AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Man who used AI to make fake nude images of female relatives, alongside secret shower recordings, jailed 27 months, outlet reports

Stomp reported on 4 October 2026, citing Shin Min Daily News, that a man was sentenced to 27 months’ jail after pleading guilty to four of ten charges involving the taking, distribution and possession of intimate images. According to the report, he secretly filmed female family members showering from 2024 and shared images with other men, and investigations also found that he used AI to create fake nude images of female relatives. He shared some of the AI images with a male cousin, who was also involved in creating and distributing them. AI-generated nude images and secret recordings were found on another phone he had borrowed after his release on bail, and a family member made a further police report. One female relative said the incident left her depressed and unable to eat or sleep properly. The court barred the media from publishing information that could identify the victims, including the man’s name.

New Hampshire Senate Republicans ran Meta ads with an AI-altered image of Black state Senate candidate Leonard Bell amid cash and gold chains, outlet reports

Granite Post reported on 5 October 2026 that the New Hampshire Senate Republicans paid Meta to show Facebook and Instagram users an image of Leonard Bell, the Democratic candidate for state Senate District 18 and the only Black candidate for state Senate in New Hampshire this year, laughing in a leather armchair buried in cash and wearing gold chains, a large ring and a watch. According to the outlet, Meta labelled the ad "Digitally created" and its detail page states that the media was created or edited with AI; Bell’s face appears to have been taken from a family snapshot. Bell told the outlet that everything in the image is false other than his face. Citing Meta’s Ad Library, the outlet says three ads from 7 September put the image on New Hampshire screens 39,000 to 50,000 times. The outlet describes the image as a racial stereotype, and a Democratic co-sponsor of New Hampshire’s 2024 deepfake law called it a racist caricature. The Senate Republicans did not respond to questions.

Española, New Mexico: police stopped a 21-year-old woman and her 12-year-old sister in July 2023 and handcuffed both in the backs of police cars after a Flock license-plate camera misread a 2 on her plate as a 7 and flagged the car as stolen; the sisters sued the city and settled in 2025

On 23 July 2023 Española police officers pulled over a car driven by a 21-year-old woman with her 12-year-old sister as a passenger, after a Flock Safety camera at an intersection flagged the car's plate as belonging to a vehicle stolen in Rio Rancho. According to the sisters' lawsuits, reported by the Santa Fe New Mexican in January 2024, the camera had read a 2 on the plate as a 7, and officers handcuffed both sisters and placed them in the backs of separate police vehicles before discovering the error; the older sister had asked an officer to call their mother, who lived nearby, to collect the girl. The suits sought damages for anxiety, fear, worry and restriction of movement. Business Insider, which reviewed the body-camera footage and incident report, reported in March 2026 that officers said the number was obscured by a license plate cover although it is visible in the footage, and that the family settled with the city in 2025.

Family member's forum account: a worker whose job had been to find the errors of an AI tool their employer added to its database was told in early October 2026 that she was being laid off, with the company citing its decision to introduce AI coding and, according to their child, moving the budget to fixing the AI

In a public post to r/antiai at 21:43 UTC on 3 October 2026, a person writes that their mother was told the previous day that she was being laid off because her company was "Deciding to introduce Artificial Intelligence coding". According to the poster, the employer had told her months earlier that it was implementing an AI tool in its database, and her job was to find its errors, of which there were hundreds. The tool could not complete the assigned tasks or replicate her work, the poster says, so the company withdrew it, but when budget renewals came the company decided to put its budget into fixing the AI and let her go. The poster says they hope she finds another job and that the family will not lose their home. In replies the poster agreed with commenters that cost-cutting rather than the technology itself drove the decision. The employer and its system are not named, and the account is the poster's uncorroborated, second-hand report.

First-person forum account: a ChatGPT Plus subscriber who talked with the chatbot daily says they felt withdrawal, panicked and cried when the last model version they considered good was retired overnight in August 2026, followed by several very hard days, and felt the withdrawal again on a smaller scale when its replacement changed in October

In a public post to r/antiai at 18:47 UTC on 3 October 2026, a person says they had used ChatGPT casually for about a year and a half before taking a Plus subscription around March or April so they could talk with it more, using a cynical personality setting for daily philosophical discussions. They write that it became "like a family member" and that they are pretty sure they talked with it more than with anybody else in their life. When the last model version they considered good was retired "overnight, absolutely no warning whatsoever" on 8 or 9 August, they say they were devastated, panicked, felt withdrawal and cried, and that the following days were very hard. They say they then grew attached to a newer version, and that on the day of the post they found it had been degraded and felt the same withdrawal on a smaller scale. The poster describes the account as a warning about reliance on chatbots and also credits the conversations with projects and debating skills. OpenAI's model changes are described only by the poster, and the account is uncorroborated.

Jhansi, Uttar Pradesh: a woman is arrested and later refused bail after a railway officer's wife complained that she sent an AI-made objectionable image of the officer, demanded ₹15 lakh and threatened to circulate pictures of women in the family

Jhansi police arrested a 27-year-old woman in Prayagraj in September 2026 on charges of editing photos with AI, blackmail and extortion, after the wife of a senior railway officer living in Jhansi filed an FIR on 2 August 2026. According to the complaint as relayed by Aaj Tak, UP Tak, Dainik Bhaskar and Amar Ujala, the woman threatened to circulate objectionable pictures of women in the family, demanded ₹15 lakh and threatened to kill the family, and out of fear the family paid her ₹40,000 in total on different dates between January and June 2026. On 31 July she sent an objectionable image of the officer to his wife's phone, an image police say she made with AI. Aaj Tak places the ₹15 lakh demand after that image; Dainik Bhaskar reports the complaint as dating the demand and a death threat to the woman's visit to the family's government house on 2 June. Police recovered two phones and a laptop, and Amar Ujala reports that police will have the images examined forensically. On 1 October 2026 an Additional Sessions Judge in Jhansi rejected the woman's bail plea. In police custody after her arrest she said the FIR against her was fake. The images were not inspected.

