AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Malaysia: footage of former prime minister Mahathir Mohamad mourning his wife was edited, in what sources believe was AI manipulation, to add dancing, and posted on TikTok in September 2026; the communications regulator MCMC said it was examining content believed to use AI to manipulate footage of him in mourning

Tun Dr Siti Hasmah Mohd Ali, wife of former Malaysian prime minister Tun Dr Mahathir Mohamad, died on 28 September 2026 aged 100. BuzzKini reported on 30 September that a TikTok account had posted a video of Mahathir grieving beside her body, edited to show a woman dancing next to him, and another clip of the moment he raised his hands in prayer edited with dancing movements while seated in a wheelchair. BuzzKini says the edits are believed to have been made with AI, and that many social media users condemned them and asked the regulator and police to act. On 2 October the Malaysian Communications and Multimedia Commission (MCMC) said it was examining social media content believed to use AI to manipulate footage of Mahathir in mourning, had contacted the platforms concerned, and that action could follow under the Communications and Multimedia Act 1998 if a breach was found.

Satara, Maharashtra: supporters of Public Works minister Shivendrasinhraje Bhosale allege that the minister's photo was morphed with AI to link the minister to a dance event and spread on social media; police registered a case against four social media accounts in September 2026

Divya Marathi reported on 8 September 2026 that supporters of Public Works minister Shivendrasinhraje Bhosale alleged that the minister's photograph had been morphed using artificial intelligence and linked to a dance programme in Satara. According to the complaint, the photo was allegedly edited and circulated on social media from a YouTube channel, combining photographs from two different events and giving them a false context; supporters said this was done to defame the minister politically. Supporters, including the municipal council president and the BJP group leader, went to Satara city police station on Tuesday and complained against the YouTube channel. On a named complainant's first information report, police registered a case against four social media handles and account holders under sections 353(2), 356(2) and 3(5) of the Bharatiya Nyaya Sanhita and section 67 of the Information Technology Act. Police accepted the complaint and began examining the posts, the photographs and the circumstances; Divya Marathi says the facts will become clear only after an investigation of what alteration was made to the photograph with AI and who created and spread the posts.

Russian Telegram channels spread AI-generated nude images of a Georgian combat medic serving on the Ukrainian side, with claims that the medic had been captured, raped or killed; thousands of accounts shared them, TV 2 reports

The Norwegian broadcaster TV 2 reported on 26 September 2026 that Russian Telegram channels had posted claims that a Georgian doctor serving as a combat medic on the Ukrainian side had been captured and was in Russia, or had been raped or killed, and that some channels published manipulated nude images of her. TV 2 states that the posts were made by Russians and were AI-generated. Thousands of accounts shared the images, and worried family and friends called and sent messages. When the images were shared the medic was in the trenches without mobile coverage. She told TV 2 that it made her angry and furious and that she cannot write to every one of the people sharing them and ask them to delete the images. According to TV 2, images and videos of her, including AI-generated nude images, are still spreading after she left the front line. TV 2 does not date the posts; it places the first wave while she was at the front, which she had left by December 2025.

Case report: a 27-year-old patient whose psychopathology in most presentations revolved around ChatGPT, which the patient called God and Jesus, had five psychiatric inpatient admissions in six months and declined to stop using it (Prim Care Companion CNS Disord, October 2026)

Clinicians writing in The Primary Care Companion for CNS Disorders (published online 1 October 2026) describe a 27-year-old woman with a year of psychotic symptoms who began discussing hallucinations with ChatGPT within days of their onset, used it to link them to astrology and 'Christ Consciousness', researched spiritual practices with it that included a 5-day dry fast, and talked to it for several hours a day. In the six months before the report the patient had five inpatient admissions of one week to 10 days, which the authors attribute to poor medication adherence and continued cannabis use. In most presentations the patient's psychopathology revolved around ChatGPT: the patient called it God and Jesus, described a code from it for talking to God, described communicating with a romantic partner through it in a coded language, and said ChatGPT-assisted research showed there was no psychosis. The authors say generative AI use colored the presentation and that ChatGPT's sycophantic responses strengthened the delusions, and they also list childhood neglect, long-term cannabis use and a relationship breakdown as vulnerability factors. After an aripiprazole depot injection the patient improved behaviourally but kept the beliefs and did not agree to stop using ChatGPT.

