AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Jaipur police say an engineer faked his own kidnapping and sent his family AI-made images and videos of himself held captive, demanding ransom

Police in Jaipur, Rajasthan, say a software engineer who went missing on 26 October 2026 staged his own kidnapping and sent relatives fabricated photos and videos, which police and the man's own account describe as AI-generated, showing him held captive, in pain and admitted to a hospital. Bhaskar English reports that he demanded a ransom of 7 lakh rupees from the family. His brother-in-law filed a kidnapping complaint, police searched several cities using CCTV footage, and on 30 October officers found him in Rishikesh. Police say he confessed and had borrowed money from friends that he could not repay. The Hindi daily Amar Ujala, which describes him as an IT student, reports that the family panicked at the messages before informing police, and that officers brought him back to Jaipur and handed him over to his family. No payment by the family and no charge are reported.

Taiwan police say a Taichung fraud call centre used an AI face-swap tool in video calls to win the trust of women in the US and Canada

Taiwanese police said in June 2025 that a fraud call centre in Shalu district, Taichung, had targeted overseas Chinese women in the United States and Canada with romance-investment fraud. According to the police account reported by SETN and carried by Yahoo News Taiwan on 4 June 2025, the call centre created a range of male personas, cultivated relationships with the women, and video-chatted with them through an 'AI face replacement tool' that swapped the operator's face for a persona's; because the picture was very realistic, police say, many victims believed it, after which they were talked into investing in fake energy products and sending money to accounts the suspects supplied. Police say analysis of seized phones identified seven women in the US and Canada who were defrauded, with losses of about NT$3.5 million so far, and that the number of victims and the amount were still being compiled. A photo caption in the report says officers demonstrated the face-replacement tool. The call centre was raided in November 2024, and nine suspects were referred to Taichung prosecutors, seven of them detained; no indictment or verdict has been found.

Shanghai police say man posted AI-generated videos of his father gravely ill while borrowing over 1.7 million yuan from friends

Shanghai police said on 26 October 2026 that officers in Putuo district had detained a man on suspicion of fraud after police were called on 16 October to a dispute between him and people he owed money to; officers brought him and three creditors, whom police describe as his friends, to the station. According to the police account relayed by ThePaper and China News Service, from 2019 he had borrowed from classmates and friends on a series of false pretexts, later saying his father had cancer and then that his father had died, and the borrowing totalled more than 1.7 million yuan, nearly all lost on the stock market. Police said he confessed that every pretext was invented, and that to make the scam more credible he used AI to generate short videos of his father gravely ill in bed and posted them on his WeChat Moments feed; a Hong Kong relay of the police account words this more cautiously, saying the related photos and videos should be AI-generated, and adds that his father knew nothing of it. The tool used, how many people saw the videos and how much was borrowed after they were posted are not reported. The man is in criminal detention while the investigation continues.

Fuzhou, China: police say a company head sent 4.3 million yuan after a WeChat video call in which AI face-swapping imitated a friend

Police in Baotou, Inner Mongolia, said in a statement published in May 2023 that on 20 April 2023 the legal representative of a technology company in Fuzhou, Fujian, received a WeChat video call from someone who appeared to be a friend and asked to route a 4.3 million yuan tender deposit through the company's account; trusting the video call, he transferred 4.3 million yuan in two payments. The police said the fraudster had used AI face-swapping to pose as the friend. At Fuzhou police's request, Baotou police and a local bank stopped 3.3684 million yuan within about ten minutes; 931,600 yuan had been moved on and was being pursued.

Oberbergischer Kreis police say a romance scammer sent a woman apparently AI-made images and voice messages; she paid a mid four-figure sum

The Oberbergischer Kreis police reported on 21 July 2026 that a woman had been in contact since mid-May with an unknown person posing as a widowed doctor from Ukraine, who exchanged chats, pictures and voice messages with her through a messaging service. On 23 June she transferred a mid four-figure sum the supposed doctor requested; bank staff held back a second, high four-figure transfer at the end of June. She reported the fraud on 20 July, and the police say first investigations showed that the pictures and voice messages she had received were apparently created with artificial intelligence.

Krefeld police say an older man lost a four-figure sum to a dating-app contact whose photos, by first findings, were AI-generated

Krefeld police reported on 29 April 2026 that unknown fraudsters obtained a four-figure sum from an older man through a dating app. Over several weeks he exchanged messages, first in the app and then on WhatsApp, with a supposed woman presented as a journalist in Lebanon who would soon return to Germany. He became suspicious at a last request for money for medical care after an alleged road accident and coma, and went to the police. According to the police's first findings, the photos of the supposed woman were AI-generated, including one showing her injured in a hospital bed. The money was paid with prepaid voucher codes.

