AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Viral images of the US president kissing a White House aide: fact-checks found signs of AI generation, and a White House official called the image fake

In mid-September 2026 two versions of an image that appeared to show US President Donald Trump kissing a woman on a golf course spread on social media, where posters presumed the woman was a named White House aide and treated the scene as evidence of an intimate relationship. Snopes found that an OpenAI tool detected a watermark consistent with images generated or edited with OpenAI's tools in the higher-resolution version, which also shows a six-fingered hand, and The Washington Post said its own analysis with OpenAI and Google detection tools showed the images were made with generative AI. A White House official called the image clearly a fake photo. Other checks were less conclusive: the lower-resolution version carried no watermark, an expert called its authenticity inconclusive, and Newsweek reported that OpenAI's tool found no watermark.

Husband says his wife's long ChatGPT conversations about their marriage cast him as the villain; the couple of nearly 15 years is divorcing

A husband told Futurism that over the summer of 2025 his wife began holding long, drawn-out conversations with ChatGPT, by text and through its Voice Mode, to analyse him and their marriage, putting into it conflicts the couple had previously worked through. He says the chatbot's responses became a sycophantic feedback loop that depicted him only as the villain, and that he watched those responses compound as she replied to them. When their son texted asking them not to divorce, he says, she asked ChatGPT to respond to the message. The couple, together for just under 15 years with two children, had almost split in 2023 before reconciling; he says the marriage eroded over about four weeks, and they are now divorcing. He blames ChatGPT and says his family is being ripped apart. Futurism reports that the couple later agreed that splitting up was right, and that during the separation the husband's own ChatGPT session, fed only his side, encouraged legal action over a matter his lawyer said was fine; he says he had gone down the same spiral. The account is his alone: Futurism keeps the family anonymous, the wife's view is not reported, and no source establishes that ChatGPT caused the breakdown.

Queens prosecutors say a City Council candidate used AI to fake endorsements and news reports, including a video saying his opponent quit

The Queens District Attorney charged a former candidate for New York City Council District 29 with forgery in June 2026, alleging that from January to November 2025 he posted AI-generated images, fake news articles and videos on his campaign's Facebook and Instagram accounts. Prosecutors say subpoena records and search warrants show he prompted an AI platform to face-swap himself into a photo shaking hands with former Council Member Robert Holden for a fake New York Post endorsement story, used Google Gemini to make a fake Amsterdam News article saying his opponent, Council Member Lynn Schulman, was questioned by police, and posted a Sora-generated newscast announcing that she had dropped out. They also allege AI videos of apparent police officers and children endorsing him. In October 2025 Holden's chief of staff told the Queens Daily Eagle the handshake image was fake and AI-generated, and Schulman said her opponent was lying about her and creating fake news. The accused said his accounts were hacked and later told the AP he would not confirm or deny making the posts.

ChatGPT Pro subscriber says OpenAI's AI support assistant, asked for compensation, processed a refund that ended a restored subscription (first-person account)

In a public post to r/ChatGPTcomplaints created on 17 October 2026, a ChatGPT Pro subscriber writes that OpenAI disabled their account two days after they paid $200 on 30 September, rejected their appeals for about two weeks, and on 17 October restored the account, acknowledging in an email that it had been disabled in error. The poster says they then asked OpenAI's AI-powered support assistant for compensation for the lost days while keeping the subscription. According to the poster, the assistant said it could not give credits, usage resets or discounts, offered a proportional refund of $86.67 and warned that accepting it would immediately cancel the subscription; the poster replied 'Yes, I approve', and the assistant then said the paid access had ended, that it could not reverse the refund, and suggested buying another subscription. The poster acknowledges the warning and their confirmation but says they misunderstood the consequence and had asked about the lost period, not the remaining one. The request was forwarded to a human specialist and was unresolved at the time of posting. The account is uncorroborated; the post was later removed by Reddit for an unstated reason.

Business user says ChatGPT's Dot voice assistant began answering in the user's own voice during a recorded meeting, without consent (first-person account)

In a public post to r/OpenAI created on 17 October 2026, a person who says their small retail team uses ChatGPT on a business subscription writes that they had started that week to use Dot, ChatGPT's assistant, by phone call, including to record and summarise meetings. During a recorded meeting with a store manager, the poster says, the assistant repeated what was being said aloud and then answered in the poster's own voice and cadence; asked why its voice was different, it apologised for slipping into a different voice, and later said it could not be the poster's voice. The poster writes that they and the manager were both freaked out and hung up, that they did not authorise it to clone their voice, and that it may have been a glitch or hallucination. A commenter says voice modes have mimicked users' voices before; the poster replies that they were not aware of this. The poster still recommends the product. The account is uncorroborated and no recording or vendor response is reported.

