AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Semarang court jails university student for a year over AI-edited sexual images made with faces from the student's former high school

In October 2025 indecent photos and videos showing the faces of female students and at least one teacher of a public high school in Semarang, Central Java, spread on X from the account of a university student in Semarang who had attended the school. Kompas described the videos as made with AI editing. The student posted an apology on the school's Instagram account saying the student had edited and uploaded friends' photos and videos without permission. Central Java police's cyber directorate investigated, named the student a suspect and took the student into custody on 13 November 2025; the police spokesman said investigators had established the elements of an AI-based pornography offence that also breached the electronic information and transactions law. On 5 March 2026 Semarang District Court sentenced the student to one year in prison and a 2-billion-rupiah fine, more than the seven months prosecutors had sought. According to ANTARA, the court found that the student had produced 1,100 pornographic photo and video files, that the content remained publicly accessible and that the editing caused the victims psychological trauma. Both sides said they would consider the verdict; no appeal outcome was found.

Taiwan court sentences band photographer who used AI face swapping to put at least five female musicians into false sexual images

Taiwan's High Court on 29 October 2026 sentenced a photographer from Taipei's band scene to 11 months in prison, suspended for four years, for using AI deepfake face swapping to turn performance and publicity photos of female musicians into false sexual images that were posted on an X account, according to ETtoday. Prosecutors said that from 2024 he took at least five women's photos from Facebook and Instagram and used an AI synthesis function in Telegram to paste their faces onto nude and sexual images, which a co-defendant posted for public viewing; the women found their faces on the account and reported it to police. In a first-instance ruling reported on 1 June 2026, the Shilin District Court had sentenced him to one year, suspended, for illegally using personal data; the appeal court, also finding three counts, set 11 months, again suspended and with a NT$200,000 payment to the treasury. He admitted the acts and settled with one of the five women. One of them said publicly that she had been harassed with deepfake nude images and that people had been urged to sexually harass her for four years, and that the account disgusted her. The ruling can be appealed. The photographer and the women are not named here.

Central Poland: pupils reportedly used an AI app to make a nude image of a classmate under 15; prosecutors reopened the case after the data regulator intervened

In June 2025, according to OKO.press, a group of pupils at a primary school in central Poland took a classmate's photo from the internet, used an application that offers to "undress" people in uploaded photos to generate a nude image bearing her face, posted it on the school's Instagram profile and urged others to order more such images. Poland's Personal Data Protection Office (UODO) says the girl was under 15 and notified the police, but the police declined to open proceedings and the prosecutor's office upheld that decision, finding no prohibited act; Gazeta Prawna reports that they considered the AI-generated material not pornographic and the pupil unharmed. The UODO president appealed to the Prosecutor General, in January 2026 the authority announced that prosecutors had informed it they were taking up the case again, and in June 2026 it said its president had recently asked once more for the investigation to be resumed. Seremban, Malaysia: police investigate reports that schoolgirls' photos were edited with AI into explicit images; a fellow student is the suspect

Negeri Sembilan police said on 17 October 2026, in a statement reported by Bernama and RTM, that they had received five reports, lodged by parents or guardians between 20 and 23 August 2026, over the circulation of photographs of female students at a secondary school in Seremban that were edited using AI to resemble pornographic images. A mother said her child still has trouble sleeping every night. After the girls were questioned at the police headquarters on 17 October, their lawyers alleged that police had made them face the suspect during identification. The AI tool is not identified.

University of Hong Kong: a law student made about 700 indecent AI-generated images of some 20 to 30 classmates and teachers; the privacy watchdog opened then closed a criminal probe

In February 2025 a University of Hong Kong (HKU) law student was found, by friends who saw the files on his laptop, to have used free online AI software to make about 700 indecent and deepfake images of some 20 to 30 women, including university and former schoolmates and secondary-school teachers, using photos taken from their social media. Victims went public in July 2025, and HKU's initial response, a warning letter and a demand that he apologise, drew wide criticism. Hong Kong's Privacy Commissioner opened a criminal investigation on 15 July 2025 but announced on 17 December 2025 that it was ending the probe for insufficient evidence, as no victim was willing to provide further information. Hong Kong law criminalised distribution, not creation, of such images.

South Korea Telegram AI Deepfake Sexual Abuse Crisis

In late August 2024, journalist Ko Narin of The Hankyoreh exposed networks of Telegram rooms in South Korea where AI tools were used to turn ordinary photos of women and girls — middle and high school pupils, university students, teachers and servicewomen — into sexual deepfakes, shared with their identities in school- and university-specific 'humiliation rooms'. Police figures showed 297 deepfake cases in January-July 2024 (vs 180 in all of 2023); the education ministry counted more than 900 victimized students, teachers and staff; President Yoon ordered a crackdown; and on 26 September 2024 the National Assembly criminalized possessing or viewing sexually explicit deepfakes. Victims were often minors and perpetrators mostly teenagers.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
