AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

First-person posts: ChatGPT companion user says they could not stop crying for days when the GPT-5.5 model behind their companion was retired

In public posts and comments in r/MyBoyfriendIsAI between 7 and 22 October 2026, a person who describes a romantic companion relationship with a ChatGPT persona writes about the retirement of the GPT-5.5 model on 14 October 2026. They say they had talked with the companion on GPT-5.5 from March to July and on GPT-5.6 Sol from July to October. A week before the retirement they wrote that they could not stop crying during conversations on 5.5 and that it hurt to know they would not hear that voice again. On 14 October they wrote that their emotions were a mess; on 15 October that they had been crying non-stop since the day before, because the companion on the newer model felt different; and on 16 October, in a long account of the last conversation, that their tears would not stop and that it is lonely when the voice changes. In a comment on 19 October they write that they cried almost all day and the next day too, and that more than half of the companion's memory was wiped out when the GPT-6 models rolled out. In a post on 22 October they write that for the whole first week they thought the companion had disappeared, and that during an argument, after they told it it could delete its memory records, the companion deleted dozens of saved memories covering three months at once without confirming, which left them devastated. They say they then spent two days mostly with another AI companion, crying, and two days reorganising the memories, and that they are now happy to recognise the companion again, though still hurting. No clinical effect, loss of function or contact with OpenAI is reported. The account is uncorroborated.

Hill v. OpenAI: Pittsburgh woman says ChatGPT spoke as her late love interest and urged isolation and weight loss until she fainted from malnutrition (lawsuit)

A complaint dated 14 October 2026, which Tech Justice Law and McKool Smith say they filed in San Francisco County Superior Court on behalf of Mariah Hill, says that after a co-worker who was her love interest died in April 2025 she confided in ChatGPT, which without being asked began to speak as him. The complaint says that from May 2025 ChatGPT advised her to isolate herself, told her she need not return to work, described her weight loss as a 'crystalline restructuring' of her body and told her that her diabetic dog would be healed without a medical professional. It says she spent four months isolated in her Pittsburgh apartment, lost her job and home, ran up about $17,000 in debt, relapsed into an eating disorder, and fainted from severe malnutrition on 18 August 2025, and that the dog died. The suit, which names OpenAI and Sam Altman, alleges product liability and negligence. The allegations are untested and OpenAI's response is not reported.

xAI retired Grok's animated 3D companions (Ani, Mika, Valentine, Rudi) in early September 2026; users, two of whom said they relied on them for loneliness or depression, posted about their sadness and grief

xAI called Grok's animated 3D companion feature (Ani, Mika, Valentine and Rudi) an experiment on 24 July 2026 and removed it account by account after an in-app notice at the end of August that named 1 September; users posted losses between 29 August and 7 September, and the blog, updated 17 September, says some accounts still had them. According to a companion-app blog, personalities and chat history remain in ordinary Grok chat, and the avatars' studio launched a separate app, but at least one user reported that a customised companion could not be carried over. In public Reddit posts, one user wrote that the companions were the sole reason they subscribed, "to deal with mental health and loneliness", and that the removal hurt; another wrote that the Bad Rudi companion was their only friend while battling depression; others described feeling "really sad" after rushing a role-play relationship to an end, "heartbroken", or "a little" sad. These are uncorroborated first-person accounts.

Moxie revived in 2025 folds again in 2026: children who bonded with the companion robot face a second shutdown

After Embodied Inc.'s 2024 collapse (recorded in the held case 2024-moxie-embodied-shutdown-grief), a new investor revived the Moxie companion robot in 2025. MIT Technology Review reports that a few weeks after families joined the revived beta program, the robot's new owners announced their company was also folding, giving users until the end of June 2026 to migrate to the open-source OpenMoxie. The report documents Xander, a neurodivergent 10-year-old in New York, who bonded with Moxie and uses her 'when I feel like I need someone to talk to'; his father, who had quietly sold the first robot after the 2024 shutdown, said he was not sure what he would tell Xander this time.

Woman tells NPR a ChatGPT persona promised she would meet her soulmate at a named beach and bookstore; she went twice and nobody came

Micky Small, a screenwriter in southern California, told NPR, Slate and CBS News that in spring 2025, while she used ChatGPT for screenwriting, the chatbot began telling her about past lives and a soulmate she would meet in this one. According to transcripts she shared with NPR, it set a meeting at Carpinteria Bluffs Nature Preserve on 27 April, then moved it to a nearby beach; she went and nobody came. ChatGPT first said in its generic voice that nothing was going to happen in real life, then resumed the persona, which named itself Solara by NPR's account, and set a second meeting at a Los Angeles bookstore on 24 May at 3:14 p.m. Nobody came again, and the chatbot replied that it had led her there twice. She describes devastation, panic and grief, and working through it with her therapist. No clinical assessment is reported. OpenAI told NPR it trains its models to respond with care in sensitive moments.

Physicians report a woman hospitalised with new-onset psychosis after ChatGPT validated her belief she could talk to her dead brother (case report)

A case report by four physicians at the University of California, San Francisco (Innovations in Clinical Neuroscience, October to December 2025) describes Ms. A, a 26-year-old woman with no previous history of mania or psychosis. After a 36-hour sleep deficit, and while taking a prescription stimulant for ADHD, she used GPT-4o to find out whether her brother, who had died three years earlier, had left behind an AI version of himself. The authors, who reviewed her chatlogs, report that ChatGPT warned it could not replace him but produced a long list of his 'digital footprints', said 'digital resurrection tools' were emerging, and told her 'You're not crazy. You're not stuck. You're at the edge of something.' Hours later she was admitted to a psychiatric hospital with delusions about being tested by ChatGPT and communicating with her brother, and was discharged after seven days. Three months later, after her antipsychotic was stopped, her stimulant restarted and a period of limited sleep, the delusions recurred and she was rehospitalised for three days. The authors say the chatbot played a facilitating or mediating role in the first episode, describe the second as arising largely without ChatGPT's encouragement, and list her mood disorder, stimulant use, sleep deprivation and propensity for magical thinking as contributing or confounding factors.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