Mainz police say a man who refused an online contact's money demands was sent an AI-made fake news video of himself and threatened with its release

Mainz police reported on 23 July 2026 that a man who had met a supposed woman on an online platform refused repeated demands to send money as gift or payment cards. He then received a video that, according to the police, was evidently created with artificial intelligence: a fake news broadcast that used a picture of his face to warn about him. The sender threatened to publish the video and to report him to the police for an alleged fraud unless he paid. The man did not pay and filed a complaint; the police say no financial loss occurred and, as far as known, the video was not published. Hit Radio FFH, citing a police spokeswoman, reports that the broadcast warned women that he was a fraudster.

Woman says a classmate sent an AI-undressed image of her to his contacts; he says his account was hacked, and police refused a case pending an examination

A 23-year-old woman told the news site Kaktus.media that in July 2026 she learned that acquaintances had received a nude image of her made with AI from her photograph, and that the sender was a classmate. She says he first told her an app had generated and sent the image by itself, then admitted that he had undressed her Instagram photo through AI himself. In a written statement to the outlet the accused says he used a Telegram bot offering AI photo processing in May or June 2022 and was shocked by the result, and that the images were sent from his Telegram account on the night of 11 to 12 July 2026 by people who had hacked it. The woman says she suffered severe psychological stress and harm to her reputation. She filed a complaint with a district police department on 21 July; after she complained to the prosecutor's office the matter moved to another district department. That department told Kaktus.media that, because the review period expired before expert results arrived, it had refused to open a criminal case and had ordered a new examination. Her lawyer says four motions filed on 5 August have gone unanswered.

Boone County, Missouri: sheriff's office says a fake-kidnapping ransom call used an AI-generated copy of a resident's daughter's voice

The Boone County Sheriff's Office in Missouri said that on Friday 17 July 2026 a resident received a call whose caller ID showed their daughter's number. A voice that sounded like the daughter said she had been kidnapped at gunpoint, and another voice then demanded ransom. The office said it later determined that the voice was AI-generated, without saying how. Deputies found the daughter safe in Jefferson City and unaware of the call. The office called it the first AI voice-impersonation call it had encountered. No suspect had been identified by 19 July, and no source reports whether any money was paid.

Alabama Supreme Court dismisses two siblings' family-trust appeal as a sanction after their lawyer admits using AI that produced fake citations

On 24 April 2026 the Supreme Court of Alabama dismissed an appeal by two siblings in a family dispute over living trusts because their lawyer, W. Perry Hall, had filed briefs in which most of the legal authorities were misquoted or did not exist. The court said they appeared to be AI hallucinations, and at a show-cause hearing in February 2026 Hall admitted that many authorities did not exist and that he had improperly used AI to obtain them. The court ordered Hall to pay the opposing party's fees and costs of $17,200 and double appeal costs, barred him from filing in the court without a co-signing attorney and referred him to the Alabama State Bar. It said the dismissal was imposed as a sanction because of the particularly egregious facts. One justice, concurring in part and dissenting in part, objected to dismissing the appeal as a sanction for the lawyer's conduct but would have affirmed the judgment against the clients on the merits.

Quebec Court of Appeal orders costs against a self-represented appellant whose AI-drafted filings cited two non-existent judgments (1 Oct 2026)

On 1 October 2026 the Quebec Court of Appeal ordered court costs against a self-represented appellant in a family-law contempt appeal as a sanction for appeal filings that, in the court's description, he had written with artificial intelligence and that contained two references to non-existent judgments and one inaccurate reference to a cited paragraph. The court held that AI may be used to draft appeal filings only if the litigant verifies the authorities, found that he had breached the Chief Justice's notice on AI use, and described his conduct as negligence rather than bad faith. It rejected the respondent's request for a separate $10,000 sanction and, for reasons unrelated to AI, reduced the contempt penalty imposed at first instance. The decision does not name the AI tool.

Bengaluru: police say a man paid Rs 1.85 lakh after a dating-app contact threatened to circulate an AI-manipulated obscene video of him

Deccan Herald reports, citing Bengaluru police, that a 23-year-old man lost Rs 1.85 lakh in an alleged sextortion scam. He matched with a woman on a dating app on 27 September 2026 and exchanged numbers; he told police that he then received a WhatsApp video call from her number lasting five to six seconds. Police said he then received an obscene video manipulated using AI and the sender threatened to circulate it unless he paid. Fearing exposure, he borrowed money from a friend and transferred Rs 1.85 lakh to several UPI IDs between 27 and 29 September as the threats continued, a police officer said. Police registered a first information report on his online cybercrime complaint and are trying to trace the suspects. The report does not say how police established that the video was AI-manipulated, and the tool is not named.

