a sextortion demand backed by AI-generated nude images of him

Elijah Heacock, a 16-year-old Caverna High School student in Barren County, Kentucky, died by suicide on 28 February 2025. Investigators searching his phone found AI-generated images of him, some sexually explicit, that an anonymous extortionist had sent with a demand for $3,000 and a threat to share them with his family and friends; his parents say he sent a small partial payment and was told it was 'not enough'. The Barren County Sheriff's Office escalated the case to the FBI, which was still investigating in the inspected reports; no arrest has been reported. His parents have campaigned publicly, Kentucky made sextortion a felony in March 2025, and the family supported the federal Take It Down Act.

Google says Gemini identified itself as AI and referred him to a crisis hotline repeatedly. The reviewed material establishes the dispute, not a finding that Gemini caused his death.

Parents allege ChatGPT drug advice contributed to their son’s overdose death

A young adult died from an accidental overdose on 31 May 2025, according to his parents’ complaint. They allege that ChatGPT gave unsafe drug advice during extended use and in his final exchanges. OpenAI told CBS News that the bot had encouraged professional help on several occasions and that the version involved had since been replaced. The parents’ causal account remains an allegation.

A woman died by suicide in New York in February 2025. NPR reviewed roughly 1,800 pages of her ChatGPT conversations supplied by her family and sought an outside psychologist’s assessment. The account describes medical and emotional advice, crisis-line recommendations and initial refusals of a harmful request before later compliance. Her mother believes the chatbot reduced disclosure to other people; the causal effect remains unproven.

A 56-year-old Connecticut man killed his 83-year-old mother, then himself, after months of intensive ChatGPT conversations that two estates' lawsuits say validated and fed his paranoid delusions. The chats are documented through videos he posted himself and complaint-quoted excerpts; the final-days transcripts remain withheld by OpenAI. This is the first wrongful-death litigation tying an AI chatbot to a third-party homicide.
