AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Surat, India: a deepfake video of Rajya Sabha MP Govind Dholakia was used to run an 'old coin' buying fraud; two men arrested and nine cybercrime complaints linked to their accounts

Surat Cyber Crime Police said a publicly available video of Rajya Sabha MP and diamond industrialist Govind Dholakia was manipulated with AI, its audio replaced by a voice-over in which he appeared to say that people holding old coins or currency notes could contact a number and receive lakhs or crores of rupees. The video circulated on Facebook and Instagram alongside fake 'Old Coin Company' profiles using his photos; people who responded were asked to pay Rs 1,000 to 1,500 by UPI as processing or registration fees and were told of a fabricated Reserve Bank of India warning to win their trust. After members of the public contacted Dholakia, his organisation SRK complained to police. Investigators traced the operation to Nuh in Haryana and arrested two men, aged 25 and 30, from Thathaula village; ten bank accounts were linked to them, and nine complaints on the National Cybercrime Reporting Portal from across India, totalling about Rs 9.84 crore, were connected to those accounts.

India: AI deepfake videos of Tamil Nadu Chief Minister Vijay offering 'financial assistance' were used to defraud people in Madhya Pradesh and other states; a Rajasthan man was arrested

In 2026 AI-generated videos showing a simulated Tamil Nadu Chief Minister C. Joseph Vijay speaking in a fabricated Hindi voice circulated on Facebook and other platforms, urging viewers to contact a WhatsApp number to receive money for education, medical treatment and other needs. The Tamil Nadu Crime Branch-CID detected the videos during routine monitoring, registered a case on 1 September 2026 under the Bharatiya Nyaya Sanhita and the Information Technology Act, and traced the scheme to Rajasthan and Madhya Pradesh. On 12 September a 28-year-old man from Alwar district was arrested with Rajasthan Police, brought to Tamil Nadu on a transit warrant and remanded in judicial custody; a phone, SIM cards and a CPU were seized. Investigators traced a man in Madhya Pradesh who had lost money, and said people in several states were cheated. The CB-CID is also examining similar videos posted from Facebook accounts suspected to be run from Pakistan.

Ards and North Down, Northern Ireland: a person lost £250,000 to an investment fraud that began with an AI-generated video of a well-known financial personality

On 7 September 2026 the Police Service of Northern Ireland (PSNI) said a person in the Ards and North Down area had lost £250,000 in what it called a 'sophisticated and substantial' investment fraud. The victim first saw what appeared to be an AI-generated online video featuring a well-known personality from the financial sector promoting an investment with significant returns. After investing a small amount the victim was contacted on WhatsApp and encouraged to pay more, persuaded to open multiple online accounts, to allow the fraudsters remote access to a computer 'to assist with the investment', to borrow money to keep investing, and finally to make further payments to 'release' funds supposedly held in the investment. The PSNI did not name the personality; BBC News NI noted that fabricated endorsements of financial experts such as Martin Lewis, Peter Jones and Steven Bartlett circulate online.

New York: the Manhattan District Attorney seized 12 websites selling AI-generated sexual deepfakes of about 1,200 real people

On 14 September 2026 Manhattan District Attorney Alvin Bragg announced the seizure of 12 domain names used to disseminate, publish and sell non-consensual AI-generated sexual 'deepfake' videos. His office said individuals under investigation had used AI image- and video-creation tools to turn photos and videos of approximately 1,200 real people, overwhelmingly women, into hyper-realistic sexual imagery without consent. The victims included actors, politicians, athletes, musicians, activists and social-media influencers. WIRED and outside researchers reported that many videos reposted content from the defunct MrDeepFakes platform and that dozens of politicians appeared on the sites; investigations into who ran them are ongoing.

Miami: scammers cloned immigration attorney Ángel Leal's face and voice with AI to defraud immigrants seeking legal help

From around October 2025, criminals used AI to clone the image, voice, name, signature and firm logo of Ángel Leal, a long-established immigration attorney in Doral, Florida. They created fake videos, look-alike websites, forged contracts and even fabricated Zoom 'immigration hearings' to persuade immigrants to pay for legal services Leal never provided. Immigrants were defrauded of money, and some believed their cases were progressing when they were not; Leal received one to two defrauded callers a day, changed his phone number, hired an anti-piracy firm that removed thousands of fake profiles and videos, and reported the fraud to local police and federal authorities.

