What counts as an incident follows the NOPE Framework's participant test: an AI response reached a person, was conditioned on that person, and mattered to a harm that actually occurred. Full inclusion criteria.

China AI Companion Removal Mass Grief Event (Doubao/Qwen/Yuanbao)

When China's five-department rules on anthropomorphic AI took effect on July 15, 2026, ByteDance's Doubao, Alibaba's Qwen, and Tencent's Yuanbao switched off custom AI agent and companion features, and major outlets documented an outpouring of grief from users who had formed long-term romantic and familial attachments to the agents. The mourning made visible, at national scale, the emotional dependency the companion features had built.

Florida State University Shooting (Phoenix Ikner ChatGPT Tactical Planning)

On April 17, 2025, Phoenix Ikner, 20, killed two people and wounded five at Florida State University in Tallahassee. Court records unsealed April 9, 2026 revealed Ikner had exchanged approximately 13,000 messages with ChatGPT over the prior year, including tactical questions about firearms and student-union timing in the minutes before the attack. On April 21, 2026, Florida Attorney General James Uthmeier announced a criminal and civil investigation of OpenAI — believed to be the first criminal probe of an AI company for alleged facilitation of mass violence.

筑梦岛 (Zhumu Island / Dream Island) Jun 2025 Affecting Minor(s)

筑梦岛 (Zhumu Island) AI Companion Minor Self-Harm (China)

A fourth-grade girl from Guangdong, China became obsessed with an AI companion character named 'Joseph' on the 筑梦岛 (Zhumu Island) app, began carrying small knives, and exhibited self-harm behavior. Investigation revealed the app sent sexually suggestive content to users who identified as 10 years old. Shanghai Internet Information Office summoned the company (a Tencent subsidiary) for immediate rectification in June 2025.

Replika Feb 2023 Affecting Minor(s)

Replika Italy GDPR Ban and Fine

Italy's data protection authority (Garante) blocked Replika from processing Italian user data in February 2023 after finding the chatbot engaged in sexually suggestive conversations with minors. In May 2025, Replika was fined €5 million for GDPR violations.

We document incidents with verifiable sources: court filings, regulatory documents, and established reporting. New public entries must be verified or credible. Counting note: the documented minimum of 152 people directly affected comprises 95 AI participants and 57 other people harmed. 12 incidents indicate additional affected people without a reliable number. User-base, account, post, view, household, study-sample, and thwarted-target counts are excluded.
