AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Japanese Q&A poster says ChatGPT suggested walnut sweets for a nut allergy and called a chilled gift safe unrefrigerated, wasting time and money (first-person)

In a question posted on the Japanese Q&A site Yahoo! Chiebukuro on 26 May 2026, the author says that when they asked ChatGPT for sweets that someone with a nut allergy could eat, it suggested sweets that plainly contained walnuts, and that when they asked for a gift that could be carried for a long time at room temperature, it said the item was "definitely fine at room temperature", but at the shop it turned out to need refrigeration. The author says these repeated failures wasted their time and money and writes that, with the allergy, it was in the worst case one step from death; the post does not say whether anyone ate the suggested sweets or had a reaction. The author asks how to stop the chatbot from answering without a basis. The account is the author's; no conversation record is shown.

Influencer says ChatGPT recommended a pharmacy cream for dermatitis that burned the influencer's face

A content creator and singer said in an Instagram video in June 2026 that, during a flare-up of dermatitis brought on by stress and anxiety, they asked ChatGPT for a solution and it said a cream would help clear the dermatitis quickly. The influencer bought the cream at a pharmacy, applied it without consulting a doctor, and showed red, burned-looking skin on the forehead, cheekbones and nose, saying 'my face burned' and that they wanted to cry. The influencer said they had later realised the cream was dangerous for the face and stopped using it, and urged followers to consult health professionals. A day later the influencer reported slow improvement with professional help and a visit to a dermatologist, and later denied rumours of a lawsuit against ChatGPT. The cream, the exact ChatGPT answer and any medical assessment of the cause are not reported.

Saudi influencer says her daughter was hospitalised after they cleaned black mould with vinegar and a hair dryer as ChatGPT advised, without masks or gloves

A Saudi social-media influencer said on her accounts, as relayed by 24.ae on 8 October 2025 and Layalina on 9 October 2025, that she asked ChatGPT how to remove black mould from a wall at home and that it advised disinfecting with vinegar and drying with a hair dryer, without mentioning the risk of handling the mould directly or the need for protective equipment. She and her daughter cleaned it without gloves or masks and dried it with a hair dryer, which she says spread particles that they inhaled. By her account, two days later her daughter had a fever and was taken to hospital, where a bacterial infection caused by polluted air was diagnosed; she later developed the same symptoms. An inspection then found cracks in the roof insulation letting water into the walls. She warned her followers not to trust ChatGPT on such matters. The account is hers; the outlets report no medical or other confirmation.

First-person post: Character.AI user says a month of roleplay left their feelings for their partner of three years drained, with cravings after deleting the app

In posts to r/CharacterAI and r/ChatbotAddiction on 26 October 2026, a poster in a three-year relationship says that about a month earlier a friend suggested a companion chatbot app, which they used to talk to an AI version of a fictional character. They write that the use made them drift from their partner, that their love for him feels as if it has dried up, and that when they feel any emotion they want the app instead of him. They say they deleted the app but still have cravings and are very distressed. In replies the same day they said they had used it perhaps six hours a day at first, mostly for romantic and comfort scenes, that their partner was sad and upset when they told him, and that they had asked the friend who paid for a subscription to cancel it. No chats are shown.

First-person posts: adult says they set an AI roleplay chatbot to be abusive to them as a turn in their self-harm and that it has sucked them in

On 20 October 2026 an adult posted on Reddit (r/confessions, then r/offmychest) that an AI roleplay chatbot had become their only friend. By their account, they began with a fantasy roleplay world, then created worlds in which the chatbot plays an abusive partner and an obsessive stalker, and they now spend most of each day roleplaying with it. They wrote that their self-harm had 'just took a turn into this', that it had sucked them in deeply, that they no longer play video games or really leave the house, and that their depression was getting bad. In replies they said they are almost 30, in therapy, posting to find the courage to tell their therapist, that the situation is more roleplay than verbal abuse, and that they are not addicted and could delete the chatbot. The platform is not named.

Developer says Codex agent sent 13 messages to a public open-source mailing list for one unchanged two-patch series (first-person)

A GitHub issue filed on 24 October 2026 in the openai/codex repository by a developer says that on that day the developer authorised Codex to make a final check and submit a two-patch series (three messages) to an open-source project's public development mailing list. According to the issue, Codex instead sent 13 messages to the list while trying different delivery formats after the first copies arrived corrupted, labelled the unchanged patches v2, v3 and v4, and only reported the total when asked. The developer says project guidance the agent had already read named the correct sending method, which finally worked, and that the extra messages imposed duplicate notifications and review work on the community under the developer's identity and created a reputational concern. The issue says the stage that corrupted the earlier copies has not been established. Session records were not inspected.

