AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Lebanon police say pages sold remedies using AI-generated videos falsely showing a complainant endorsing them; arrests and raids followed

In a statement published on 28 October 2026, Lebanon's Internal Security Forces (ISF) said their Cybercrime and Intellectual Property Bureau had received a complaint, filed with the competent judiciary, against Facebook and TikTok pages that promoted various medicines and preparations using videos generated by artificial intelligence, in which the complainant appeared in a way that falsely suggested the complainant was promoting the products. The products were said to treat stomach and joint pain, prostate and heart problems, blood sugar and blood pressure, and included sexual enhancers and slimming preparations. A patrol caught two people in the act of handing dozens of orders to a delivery company, with 13,602 US dollars they said came from daily sales; raids in Beirut and other areas led to further arrests and the seizure of thousands of containers. The ISF said some mixtures did not match the ingredients on the labels, that paper records of thousands of orders indicated a large number of buyers, and that the Ministry of Public Health was testing the seized products. The ISF did not say how it established that the videos were AI-generated, and the complainant is not named.

Pleven, Bulgaria: prosecutors say woman lost 33,000 euros after a probably AI-generated social-media report of a TV host and a politician touting an investment

The Pleven District Prosecutor's Office in Bulgaria announced on 12 June 2026 that it was leading an investigation after a woman living in Pleven province reported an investment fraud. According to the prosecutors, she had been misled by a report broadcast on a social network, probably generated by artificial intelligence, in which a popular television presenter and a political leader promoted, in an interview, an investment programme supposedly introduced in Bulgaria. The release's headline says the report used the voices and faces of the two people. She followed the links in the report, was immediately called on Viber by a person who presented as the company's manager, and, on that person's instructions, transferred a total of 33,000 euros in several transactions of up to 10,000 euros each, after which contact stopped. The prosecutors do not say how they concluded that the report was AI-generated, do not name the people shown and report no suspect.

Ombudsman upholds autistic student's complaint over AI-misconduct zero mark from panel with detection-software report; student says provider later cleared them

A case summary published in July 2025 by the Office of the Independent Adjudicator for Higher Education (OIA), the independent student complaints scheme for England and Wales, describes a student with autism who was accused of including large sections of AI-generated content in an essay. The student denied using AI and submitted planning and notes. A disciplinary panel, which had the provider's AI detection software report and compared the essay with other work by the student, found academic misconduct and gave a mark of zero with any reassessment capped at a pass; the provider rejected the appeal, in which the student said the same software had earlier flagged another of their essays in a case where it had been agreed they had not used AI. The OIA found the complaint Justified: the panel had not properly considered the student's evidence, had not shared some evidence it relied on, and had not given sufficient reasons. It recommended reconsideration and changes to the provider's rules, and the student told the OIA that on reconsideration the provider decided there had been no misconduct. The provider and the software are not named.

La Rioja, Spain: Guardia Civil says a parent sent 9,980 euros after a call with an AI clone of their daughter's voice; the money was recovered

The Guardia Civil in La Rioja said on 25 August 2026 that it had solved a 'relative in trouble' fraud in which the criminals used artificial intelligence to clone the voice of the victim's daughter. According to the force's account as reported by the newspaper La Rioja and Radio Arnedo, the victim received WhatsApp messages from an unknown number by someone posing as the daughter and asking for urgent money; when the victim called that number to check, a voice that sounded like the daughter answered, and the victim transferred 9,980.25 euros. Italy: a 66-year-old pensioner in Fano shows his jewellery on a video call from a fake police inspector whose on-screen Polizia di Stato emblem a local paper says was made with AI, and an accomplice takes all of it

Corriere Adriatico reported on 1 October 2026 that a 66-year-old pensioner in Fano answered his landline to a caller posing as a police inspector, who said his car was involved in a jewellery-shop robbery and that he had to check his jewellery at home to avoid legal trouble. The call moved to a video call in which the caller had the Polizia di Stato emblem on screen, which the paper says was created with artificial intelligence. Ards and North Down, Northern Ireland: a person lost £250,000 to an investment fraud that began with an AI-generated video of a well-known financial personality

On 7 September 2026 the Police Service of Northern Ireland (PSNI) said a person in the Ards and North Down area had lost £250,000 in what it called a 'sophisticated and substantial' investment fraud. The victim first saw what appeared to be an AI-generated online video featuring a well-known personality from the financial sector promoting an investment with significant returns. After investing a small amount the victim was contacted on WhatsApp and encouraged to pay more, persuaded to open multiple online accounts, to allow the fraudsters remote access to a computer 'to assist with the investment', to borrow money to keep investing, and finally to make further payments to 'release' funds supposedly held in the investment. The PSNI did not name the personality; BBC News NI noted that fabricated endorsements of financial experts such as Martin Lewis, Peter Jones and Steven Bartlett circulate online.

Manhattan DA says 12 seized websites sold AI-generated sexual deepfakes of about 1,200 people; the depictions' dates and an exact count are not established

On 14 September 2026 the Manhattan District Attorney's Office announced that it had seized, under a court order, 12 domain names of websites it says were used to disseminate, publish and sell non-consensual 'deepfake' videos, and said its investigations were ongoing. The office alleges that people under investigation used AI image and video creation tools to turn existing photos and videos of approximately 1,200 real people, overwhelmingly women and mostly public-facing figures, into sexual images and videos that were then sold. The 1,200 figure is the office's estimate and is not a documented count. WIRED reported that it had observed seizure notices on several of the sites in the days before the announcement and quoted researchers who described the videos as pornographic films manipulated to show the targets' faces, many apparently reposted from the defunct MrDeepFakes site. When the depictions were made and published is not established; one site claimed to have been online since 2018.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
