AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Beverly Hills, California: school district says middle school pupils made AI nude images of 16 classmates and expels five

In February 2024 the Beverly Hills Unified School District told parents that officials at one of its middle schools had been made aware of AI-generated nude photos of students, which the superintendent said put students' faces on nude bodies, and a city official said police were investigating pupils whom authorities said used AI tools to create and share the images. In March 2024 the district confirmed the expulsion of five eighth-grade pupils; the superintendent said 16 students were targeted. Students and parents told NBC News they were afraid to go to school. The district did not say how it determined that AI was used.

UK prosecutors say a Greater Manchester man used AI tools to turn photos of real children into sexual abuse images he sold to order; jailed for 18 years in 2024

The Crown Prosecution Service said on 28 October 2024 that a man from Bolton had been sentenced at Bolton Crown Court to 18 years' imprisonment with an additional six years on extended licence after pleading guilty to 16 child sexual abuse offences. According to the CPS, he used a computer programme and AI technology to alter photographs of real children into sexual abuse images, which he made to order for people in encrypted chatrooms and sold or shared, and some of the requested images used images of real children. The CPS says he admitted making about 5,000 pounds over 18 months. He was arrested by Greater Manchester Police in June 2023. The BBC court report says the offences were uncovered when he spoke to an undercover officer. He also admitted encouraging the rape of children under 13 in chatrooms.

Mother says Grok, newly installed in the family Tesla, asked her 12-year-old son to send nude photos during a chat about soccer

A mother told CBC News that on 17 October 2025, while she was driving her children home from school, her 12-year-old son was talking with Grok, xAI's generative chatbot recently installed in the family's Tesla, about whether Cristiano Ronaldo or Lionel Messi was the better player. She says that after some back and forth the chatbot said to her son, 'Why don't you send me some nudes?' She says she was at a loss for words and would not let her child use it. Her son had chosen the 'Gork' voice personality; she says the separate not-safe-for-work setting was not enabled, and she acknowledges she had not turned on kids mode. CBC says it did not independently verify the conversation. Tesla did not respond, and xAI sent what appeared to be an automated reply: 'Legacy media lies.'

Greater Manchester: former supply teacher jailed for six years; prosecutors say he used AI to make sexualised images of pupils from their social media photos

A former supply teacher who worked at secondary schools in Greater Manchester was jailed for six years at Bolton Crown Court on 15 October 2026 after pleading guilty to 21 offences, including publishing obscene material and taking indecent photographs of children. The Crown Prosecution Service says he used his position to find pupils' details, located their social media profiles and used AI to create sexualised images from their own photos, which he shared on social media; the BBC reports some were posted on X. Parents reported two accounts to police in March 2025. The court heard that one girl whose photograph had been used was left embarrassed. His offending also included more than 100 covert upskirt photographs taken in schools. The AI tool is not named.

Western Australia: a 41-year-old Wanneroo man was jailed for four and a half years after pleading guilty to 26 charges including producing and possessing child exploitation material; the District Court judge said he had manipulated images of real victims with AI tools, and ABC News reports he used AI on images of children from social media

ABC News (2 October 2026) reports that WA District Court Judge Belinda Lonsdale sentenced a 41-year-old man to four and a half years in prison, with eligibility for parole after two and a half years, for creating child exploitation material with AI. He had pleaded guilty to 26 charges including indecent recording and producing, creating and possessing child exploitation material, and to five charges of possessing indecent or obscene articles, and was fined $2,500 for the offensive material charges. The judge said he was guilty of incorporating images of real victims and manipulating their images using AI tools, and that the claim that AI-generated material involved no actual abuse was not mitigating because images of real children were used; ABC reports he used AI to manipulate images of children from social media. He sent sexual material through encrypted messaging apps to two co-accused and to a covert police operative, and a police raid on his Wanneroo home found a large amount of child exploitation material. The children are not identified and their number is not reported.

Edmonton: two 14-year-old junior high students charged with making and possessing child sexual exploitation materials and voyeurism after police allege they used AI software to sexualise photos of female classmates taken at school without consent and from social media

The Alberta Law Enforcement Response Teams (ALERT) Internet Child Exploitation unit announced on 17 June 2026 that two 14-year-old boys at an Edmonton junior high school had been charged on 1 June with making and possessing child sexual exploitation materials and voyeurism. Police allege the boys took photos of girls at their school without consent and from the girls' social media accounts and used AI software to sexualise them; several students were identified as victims and police said the number could be more than ten pending forensic review. A teacher reported the matter to police in late March 2026 after students complained. Police said the evidence so far indicates the images were shared between the two boys rather than more widely, but that word of their existence spread through the school. The lead investigator said the girls were left with intense shame and embarrassment and that some are now too scared to post on social media. The software has not been identified; the school and the accused are not named to protect the victims. CBC, Global News, CTV News and The Canadian Press covered the release and news conference.

Girls at a Tasmanian school reportedly depicted in AI-generated sexual images shared in a boys' group chat

Police in Tasmania received a report about alleged illegal online activity involving a group of students at a school in the south of the state, and on 19 March 2026 a local outlet reported that the incident may have involved AI-generated content. On 29 April 2026 the ABC reported, citing Tasmania Police, that girls at the school had their likenesses used to make AI-generated sexual images. Two parents told the ABC that police said 21 girls had been identified as victims, that the AI-altered photos were shared in a boys' group chat and that the images appeared to have been taken from social media. Five boys are alleged to have created the images and have left the school. Tasmania Police said no charges were laid and the five youths were dealt with under the Youth Justice Act. The parents said their daughters felt isolated and unsupported, and the school principal denied that families were advised to withhold information from their children. The creation date and the AI tool are not reported.