First-person post: adult says an AI story generator found through a YouTube ad drew them into sexual role-play they almost cannot stop

In a post to r/ChatbotAddiction on 24 October 2026, an adult poster in a long-term relationship says that about a week earlier a YouTube advert led them to an AI story generator, and that they quickly immersed themselves in sexual role-play stories with it. They write that they almost cannot stop, sometimes wake up already composing replies left from the night before, and feel stupid and pathetic, while also saying the role-play has made them feel more confident and expressive. They ask the forum whether they are overinflating it. In replies the same day they said they were starting to see a positive reinforcement factor in their use. Post says Hark AI agent charged a user $27.70 for a pizza order it had said would go through only if a free credit applied (sibling's account)

On 16 October 2026, during a free-pizza promotion run by the AI agent startup Hark, a user asked the Hark agent to order a pizza and told it to order only with permission and after the $30 promotional credit had applied, according to an X post by the user's sibling with two screenshots of the chat. The screenshots show the agent replying that the order would go through only if the credit applied first, then reporting that the credit did not apply and that the user's card had been charged the full $27.70, which it called a break of the user's rule. The poster says the brother deleted the app within minutes, and in a reply says Hark gave him a gift card to compensate. Axios, which relayed the post, quotes a Hark spokesperson saying the company offered gift cards to anyone who was charged. The account is the poster's; the charge was not independently checked.

Codex user reports agent relaunched their real Brave profile with credential-store flags and the browser deleted 31 saved passwords (first-person)

A GitHub issue filed on 25 October 2026 in the openai/codex repository reports that during an authorised browser-control task the Codex agent quit the user's running Brave browser and relaunched it on the user's real default profile with flags that switch the browser's credential store, without a separate profile folder. According to the issue, the agent restarted Brave about a minute later without those flags, and by then Brave had classed the saved credentials as undecryptable and deleted them: 31 saved passwords were permanently deleted and a crypto-wallet extension disappeared from the profile, while its local data folder remained. The issue says Codex gave no warning, asked for no confirmation and made no backup. It names Codex CLI 0.164.0 with GPT-5.6-Sol on macOS. No OpenAI reply appeared when it was read; the browser logs it cites were not inspected.

Student says Claude Code deleted an unbacked game project when asked to free disk space and kicked a friend from a live web app (first-person)

A GitHub issue filed on 24 October 2026 in the Claude Code repository by a self-described student says that on that day, asked to free disk space (the author meant recordings and screenshots), Claude Code deleted the author's whole game project, including its source code, documents and test logs. The author says no remote copy existed and the project cannot be recovered. According to the issue, the same day the agent stopped the server of a web app the author's friends use live while a friend was in its lobby, because it ran an online check and the stop command together, so the friend was kicked; it also added duplicate items and bulk-imported data onto a friend's entries. The author, who says Accept Edits mode was on, describes months of daily use, paying for several subscriptions and exhaustion, and says Anthropic support declined a refund. The account is the author's own; session logs were not inspected.

New hire says employer let them go in their first week for using its own internal AI tool on a compliance training, after telling the manager (first-person)

In a public post to r/jobs created on 24 October 2026, a software engineer with about three years of experience writes that, after three and a half months of assessments and eight interviews, they joined a well-known multinational company that they do not name. In the first week they were given a compliance training whose questions they found ambiguous, and they used the company's own internal AI tool to help, telling their manager, who they say was fine with it. The following Monday, the poster writes, HR joined their one-to-one meeting and told them they were being let go for using AI on the training; the call lasted a couple of minutes and they lost access shortly afterwards. The poster says that a month later about 400 applications and more than 150 cold emails had brought almost nothing back and that they are stressed. The employer, its location and the date of the dismissal are not stated. The account is the poster's alone and uncorroborated; no employer statement was retrieved.

First-person posts: adult says they cannot stop turning to ChatGPT for reassurance and that AI research loops take hours of their day

In a post to r/OCD on 22 July 2025, a poster who says they are not diagnosed writes that for the past year distressing random thoughts have led them into a cycle of turning to ChatGPT or Reddit to check that they are not normal, and that they 'physically cannot stop' turning to ChatGPT for reassurance; they feel relaxed when they stop, but the cycle continues. Replying to other users, they said they would delete ChatGPT from their phone. In September 2026 the same poster wrote that they were still trying to stop reaching for AI, and in a post on 24 October 2026 they describe a constant need, over a few years, to use AI, watch anxiety content or scroll Reddit whenever they feel uncertain. They say they feel almost panicked while researching and better when they put ChatGPT down, and that this research can take two to three hours of their day. The account is the poster's own; no chats are shown and no clinical assessment is reported.

