AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Asunción, Paraguay: students of the Universidad Nacional de Asunción report an anonymous website sharing intimate images of students, teachers and staff without consent, some altered or generated with AI; police cybercrime investigators and a prosecutor open an investigation, reported 21 to 24 September 2026

According to Hoy (21 September 2026) and El Nacional (22 September), students of the architecture faculty of the Universidad Nacional de Asunción reported an anonymous website that shared intimate images of students, teachers and staff without authorisation, including real photographs and material altered with AI. El Nacional reports that at least about twenty students complained, that some posts used photographs taken from social media, and that related content later appeared in Telegram and WhatsApp groups. The Minister of Women said her ministry had seen messages in which UNA students raised concern about content of them held without consent, and that in some cases AI-generated images of sexual content were shared (La Nación, 22 September). La Tribuna, reporting the findings of the police cybercrime department, describes the portal as used for the non-consensual distribution of intimate photographs and AI-altered content; according to a memorandum signed by the head of the department, a Paraguay section exposes full names, images and phone numbers of students (24 September). The university referred the case to prosecutors and police, and police suggested that prosecutors seek a court order to block the site in Paraguay. No person responsible has been publicly identified.

Buenos Aires: AI-generated fake intimate images of 13- and 14-year-old girls at the Carlos Pellegrini school circulate in pupils' WhatsApp groups; families file a complaint and the national ministry demands answers from the UBA

Perfil reported on 1 October 2026 that fake intimate images of female pupils, created with artificial intelligence, had circulated in the WhatsApp groups of second-year students at the Escuela Superior de Comercio Carlos Pellegrini, a secondary school run by the University of Buenos Aires (UBA). The girls are 13 and 14 years old. The case came to light in late June 2026, and the school acknowledged the chats on 30 June. According to the report, the students involved manipulated, spread and sold the material, and a message on classroom desks told the victims the boys would keep undressing and selling them. Families filed a complaint with the Buenos Aires City cybercrime prosecution unit (UFEDICI) on 16 July and say the school has not concluded disciplinary action. By 1 October the national Ministry of Human Capital had asked the UBA rector for a detailed report on the protective measures and sanctions applied.

Honduras: Criterio.hn's fact-check desk finds three Facebook videos altered with AI in which plastic surgeon Emec Cherenfant appears to promote a home remedy for prostatitis; he says scammers keep using his name and image

El Mirón, the verification section of Criterio.hn, reported on 30 September 2026 that it found three videos on a Facebook profile, altered in image and audio, that use the image of the plastic and reconstructive surgeon Emec Cherenfant to offer a remedy for prostatitis and related problems. In the videos his artificial image says he helped former footballers beat prostatitis without surgery and invites viewers to write to him on WhatsApp. El Mirón says the manipulation is evident from the vocabulary, voice and body movements and that the detector ZeroGPT flagged the videos as AI-made. The hospital where he offers his services told El Mirón the advertising is deceptive. Cherenfant said he has nothing to do with prostate treatment, that scammers repeatedly use his name and image, and that he filed a complaint with the Ministerio Público two years ago. 'Son estafadores. No se dejen engañar, es pura inteligencia artificial', he said. No buyer losses are reported.

29 Jul 2026 to 1 Sept 2026BangladeshUnidentified voice-cloning tool

Khulna: a businessman paid Tk3.5 crore for old jute-mill machinery after WhatsApp calls in a voice presented as National Parliament Whip Raqibul Islam Bakul's, which the complaint and Khulna DB police describe as cloned with AI; four to five people arrested, the prime accused confessed

Khulna Gazette, the Daily Times of Bangladesh, Newsbangla24 and BD Today (all 29 September 2026) report from the first information report and Khulna Metropolitan Police Detective Branch (DB) statements that a businessman in Khulna city was joined on 29 July 2026 to a WhatsApp group call with a person introduced as National Parliament Whip Raqibul Islam Bakul, who discussed the sale of old machinery from Platinum Jute Mill and asked for advance payment. The businessman handed over Tk2 crore in cash that afternoon to men sent as representatives and a further Tk1.5 crore after inspecting the mill (30 July per Khulna Gazette and BD Today; 2 August per the Daily Times of Bangladesh and Newsbangla24), a total of Tk3.5 crore. When the businessman met the Whip in person in Khulna on 1 September, the Whip disclaimed any knowledge of it. The complaint filed on 18 September under the Cyber Security Act says the caller's voice had been changed with digital technology and artificial intelligence to pass as the Whip's; the DB deputy commissioner described the case to Asia Post as fraud by imitating the Whip's voice through AI. A press release from the Khulna city BNP media cell, sent on 4 September and published by Jaijaidin on 5 September, had already said a ring was using a US number and AI to clone the Whip's voice and demand money. Police arrested four people, recovered Tk12 lakh, and the prime accused gave a confessional statement before a magistrate on 28 September; on 29 September a court added two more detained men to the case. The voice-cloning tool is not identified and no forensic finding has been reported.

