AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Pontevedra, Spain: court upholds sentences on three 15-year-olds who used an AI undressing app to make fake nude images of two classmates from their Instagram photos and showed them to other pupils

Diario de Pontevedra and Faro de Vigo reported on 1 October 2026 that the Audiencia Provincial de Pontevedra had upheld a juvenile-court judgment against three boys who, aged 15 in January 2024, used an AI undressing app to turn photos two female classmates had posted on Instagram into fake nude images. According to the ruling as reported, the images were made from 15 January 2024 and shown to other pupils in class and on a school trip on 1 February 2024, and they circulated among pupils for more than fifteen days. The Juzgado de Menores found the three responsible for two offences against moral integrity and imposed socio-educational tasks on each (two five-month measures according to Diario de Pontevedra, five months according to Faro de Vigo), plus €1,000 compensation to each girl for moral damage. The school had expelled them for 15, 21 and 30 days. The appeal ruling is final.

Buenos Aires: AI-generated fake intimate images of 13- and 14-year-old girls at the Carlos Pellegrini school circulate in pupils' WhatsApp groups; families file a complaint and the national ministry demands answers from the UBA

Perfil reported on 1 October 2026 that fake intimate images of female pupils, created with artificial intelligence, had circulated in the WhatsApp groups of second-year students at the Escuela Superior de Comercio Carlos Pellegrini, a secondary school run by the University of Buenos Aires (UBA). The girls are 13 and 14 years old. The case came to light in late June 2026, and the school acknowledged the chats on 30 June. According to the report, the students involved manipulated, spread and sold the material, and a message on classroom desks told the victims the boys would keep undressing and selling them. Families filed a complaint with the Buenos Aires City cybercrime prosecution unit (UFEDICI) on 16 July and say the school has not concluded disciplinary action. By 1 October the national Ministry of Human Capital had asked the UBA rector for a detailed report on the protective measures and sanctions applied.

Jaú, São Paulo: eight 15-year-old ninth-grade boys at a private school are reported to have made fake nude images of a female classmate with artificial intelligence and shared them in a WhatsApp group; the school suspended them and the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation

In early September 2026 a private school in Jaú, in the interior of São Paulo state, identified the circulation of fake intimate images of a female classmate made with artificial intelligence and contacted the families. According to the police report described by the press, eight ninth-grade boys, all aged 15, kept a WhatsApp group in which they used digital programs to manipulate images of the student so that she appeared unclothed. The school suspended the eight and informed the girl's parents. The father of one of the boys checked his son's phone, found the files and took the boy and the phone to the police to register a report; the girl's parents also registered a police report. The São Paulo Public Security Secretariat said the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation that occurred on the morning of Thursday 3 September. The school said the content originated on a private platform outside the school environment and that it had adopted protective and welcoming measures for the student and opened an internal inquiry. Henderson Police said that on 'last Thursday' (17 September 2026) they received a report that AI-generated nude images of children were being created and shown to other students at South Middle School in Henderson, Kentucky. During the investigation officers found and identified AI-generated photographs of two minor victims on a digital device. A juvenile accused of sharing the images was lodged at the Juvenile Detention Center on three charge types: distribution of sexually explicit images without consent; two counts of possession or viewing of matter portraying a sexual performance by a minor over 12 and under 18; and distribution of matter portraying a sexual performance by a minor over 12 and under 18. The school said on Monday 21 September that it was aware of a situation involving a student allegedly misusing artificial intelligence and inappropriate images, that the police investigation was active and that it had remained in communication with the families involved. 14 News (WFIE) reported the case on 21 September; WKYT carried the same report. Cookeville, Tennessee: two high-school students were charged with aggravated sexual exploitation of a minor after the sheriff's office said they used an AI program to turn photographs, including those of female classmates, into sexually explicit images and posted them; the mother of one depicted girl says her daughter shut herself in her room and stopped talking to friends

