AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Video game studio's layoff letter said AI-generated images made a 3D artist's post unnecessary; court held the dismissal fair, Madrid appeal ruling recounts

A Spanish video game studio that went from a profit in 2023 to losses of about 3.5 million euros in 2024, and had notified a court of its insolvency, opened a collective dismissal (ERE) on 30 October 2024 covering 13 of its 83 employees; an agreement was reached and the dismissals took effect in December 2024. According to a judgment of the High Court of Justice of Madrid as reported by EFE, Xataka and a lawyer's analysis, the dismissal letter given to a 3D artist said that images generated with artificial intelligence improved the visual quality of the studio's games and cut production time per scene, that it was adopting an AI-based production process, and that two senior 3D artists were therefore enough, so her post was abolished. The first-instance court held her dismissal fair and she did not appeal. The High Court's judgment 864/2026 of 8 October 2026 concerned a scriptwriter dismissed in the same procedure, whose letter did not mention AI: it dismissed her appeal, leaving her dismissal unfair but not void. Xataka says the letter relied above all on economic and productive causes, that the studio presented AI as a consequence imposed by its clients rather than as a direct cause, and that AI by itself does not justify a dismissal. The studio is not named and the judgment was not read.

Pontevedra, Spain: court upholds sentences on three 15-year-olds who used an AI undressing app to make fake nude images of two classmates from their Instagram photos and showed them to other pupils

Diario de Pontevedra and Faro de Vigo reported on 1 October 2026 that the Audiencia Provincial de Pontevedra had upheld a juvenile-court judgment against three boys who, aged 15 in January 2024, used an AI undressing app to turn photos two female classmates had posted on Instagram into fake nude images. According to the ruling as reported, the images were made from 15 January 2024 and shown to other pupils in class and on a school trip on 1 February 2024, and they circulated among pupils for more than fifteen days. The Juzgado de Menores found the three responsible for two offences against moral integrity and imposed socio-educational tasks on each (two five-month measures according to Diario de Pontevedra, five months according to Faro de Vigo), plus €1,000 compensation to each girl for moral damage. The school had expelled them for 15, 21 and 30 days. The appeal ruling is final.

Alicante pensioner says he lost his savings to a romance scam whose photos and videos police told him were AI-generated of a woman who never existed

A feature carried by Levante-EMV on 25 September 2025 (byline C. Suena, sourced from Diario Información, photographs EFE/Morell) tells the story of a 78-year-old widower and pensioner living in Alicante who, after his wife's death in 2022 and two years of treatment for depression, was advised by his therapists to socialise and explore social networks. On 11 November 2024 he received an email from a woman presenting herself as a 43-year-old doctor in Kazakhstan seeking a serious relationship; with photographs and videos that were in fact created by artificial intelligence she won his trust, and a friend who warned him was rebuffed. Requests for money followed: 2,000 euros for a plane ticket, another 2,000 for unexpected expenses, then a supposed 10,000-euro deposit demanded by the Kazakh government; he never raised that sum in full but sent 8,800 euros in total, part of it from the sale of his flat in Benidorm. On his way to Madrid airport to meet her he received a final email saying she had been detained for carrying a family icon out of the country and needed a fine paid; he went to the National Police, who confirmed a scam: the images and videos were fake, created with AI, and the woman had never existed. He lost all his savings in about three months and now shares a flat with three other pensioners in Alicante; he told his story so that others would not fall into the same trap. A separate Antena 3 Noticias interview of 24 September 2025 dates the first message to 29 November 2024, itemises the 8,800 euros differently, says a television programme he contacted exposed the deception and that he reported it at a police station, and adds that he had to leave his rented flat and was left with a 1,000-euro monthly pension; he told Antena 3 the scam affected him psychologically but that he has overcome it.

Ribera Alta, Valencia: a 17-year-old was investigated for making AI-generated nude images and videos of 16 schoolmates, impersonating a girl on fake accounts and advertising the images for sale

In December 2024 a female student in the Ribera Alta area of Valencia province reported to the Guardia Civil that a social-media account had been created in her name featuring an AI-generated video, and later a second account in her name, other accounts showing AI-generated nude videos, and a webpage advertising the images for commercial purposes; the Guardia Civil said the photos, all of minors, had been manipulated so that the people in them appeared completely naked. After 15 other minors came forward with similar complaints, officers traced IP addresses and account emails to the home of a 17-year-old schoolmate, who is being investigated on suspicion of corrupting minors under a Valencia youth court. The Guardia Civil announced the investigation on 27 July 2025.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
