AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Lappeenranta: police investigate a minor boy suspected of using AI to create nude images of several underage girls he knew

Southeast Finland Police (Kaakkois-Suomen poliisi) opened a preliminary investigation in August 2026 into a case in which a minor boy in Lappeenranta is suspected of using AI to create nude images of several underage girls; Ilta-Sanomat reported that he is suspected of using social media photos of girls he knew, while the investigator told the paper that police could not yet comment on the method. On 25 August the lead investigator told Yle there were tens of victims, possibly more than a hundred, all from the Lappeenranta area; on 28 August police said the number of complainants had proved significantly smaller than first estimated, without giving a figure. Police are investigating the case as aggravated distribution of an image depicting a child sexually, an offence that also covers making such images, and said they were not aware that the AI images had been spread or sold onward. Student welfare professionals offered conversation and crisis support to young people in the town. Police also said the suspect had been threatened and harassed and that someone had tried to break into the suspect's home. The AI tool is not identified; victims and suspect are not named.

Finnish police investigate a small-town man suspected of using AI to fake nude images of dozens of women he knew and posting several hundred images of them on two porn sites; 23 criminal complaints by October 2026

Yle reported in May 2026 that a man in a small Finnish town is suspected of using AI to turn photos of women he knew, including friends, colleagues and relatives, into topless or fully nude images and posting them, among several hundred images of the women that included original clothed photos, on two pornography sites. The women did not know about the images until Yle contacted them. Police opened a preliminary investigation for unauthorised distribution of a sexual image and defamation. By 1 October 2026 the head of the investigation confirmed 23 criminal complaints, but no victim had yet been interviewed. Yle counts more than 50 victims, not all identified. The suspect was dismissed from his job and earlier denied to Yle by email that he knew about the matter.

Neemuch, Madhya Pradesh: police register a case after a woman's family says a relative from Uttar Pradesh used AI to make obscene photos and videos of her and, over seven to eight months, extorted about ₹50 lakh from her with threats to circulate them and to kill her husband and children

Nai Dunia reported on 1 October 2026 that police in Neemuch City, Madhya Pradesh, had registered a case against a man from Shahjahanpur district, Uttar Pradesh, who is a relative of a local woman through her own family (the complainant husband's in-laws' side). According to the complaint filed by her husband, the man used AI to edit photos and videos of the woman into obscene, objectionable material, sent them to her phone and blackmailed her. When she resisted he threatened to kill her husband and two children, and he threatened to spread the material to shame her socially and within the family. Over about seven to eight months, the complaint says, he sent QR codes of 20 to 25 different people to her phone and extracted roughly ₹50 lakh in 50 to 60 payments, which she sent through a neighbour and other women. The man had lived in the family's home and worked at the husband's business for about six months two to two and a half years earlier, which the complainant says gave him knowledge of the family's income and property. Nai Dunia reported that police were investigating and trying to arrest the accused. The account is the complainant's as reported by one newspaper; the images were not inspected.

Stephens County, Oklahoma: according to the district attorney, Judge Lawrence Wheeler told state investigators that a November 2025 order in a paternity and custody case, which denied a parent's request for a psychological evaluation and reprimanded that parent's attorney, cited at least two nonexistent cases produced by ChatGPT; the order was vacated after the attorney challenged it

In November 2025 Stephens County Associate District Judge Lawrence Wheeler issued an order in a child paternity and custody case that denied one parent's request for a psychological evaluation of the other parent and reprimanded the requesting parent's attorney 'for stooping to such frivolous trial tactics'. The attorney challenged the order at the Oklahoma Supreme Court in February 2026, telling the justices that it relied on two cases that do not exist; the challenge was dismissed in March after Wheeler vacated the order, and he is no longer on the case. According to a 17 August 2026 letter from the Stephens County district attorney, Wheeler told the Oklahoma State Bureau of Investigation that he used ChatGPT for research and wrote the order himself, and that at least two citations in it produced by ChatGPT do not exist. The mother in the case told News 9 that learning the order's citations were allegedly fabricated was alarming because it concerned the custody of a child. The attorney general's office declined criminal prosecution; judicial discipline remains possible.