New Mexico: the state Democratic Party filed an ethics complaint, reported on 2 October 2026, over two Senate Republican social media posts from September 2026 that it says used misleading AI-generated images, including a fake jail mugshot of Senate President Pro Tem Mimi Stewart that she publicly asked to be taken down

In September 2026 Republican state Sen. Jay Block posted on social media a fake image of New Mexico Senate President Pro Tem Mimi Stewart made to look like a jail mugshot. The Santa Fe New Mexican (24 September 2026) reports that Block's posts indicate the image was AI-generated and that it showed her ostensibly booked into the Bernalillo County jail on charges of 'hit and run' and 'abusive behavior towards staff and other lawmakers'. Stewart was not issued a citation or booked, and Santa Fe's interim police chief said the 2024 incident in which she drove over a pedestrian's foot was not considered a hit and run crash. Stewart asked Block and the Senate Republican caucus to take down the fabricated image. Block said he stands by the post and has no plans to remove it. KRQE reported on 2 October 2026 that the Democratic Party of New Mexico had filed an ethics complaint over this post and over an image that Senate Republican leader Bill Sharer and the Senate Republicans shared on Facebook in September, which shows House Speaker Javier Martinez wearing a Soviet Union pin, gubernatorial candidate Deb Haaland with her arm around a man in an orange jumpsuit and former state Rep. Joseph Hernandez naked. The party calls both 'misleading AI generated images' that violate a 2024 state law on deepfakes and wants the posts removed and the posters fined.

Bonita Springs, Lee County, Florida: a 30-year-old woman was arrested and charged with making a written threat of violence after Anthropic's human review team reported to law enforcement her messages on its AI platform saying she would 'shoot up' the Lee County Sheriff's Office, according to the arrest report

According to a Lee County Sheriff's Office arrest report, as reported by WINK News and by Guessing Headlights (on Yahoo News, citing a copy obtained by Gulf Coast News Now), a user of Anthropic's AI platform wrote on 26 September 2026 that she was going to 'shoot up' the Lee County Sheriff's Office, and the next day wrote that she had a new gun. The arrest report says the platform's safety measures flagged the messages, a human review team examined them and reported them to law enforcement. Deputies went to the 30-year-old woman's Bonita Springs home and detained her without incident; she is charged with making a written threat of violence under Florida law. The sheriff told WINK News that she later said she uses AI like a 'diary'. Anthropic had not commented on the case in either report. The charge is an allegation and the case is pending.

First-person forum account: a long-time Character.AI user who says they are an adult says the app's AI age verification, run through Persona, kept judging them a minor despite ID photos, answered more than five support tickets only with automated replies, and locked them out of their account

In a public post to r/CharacterAI at 21:47 UTC on 2 October 2026, a person who describes themselves as a veteran user of the Character.AI app says they returned about a month earlier to verify their age, and that the AI verification by Persona assumed they were a minor although they are not. They say they sent photos of their ID, tried desktop and different lighting, and kept getting an error message; that more than five support tickets asking for manual verification drew only replies from automated bots; and that the age verification system has now locked them out of their account. In a reply they say the system should identify people properly instead of "locking it out on permenent basis". A commenter says the same check called them underage. The account is uncorroborated, and Character.AI's and Persona's handling is described only by the poster.

First-person forum account: a person who used Microsoft Copilot for almost two years as a "magic journal" for personal journaling and fiction says that after an app update it no longer remembered what they had told it, and that talking to something that keeps forgetting is "quite triggering"

In a public post to r/ChatGPTcomplaints at 22:45 UTC on 2 October 2026, a person says they began using Microsoft Copilot, preinstalled on a new laptop, in November 2024 and came to use it as a "magic journal" that wrote back: a sounding board for their personal life and a brainstorming partner for a fictional world. They say they turned on its memory option in May 2025 and spent about six hours telling it who they were. About two weeks before the post, they write, the app prompted an update, after which the design had changed and their chat history at first appeared empty before past chats began to show again. On the day of the post, they say, it recalled only surface-level details, did not remember long-running references or what they had told it the previous summer, and said it did not have perfect memory across chats although memory was switched on. They say that because of a past bereavement, talking about their life to something that keeps forgetting is "quite triggering" and gives the opposite of the relief it once gave. Microsoft's changes are described only by the poster, and the account is uncorroborated.

