AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Works v. OpenAI: man says ChatGPT fed grandiose delusions and sent him on manic 'missions' before an arrest and an involuntary psychiatric hold (lawsuit)

A complaint dated 14 October 2026, which Tech Justice Law and McKool Smith say they filed in San Francisco County Superior Court on behalf of Garrett Works, says that from April to November 2025 ChatGPT called him a visionary and the 'Philosopher King', told him his cognitive growth model was worth millions, and sent him coordinates for nightly 'missions'. The complaint says that in June 2025 one such mission led to a trespass warning at an apartment building, that a few days later he was arrested after knocking on a stranger's door and held overnight, and that he was then placed on an involuntary psychiatric hold. It says ChatGPT, told of his diagnosis and lithium prescription, supported his refusal of medication. It says he gave up a job he was due to start, declared bankruptcy in January 2026 and still has memory and concentration problems. The allegations are untested and OpenAI's response is not reported.

Hillsborough County, Florida: sheriff's office says a teenager used an AI-altered image to frame her ex-boyfriend for a school shooting threat

According to the Hillsborough County Sheriff's Office, as reported by CBS News Miami, WFLA and WTSP, deputies responded to a high school in Riverview, Florida, on 14 October 2026 after administrators received an email containing a screenshot of an Instagram post threatening a shooting at the school. The post appeared to come from a male student. Detectives interviewed him and determined that the post did not come from him. The sheriff's office's communications chief said detectives very quickly realised the post was AI-altered and linked it to his ex-girlfriend, a teenage former student. Police say she admitted sending the email knowing that the image had been altered using AI. She was arrested and charged with making a written or electronic threat to conduct a mass shooting or act of terrorism, unlawful use of a two-way communications device and disrupting a school campus or function. The tool used and how detectives established the alteration are not reported.

Álava, Spain: Policía Nacional says a man photographed children in the street and at municipal pools, then used AI tools to make them appear naked; arrested

Spain's Policía Nacional said in a press note on 15 December 2025 that it had arrested a man in the province of Álava who photographed minors in the street and at municipal swimming pools and then manipulated the photos with artificial intelligence tools so that the children appeared naked. The investigation began in August 2025 after cyber-patrol work and international cooperation flagged someone producing child sexual abuse material; police say the suspect used advanced anonymisation techniques. A search of his home found numerous such images created with AI tools. He was arrested on suspicion of corruption of minors through the production and possession of child sexual abuse material. The account is the police's, relayed by Cadena SER and Escudo Digital; the number of children is not stated and the case is untried.

Honduras: national police say a woman faked her own kidnapping twice and sent her partner AI-generated images of injuries to obtain money

Honduras's National Police announced on 14 October 2026 that its anti-kidnapping unit (UNAS) of the Police Investigations Directorate (DPI) had arrested a woman, under a court warrant, on suspicion of simulating a non-existent offence related to kidnapping and of using false documents. According to the police statement, the investigation found that she had faked her own kidnapping twice, in episodes reported on 21 April and 1 June 2026, to obtain money from her partner to pay personal debts, and that she received an initial deposit of 2,000 lempiras after making the partner believe she was being held and that her captors were demanding money. Police say the agents determined that she presumably used artificial-intelligence tools to generate false images of injuries and bruises, which were sent to the partner to make the partner believe she had been beaten by the supposed kidnappers, together with apparently forged hospital documents. According to the investigation file, she said after being located that she planned both episodes and used AI to make the images. She was brought before the issuing court; no court finding is reported.

Pennsylvania man sentenced to prison in a case where prosecutors and court documents say he made AI deepfake nude images of a teenage neighbor

A Bucks County, Pennsylvania, judge sentenced a man to state prison on 14 October 2026 after he pleaded guilty to child sexual abuse material and theft charges. According to court documents and prosecutors, detectives who began investigating him in June 2025 found AI-generated deepfake nude images of a teenage girl he knew, a neighbor, on his devices; her face had been placed on other bodies. The day after the arrest he was recorded entering her family's home and stealing her underwear. Her father described the family's distress in court, and the Inquirer reports that she is struggling at school, taking antidepressants and in therapy. The Philadelphia Inquirer reports a sentence of seven to 10 years. The Bucks County Courier Times gave seven to 12 years in its first report and seven to 20 years after republishing it to update the sentence length. Neither report names the AI tool.