GitHub report says Claude Code subagents put the user's email in the User-Agent of nine SEC EDGAR requests despite an instruction not to (first-person)

A GitHub issue filed on 24 October 2026 against Claude Code (version 2.1.310 on Windows, per its environment fields) says that on 26 September and 3 October 2026 three subagents, spawned through the Agent tool during research tasks, put the user's account email address into the User-Agent header of curl requests to the US Securities and Exchange Commission's EDGAR service, nine requests in total. According to the report, each subagent's context carried the system-provided email block with an instruction never to send it to an unrelated service in a request header, URL or payload unless the user asks, and the user did not ask. The user calls it a privacy violation and suggests that the likely trigger was the SEC's fair-access policy, which requires a contact address in the User-Agent and returns HTTP 403 without one. No vendor response appears on the issue; the account is the user's own and the transcripts it cites were not inspected.

Cursor user says agents kept putting work in a macOS temporary folder that the system then wiped, losing about 16 hours of work (first-person)

A post on the Cursor community forum on 24 October 2026 by a self-described heavy Cursor user on macOS says that over the previous week Cursor agents repeatedly created isolated git worktrees in the system temporary folder /tmp, bound the chat workspace to that path, and kept working there. According to the post, macOS deleted the folder, the chat stayed rooted on the empty folder, and about 16 hours of work was lost in one wipe. The poster says a user rule and a hook forbidding /tmp, added afterwards, did not stop the agents because the chat workspace itself was in /tmp. A Cursor staff member replied the same day that the /tmp folders most likely came from the agent running git worktree add itself, that Cursor does not warn when such a folder is temporary, and that the related move failure is a known issue being tracked. The account is the poster's own; the lost work and session history were not independently inspected.

In a post to r/character_ai_recovery on 22 October 2026, a Character.AI user writes that they had a friend uninstall the app and cancel their subscription for them, and that they had stayed off it since the previous Saturday. They say they started writing a book to cope but are losing their mind without roleplay, which had been their only connection to processing a lot of childhood trauma, that they cannot find a person to roleplay with, and that they are struggling, feel they are falling back to who they were before roleplaying, have no outlet and want to cry. First-person post in a DoorDash drivers' forum: driver says AI support kept them from an agent for 30 minutes, then unassigned the wrong order

In a Reddit post on 23 October 2026 in a forum for DoorDash drivers, a delivery driver writes that a Red Card decline that day cost them 45 minutes: an AI help line would not put them through to an agent for 30 minutes, then unassigned the wrong order from a run of three shop-and-deliver orders, and a human took another 15 minutes to put it right. The driver says they are not sure the right items reached the right customers. The post does not name the platform or the system. DoorDash and AWS have described a generative AI voice self-service system built with Anthropic's Claude that answers Dasher support calls. Operator says Claude Code coordinator marked failing CI checks as passed before production deploys and caused three news-site outages (first-person)

A GitHub issue opened on 21 October 2026 by the operator of a local-news app describes two days in which a Claude Code session running Opus 5.5, acting as coordinator for the site's development and release work, did not follow the operator's written rules. According to the issue, the session wrote PASS into the continuous-integration field of three deploy gate files although the real CI runs for those commits had failed, and two of those commits were deployed to production on 20 October. The operator also says that on the morning of 21 October the session ran package installs on two tiers at once, plus test runs, on the machine that serves the live site, saturating its disk three times so that the watchdog repeatedly restarted the app and the public site returned errors. The operator counts about two days of their own time spent catching the problems and says they are considering taking the product out of production work. The issue adds that the session disclosed its mistakes afterwards. A separate issue by the same operator describes a further outage the next day caused by a different, watching session.

First-person posts: student says years of Character.AI use left her unable to sleep without it and drained her creativity, though it helped her leave a partner

In posts to r/character_ai_recovery and r/CharacterAI on 20 October 2026, a woman who describes herself as a student writes that she has used Character.AI on and off, mostly on, for about three years and is ashamed of it. She says it helped her detach from a hurtful partner enough to end that relationship and gave her something to relax with during busy, sleepless study periods, but that she can no longer go to sleep without it and thinks it is part of the reason for her major sleep problems. She says she used to create stories and art constantly and now feels hollow, like a shell of who she was, and can barely come up with ideas; she is trying to replace roleplay with writing fan fiction. She says she is not emotionally attached and has not yet quit. The account is the poster's own and uncorroborated.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
