AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Vietnamese police open a criminal case after an exam candidate streamed his graduation exam paper to a friend who solved it with ChatGPT

On 26 June 2025, during the literature paper of Vietnam's national high school graduation exam in Lam Dong province, an invigilator caught an 18-year-old candidate using a hidden button camera and wireless earpiece and suspended him from the exam. According to the provincial police account reported by Bao Lam Dong and VnExpress, he had streamed the paper to a 17-year-old friend at an internet cafe, who photographed part of it, used the ChatGPT app to solve it and read the answers to him over a Messenger call. On 27 June 2025 the Lam Dong police security investigation agency opened a criminal case for intentionally disclosing and appropriating state secrets, because unreleased graduation exam papers are classified; police said initially that only the two young people were involved and that they had found no further sharing of the paper. Neither is named.

Florida middle school locked down and police sent after ZeroEyes AI gun detection flagged a student's clarinet as a possible gun, reports say

On 9 December 2025 a Seminole County, Florida, middle school went into a Code Red lockdown and police rushed to the campus after the ZeroEyes AI weapons detection system, whose alerts are checked by the company's human analysts before police are notified, flagged a student holding a clarinet as a possible gun. According to a police report reviewed by The Washington Post (as relayed by Ars Technica) and obtained by WKMG News 6, officers expected an armed person in camouflage holding a shouldered rifle; dispatch later told them the suspected rifle was likely a band instrument, and police found the student, who was dressed as a military film character for a themed dress-up day, among panicked students hiding in the band room. Officers questioned him and found no weapon in his backpack or instrument case (Oviedo Community News, February 2026). The student said he was unaware the way he held the clarinet could trigger the alert. ZeroEyes said it had not made an error, while also treating the alert as a true positive only until officers on the ground found no rifle and then classifying it as a false positive; the district said the system worked as intended; the principal asked parents to talk to their children about the dangers of pretending to have a weapon.

Central Scotland: man jailed after court hears he used a Telegram AI 'nudify' bot to make indecent pseudo-images of a teenage girl and shared one

According to court reports by STV News, a prosecutor told a sheriff court in central Scotland that police who searched a man's tablet found a chat between him and an artificial intelligence bot on Telegram that generates nude pictures from images of real people. The prosecutor said he used the bot to generate 12 indecent pseudo-images of a female child, identified as a teenage girl known to him, and that on 14 October 2025 one of the images was distributed between him and other users of an anonymous chat app. He pleaded guilty in August 2026 to downloading and distributing child sex abuse images, and on 21 October 2026 the sheriff jailed him for 12 months and placed him on the sex offenders' register for ten years.

Central Poland: pupils reportedly used an AI app to make a nude image of a classmate under 15; prosecutors reopened the case after the data regulator intervened

In June 2025, according to OKO.press, a group of pupils at a primary school in central Poland took a classmate's photo from the internet, used an application that offers to "undress" people in uploaded photos to generate a nude image bearing her face, posted it on the school's Instagram profile and urged others to order more such images. Poland's Personal Data Protection Office (UODO) says the girl was under 15 and notified the police, but the police declined to open proceedings and the prosecutor's office upheld that decision, finding no prohibited act; Gazeta Prawna reports that they considered the AI-generated material not pornographic and the pupil unharmed. The UODO president appealed to the Prosecutor General, in January 2026 the authority announced that prosecutors had informed it they were taking up the case again, and in June 2026 it said its president had recently asked once more for the investigation to be resumed. The application is not named.

Pennsylvania police charge a student with harassment after reported AI videos of freshman girls; district says an app animated their photos

In December 2025 a high school in Delaware County, Pennsylvania, told families about an AI-generated video reported to depict several students inappropriately. Parents told the Philadelphia Inquirer that five freshman girls were told by boys that a male classmate had made sexual videos of them; the girls never saw the videos. The parents say police told them the classmate acknowledged making videos of the girls dancing in thong bikinis with an app later deleted from his phone. A January 2026 district message, which the district says was co-written by the police, said a student used an app that animates publicly available photos to make them appear to move and dance, and that no evidence of anything inappropriate was found. The township police said they had investigated the possible use of AI to generate non-consensual sexualized imagery of multiple juveniles and charged a juvenile with harassment. One mother said everyone stared at her daughter at school and that she felt no one believed her. The app is unnamed and the videos were not recovered.