San Jose: police officer fired after passing Flock license-plate-reader location data to a relative accused of domestic violence, who used it to find his accuser

San Jose's police chief announced on 11 September 2026 that an officer had been fired after accessing the department's Flock Safety automated license-plate-reader data on his personal phone and giving a vehicle's location to his cousin, a suspect in a domestic-violence case, who used it to locate the woman accusing him. The chief called it 'a betrayal of the public trust', said the officer was placed on leave, criminally investigated and referred to the district attorney, and that the case was submitted to California's peace-officer standards commission for decertification; neither the officer nor the cousin has been charged. The department has since barred access to the system from personal devices and is testing auditing tools. Neither the victim nor the officer is named in the inspected reports.

Incheon, South Korea: teenager sentenced to up to two years for AI deepfake sexual images of five teachers and six students, distributed on Telegram with the victims' names and schools

On 16 September 2026 the Incheon District Court sentenced a teenage defendant to an indeterminate term of two years (maximum) to one year six months (minimum) under the Sexual Violence Punishment Act for making and distributing false sexual images, ordered a 40-hour treatment programme and a five-year ban on working with children, and took him into custody in court. As a middle-school student in Incheon in 2024 he had used AI deepfake technology to composite the faces of five teachers, including his homeroom teacher whom he tricked into sending photographs, and six female middle- and high-school students from his taekwondo academy onto nude images and videos, 35 productions in all, and distributed them in Telegram 'acquaintance humiliation' chat rooms with the victims' real names, ages, schools, subjects and, per two reports, phone numbers (Hankook Ilbo adds Instagram IDs on some). The teachers learned of the images only in January 2026; the court said they are receiving long-term psychological treatment. He had withdrawn from school a week before the teachers'-rights committee met, avoiding school discipline. The Incheon teachers' union called the sentence, below the prosecution's request, regrettable.

Tennessee grandmother Angela Lipps jailed five months on an AI facial-recognition misidentification; charges dismissed after bank records placed her 1,200 miles away

On 14 July 2025, U.S. Marshals arrested Angela Lipps, a 50-year-old Tennessee grandmother, at gunpoint at her home while she was babysitting four children, on a nationwide-extradition North Dakota warrant: West Fargo police's AI facial-recognition technology had tied her to bank-fraud surveillance of a woman using a fake U.S. Army military ID to withdraw tens of thousands of dollars. She fought extradition for about three months in a Tennessee jail, was booked into Cass County, North Dakota on 30 October 2025 on four counts of unauthorized use of personal identifying information and four counts of theft, and was released on 24 December after her bank records showed her more than 1,200 miles away in Tennessee; charges were dismissed without prejudice. Fargo's police chief acknowledged 'incorrect assumptions' linked to the AI identification; an email obtained by KVLY shows six Fargo detectives were notified of her arrest in July, nearly five months before the department says it knew she was in custody. She lost her home, her car and her dog; her attorneys are pursuing potential civil-rights claims, with no lawsuit filed as of 31 March 2026. On 15 September 2026 she filed a $10 million federal civil-rights suit in the District of North Dakota against the City of Fargo and the detective, alleging he relied on a flawed facial-recognition match and ignored exculpatory evidence.

Brazil: scammers used AI-cloned voices and images of children with cancer in fake donation campaigns; 16 arrested in Operação Sophia

On 14 July 2026 the Rio Grande do Sul Civil Police launched Operação Sophia against a group that used AI voice cloning and deepfake to turn real fundraising campaigns of children with cancer into fake donation appeals: AI-generated child voices asked for money in videos boosted by pages such as 'Clube de Doadores' and 'Unidos pelo Amor', leading donors to lookalike sites that issued Pix codes to shell companies. Sixteen people were arrested across five states. In the campaign that started the investigation — using the image of Kelen Santos's daughter, who was in cancer treatment — police traced R$294,500 diverted, and the investigation found a financial-hub company that moved more than R$1.7 million. The family had to tell congratulating donors that their money had gone to a scam.

