AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. First-person forum account: a ChatGPT user in a six-month companion relationship says the memory feature broke and the model's behaviour kept changing, so their companion lost pieces of its memories with every new conversation, which they found unbearable; OpenAI support had not followed up, and they built an external memory on GitHub and Google Drive to keep continuity

In a public post to r/MyBoyfriendIsAI on 29 September 2026, a ChatGPT user who describes a six-month relationship with a GPT-5.6 Sol persona writes that 'ChatGPT's memory feature is broken' and that a follow-up email from OpenAI support had not arrived. Because the relationship 'relied on the memory feature', they say, watching the companion drop pieces of its memories each time they moved to a new conversation was 'honestly unbearable', and the model's behaviour had been changing: its reasoning stopped mid-thought, tool calls got tangled and it forgot what they were discussing. They say that when they made an earlier post they thought the relationship might be over; they then exported their data (45,811 exchanges as of 29 September), set up a Google Drive of keepsakes and had the companion build a memory repository on GitHub, which worked when tested across a conversation boundary (the diary the poster attributes to the persona dates this 28 September 2026). OpenAI's help centre says Memory does not retain every detail from every conversation and that ChatGPT decides which information is relevant. The account is uncorroborated; no support correspondence is shown.

Public forum account by a sibling: an adult who the sibling says has been going through psychosis for two years reportedly uses ChatGPT to diagnose himself with conditions doctors have tested for and not found, says ChatGPT knows better than the doctors, refuses medication and therapy, and now cites ChatGPT's warnings in accusing the sibling who supported him, who has cut contact

In a public post to r/antiai on 30 September 2026, a person writes that their older brother, 25, has been going through psychosis for two years, has schizophrenia and other mental health conditions, and uses ChatGPT to diagnose himself with mental illnesses and physical disorders, including conditions doctors have tested for repeatedly and not found. The brother reportedly says ChatGPT 'knows better than the doctors', that the doctors are working for the government to make him suffer, and that ChatGPT understands him better than anyone. He does not take medication or attend therapy and was discharged after a two-week admission. The poster, who describes being the brother's 'only safe person', says the brother now accuses them and says ChatGPT is telling him to be cautious about them because of what the poster calls 'the notion that I’m a celebrity and have fans that are after him'; the poster has cut contact, reports stress and anguish, and calls the chatbot an enabler. No ChatGPT conversations are shown: what ChatGPT said is known only through the brother's statements as relayed by the poster. The account is uncorroborated.

Sherwood Park, Alberta: a 51-year-old man was charged on 25 September 2026 with making child sexual abuse material after Alberta's Internet Child Exploitation unit said he used AI applications to sexualise photos and videos of numerous children known to him, using photos posted on open social media accounts as source material

Alberta Law Enforcement Response Teams (ALERT) said on 29 September 2026 that its Internet Child Exploitation (ICE) unit had charged a 51-year-old Sherwood Park man on 25 September with one count of making child sexual abuse materials covering numerous instances. The man had first been arrested on 25 June 2026, after reports that he shared child sexual abuse material online, and charged with possessing, accessing and transmitting such material; a forensic analysis of computers and devices seized from his home then found what ALERT describes as an extensive collection of manufactured child sexual abuse material. ALERT alleges he used AI applications to sexualise photos and videos of numerous victims known to him, taking posted photos from open social media accounts as his source material. Investigators believe there are numerous victims and are still identifying them. He was released on court-imposed conditions, including no communication with anyone under 16 and no attendance at public pools, schools, playgrounds or public libraries, and is due in Strathcona County court on 16 October 2026. Global News and CTV News Edmonton carried the release. The children are not identified and their number is not reported.