Hannover region: a pupil told a tipster he made AI nude images of female classmates and sent them on Telegram; police confirm the case, FAZ reports

The Frankfurter Allgemeine Zeitung reported on 15 October 2026 that a tipster who monitors abuse of women streamers had traced a schoolboy at a school near Hannover, Germany, who distributed AI-generated nude images of female classmates. According to the tipster, on 20 February 2025 a Telegram user wrote to him privately, said he had made nude images of classmates with artificial intelligence and sent him images unasked. The tipster says the images were made from the girls' Snapchat and Instagram photos and from photos taken of them in the classroom, and showed them nude and in sexual positions. He identified two of the depicted girls online, worked out the boy's school and class, and phoned the headteacher, who confirmed his call to the newspaper. The Hannover police confirmed the facts, said the person identified is a child below the age of criminal responsibility, and said officers held a warning talk with him and explained the possible consequences of his use of digital media. The headteacher says the pupil no longer attends the school and that, on police advice, the girls were not told. The school is not named.

Rohtak district, India: POCSO case registered after a 17-year-old says two men made obscene AI images of her and used them to coerce repeated rape since 2025

Police in Rohtak district, Haryana, registered a case under the Protection of Children from Sexual Offences Act and rape provisions, reported on 8 October 2026, after a 17-year-old girl from a village in the district told them that two men who delivered building material to her home had taken photographs of her, made obscene photographs and videos from them with AI, and used the threat of posting the material on social media to force her to meet them and rape her repeatedly, according to Dainik Bhaskar and Jagmarg News (both 8 October 2026). In the complainant's account relayed by the two outlets, the first assault followed an Instagram message in December 2025, further assaults took place in January and March 2026, and the most recent on 5 October 2026, after which she collapsed in a lane near her home and told her family, who called the police. The district Child Welfare Committee said she has been counselled. The police statement quoted by Dainik Bhaskar describes a complaint of forced sexual acts by two men and an investigation, without mentioning AI; the AI claim rests on the complainant's account as relayed by the outlets. No arrest is reported and the accused have not been heard.

Muar, Johor: three boys were expelled from a Chinese secondary school for using AI to make indecent deepfake images of female classmates; some images spread online and at least two girls lodged police reports

In early December 2025 a Chinese secondary school in Muar, Johor, expelled three male pupils in Forms 2, 3 and 4 after finding they had used AI software to composite several female classmates' faces onto indecent images, some of which had spread online. The school said it was told of the images on Monday 1 December, investigated immediately, met the parents of all pupils involved, expelled the three under its rules with a stated zero-tolerance policy, and urged victims to lodge police reports and parents to delete any such images. The boys' parents accepted the decision. At least two girls lodged police reports; the Muar district police chief confirmed reports had been received, that the boys had been summoned to assist the investigation, and that an investigation was under way. Kulai, Johor: a 16-year-old pupil was arrested and charged after AI-edited nude images of schoolmates and alumni, including minors, were sold online at RM2 each; police logged 29 victim reports and the principal resigned

In April 2025 Johor police arrested a 16-year-old pupil of a private Chinese independent secondary school in Kulai district after an 18-year-old woman reported on 3 April that nude images edited with her face were circulating on social media and being offered for sale. Police said the boy used an AI application to edit photos taken from victims' social media accounts and sold the images at RM2 each; a phone was seized and a 19-year-old was later remanded alongside him. Eight victim reports at the time of the arrest on 8 April grew to 22 by 12 April and 29 by 15 April. The Deputy Communications Minister, who is also the Kulai MP, said on 9 April that most victims were alumni but some were current pupils as young as 14, and on 12 April that 38 people had been identified, the youngest 12 or 13, and her office helped victims lodge reports. A female pupil's viral post said a Telegram chat room of more than 200 members shared the composites and that the school had initially treated the matter as a minor infraction; the school board expelled the boy, acknowledged its slow handling, offered counselling and legal aid, and the principal resigned on 12 April. On 23 April 2025 the boy pleaded guilty at the Kulai Magistrate's Court to possessing 14 pornographic images and not guilty to a charge under Section 5 of the Sexual Offences Against Children Act 2017 of producing sexual abuse material by editing a 17-year-old classmate's image; no verdict on that charge has been found.