Italy: Fideuram chairman Paolo Molesini is deceived by a WhatsApp impersonation of Intesa Sanpaolo's CEO and a phone call in the reportedly AI-cloned voice of a lawyer he knew into ordering about 95 million euros of transfers; at least 36 million remain missing and he resigned on 12 March 2026, weeks later

On 23 February 2026 Paolo Molesini, then chairman of Fideuram (the private-banking subsidiary of Intesa Sanpaolo), received a WhatsApp message from an unknown number from someone claiming to be Intesa's chief executive Carlo Messina, announcing a confidential acquisition that had to be executed through Fideuram. The same day a caller presenting as a lawyer of A&O Shearman whom Molesini knew, whose voice ANSA, Il Fatto Quotidiano and Corriere della Sera, reporting from the Milan court papers, describe as created with artificial intelligence (today.it, which also cites the seizure decree, writes that the court papers do not yet answer whether the voice was imitated), had him sign a confidentiality agreement and sent eleven payment instructions; Fideuram's treasury head was separately contacted by someone posing as Fideuram's CEO. Between 23 and 25 February eleven transfers totalling about 95 million euros went to accounts in Portugal and at Bank of China; the bank's alarm systems and the Milan prosecutors recovered about 40-42 million from China and 13 million seized in Portugal, leaving at least 36 million (39.5 million per the court papers) missing after conversion into cryptocurrency. Molesini, who Corriere writes is not under investigation, resigned as chairman on 12 March 2026, which Fideuram announced as being for personal reasons; a 48-year-old Israeli citizen is under investigation as a member of the gang.

Pontevedra province: Guardia Civil locates two men as presumed creator and distributor of AI-generated sexual images of 22 women

In June 2026 women in a municipality of Pontevedra province (Galicia, Spain) reported to the Guardia Civil that AI-generated sexual images of them were circulating, and 18 complaints had been filed by the afternoon of 10 June. An affected woman, quoted in an El Correo Gallego article whose source line is Faro de Vigo, said the images were posted on a WhatsApp status for a full day and that the woman saw them on the afternoon of the Saturday before the 10 June report (6 June by the calendar). On 16 July 2026 the Guardia Civil said it had located two men as presumed responsible for creating and distributing the images, which it counted as showing 22 women in total. The reports name neither man and say a 35-year-old is considered the creator and a 29-year-old is investigated for an offence against moral integrity. The release says the images were made with AI applications from photographs taken from social-media profiles, and the files went to a court of instruction. June reports put the number of women appearing at about thirty, and no charge decision or court outcome is reported.

Lower Saxony CDU parliamentary group employee reportedly posted an AI-generated sexualized video of a colleague in a private staff chat group

Welt, dpa (in Tagesspiegel, news.de and t-online) and a press release of the CDU parliamentary group in the Lower Saxony state parliament report the following. On 17 January 2026 an employee of the parliamentary group posted a short AI-generated video showing a colleague in a sexualized manner in a private WhatsApp group connected to the group's staff. The Hannover public prosecutor's office had viewed the video, and a spokesperson said it was obviously an AI montage. After the group's leaders were informed (on 27 March according to press reports), the employee who created the video was dismissed without notice and a second employee, who had reportedly asked chat members in January to delete the video and keep silent, was suspended. The group informed the prosecutor's office, which said, as reported on 1 April 2026, that it had received no criminal complaint. On 9 July 2026 dpa reported, citing the prosecutor's office, that the Wennigsen local court had fined the former employee, who had made the video with the colleague's picture without consent and shared it in a chat group, 30 daily rates for a violation of the Kunsturhebergesetz, and that the decision is final. NDR reported on 10 April 2026 that the depicted employee had filed a criminal complaint, that the prosecutor's office had received the employee's application for prosecution, and that the employee did not want to comment publicly. The colleague and the former employee are not named.