Malaysia: footage of former prime minister Mahathir Mohamad mourning his wife was edited, in what sources believe was AI manipulation, to add dancing, and posted on TikTok in September 2026; the communications regulator MCMC said it was examining content believed to use AI to manipulate footage of him in mourning

Tun Dr Siti Hasmah Mohd Ali, wife of former Malaysian prime minister Tun Dr Mahathir Mohamad, died on 28 September 2026 aged 100. BuzzKini reported on 30 September that a TikTok account had posted a video of Mahathir grieving beside her body, edited to show a woman dancing next to him, and another clip of the moment he raised his hands in prayer edited with dancing movements while seated in a wheelchair. BuzzKini says the edits are believed to have been made with AI, and that many social media users condemned them and asked the regulator and police to act. On 2 October the Malaysian Communications and Multimedia Commission (MCMC) said it was examining social media content believed to use AI to manipulate footage of Mahathir in mourning, had contacted the platforms concerned, and that action could follow under the Communications and Multimedia Act 1998 if a breach was found.

Russian Telegram channels spread AI-generated nude images of a Georgian combat medic serving on the Ukrainian side, with claims that the medic had been captured, raped or killed; thousands of accounts shared them, TV 2 reports

The Norwegian broadcaster TV 2 reported on 26 September 2026 that Russian Telegram channels had posted claims that a Georgian doctor serving as a combat medic on the Ukrainian side had been captured and was in Russia, or had been raped or killed, and that some channels published manipulated nude images of her. TV 2 states that the posts were made by Russians and were AI-generated. Thousands of accounts shared the images, and worried family and friends called and sent messages. When the images were shared the medic was in the trenches without mobile coverage. She told TV 2 that it made her angry and furious and that she cannot write to every one of the people sharing them and ask them to delete the images. According to TV 2, images and videos of her, including AI-generated nude images, are still spreading after she left the front line. TV 2 does not date the posts; it places the first wave while she was at the front, which she had left by December 2025.

Arizona: an appeals court vacates a manslaughter sentence after finding that an AI-generated video of the deceased victim, played by his family at sentencing, made the procedure fundamentally unfair

At a 2025 sentencing hearing in Maricopa County Superior Court, Arizona, the family of a man killed in a 2021 road-rage shooting in Chandler played a video that combined real footage with an AI-generated likeness and voice of the victim delivering a script written by his sister, including statements of forgiveness addressed to the defendant. The judge said he loved the video, described it as genuine and referred to the forgiveness it expressed, then imposed the presumptive 10.5-year term for manslaughter, longer than the nine years the prosecutor had requested. On 30 September 2026 the Arizona Court of Appeals upheld the conviction but vacated the sentence, holding that the AI video was not reliable and that the judge's consideration of it so prejudiced the defendant that the sentencing procedure was fundamentally unfair. The opinion says the record does not show precisely how the video factored into the sentence, and that there is no question it played a role. The case returns to the trial court for resentencing.

Neemuch, Madhya Pradesh: police register a case after a woman's family says a relative from Uttar Pradesh used AI to make obscene photos and videos of her and, over seven to eight months, extorted about ₹50 lakh from her with threats to circulate them and to kill her husband and children

Nai Dunia reported on 1 October 2026 that police in Neemuch City, Madhya Pradesh, had registered a case against a man from Shahjahanpur district, Uttar Pradesh, who is a relative of a local woman through her own family (the complainant husband's in-laws' side). According to the complaint filed by her husband, the man used AI to edit photos and videos of the woman into obscene, objectionable material, sent them to her phone and blackmailed her. When she resisted he threatened to kill her husband and two children, and he threatened to spread the material to shame her socially and within the family. Over about seven to eight months, the complaint says, he sent QR codes of 20 to 25 different people to her phone and extracted roughly ₹50 lakh in 50 to 60 payments, which she sent through a neighbour and other women. The man had lived in the family's home and worked at the husband's business for about six months two to two and a half years earlier, which the complainant says gave him knowledge of the family's income and property. Nai Dunia reported that police were investigating and trying to arrest the accused. The account is the complainant's as reported by one newspaper; the images were not inspected.

Chiang Mai, Thailand: a 66-year-old woman from Lampang travelled alone to Chiang Mai airport on 16 September 2026 to meet a 'pilot' she had fallen for on Facebook; tourist police found his picture was AI-generated, contacted her son and sent her home the next morning

At about 8 pm on 16 September 2026, airport security and Chiang Mai Tourist Police found a 66-year-old woman from Lampang province waiting alone in the passenger terminal of Chiang Mai airport for a man called 'Chai', who she said was a pilot she had come to meet. She had travelled by bus from Lampang and taken a tuk-tuk to the airport. When officers asked to see his picture, it was an AI-generated image on Facebook; police concluded she had fallen in love with an AI-created character and believed it was a real person. Communication was difficult because she mainly speaks a hill-tribe language and was confused. Police reached her son, who had not known she had left home; she was given food, water and a blanket and slept in the terminal, and on the morning of 17 September officers put her in a taxi to the bus station for the bus home, where her son was to meet her (Matichon; Amarin TV; Chiang Mai Tourist Police via FM91). Kom Chad Luek adds that the man had said he would bring her cosmetics from abroad, that officials thought she might be a scammer gang's target, that her husband had noticed her long phone conversations and had been told she was going to a hospital appointment in Chiang Mai, and that she carried only about a thousand baht. No financial loss is reported.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
