AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Beijing prosecutors say a man used an AI undress tool to forge about 7,000 nude images of women, including students and colleagues, and sold them to 351 buyers

Beijing police and the Haidian District People's Procuratorate, as reported by CCTV in June 2024 and again in July 2026, said a 24-year-old technician at an internet company used AI "one-click undress" software to make fake nude images from ordinary photos that customers sent him, advertising the service on an overseas social-media platform at 1.5 yuan an image. Prosecutors said that over a few months he made nearly 7,000 such images and sold them to 351 people, and that the women depicted included students, teachers and colleagues of the customers. The case came to light after a police report in Haidian district. The procuratorate charged him with spreading obscene material for profit and also filed a public-interest suit because the images infringed the rights of unspecified people. He admitted knowing the images could be used for extortion or to damage people's reputations. CCTV reported in July 2026 that in 2024 he was sentenced to three years in prison for making and selling obscene material for profit; the result of the public-interest suit is not reported in the sources read.

Baltimore County, Maryland: police say a high school's athletic director framed the principal as racist with an AI-cloned voice recording

Police in Baltimore County, Maryland, said in April 2024 that a high school principal had been framed as racist by a fake recording of his voice containing racist and antisemitic comments, which appeared in teachers' email inboxes and spread on social media. Court records cited by the Associated Press say an expert consulted by detectives found traces of AI-generated content with human editing; a second expert found spliced recordings and told AP he had not confirmed the clip was fully AI-generated. The principal went on leave while police guarded his house. The school's athletic director, investigated for using AI to make the recording in retaliation, entered an Alford plea, maintaining his innocence, and was sentenced in 2025 to four months for disturbing school operations, CBS Baltimore reports.

Campeche, Mexico: pupils accuse a high-school classmate of making AI nude images of female classmates and protest school inaction, a local paper reports

La Jornada Maya reported on 5 October 2026 that pupils at an upper-secondary school (preparatoria) in Campeche, Mexico, accused a classmate of making images of female classmates without clothes using digital applications, which the paper describes as AI used to make obscene photos and as images that are not real photos but use the girls' faces. The pupils reported him to teachers and administrators. According to the paper, no action to protect the girls was known, and on Monday classmates and some of the affected girls protested during the school's official ceremony, where the sound was cut off when they tried to use the microphone. Afternoon-shift pupils later held a peaceful demonstration at the school entrance and parents were seeking a response. No complaint to prosecutors or police is reported.

UK: a trades-college staff member says a student reported that a video showing her undressing, which appeared to be a manipulated version of an apparently real video of her talking with a student, was circulating among classmates; she stepped away from the class for weeks and says the atmosphere had chilled on her return

WIRED (Caroline Haskins, 24 August 2026) reports the account of a staff member at a UK college that trains 16-to-19-year-olds in trades such as construction, whose job combines administration with a counselling role; WIRED identifies her by first name only, which is not repeated here. She says administrators told her this winter that a student had reported two videos circulating among classmates: one apparently real, showing her talking with a student, and one that appeared to be a manipulated version showing her taking her clothes off and dancing in underwear or a bikini. The student she suspected denied making it when questioned and soon dropped out. The college let her step away from the class for a few weeks and educated students about deepfakes; she says the atmosphere had chilled when she returned and that the episode has made her more cautious. The college did not respond to WIRED.

Arizona: a sixth-grade teacher says his students posted an AI-generated image of him kissing another male teacher in the comments of his public TikTok account; the school identified a student who admitted making it, who was suspended for three days

WIRED (Caroline Haskins, 24 August 2026) reports the account of an Arizona-based sixth-grade teacher, who spoke to WIRED by name (not repeated here). He says his students circulated an AI-generated photo showing him and another male teacher kissing in the comments of his public TikTok account. He reported the accounts to TikTok as belonging to children under 13, and says TikTok did not remove them. He says the school immediately investigated and identified a student, who admitted creating and sharing the image and received a three-day suspension; WIRED says the district handbook makes that the most severe punishment children of that age can receive for a first-time sexual harassment offense. He told WIRED that reflecting on the experience had sent him into a spiral, and said his concern was mainly the permissive culture around AI among his students. The district did not respond to WIRED. WIRED gives no date for the episode and does not name the school, district or AI tool.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