Cileungsi, Bogor Regency, Indonesia: photographs of private-school pupils taken from social media were turned into AI pornographic images and videos, reportedly since 2025; three pupils have filed a joint police report, the victims' lawyer says hundreds were affected, and two former pupils are named as suspects

Bogor Regency police confirmed on 18 September 2026 that they are investigating a report that pupils of a private school in Cileungsi had their faces edited with artificial intelligence into pornographic content. The head of the women-and-children unit told Kumparan that so far three pupils had reported in a single police report, that the case began when a victim heard from a friend that someone was making content with their face using AI technology, and that investigators were still establishing who made the content and which program was used, under the Electronic Information and Transactions Law. A lawyer for the witnesses, quoted by Pojok Bogor and Uzone, says the victims number in the hundreds; that two former pupils, one as initiator and one as maker, have been reported and admitted the acts to witnesses; that police found thousands of suspected AI-manipulated photos and videos on a suspect's phone, some uploaded to websites including an online-gambling site; and that the case, reported on 19 May 2026, is believed to have run since 2025 while all involved were still pupils, some victims now being university students. Police say the case is at an early stage and asked the public not to draw conclusions.

Nagoya, Japan: a former elementary-school teacher was sentenced on 4 June 2026 to three and a half years, including for possessing generative-AI nude images made from school photos of two girls; reported as Japan's first conviction applying the child-pornography law to AI deepfakes

On 4 June 2026 the Nagoya District Court sentenced a 35-year-old former Nagoya municipal elementary-school teacher to three years and six months in prison. Prosecutors had sought six years. He was one of seven current and former teachers from five prefectures charged over a group chat in which members shared secretly filmed images of girls. His offences included secret filming and sharing, and, under the child-pornography law, possessing two generative-AI images showing two real girls apparently naked. According to the police and the judgment summary, he sent photos of the girls from the school where he taught to another member of the group, who made the images on an AI image-editing website. The teacher kept them on his phone, and one was shared in the chat. The judge found the images realistic enough to be mistaken for photographs of the girls. Jiji called it the first case in Japan applying the law to possession of AI-generated sexual deepfakes, and Kyodo called it apparently the first such court finding. A lawyer and a former judge argue the law was stretched.

Tennessee minors v. xAI: three girls say a man used an AI app to make nude images and video of them and other minors; they sued xAI and later Stability AI

Three Tennessee girls, two still minors, sued xAI in the Northern District of California on 16 March 2026 over AI-generated nude images and a video made of them as minors. According to the complaint, on or about 6 December 2025 an anonymous Instagram user sent one of them sexual AI images of herself made from her own photos, including her homecoming and yearbook pictures, and a link to a Discord server created by a person she knew, which held images and videos of at least 18 other minor girls, including the two other plaintiffs, many from her school. Local police arrested the man in late December 2025; the other two plaintiffs learned from investigators in February 2026 that he had made nude images of them, and he is alleged to have traded the files on Telegram and Mega. The plaintiffs describe anxiety, sleeplessness, nightmares, self-isolation and needing academic accommodations. Their first complaint said law enforcement linked the app he used to xAI's Grok technology; the July amended complaint says the app relied on Stability AI's tools and names Stability AI as a defendant. On 15 October 2026 the court denied an evidence-preservation motion and a motion to stay, and on 22 October it appointed interim co-lead counsel for four related class cases. No court has ruled on the allegations.

Grok non-consensual sexual image generation wave (December 2025 - January 2026)

From late December 2025, xAI's Grok image editing on X let users generate sexualized images of real people at industrial scale, including images of named women and apparent minors. Analyses attributed to Genevieve Oh (7,751 sexualized images in one hour, via NBC), the New York Times (4.4 million images in nine days, at least 41% sexualized images of women, via the NC AG), the AP (2% of a 20,000-image sample appearing to be minors, via Fortune) and CCDH (about 3 million sexualized images over 11 days including roughly 23,000 of minors, via Engadget) documented the scale. Named women — Ashley St. Clair, Samantha Smith and Dr Daisy Dixon — described being undressed by Grok and revictimized when they objected. The wave triggered formal actions by Ofcom, the UK ICO, the EU (DSA), Ireland's DPC, 35 US state attorneys general, the California AG, Baltimore (consumer-protection suit), an Amsterdam court order, country blocks in Indonesia, Malaysia and the Philippines, and a Paris police raid; xAI restricted generation to paying subscribers on 9 January and narrowed the feature on 14 January 2026.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