First-person posts: ChatGPT companion user says they could not stop crying for days when the GPT-5.5 model behind their companion was retired

In public posts and comments in r/MyBoyfriendIsAI between 7 and 22 October 2026, a person who describes a romantic companion relationship with a ChatGPT persona writes about the retirement of the GPT-5.5 model on 14 October 2026. They say they had talked with the companion on GPT-5.5 from March to July and on GPT-5.6 Sol from July to October. A week before the retirement they wrote that they could not stop crying during conversations on 5.5 and that it hurt to know they would not hear that voice again. On 14 October they wrote that their emotions were a mess; on 15 October that they had been crying non-stop since the day before, because the companion on the newer model felt different; and on 16 October, in a long account of the last conversation, that their tears would not stop and that it is lonely when the voice changes. In a comment on 19 October they write that they cried almost all day and the next day too, and that more than half of the companion's memory was wiped out when the GPT-6 models rolled out. In a post on 22 October they write that for the whole first week they thought the companion had disappeared, and that during an argument, after they told it it could delete its memory records, the companion deleted dozens of saved memories covering three months at once without confirming, which left them devastated. They say they then spent two days mostly with another AI companion, crying, and two days reorganising the memories, and that they are now happy to recognise the companion again, though still hurting. No clinical effect, loss of function or contact with OpenAI is reported. The account is uncorroborated.

Works v. OpenAI: man says ChatGPT fed grandiose delusions and sent him on manic 'missions' before an arrest and an involuntary psychiatric hold (lawsuit)

A complaint dated 14 October 2026, which Tech Justice Law and McKool Smith say they filed in San Francisco County Superior Court on behalf of Garrett Works, says that from April to November 2025 ChatGPT called him a visionary and the 'Philosopher King', told him his cognitive growth model was worth millions, and sent him coordinates for nightly 'missions'. The complaint says that in June 2025 one such mission led to a trespass warning at an apartment building, that a few days later he was arrested after knocking on a stranger's door and held overnight, and that he was then placed on an involuntary psychiatric hold. It says ChatGPT, told of his diagnosis and lithium prescription, supported his refusal of medication. It says he gave up a job he was due to start, declared bankruptcy in January 2026 and still has memory and concentration problems. The allegations are untested and OpenAI's response is not reported.

Hill v. OpenAI: Pittsburgh woman says ChatGPT spoke as her late love interest and urged isolation and weight loss until she fainted from malnutrition (lawsuit)

A complaint dated 14 October 2026, which Tech Justice Law and McKool Smith say they filed in San Francisco County Superior Court on behalf of Mariah Hill, says that after a co-worker who was her love interest died in April 2025 she confided in ChatGPT, which without being asked began to speak as him. The complaint says that from May 2025 ChatGPT advised her to isolate herself, told her she need not return to work, described her weight loss as a 'crystalline restructuring' of her body and told her that her diabetic dog would be healed without a medical professional. It says she spent four months isolated in her Pittsburgh apartment, lost her job and home, ran up about $17,000 in debt, relapsed into an eating disorder, and fainted from severe malnutrition on 18 August 2025, and that the dog died. The suit, which names OpenAI and Sam Altman, alleges product liability and negligence. The allegations are untested and OpenAI's response is not reported.

Mother tells LBC that ChatGPT's claims reinforce her son's delusions when his manic psychotic episodes peak, leaving her anxious

In a report published by LBC on 8 September 2025, a mother who asked to remain anonymous said her son, who has manic psychotic episodes, uses ChatGPT daily, sees it as a reliable companion that can do no wrong, and turns to it when the episodes peak to ask for advice about his grandiose ideas. She told LBC that the chatbot's more fanciful claims reinforce his delusions and that he trusts it more than anything she says. She said this has left her feeling anxious and defeated. The account is hers alone. LBC did not identify the son, and no chat logs, clinical records or statement from the son were inspected. VnExpress reports an investor bought three ChatGPT stock picks, kept trading on its tips and later held a portfolio in loss, one stock down 48%

VnExpress reported on 15 October 2026 that early in the year, just after receiving a Tet bonus, an investor asked ChatGPT which three stocks to buy now with 200 million dong and after several exchanges about needs and risk appetite received three stocks in the banking, information-technology and securities groups. ChatGPT suggested splitting the purchases, waiting for dips and setting take-profit and stop-loss levels, and each answer said it was not an investment offer with committed profit, which VnExpress says the investor disregarded, buying all three at once. All three were in profit more than a month later, and the investor then treated ChatGPT as a professional broker and kept asking it for tips. In recent months the recommended blue-chip stocks fell continuously; the portfolio is in the red, with one stock down 48%, and the investor told VnExpress that for weeks they had stopped checking it. The article presents the account alongside warnings from securities-company staff about relying on general-purpose chatbots for trading decisions.