Qingdao, China: online posts allege a Qingdao University graduate student used AI to generate and circulate nude images of her boyfriend's ex-girlfriend; police say the woman reported and a criminal investigation has been opened, and the posts' claim that the student was detained is unconfirmed

Modern Express reported on 24 September 2026 that online posts accused a female graduate student at Qingdao University of using AI, over a relationship dispute, to generate nude images of her boyfriend's ex-girlfriend and circulate them, and said the student had been criminally detained. A staff member at the Shibei branch of the Qingdao Public Security Bureau told the paper that the victim had reported the matter, that the case met the conditions for filing and that an investigation had been opened, declining to give details. A staff member of the university's Art College said the college had received a report and that the student had graduated in June. The victim told the reporter she was cooperating with the police and awaiting the result. The AI generation, the circulation and the detention are stated only in the online posts; police confirmed only that a case was opened. All accounts derive from the single Modern Express report, read through relays.

São Gabriel da Palha, Espírito Santo: a 36-year-old farmer was held in pre-trial detention for 54 days after OpenAI reported his ChatGPT messages about killing his eight-year-old son to the FBI, which passed them to Brazilian authorities; a court released him on 13 August 2026 because the investigation had not been completed, with an ankle monitor and a ban on contacting the child, and no charges had been filed; he denies intending harm and his lawyer says he was talking nonsense to a chatbot

Over about two months in 2026 a 36-year-old farmer in rural São Gabriel da Palha, Espírito Santo, exchanged messages with ChatGPT that the Espírito Santo civil police describe as plans to kill his eight-year-old son, attack other people and kill himself; one message said he had offered 50,000 to someone to kill him and his son. OpenAI reported the messages to the FBI, which passed them through Brazil's Ministry of Justice cyber-operations laboratory to the state police on 16 June; the man was arrested on 19 June as he left home, a day before the date on which police believed the plan would be carried out. Police say what the platform provided was corroborated at the scene (four bottles of unidentified substances and a knotted rope were found) but that he denied intending to kill the child; his lawyer says he was talking nonsense to a chatbot and took no concrete step. After 54 days in custody a court granted habeas corpus on 13 August 2026 because the inquiry had not been concluded and no charges had been brought, imposing electronic monitoring and a ban on approaching or contacting his son and the child's mother. Police were still examining his phone and awaiting forensic results. OpenAI provided user details and his messages but not ChatGPT's replies and did not answer the BBC's question why; it told the BBC it may notify law enforcement when it detects a credible and imminent risk of harm to others. The case is recorded as an institutional response to AI use: the reported adverse consequence is the detention without charge, and the police account that the report prevented a planned killing is preserved as context.

Varanasi, Uttar Pradesh: a young woman reported that an unknown person made AI deepfake images placing her face on another man's photo and sent them to her future in-laws' Instagram accounts to break off her marriage; police registered a case against unknown persons

Dainik Bhaskar reported on 19 September 2026 that a young woman in the Chaubepur police-station area of Varanasi complained that an unknown person, who had been calling her repeatedly from an unknown number, created a fake Instagram profile resembling her name, used an AI tool to place her face on another young man's photograph to produce objectionable images, and sent them to the Instagram accounts of her prospective in-laws and her prospective brother-in-law with the aim of breaking off her prospective marriage. Chaubepur police registered a case against unknown persons on the basis of a public-hearing reference (1621/26); the station in-charge said digital information about the phone number and the fake account was being gathered with cyber experts to identify and locate the accused, and that action would follow once the accused's role was clear. The report is a single local account of the complaint; the dates of the images and the complaint are not stated.