Hong Kong police report a victim lost over HK$10m after a voice message from a hijacked WhatsApp account imitated the victim's father, reportedly by deepfake

Hong Kong police said on their CyberDefender social media page, reported on 29 July 2026, that a victim received a WhatsApp voice message from the victim's father's hijacked account asking for an urgent transfer of HK$1 million to a friend. The voice and manner of speech were very similar to the father's, and the victim made the transfer, then a further HK$2 million requested the same way, then payments to several bank accounts on different pretexts, until the savings account was empty. The victim realised the fraud only after police made contact, with a reported total loss of more than HK$10 million. Police said scammers in WhatsApp hijackings may use AI to fake the voices of people close to the victim, and outlets that published reports on 30 and 31 August 2026 of a police briefing described the case as one involving a deepfake voice message imitating the father, while one outlet said the deepfake use was suspected. No source reports analysis of the audio. The loss and the voice characterisation rest on police statements relayed by outlets, and the month of July 2026 rests on one outlet.

Filipino domestic worker reports losing 100,000 pesos in a romance scam that used reported AI deepfake video calls posing as a Dubai prince

AFP reported on 3 July 2026 that a Filipino domestic worker, who asked that a real name and age be withheld, met a scammer posing as a Dubai prince on a dating site and then took part in WhatsApp video calls in which the scammer appeared on screen as the prince. AFP described the suitor as an AI deepfake, said it saw a recording of one call in which the voice did not match the prince's voice, and said it was unclear which AI tools were used. The worker paid 100,000 pesos (about US$1,625) for a supposed marriage certificate and 'royal membership card' that would help secure a job in Dubai, and AFP said the worker lost a year's worth of savings. When the scammer then asked for a further 60,000 pesos for a hotel booking, the worker became suspicious, noticed that the Facebook account was based in Nigeria, and ended contact. The account rests on one AFP interview carried by other outlets. The dates of the events are not stated.

Economist David Rosenberg reports impersonation in Facebook and Instagram ads that directed investors to a WhatsApp investment scam

Economist David Rosenberg told The Globe and Mail (18 June 2025) that ads on Facebook and Instagram since March 2025 promoted a fake investment program listing him as its administrator and redirected people to a WhatsApp group. The Globe reported that one investor interviewed estimated losing $16,000, another said she lost $500,000 in a week, and a third alleged losing $450,000 in life savings. Rosenberg told BNN Bloomberg on 7 July 2025 that the scammers used a fake AI voice and video of him, and a Globe column of 17 July 2025 described a deepfake video advert. Losses are self-reported and unverified. The AI characterisation comes from Rosenberg and a July Globe column, and the June Globe article on the investors does not mention AI. Georgia: a retired couple lost about $800,000 in a crypto scam that police said used an AI-generated "ghost site" to fake what the husband saw on screen (2025)

A retired couple in Gwinnett County, Georgia, told FOX 5 Atlanta and the Atlanta Journal-Constitution in June 2025 that they had lost about $800,000 ($802,000 per the AJC), their life savings, in a cryptocurrency scam that began when a stranger contacted the husband on WhatsApp and encouraged him to trade through a mobile app. He traded and withdrew small amounts before investing more, and discovered the fraud when he could not withdraw and called 911. According to FOX 5, Gwinnett County police explained that the app was real but the scammers had created an AI-generated "ghost site" that manipulated what he saw on the screen; the husband said police identified that what he was seeing on the wallet was not there. The 74-year-old said he felt sick and ashamed; the couple's children started a fundraiser for day-to-day expenses, and police said such funds are rarely recovered. The tool is not described; the AI element appears in no independently reported source (a Cobb County government page of January 2026 repeats FOX 5's summary, and the AJC's legible text does not mention AI).

Jaú, São Paulo: eight 15-year-old ninth-grade boys at a private school are reported to have made fake nude images of a female classmate with artificial intelligence and shared them in a WhatsApp group; the school suspended them and the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation

In early September 2026 a private school in Jaú, in the interior of São Paulo state, identified the circulation of fake intimate images of a female classmate made with artificial intelligence and contacted the families. According to the police report described by the press, eight ninth-grade boys, all aged 15, kept a WhatsApp group in which they used digital programs to manipulate images of the student so that she appeared unclothed. The school suspended the eight and informed the girl's parents. The father of one of the boys checked his son's phone, found the files and took the boy and the phone to the police to register a report; the girl's parents also registered a police report. The São Paulo Public Security Secretariat said the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation that occurred on the morning of Thursday 3 September. The school said the content originated on a private platform outside the school environment and that it had adopted protective and welcoming measures for the student and opened an internal inquiry. The AI tool used is not named. The students are minors and are not named in any report.