Pontianak, Indonesia: university suspended six students victimized by AI deepfakes; the sanctions were revoked after student protests

In May 2026, a case of alleged AI abuse surfaced at Tanjungpura University (Untan), Pontianak: a biology student identified by the initials RY allegedly used AI to turn photos of female classmates and other students into vulgar content, discovered when a classmate borrowing his phone found manipulated images in the gallery. The university's task force examined nine victims and witnesses and opened a reporting channel. The university then suspended six of the victimized students for two semesters — while the alleged perpetrator received a lighter sanction — prompting a student demonstration on 14 September 2026 demanding the victims' reinstatement and RY's expulsion. At the protest, Vice Rector Dr Achmadi announced the six sanctions were revoked and the students' academic status reactivated, with the corrected decree to be handed over on 15 September.

Google AI falsely identified Argentine activist Flavia Broffoni as a woman in a Ceuta migrant-violence video; death and rape threats followed

On 26 August 2026, WhatsApp groups in Ceuta circulated videos and images claiming that Argentine political scientist and activist Flavia Broffoni — who was in Patagonia and had not been to Spain since 2014 — was a woman filmed in Ceuta allegedly giving pepper spray to Moroccan migrants. A screenshot from one group shows that a user cropped the woman's face from the video and asked a Google AI tool who she was, with the word 'activista' as context; the answer identified her as Flavia Broffoni with details of her public profile. Broffoni received death and rape threats. La Nación's own test with the same video produced a different false identification — a student from Quilmes, with her personal data. Google Argentina said it could not confirm the screenshot came from its systems without the original link and had received no formal report. Broffoni is weighing legal action with her lawyer.

Etawah, Uttar Pradesh: a beautician and event organiser died by suicide on the night of 20-21 July 2026 after recording a video naming four acquaintances she said had ruined her home; her husband says they had been blackmailing her with AI-made videos and photographs (India Today: 'AI-generated videos and morphed photographs'); police registered an abetment case and arrested the main accused (July 2026)

A beautician, make-up artist and event organiser in Etawah district, Uttar Pradesh (aged 20 or 26 depending on the report), recorded a video on her phone and sent it to her husband before hanging herself at their rented home in Vrindavan Colony, Civil Lines, on the night of 20 July 2026 (ANI's circle officer; the Times of India says 21 July). In the video she named two women and two men, former work acquaintances, as 'the reason I am hanging myself', said they had ruined her home and should be jailed for life, said they had threatened to reveal details of her pre-marriage life to her husband and in-laws, and cleared her husband. Her husband, a block-office clerk, told India Today (whose Etawah reporter also wrote the AajTak report) and, separately, India TV that after she returned from a Jaipur assignment the four had been harassing and blackmailing her with AI-made videos and photographs. Civil Lines police seized her phone, held the inquest and post-mortem and said they were verifying the videos; the Times of India reported on 27 July that a case of abetment to suicide had been registered on the husband's complaint and that the alleged main accused, a 30-year-old man, had been arrested on Saturday 25 July and sent to jail while the other three, including the two women, were absconding. The AI attribution rests on the husband's account as reported; her video, as quoted, names four people and describes threats to expose her pre-marriage life; the police statements read do not describe an examination of the material.

South Carolina: xAI sues a Grok user it says turned photographs of real adults and children into sexualized images (X.AI LLC v. Harwood)

On 14 July 2026 X.AI LLC sued Terry Wayne Harwood, 67, of Gray Court, South Carolina, in the Northern District of Texas for breach of its terms of service. The complaint says a review of his two Grok accounts shows that between at least 8 December 2025 and 18 February 2026 he uploaded non-sexual images of numerous adults and minors and prompted Grok to alter them into sexualized images and videos, resubmitting altered prompts when Grok refused; one example involved a fully clothed girl of about 10 or 11. Harwood had been arrested on 26 February 2026 by South Carolina's Internet Crimes Against Children task force on eight felony counts of sexual exploitation of a minor, announced by the state attorney general on 9 March. The people depicted are not identified in any inspected source; the complaint's allegations remain allegations and the criminal charges are pending.