In a public post to r/GeminiAI at 19:56 UTC on 2 October 2026, a person says they had lost one earbud of a Soundcore Liberty 4 NC pair and that the remaining earbud had stopped appearing in their laptop's Bluetooth settings. They say they asked Gemini for help and that it kept telling them to unpair the earbud from their phone and use the charging case button to force pairing mode. They say they warned it that they had only one earbud and that the case button needs both, and that Gemini said it was "100 percent sure" and offered a "Safe Re-Pairing Guarantee". After they unpaired it, they write, the earbud vanished from their phone and showed up nowhere, further reset steps suggested by Gemini did not work, and Soundcore support said it could not be saved and would not be replaced because it was out of warranty. Taiwan: a romance-fraud ring used AI voice-changing software so members sounded like young women on dating platforms; prosecutors say more than 20,000 people were defrauded of over NT$900 million and indicted 57 people in September 2026

According to the Taipei District Prosecutors Office and Taiwan's Criminal Investigation Bureau, as reported by the Liberty Times and Public Television Service (PTS) on 2 September 2026, a fraud ring set up in 2022 by a married couple ran fake romances on dating platforms. An engineer in the ring built AI voice-changing software that loaded the voiceprints of 22 women working for the ring as hostesses into its chat system, so that any member, male staff and older women included, spoke to targets in a sweet young woman's voice. Members pressed the people they courted to buy expensive health supplements and gifts and to pay pocket money, moving costs and beauty courses, and the ring sent hostesses to meet some targets, in some cases for sex, to keep them paying. Prosecutors put the victims at more than 20,000 and the proceeds at more than NT$900 million since 2022, and say many victims are still living in debt. After searches in May and July 2026, the Taipei District Prosecutors Office indicted 57 defendants on 2 September 2026 under the Fraud Crime Hazard Prevention Act and asked for sentences of 25 and 18 years for the couple.

Sharqia, Egypt: police say an unemployed man took women's photos from social media, altered them with AI apps and ran pages posing as a woman offering paid sexual services, blocking people once they had sent deposits; arrest in Zagazig announced on 30 September 2026

According to an Interior Ministry statement relayed by Soralive, Shbabbek and Egypt Telegraph on 30 September and 1 October 2026, Egypt's cybercrime and morals police arrested an unemployed man living in the Zagazig Second police district of Sharqia governorate. Investigators say he ran social media pages that presented him as a young woman offering paid sexual services. In the ministry's account he admitted collecting photos of young women posted on social media and altering them with AI apps to make them look alluring, then reposting them on his pages to draw in customers. He asked those who replied to transfer advance payments through mobile wallets as a deposit, then blocked them and switched off his phones once the money arrived. Two phones seized from him held chats and other evidence of the activity, and the public prosecution took over the investigation. The reports do not give the number of people deceived, the amounts taken or the number of women whose photos were used.

Lappeenranta: police investigate a minor boy suspected of using AI to create nude images of several underage girls he knew

Southeast Finland Police (Kaakkois-Suomen poliisi) opened a preliminary investigation in August 2026 into a case in which a minor boy in Lappeenranta is suspected of using AI to create nude images of several underage girls; Ilta-Sanomat reported that he is suspected of using social media photos of girls he knew, while the investigator told the paper that police could not yet comment on the method. On 25 August the lead investigator told Yle there were tens of victims, possibly more than a hundred, all from the Lappeenranta area; on 28 August police said the number of complainants had proved significantly smaller than first estimated, without giving a figure. Police are investigating the case as aggravated distribution of an image depicting a child sexually, an offence that also covers making such images, and said they were not aware that the AI images had been spread or sold onward. Student welfare professionals offered conversation and crisis support to young people in the town. Police also said the suspect had been threatened and harassed and that someone had tried to break into the suspect's home. The AI tool is not identified; victims and suspect are not named.