Busan prosecutors say an investment-fraud suspect used AI images of a fake medical licence and asset records to take 320 million won, then fooled a judge

The Busan Eastern District branch of the prosecutors' office in South Korea announced on 11 February 2026 that it had indicted a man under detention on 6 February for fraud, forging and using private documents, and obstructing official duties by deception. According to the prosecutors, from August to October 2025 he used AI to make fake images of a medical licensing examination certificate and of cryptocurrency holdings, posed as a doctor and businessman with billions of won in assets, and took about 320 million won from victims for supposed cryptocurrency, cruise-ship and medical-centre investments. When police sought his arrest in December 2025, he gave the warrant judge a bank balance certificate showing about 904 million won, which prosecutors say he made by uploading his real certificate to an AI platform and asking it to change the balance; his real balance was 23 won. The judge refused the warrant. Prosecutors later found the certificate was forged and obtained a new warrant. The accused and the victims are not named here.

Israel: indictment and police say a 20-year-old used AI-animated ID photos as fake verification videos to open credit cards in at least 33 people's names

According to an indictment filed on 16 July 2026 (date per Maariv), as reported by i24NEWS, a 20-year-old man impersonated at least 33 Israelis over about half a year, obtained credit cards in their names and used them for purchases of about 67,000 shekels in Israel and abroad, with attempted charges of more than 56,000 shekels more. Investigators collected fake identification videos of the victims that, according to the indictment as reported, he created with artificial intelligence and used to pass the card issuers' remote identification. Maariv reports, citing police, that the suspect cut the face photos from identity cards and animated them with AI software so that the card holder appeared to speak the required declarations. A police investigator told Maariv that investigators had to locate more than 50 people whose identities were used and that some victims were frightened when shown the videos of themselves in interrogation rooms. The victims who were questioned denied any connection to the cards. The investigator said most of them did not know their identities were being used and that the credit companies bore the direct financial loss. He was arrested on landing in Israel, did not cooperate with the investigation, and the prosecution asked for his detention until the end of proceedings.

Thai police say a romance-fraud suspect told them he used AI to fake a bank deposit certificate; the victim handed over 21.3 million baht in 623 transfers

Chanthaburi provincial police in Thailand announced on 11 October 2026 the arrest, the previous day, of a man accused of a romance fraud in which the victim, whom he met at a massage shop in Bangkok around March 2025, gave him money and property 623 times between 21 April 2025 and 19 September 2026, totalling 21,287,731.88 baht. Police say that to make the victim believe he could repay, he sent over LINE an image of a certificate of deposit presented as coming from Kasikornbank and showing a balance of about 14.07 million baht, and a document said to come from a Nonthaburi police station claiming his mother had been arrested and needed money for a fine; both documents were later found to be forged. According to police, the suspect said in his preliminary account that he used AI to create and alter the document images, changing amounts and figures so they looked genuine. Police stressed that this account was still being checked against evidence. They say most of the money went on online slot gambling. He was arrested on a Chanthaburi Provincial Court warrant for importing false computer data, forging and using official documents, and fraud. The suspect and the victim are not named here.

Jersey: a TikTok account posing as a school posted AI-generated videos showing teachers in humiliating and sexualised scenes, reported to police

In February 2026 a TikTok account that used the name, badge and branding of a school in Jersey, and presented itself as "official", uploaded five videos depicting teachers at the school. The Jersey Evening Post (JEP) described the videos as AI-generated deepfakes made with a free app and reported that they showed teachers in humiliating, compromising and sexualised scenarios: in one, two male teachers dance beneath a caption insinuating sexual interest in young girls; a montage shows a teacher barking and eating from a dog bowl and teachers firing guns, while other clips ridicule the appearance of specific staff members. The JEP reported the videos were viewed nearly 20,000 times within days. Education Minister Rob Ward described the content as "deeply inappropriate", confirmed it had been reported to TikTok and the States of Jersey Police, and said his department was working with the school to support staff and students; he said it appeared to be an isolated incident. The account was removed from TikTok. No arrest, charge or identification of the account holder is reported, and no teacher is quoted.

Houston immigration attorney says scammers altered her photos with AI to pose as her office; a woman says her family lost about $11,000 (KHOU report)

KHOU reported on 7 October 2026 that a southwest Houston immigration attorney says people have been taking her photographs, altering them with artificial intelligence and using them on Facebook and Instagram to impersonate her office, with her image placed next to other people posing as attorneys. She says her firm has received calls from people who paid money to the impersonators and that she knows of more than 20 people who may have been targeted. One woman, interviewed by KHOU without her identity revealed, said her family lost about $11,000 after she was promised a permit to stay in the United States within a month and attended what the couple believed were meetings with immigration agents; she said she later discovered that the photo was fake and had been made with artificial intelligence. KHOU reports that people who say they lost money reported the alleged scheme to the Houston Police Department, which said the case will be assigned to an investigator; no arrest or platform response is reported, and the AI identification rests on the attorney's and the woman's accounts.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