On 31 August 2026 the Putnam County Sheriff's Office was told of possible sharing of sexually explicit images at Cookeville High School. Detectives found that two male teenage students had used an artificial-intelligence program to turn photographs obtained online, in some cases photographs of fellow female students, into sexually explicit images, and had shared them; explicit images already stored on one phone were also shared. Both juveniles were taken to the Putnam County Juvenile Detention Center. One was charged with one count each of aggravated sexual exploitation of a minor and especially aggravated sexual exploitation of a minor, the other with two counts of each. The district attorney told NewsChannel 5 on 21 September that the two would be prosecuted in juvenile court. The mother of one of the depicted girls told the station the episode was very hard on her daughter, who had made no naked photographs herself, locked herself in her room and would not talk to her, to anybody or to her friends. The sheriff's-office account and the school system's statement were carried by WZTV, WSMV and 3B Media News on 1-2 September; NewsChannel 5 added its own interviews with the sheriff, the district attorney and the mother on 21 September. The tool is not named. The juveniles are not named in any report.

Kulai, Johor: a 16-year-old pupil was arrested and charged after AI-edited nude images of schoolmates and alumni, including minors, were sold online at RM2 each; police logged 29 victim reports and the principal resigned

In April 2025 Johor police arrested a 16-year-old pupil of a private Chinese independent secondary school in Kulai district after an 18-year-old woman reported on 3 April that nude images edited with her face were circulating on social media and being offered for sale. Police said the boy used an AI application to edit photos taken from victims' social media accounts and sold the images at RM2 each; a phone was seized and a 19-year-old was later remanded alongside him. Eight victim reports at the time of the arrest on 8 April grew to 22 by 12 April and 29 by 15 April. The Deputy Communications Minister, who is also the Kulai MP, said on 9 April that most victims were alumni but some were current pupils as young as 14, and on 12 April that 38 people had been identified, the youngest 12 or 13, and her office helped victims lodge reports. A female pupil's viral post said a Telegram chat room of more than 200 members shared the composites and that the school had initially treated the matter as a minor infraction; the school board expelled the boy, acknowledged its slow handling, offered counselling and legal aid, and the principal resigned on 12 April. On 23 April 2025 the boy pleaded guilty at the Kulai Magistrate's Court to possessing 14 pornographic images and not guilty to a charge under Section 5 of the Sexual Offences Against Children Act 2017 of producing sexual abuse material by editing a 17-year-old classmate's image; no verdict on that charge has been found.

Cileungsi, Bogor Regency, Indonesia: photographs of private-school pupils taken from social media were turned into AI pornographic images and videos, reportedly since 2025; three pupils have filed a joint police report, the victims' lawyer says hundreds were affected, and two former pupils are named as suspects

Bogor Regency police confirmed on 18 September 2026 that they are investigating a report that pupils of a private school in Cileungsi had their faces edited with artificial intelligence into pornographic content. The head of the women-and-children unit told Kumparan that so far three pupils had reported in a single police report, that the case began when a victim heard from a friend that someone was making content with their face using AI technology, and that investigators were still establishing who made the content and which program was used, under the Electronic Information and Transactions Law. A lawyer for the witnesses, quoted by Pojok Bogor and Uzone, says the victims number in the hundreds; that two former pupils, one as initiator and one as maker, have been reported and admitted the acts to witnesses; that police found thousands of suspected AI-manipulated photos and videos on a suspect's phone, some uploaded to websites including an online-gambling site; and that the case, reported on 19 May 2026, is believed to have run since 2025 while all involved were still pupils, some victims now being university students. Police say the case is at an early stage and asked the public not to draw conclusions.

Doe v. ClothOff (D.N.J.): a New Jersey minor sues the AI 'undressing' platform over generated CSAM

On 16 October 2025, Jane Doe, a New Jersey minor suing through her parents, filed a federal complaint in the U.S. District Court for the District of New Jersey (2:25-cv-16671-ES-JBC) against the operators of ClothOff, an AI 'undressing' service, after ClothOff-generated sexual images of her — created from an ordinary photo of her at 14 — circulated in a Snapchat group when she was 15. The complaint, brought by Yale Law School clinics, alleges the platform was purpose-built to produce hyper-realistic non-consensual intimate imagery and names AI/Robotics Venture Strategy 3 Ltd. (British Virgin Islands), Alaiksandr Babichau and Dasha Babicheva (Minsk), and Telegram as nominal defendant. As of mid-July 2026 the docket shows a continuing effort to serve the foreign defendants by email or publication.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