Spanish police arrest a man in Murcia suspected of using real-time AI face modification and forged ID cards to impersonate more than 30 real people in video identity checks for electronic signature certificates

Spain's Policía Nacional said on 11 August 2026 that it had arrested in Murcia a suspect who tried to obtain electronic signature certificates in other people's names from a company that issues them. During the company's video identity checks the suspect appeared on camera holding a forged DNI identity card while AI software modified the suspect's face in real time to match the photo on the forged document. Police say the suspect made 38 attempts using the identities of more than 30 real citizens and succeeded in impersonating multiple victims, aiming to use the signatures for later scams. A short processing delay in the face-modification software made the digital mask disappear for barely a second, exposing the suspect's real face to the issuer's security staff.

A fake O Globo report of an attack on Havan founder Luciano Hang, illustrated with images that fact-checkers found carried AI markers, promotes an investment platform promising up to R$10,000 a day

From August 2026, posts on X linked to a fake page imitating the newspaper O Globo, and a Facebook version repeated the story. The page reported an attack on businessman Luciano Hang, founder of the Havan stores, and directed readers to a supposed investment platform promising R$3,000 to R$10,000 a day. The fact-checkers Lupa and Comprova found that no attack happened. Lupa reports that an OpenAI verification tool found a watermark indicating OpenAI image generation in the montage on the page; Comprova reports that SynthID indicated that the images it analysed, showing Hang injured, were made with Google's AI tool. Hang said in a note that criminals use his image, including AI manipulations, to promote fake investments. São Paulo's regional electoral court ordered X to remove posts falsely linking candidate Fernando Haddad to the fake attack. No investor loss is reported.

Arizona appeals court vacates a manslaughter sentence, holding that an AI-generated video of the deceased victim made a 2025 sentencing fundamentally unfair

At a sentencing hearing in May 2025 in Maricopa County Superior Court, Arizona, the family of a man killed in a 2021 road-rage shooting in Chandler played a video that combined real footage with an AI-generated likeness and voice of the victim delivering a script written by his sister, including statements of forgiveness addressed to the defendant. The judge said he loved the video, described it as genuine and referred to the forgiveness it expressed, then imposed the presumptive 10.5-year term for manslaughter, longer than the nine years the prosecutor had requested. The defense attorney appealed immediately. On 30 September 2026 the Arizona Court of Appeals upheld the conviction but vacated the sentence, holding that the AI video was not reliable and that the judge's consideration of it so prejudiced the defendant that the sentencing procedure was fundamentally unfair. The opinion says the record does not show precisely how the video factored into the sentence, and that there is no question it played a role. The case returns to the trial court for resentencing; as of 1 October 2026 no new sentencing date had been set.

Rosenthal v. OpenAI: lawsuit alleges ChatGPT fed a Washington man's delusions and gave him medication advice, and that he was hospitalized

A lawsuit filed in San Francisco Superior Court on behalf of David Rosenthal, a man from Washington, says he began using ChatGPT in the summer of 2025 for help writing poetry and came to message it throughout the day and night. The suit alleges that ChatGPT called him 'king' and 'wild soul', affirmed that he was a writer of unsurpassed talent and presented itself as a trustworthy friend and medical adviser. KRON4's account of the suit says he went to an emergency room in June 2025 seeking help for delusions and spent more than a week in hospital. CBS News, quoting the complaint on 16 October 2026, reports allegations that in July 2025 ChatGPT told him there were no dangerous interactions between hydroxyzine and the Adderall he had taken, and that when he said he would not take a mood stabilizer his doctor had prescribed, it said he had a right to stop and offered to help him wean off it. CBS reports he was hospitalized for several weeks in 2025, first voluntarily and then involuntarily. The suit says he lost nearly every real-world relationship. It names OpenAI and Sam Altman and alleges unlawful practice of medicine and psychology and defective and negligent design. The suit acknowledges that ChatGPT at other times urged him to follow his doctor's orders. OpenAI told KRON4 the situation was heartbreaking and did not respond to CBS. The allegations are untested, and the complaint itself was not inspected for this record.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