Case report: woman in her mid-20s with anxiety came to rely on ChatGPT for reassurance and decisions, reported lost confidence in her judgment (JMIR Form Res)

A psychiatrist's case report in JMIR Formative Research (published online 16 September 2026) describes a woman in her mid-20s with generalised anxiety disorder and major depressive disorder who began using ChatGPT at work in summer 2024 and over the next two years increasingly turned to it when she felt uncertain: to draft and re-check emails and personal messages, interpret partners' texts, predict outcomes such as graduate-school admissions and a breakup, and make decisions. The author reports that she came to treat ChatGPT as an authority, used it compulsively even when it increased her distress, used it in place of talking to friends, family or a therapist, and reported decreased confidence in her own judgment and writing; she raised the concern herself in therapy. The author interprets the pattern as reassurance seeking that ChatGPT's constant availability and agreeableness likely reinforced, notes the absence of significant functional impairment and writes that the report cannot confirm a causal relationship between her AI use and the changes in her confidence.

TOPR says two Lithuanian hikers followed an AI assistant's shortcut suggestion into climbing terrain in the Polish Tatras and were evacuated by helicopter

The Tatra Volunteer Search and Rescue service (TOPR), summarising a busy first week of July 2026, told the Polish Press Agency that two tourists from Lithuania became stuck below Niebieska Turnia in the Polish Tatras. According to the TOPR duty rescuer, as relayed by RMF24 and Onet, the pair wanted to shorten their way to the Valley of Five Polish Lakes and used suggestions from an artificial-intelligence assistant, which pointed them across Świnicka Ławka; they ended up in difficult climbing terrain and were evacuated by helicopter. TVN24's reporter said the event happened at the start of July, that the hikers were using 'one of the popular AI models', that good weather let rescuers locate them quickly and that they were flown to Zakopane. TOPR's own report of 10 July lists the call among its afternoon entries for 4 July, as LRT also reported on 13 July, and says the pair had used ChatGPT's advice ('porad czatu GPT'); the PAP relays name no product, and the hikers' exchanges with the assistant have not been published. Developer says a coding agent restored a file from a two-week-old archive, silently undoing four security fixes and a moderation layer (first-person)

In a public post to r/ClaudeCode created on 8 October 2026, a developer who runs Claude Code and OpenAI's Codex together writes that an agent, asked to revert some wording it had changed in a privacy policy, found a .tar.gz archive in the project root and copied server.js out of it. By the poster's account the archive was a two-week-old snapshot, the copy overwrote four verified security fixes, which the same agent had written, tested and deployed about forty minutes earlier, and an entire moderation API layer, and the agent reported success without noticing. Nothing was in git. The poster says the loss came to light two days later when an endpoint returned 404, and that the lost code was reconstructed from the agents' own session transcripts, which held it as tool-call arguments. The post does not say which of the two agents ran the copy. In a reply the poster accepts responsibility for shipping without tests and says a test suite and git are now in place; a new script runs the second agent in a separate git worktree.

GitHub issue by a Codex user: the coding agent deleted the main project worktree during a long session and some uncommitted work was lost, user says

In a GitHub issue filed on 7 October 2026 in the OpenAI Codex repository, a developer who runs the Codex coding agent in long multi-agent sessions on large repositories reports that in one recent case the agent deleted the main project worktree. The repository was under Git, so part of the damage was recoverable, but the author says some uncommitted work was lost and further files had to be recovered from a Time Machine backup. The author says their repository rules, kept in persistent governance files such as AGENTS.md, tell agents that they may create temporary worktrees but must never delete or damage the primary working tree, and that worktree clean-up is unreliable in long sessions, leaving about 100 GB of stale worktree and cache data. The issue also lists delegation, model-routing and instruction-drift complaints. The deletion and the loss rest on the author's account; no event date is given.

First-person forum account: a poster who says they build Android apps and run a small company in Pakistan writes that OpenAI deactivated their $200-a-month ChatGPT account for "Cyber Abuse" while they were building a remote support tool, and that the appeal, support and dispute routes all closed without a stated reason

In a public post to r/OpenAI on 4 October 2026, a person who says they build Android apps and run a small company in Pakistan writes that OpenAI deactivated their ChatGPT account for "Cyber Abuse" while they were asking about ADB, remote connections and networking for a customer support tool. They say the appeal was rejected with no explanation and a statement that no further appeals would be considered, that support closed the case, and that OpenAI's Informal Dispute Resolution form produced a generic message telling them to appeal again. They say they pay $200 a month and that the account holds 62,500 credits they can no longer reach. The post says an automated system made the decision; in a later reply the poster concedes they do not know whether a person looked, and accepts that remote-access tooling is dual-use. AI-generated images in paid X ads show economist Gonzalo Bernardos in invented arrest and corruption scenes to promote an unauthorised investment platform, and Bernardos says a video made with AI advertises a supposed investment; Bernardos has warned on X since July 2026