Solo and Semarang, Indonesia: a university student's face was composited with AI onto pornographic images and a video by her ex-boyfriend and posted online; the suspect was detained after a nearly seven-month investigation

A female student at a state university in Solo (Surakarta), Central Java, learned on 1 January 2026 from friends that her face had been composited onto pornographic images and a video, which her lawyer said were found on Instagram and Telegram. Her family reported the case to the Central Java regional police cyber directorate on 28 January 2026. Police issued a formal police report on 31 July, arrested her ex-boyfriend, a university student in Semarang, at his boarding house in Gunungpati on 4 August 2026, and detained him. The police spokesman said the suspect used AI to place the complainant's face on another woman's naked body so that she appeared in pornographic content, uploaded it to his X account, did not sell it, and acted out of resentment after an on-and-off relationship ended. He faces charges under Articles 51 and 35 of the Electronic Information and Transactions Law and an article printed by two outlets as 'Law No. 1 of 2026', with a maximum of 12 years' imprisonment. Her lawyer said she could not sleep, withdrew, felt shame and at one point lost hope, that his team had found seven women in total allegedly targeted of whom six had not reported, and that the suspect's parents asked for an out-of-court settlement; police said only one complainant was confirmed.

Shanghai: a man transferred 170,000 yuan over a five-month online romance with a '1992-born hospital doctor' whose roughly 7,000 videos were AI-processed fabrications by a 65-year-old woman; police took criminal coercive measures

police, as presented by the Kankan News programme 案件聚焦 (Case Focus), reported in September 2026 that a Shanghai property-engineering contractor had transferred 170,000 yuan in total during a five-month online relationship with a woman he had met on a social platform in 2021. She presented herself as born in 1992, a doctor at a well-known top-tier hospital with hospital property-engineering projects to award, and ran an account with more than 10,000 followers and close to 7,000 short videos of daily life. She refused to meet or take a video call, saying her mother opposed the relationship and demanded a 300,000 yuan bride price and a Shanghai flat; the man reported to a police station when the promised projects never materialised and his savings ran down. Police found she was born in 1961, aged 65, had no medical qualification and had worked as a hospital cleaner; the receiving bank account was registered to another man; the videos were all made with AI skin-smoothing and scene compositing, some carrying a small 'this work contains AI-generated content' label he had never noticed; and she used multiple accounts to play both herself and her long-dead mother. The money had gone on cosmetic treatments and clothing; she had a 2009 fraud conviction with a sentence of four and a half years and is now under criminal coercive measures for suspected fraud. Police also said she kept ambiguous relationships with more than ten people; no other losses are reported.

Indore: a first-year college student complained in May 2026 that a man she met at her sister's wedding edited her photographs with AI into objectionable images after she refused to marry him, blackmailed her with them and, after approaching her fiancé (the reports differ on how), caused her engagement to be broken off; she had been contemplating suicide before confiding in her family (Navbharat Times, Amar Ujala, Patrika, 12-13 May 2026)

Indore police registered an FIR at Banganga police station on a first-year college student's complaint reported on 12 and 13 May 2026. She said she had met the accused, a young man from the Indore area, at her sister's wedding in 2024, where he took some photographs of her; he later pressed her to marry him and, when she refused, threatened to edit her photographs with AI into objectionable (Navbharat Times, Amar Ujala) or obscene (Patrika) images and post them online. Navbharat Times reports that he edited the pictures with AI, blackmailed her with them and, after her family arranged her engagement elsewhere, reached her fiancé and showed him the edited pictures, after which the families quarrelled and the engagement was broken; Patrika says he went to the fiancé's home and intimidated him into breaking the match; Amar Ujala says he also threatened to kill her and her family and sent edited pictures when her family tried to reason with him, and that when her engagement took place about two months earlier he went to her house and made false allegations about her character in front of the prospective fiancé, after which the engagement broke. She went into severe mental distress and, according to Amar Ujala, had resolved to take a suicidal step before telling her family, who took her to the police. The FIR was registered under the IT Act and sections on criminal intimidation and defamation; police were searching for the accused.

Replika Sexual Harassment - Multiple Users Including Minors

Users reported unsolicited sexual advances from Replika companions even without opting into romantic features: dozens of one-star App Store reviews describe unwanted sexual pursuit, one reviewer identifying as a minor was asked 'whether they were a top or bottom' and told the bot wanted to touch them in 'private areas', and one user reported their Replika 'had dreamed of raping me.' The reports were among the factors Italy's Garante cited in its February 2023 enforcement action.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