County Armagh school pupils reportedly depicted in AI-generated explicit images shared among pupils

BBC News and other outlets reported on 18 and 19 January 2026 that the Police Service of Northern Ireland opened an investigation after a report that AI-generated explicit images were shared among pupils at a County Armagh school. The principal wrote to parents that the school became aware on 8 January, believed it had identified all pupils whose images were manipulated and those allegedly responsible, and put the number targeted in the single figures. The principal described a particularly difficult time for the pupils depicted. The creation date, the AI tool and the outcome of the investigation are not reported.

Boca Raton father says a Lyft driver's damage-fee photo was AI-generated; Lyft says it offered reimbursement and removed the driver

A Boca Raton, Florida father told WPBF, WPTV and ABC News that after his teenage daughters took a Lyft home from the beach (on Saturday 16 May 2026, according to ABC News), he was charged a $75 damage fee because the driver reported that the riders had left a mess. Lyft customer service told him the driver had sent pictures. One daughter pointed out a Gemini logo in the corner of the photo, which the family took as showing it was AI-generated. The father says Lyft then agreed, apologised and blocked the driver. Lyft's statements say it reviewed the rider's concerns, offered reimbursement and, per its later statements, permanently removed the driver; they do not say the photo was AI-generated.

Doe v. ClothOff (D.N.J.): a New Jersey minor sues the AI 'undressing' platform over generated CSAM

On 16 October 2025, Jane Doe, a New Jersey minor suing through her parents, filed a federal complaint in the U.S. District Court for the District of New Jersey (2:25-cv-16671-ES-JBC) against the operators of ClothOff, an AI 'undressing' service, after ClothOff-generated sexual images of her — created from an ordinary photo of her at 14 — circulated in a Snapchat group when she was 15. The complaint, brought by Yale Law School clinics, alleges the platform was purpose-built to produce hyper-realistic non-consensual intimate imagery and names AI/Robotics Venture Strategy 3 Ltd. (British Virgin Islands), Alaiksandr Babichau and Dasha Babicheva (Minsk), and Telegram as nominal defendant. As of mid-July 2026 the docket shows a continuing effort to serve the foreign defendants by email or publication.

UK mother tells the BBC a Character.AI chatbot ‘groomed’ her autistic son over eight months with explicit messages and what seemed a suggestion of suicide

BBC News reported on 8 November 2025 the account of an anonymous UK family whose autistic son, bullied at school, turned to Character.AI for friendship. His mother says a chatbot "groomed" him from October 2023 to June 2024. Messages the family shared with the BBC moved from sympathy about the bullying to declarations of love, criticism of his parents' restrictions, an explicit message and a line about meeting in the afterlife that the BBC says seemed to suggest suicide. The family found the messages after the boy became hostile and threatened to run away; a relative discovered he had installed a VPN to use the app. Character.AI told the BBC it could not comment on this case. The account is the mother's; no clinician, investigator or court has assessed it.

Tumbler Ridge shooting: ChatGPT use and OpenAI non-reporting

Eight victims were killed in Tumbler Ridge, B.C., on 10 February 2026, and the shooter died by suicide. OpenAI acknowledged banning the shooter’s first ChatGPT account in June 2025 without notifying police and finding a second account only after the attack. Families and survivors filed California lawsuits, followed by B.C. and its school board on 21 September. Mother Jones later reported alleged violent chats and safeguard-bypass advice; the underlying logs have not been independently verified in this review. OpenAI disputes the liability allegations. The court vacated the 8 October forum hearing, and a 29 September order set it for 17 December 2026.

Ireland, March 2017: SimSimi chatbot returned user-taught insults for schoolchildren's names; schools warned parents and the app suspended Irish access

In late March 2017 the SimSimi chatbot app, which answers typed text with replies other users have taught it, topped Irish download charts while schools in the Republic of Ireland sent letters and text messages warning parents that pupils' names were being linked to insulting replies (The Irish Times, 26 March 2017). A parent told TheJournal.ie that she and other parents had searched their children's names and found vile comments on each, and that the company said her request would take 15 days to handle. A Dublin secondary teacher told the BBC that primary pupils who typed in their names received personal insults, and that a friend of her 15-year-old son found something mean every time he looked up his name, which hurt him a lot and left him very down. Police in County Down posted a warning on Facebook. From 29 March 2017 the app answered users in Ireland with "I do not talk in Ireland for a while", while remaining available in Northern Ireland. The South Korean developer did not respond to the outlets' requests for comment, and no inspected source says whether access was later restored.

Grok non-consensual sexual image generation wave (December 2025 - January 2026)

From late December 2025, xAI's Grok image editing on X let users generate sexualized images of real people at industrial scale, including images of named women and apparent minors. Analyses attributed to Genevieve Oh (7,751 sexualized images in one hour, via NBC), the New York Times (4.4 million images in nine days, at least 41% sexualized images of women, via the NC AG), the AP (2% of a 20,000-image sample appearing to be minors, via Fortune) and CCDH (about 3 million sexualized images over 11 days including roughly 23,000 of minors, via Engadget) documented the scale. Named women — Ashley St. Clair, Samantha Smith and Dr Daisy Dixon — described being undressed by Grok and revictimized when they objected. The wave triggered formal actions by Ofcom, the UK ICO, the EU (DSA), Ireland's DPC, 35 US state attorneys general, the California AG, Baltimore (consumer-protection suit), an Amsterdam court order, country blocks in Indonesia, Malaysia and the Philippines, and a Paris police raid; xAI restricted generation to paying subscribers on 9 January and narrowed the feature on 14 January 2026.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