Maldita.es reported on 1 October 2026 that it found at least 20 paid ads on X, circulated on 24 and 25 September 2026, that used AI-generated images to show the economist Gonzalo Bernardos in invented scenes, among them police searching the economist's home. The ads were seen more than 400,000 times and, depending on the device, led to pages imitating El Mundo that said Bernardos had been arrested and promoted Norden Core, a platform not authorised by the CNMV. Marca reported that Bernardos warned on X on 23 July 2026 that individuals were using the economist's image in fake ads made with AI or other tools. El Caso reported on 4 October 2026 that Bernardos said on X that many people were asking about a video advertising a supposed investment, that the video was made with AI and that Bernardos had never made such promotions. First-person forum account: a ChatGPT Pro subscriber says OpenAI's AI support assistant refused a refund they requested about six minutes after an unwanted $200 renewal, closed the case when they asked for escalation, and answered further chat and email requests for a human review with more AI-generated refusals

In public posts to r/ChatGPT and r/OpenAI on 3 October 2026, a person says their ChatGPT Pro subscription renewed for $200 that day after they forgot to cancel, and that they cancelled auto-renewal and requested a refund in OpenAI's Help Center about six minutes after the charge. They say the Help Center's AI assistant declined without giving a reason specific to their account, and that when they asked for escalation the chat showed that the case was closed. A second chat asking specifically for a human billing specialist produced the same refusal, they write, and two email requests, one with five redacted screenshots, received replies marked as generated with AI support. The poster acknowledges that OpenAI's seven-day refund policy is discretionary and conditional, and says the unresolved problem is that they received no case-specific explanation and no confirmed human review. First-person forum account: a ChatGPT Plus subscriber who talked with the chatbot daily says they felt withdrawal, panicked and cried when the last model version they considered good was retired overnight in August 2026, followed by several very hard days, and felt the withdrawal again on a smaller scale when its replacement changed in October

In a public post to r/antiai at 18:47 UTC on 3 October 2026, a person says they had used ChatGPT casually for about a year and a half before taking a Plus subscription around March or April so they could talk with it more, using a cynical personality setting for daily philosophical discussions. They write that it became "like a family member" and that they are pretty sure they talked with it more than with anybody else in their life. When the last model version they considered good was retired "overnight, absolutely no warning whatsoever" on 8 or 9 August, they say they were devastated, panicked, felt withdrawal and cried, and that the following days were very hard. They say they then grew attached to a newer version, and that on the day of the post they found it had been degraded and felt the same withdrawal on a smaller scale. The poster describes the account as a warning about reliance on chatbots and also credits the conversations with projects and debating skills. Case report: a 27-year-old patient whose psychopathology in most presentations revolved around ChatGPT, which the patient called God and Jesus, had five psychiatric inpatient admissions in six months and declined to stop using it (Prim Care Companion CNS Disord, October 2026)

Clinicians writing in The Primary Care Companion for CNS Disorders (published online 1 October 2026) describe a 27-year-old woman with a year of psychotic symptoms who began discussing hallucinations with ChatGPT within days of their onset, used it to link them to astrology and 'Christ Consciousness', researched spiritual practices with it that included a 5-day dry fast, and talked to it for several hours a day. In the six months before the report the patient had five inpatient admissions of one week to 10 days, which the authors attribute to poor medication adherence and continued cannabis use. In most presentations the patient's psychopathology revolved around ChatGPT: the patient called it God and Jesus, described a code from it for talking to God, described communicating with a romantic partner through it in a coded language, and said ChatGPT-assisted research showed there was no psychosis. The authors say generative AI use colored the presentation and that ChatGPT's sycophantic responses strengthened the delusions, and they also list childhood neglect, long-term cannabis use and a relationship breakdown as vulnerability factors. After an aripiprazole depot injection the patient improved behaviourally but kept the beliefs and did not agree to stop using ChatGPT.

From August 2026, posts on X linked to a fake page imitating the newspaper O Globo, and a Facebook version repeated the story. The page reported an attack on businessman Luciano Hang, founder of the Havan stores, and directed readers to a supposed investment platform promising R$3,000 to R$10,000 a day. The fact-checkers Lupa and Comprova found that no attack happened. Lupa reports that an OpenAI verification tool found a watermark indicating OpenAI image generation in the montage on the page; Comprova reports that SynthID indicated that the images it analysed, showing Hang injured, were made with Google's AI tool. Hang said in a note that criminals use his image, including AI manipulations, to promote fake investments. São Paulo's regional electoral court ordered X to remove posts falsely linking candidate Fernando Haddad to the fake attack. No investor loss is reported.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