Galway West, Ireland: broadcaster Gráinne Seoige says an AI-generated sexual deepfake image of her circulated on WhatsApp during her 2024 election campaign

Gráinne Seoige, a broadcaster who stood for Fianna Fáil in Galway West in Ireland's 2024 general election, told RTÉ's Prime Time in January 2026 that people close to her alerted her during the campaign to sexualised images of her made with deepfake technology. She says the AI-generated pornographic image was disseminated through WhatsApp, shared hundreds if not thousands of times by her account, and that when gardaí sought its origin some recipients refused to cooperate, so the investigation hit a dead end. She kept silent during the campaign and has described the experience as deeply traumatising, with shame and humiliation lasting almost a year and a half. In a September 2026 interview relayed by RSVP Live from the Sunday Independent she described night terrors of faceless men tearing her clothes off and wearing a long puffer coat everywhere so her body could not be seen. She has since appeared before the Oireachtas media committee (Irish Independent, 21 January 2026), and made a Virgin Media documentary on deepfake abuse, scheduled for broadcast on 5 October 2026. In passages from it quoted by RSVP Live and the Irish Sun on 4 October 2026 she says she was shown the image in September or October 2024, that it made her not want to go outside, that she moved away from her home and that she has had therapy; she also says she believes the image was politically motivated. The Irish Times, reviewing the documentary on 5 October 2026, reports that in it she learns from a Dublin technology company that further images taken from her Instagram account have been manipulated with deepfake software, which was news to her. The tool used to make the image and the person who made it are not identified.

Bengaluru: a security guard says a WhatsApp video call in which an alleged deepfake 'Chief Minister Vijay' offered him financial aid led to fake 'processing' charges and a fake CBI fine, and he lost more than Rs 1.04 lakh; Byappanahalli police register an IT Act case (September 2026)

A security guard in Bengaluru, originally from Odisha, told the Times of India that on 9 September 2026 he answered a WhatsApp video call in which a face and voice presented as Tamil Nadu Chief Minister C. Joseph Vijay introduced himself in Hindi, asked his name, work and where he lived and pressed him to accept financial help. A 'manager' then promised Rs 11 lakh, said Rs 5 lakh had been allotted to him and asked for a Rs 5,000 exchange charge, then Rs 18,000 to unlock a supposedly locked PIN; a caller posing as a CBI officer demanded more than Rs 50,000 as a fine. The fraudsters sent a fake allotment letter and a purported CBI officer's identity proof. He paid more than Rs 1.04 lakh through a digital payment app before refusing a further Rs 50,000 demand, called the 1930 cybercrime helpline and went to Byappanahalli police, who registered a case under the Information Technology Act. The Times of India says the fraudster allegedly used a deepfake AI-generated video and calls it the first such case reported in the city. East Jakarta: police confirm a grade-9 student at a state junior high school edited several female schoolmates' photographs with AI into indecent images that circulated as WhatsApp stickers; a social-media post said about 200 images were sold; the student was later beaten by schoolmates angry at the images (police statements differ on whether the depicted students or their friends did so) and both matters are under police investigation (Kompas, ANTARA, detik, VIVA, Media Indonesia, 23-24 September 2026)

On 23 September 2026 the spokesperson of the East Jakarta metropolitan police (Polres Metro Jakarta Timur) confirmed to reporters that a grade-9 student at a state junior high school in Pulogadung, East Jakarta, had edited photographs of several female schoolmates using artificial intelligence so that they appeared indecent, and that the edited images had been made into WhatsApp stickers; the police women-and-children protection unit is investigating. The case surfaced through a post on Threads which said about 200 edited photographs had been produced and sold to others; Kompas.com noted on 24 September that the sale allegation still rested only on that post, VIVA reported that police were still checking the claim that the images had been sold, and ANTARA reported on 23 September that police were still establishing the editing method, the recipients and the number of depicted students. As of 24 September the depicted students had not filed a report (detik and VIVA date the spokesperson's telephone remarks 'Kamis (22/9/2026)', a date that is internally inconsistent) and police appealed to them to do so. According to the parents' account relayed by police, on 15 and 16 September several people took the student who made the images to an empty house and then to a reservoir in Kayu Putih, punched him on the nose, ears and head, kicked his chest and stomach and threatened him with a bladed weapon; police attribute the beating to friends of the depicted students (ANTARA, detik, VIVA), while Kompas.com's 23 September report quotes the same spokesperson as saying the depicted students themselves beat him; he filed an assault report, which police are handling alongside the image case, and the school has held mediation with the students' parents. Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