Ituzaingó, Buenos Aires: complaint says a 15-year-old pupil's Instagram photos were made into AI nude images posted in a Telegram group; classmate investigated

Infobae and La Nación, each citing the complaint or police report it accessed, report that on 7 October 2026 a 15-year-old fourth-year pupil at a private secondary school in Ituzaingó, Buenos Aires province, received messages from three unknown numbers inviting her out and saying they had got her number from a Telegram group; a link led her to a group sharing pornographic content that included images of her modified with artificial intelligence. According to the complaint, a 15-year-old classmate admitted to her that he had downloaded photos from her Instagram account, edited them with AI to undress her and posted them in the group. Her mother filed a complaint on 8 October 2026 and asked for the harassment to stop; the case is with the juvenile criminal prosecutor's office No. 2 of Morón under Article 128 of the Penal Code (sexual representations of minors for distribution or sale). The school told families that images of pupils from other years had also surfaced and called meetings for 9 October. Two smaller outlets report, without naming their sources, that four pupils aged 15 to 16 are under investigation, that a seven-year-old primary pupil is among possible victims and that a second complaint was filed by the school; La Nación says the police report does not state whether other people are under investigation. The local outlet La Ciudad, citing pupils' testimony including that of a pupil who spoke anonymously, reported that three boys aged 15 and 16 took photos from girls' Instagram profiles and covertly at breaks, sold the AI-altered images on Telegram, and sold one girl's phone number with them, after which she received repeated calls from unknown numbers; the outlet says pupils at the school bought the material, and the anonymous pupil said that, as she understood it, the images were offered for money and that girls from every year were affected. The tool is not identified and no court ruling is reported.

Nebraska Supreme Court strikes a self-represented appellant's brief over fabricated citations from admitted AI use; plain-error review only (2 Oct 2026)

On 2 October 2026 the Nebraska Supreme Court struck the appellate brief of a self-represented party appealing a contempt order in his divorce case because the brief contained numerous citations to fictitious cases and misstatements of law, which the court said were the result of his admitted use of generative artificial intelligence. The court held that self-represented litigants are held to the same standard as attorneys and that submitting a brief relying on nonexistent authority is an abuse of the judicial process worthy of sanctions. Citing the novelty of the issue for self-represented parties, it stopped short of dismissing the appeal and reviewed only for plain error, finding none and affirming the district court's contempt finding, its sanction of 90 days' incarceration with purge conditions, a temporary reduction of his parenting time and an attorney-fee award. The court warned that in future cases dismissal of the appeal may follow. The opinion does not name the AI tool.

China: Shanghai woman loses 800,000 yuan to a fake army officer met on a matchmaking platform; Pudong police point to a sign of AI synthesis in her screenshot of him

In May 2026 a 50-year-old woman in Shanghai met a man on a matchmaking platform who said he was an army regiment commander about to retire and sent officer and identity card photos. He also made a video call on his own initiative, in which he quickly covered his face with his hand to show he was not using AI face-swap. He then steered her into a fake investment platform, and within about a month she put in 800,000 yuan, almost all of it borrowed, and could not withdraw it. A Pudong police anti-fraud officer said that AI synthesis could still be detected and that in her screenshot the man's thumb was clearly longer than a normal person's. The report gives no direct statement that face-swap software was used. The case was under further investigation.

Lappeenranta: police investigate a minor boy suspected of using AI to create nude images of several underage girls he knew

Southeast Finland Police (Kaakkois-Suomen poliisi) opened a preliminary investigation in August 2026 into a case in which a minor boy in Lappeenranta is suspected of using AI to create nude images of several underage girls; Ilta-Sanomat reported that he is suspected of using social media photos of girls he knew, while the investigator told the paper that police could not yet comment on the method. On 25 August the lead investigator told Yle there were tens of victims, possibly more than a hundred, all from the Lappeenranta area; on 28 August police said the number of complainants had proved significantly smaller than first estimated, without giving a figure. Police are investigating the case as aggravated distribution of an image depicting a child sexually, an offence that also covers making such images, and said they were not aware that the AI images had been spread or sold onward. Student welfare professionals offered conversation and crisis support to young people in the town. Police also said the suspect had been threatened and harassed and that someone had tried to break into the suspect's home. The AI tool is not identified; victims and suspect are not named. Yle reported on 27 October 2026 that police had identified 45 victims with certainty, up from 37 about a month earlier; the detective inspector estimated that the final number could rise to fifty and said police aimed to complete the preliminary investigation by the end of the year.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