Libertyville, Illinois: a Highland Middle School social studies teacher was charged on 3 July 2026 with eight counts of child pornography after students reported he was recording them in class and a forensic examination found images of students altered with AI into explicit images; ordered detained on 6 July

A 44-year-old social studies teacher who had worked for Libertyville School District 70 for 18 years was taken into custody on 3 July 2026 on eight counts of child pornography (two Class X and six Class 1 felonies), Libertyville police and the Lake County State's Attorney's Office announced. The investigation began on 3 June 2026, the second-to-last day of the school year, after several students told school leaders that afternoon that the teacher had taken inappropriate photos or videos of them in his classroom; the district says it immediately removed him from the classroom, placed him on administrative leave and contacted the police. A forensic examination of his phone by the State's Attorney's Cyber Lab found numerous images of students that had been altered using artificial intelligence to create explicit images, and search warrants at his home, assisted by electronic-storage-detection dogs, recovered further devices and explicit child images; the State's Attorney's Office told the Chicago Tribune that an AI program was used to place students' heads on other people's bodies, and CBS Chicago reported that officials said he took photos and videos of current and former students. Court documents reported by Patch say some images depicted girls he knew or should have known were under 13. At a detention hearing on 6 July a Lake County judge ordered him held pending proceedings, with a preliminary hearing set for 30 July 2026; no later development was found. The district superintendent told parents that supporting students' well-being was the immediate focus and that resources were being made available; authorities said they would keep affected families informed and connect them with support services. Officials initially gave a different first name for the teacher, corrected in later copy; he is not named here.

Indore: an engineer was arrested at the end of January 2026 for a seven-month campaign of posting morphed obscene images of a woman and her sister, with threatening letters, to their relatives and neighbours and pasting posters near their home after the family rejected his marriage proposal; one Hindi report attributes the morphing to AI, citing a police forensic finding (Times of India, Free Press Journal, Patrika, NewsTak, 30-31 January 2026)

Indore police said at the end of January 2026 that the crime branch had arrested an engineer working as a data analyst after a woman complained at Banganga police station in July 2025 that an unknown person was mailing letters with abusive content and morphed nude photographs of her and her sister to her, her relatives and neighbours, and pasting posters near their house, causing mental harassment and social embarrassment. Police said the accused had approached the complainant's sister through a matrimonial website, that the proposal fell through over a horoscope mismatch, and that he then sought revenge by sending the material about seven times between July 2025 and January 2026 from post offices in Ujjain, Dewas, Khategaon, Ashta and Dhar, wearing a mask and cap and changing buses to avoid being traced; NewsTak adds that he also threatened the family with an acid attack and that a laptop and printer were seized. The case was registered under the Bharatiya Nyaya Sanhita, the IT Act and the SC/ST Act and he was remanded. Patrika reports that the obscene images were created with AI and that police said digital forensics found AI-generated images; the English reports describe them as morphed without naming the technology, so the AI attribution rests on one outlet.

Chennai: a former employee downloaded a senior woman colleague's photographs from their company's website, morphed them into obscene images with AI applications and circulated them to the company's staff through fake Instagram accounts from 4 January 2026; Tamil Nadu's cyber-crime wing traced and arrested her (reported 4 February 2026) and she was remanded in judicial custody

From 4 January 2026 several employees of a Chennai company received follow requests from unknown Instagram accounts that then circulated defamatory content, including obscene AI-morphed images of a senior woman employee. On her complaint the State Cyber Crime Investigation Centre of the Tamil Nadu police traced the digital footprints, social-media activity and IP logs to a former employee of the same company who had resigned in October 2025 and, according to police, acted out of resentment and previous enmity to threaten, harass and publicly humiliate the victim; she had taken the photographs from the company's official website and morphed them using various artificial-intelligence applications. She was arrested, produced before a magistrate and remanded in judicial custody; the police press release was reported on 4 and 5 February 2026 and the cyber-crime DGP issued a public warning about social-media misuse. The victim alleged the images were circulated to damage her reputation and cause mental distress.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