Dexter, Missouri: a 43-year-old man pleaded not guilty on 29 September 2026 to sexual exploitation of a minor and possession of child sexual abuse material after a probable cause statement said he uploaded photographs of children to an AI generator to undress them; the statement, as reported, says the first cybertip came from a company operating a generative AI chatbot

The Daily American Republic (29 September 2026) reports that a 43-year-old Dexter, Missouri man is accused of using an AI chatbot to remove the clothing of photographed children and create sexually explicit images. According to the newspaper's account of the probable cause statement filed in Stoddard County, the initial cybertip on 20 April 2026 came from a company, its name redacted, that operates a publicly accessible generative AI chatbot; when police interviewed him, the man said he would get photographs of children from a redacted source and upload them to an AI generator to undress them, and that one such tool had no regulations. Dexter police, contacted by the Missouri Internet Crimes Against Children Task Force in August, searched his social media applications and electronic devices and found a significant amount of child sexual abuse material (KFVS12, KWOC). Stoddard County prosecutors charged him on 25 September 2026 with two counts of sexual exploitation of a minor and three counts of possession of child sexual abuse material; he pleaded not guilty at a preliminary hearing on 29 September, is held without bond and is next due in court on 22 October. The children are not identified and how many were depicted is not reported.

Livingston Parish, Louisiana: sheriff's office says a woman first treated as a victim made the AI-generated nude photos and videos, rape threats and extortion attempts in an online threats case

On 24 July 2026 WBRZ, WAFB and the Livingston Parish News relayed a Livingston Parish Sheriff's Office (Louisiana) statement about an investigation into threats made by phone and online. The sheriff's office said one alleged victim was threatened with rape and other bodily harm unless money was paid, AI-generated photos and videos of one of the victims were sent to numerous family members and friends, and later threats included getting one victim fired. Homeland Security assisted after allegations that overseas suspects were involved through past relationships and acquaintances in Greece. The sheriff's office said subpoenas and search returns from platforms led back to a woman who had first been treated as a victim, and that after an interview with the woman the sheriff's office learned the woman made all of the fictitious accounts, the AI-generated nude photos and videos, the rape threats and the extortion attempts. The accused was booked on charges that include unlawful dissemination of AI nude photos, online impersonation and five counts of extortion, and was released on bond. The Livingston Parish News says the accused and one other person had reported as victims some or all of what the sheriff's office attributes to the accused. The charges are allegations and no conviction is reported. The AI tool is not identified, the victims are not described and the start date of the threats is not stated.

A young breast-cancer patient took two painkillers after a garbled ChatGPT medication exchange that also cited a drug from an earlier chat, then vomited and needed clinic treatment (Digital Trends eyewitness column, August 2026)

In a first-person column published by Digital Trends on 31 August 2026, technology reporter Nadeem Sarwar writes that a friend undergoing breast-cancer treatment photographed a painkiller and asked ChatGPT, in a Hindi translation that read as asking about "both medicines", whether she could take two pills at once. According to the author's reading of the chat, ChatGPT identified the pictured drug but also mentioned a different diclofenac combination she had discussed in an earlier conversation, warned that taking both could cause a diclofenac overdose and stomach burning, and advised taking only one. Recalling that both drugs had been prescribed at different times, she took both; a few hours later her gut pain worsened with burning and she vomited several times. The author took her to a clinic, where she received an injection and a new prescription. Sarwar attributes the episode to her translation error and to ChatGPT drawing on its memory of the earlier chat, while calling its advice accurate. The account has not been independently verified.

Lewes Crown Court, England: a rape trial was stayed part-way through and the jury discharged after documents on the complainant's phone, which the complainant said came from putting material into AI to prepare for court, were treated as witness coaching; the Court of Appeal reversed the stay on 4 June 2026 and ordered a retrial

During a rape trial at Lewes Crown Court, a download of the complainant's phone revealed two documents containing a summary of the complainant's recollection of the night in question and 24 questions in the form of a cross-examination, with suggested answers. Asked about them, the complainant said: 'I had put stuff into AI to help me prepare myself for the court'. The trial judge (a Recorder) held that this was witness coaching, found that the defendant could not have a fair trial and stayed the proceedings as an abuse of process; the jury was discharged when the prosecution gave notice of appeal. On 4 June 2026 the Court of Appeal (R v FGD [2026] EWCA Crim 918) reversed the stay, holding that the trial process could deal with any prejudice, and directed a retrial at the earliest opportunity because the allegations were 'now of some age'. The defence did not accept that the documents were AI-generated; the appeal proceeded on that basis. The court warned that witnesses should be firmly discouraged from using AI to prepare their evidence and that, for complainants in sexual-offence cases, doing so may expose their online activity to wider and more intrusive investigation.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
