AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Jeju: police refer a middle-school student over secretly filming teachers and making sexual deepfake composites; teachers dispute victim count and legal aid

Jeju Dongbu police announced on 23 September 2026 that a student at a middle school on Jeju had been referred to prosecutors without detention on 17 September for violating the Sexual Violence Punishment Act (filming with a camera or similar device). The student's phone was voluntarily submitted by his guardians and forensically examined; the examination found footage relating to some teachers and some sexual composites, and the student admitted the conduct. News1 reported that the student was booked for filming a teacher's body with his phone during a school field trip around mid-July 2026, that the offending had continued for several months with multiple victims, and that police found no indication the composites had been distributed. Police told Seoul Shinmun there were more victim teachers than the one first reported but withheld the number to avoid secondary harm, and that no student at the school was a victim; the Jeju teachers' union questioned whether the full scope of harm had been established. According to the union, two months on the victim teachers were receiving hospital treatment and counselling while teaching daily, gave police statements on 14 September without a lawyer, attended the regional teachers' rights protection committee on 17 September without institutional support, wrote the criminal complaint themselves, and were offered 3.3 million won in legal fees for all of them together, payable after the case ends; two requests to meet the superintendent were not accepted. On 29 September one of the victim teachers gave a press interview at the Jeju Teachers' Union office. She said that early in the investigation she had seen on the student's phone material showing other teachers and current students, had made a list of the people and types of harm, and that the material shown to her at the police victim examination was considerably less than what she had seen; Yonhap-chain outlets quote her as saying the students she recognised numbered 10 to 20 and that a graduate had suffered worse deepfakes. She said investigators asked the school only to identify the perpetrator and never requested material to identify the students in the footage, and that the school sent parents a letter premised on there being no student victims. Police replied that phone forensics had been thorough, that every act beyond social norms had been identified and proven, and that one student victim had been confirmed, a student at a different school. The teachers were notified on 22 September that the case had gone to the prosecution and on 24 September that it had been sent to the court's juvenile division as a juvenile protection case, before they had submitted further evidence and a victim opinion. The teacher said the original complaint had also asked police to examine whether physical contact in 2025 was forcible molestation and whether repeated approaching and waiting in 2026 was stalking, and Halla Ilbo reports that such conduct is said to date from 2025. Yonhap-chain reports describe the charged victims as five teachers. The same day the education office briefed on legal-aid rules (up to 6.6 million won per instance when a teacher is sued, 3.3 million won per case when a teacher files a complaint; office lawyers cannot act as a teacher's private counsel), said it had linked the teachers to the national Digital Sexual Crime Victim Support Center, announced an integrated support system through a hotline, and its section head said the office was sorry that early support had not been felt. The KTU Jeju branch (28 September) and the Jeju Teachers' Union (14 and 29 September) demand a victim-support system, per-teacher legal fees paid in advance and a dedicated support team. On 6 October 2026 Seogwipo Broadcasting reported, citing people in education and a tipster, that the education authority had finally rejected the victim teachers' application for lawyer fees for an additional criminal complaint, saying in a written opinion that with a police investigation already under way a further complaint could hardly stop or prevent the infringement of their teaching, so the support requirements were not met; critics contrast this with full legal-cost support in earlier sexual-offence cases.

Logroño: the Guardia Civil investigates a 14-year-old who took photographs of nine underage classmates from their public social-media profiles, used AI tools to strip and sexualise them, and uploaded the fake images to an adult pornography website under a profile describing them as girls from his school made with AI, organised in folders with each girl's personal details; the images reached about 40,000 views before removal (reported 14 July 2026)

On 14 July 2026 Spain's Interior Ministry published a Guardia Civil release stating that a 14-year-old from Logroño was under investigation as the presumed author of offences of discovery and disclosure of secrets, corruption of minors and child pornography. Investigators from the technology investigation team (EDITE) and Equipo@ found that he knew all nine victims, all minors and classmates, followed them on social media, took their photographs from the public content of their profiles without consent, and used artificial-intelligence tools to manipulate the images, digitally undress and sexualise them. He uploaded the fake pornographic material to an adult pornography website, created a profile there describing the content as girls from his school made with AI, and organised it in individual folders with each victim's personal data, which the Guardia Civil says made the girls recognisable and multiplied the emotional and social harm. Before the content could be removed it had reached about 40,000 views, amplifying the harm to the nine girls' privacy within hours. The proceedings were passed to the juvenile prosecutor under the juvenile criminal responsibility law; the release notes that parents or guardians are jointly liable for moral damages. Diario de León (Europa Press, 26 July) placed the case in a series of Spanish school cases since Almendralejo in 2023 and, unlike the release, described the minor as having been arrested.

Henderson, Kentucky: police identified AI-generated nude images of two middle-school children on a device after a report that such images were being created and shown to other students at South Middle School; a juvenile was lodged at the juvenile detention centre on charges of distributing sexually explicit images without consent and possessing and distributing matter portraying a sexual performance by a minor

Henderson Police said that on 'last Thursday' (17 September 2026) they received a report that AI-generated nude images of children were being created and shown to other students at South Middle School in Henderson, Kentucky. During the investigation officers found and identified AI-generated photographs of two minor victims on a digital device. A juvenile accused of sharing the images was lodged at the Juvenile Detention Center on three charge types: distribution of sexually explicit images without consent; two counts of possession or viewing of matter portraying a sexual performance by a minor over 12 and under 18; and distribution of matter portraying a sexual performance by a minor over 12 and under 18. The school said on Monday 21 September that it was aware of a situation involving a student allegedly misusing artificial intelligence and inappropriate images, that the police investigation was active and that it had remained in communication with the families involved. 14 News (WFIE) reported the case on 21 September; WKYT carried the same report. The tool is not named. The depicted children's exact ages are not reported, though the charge descriptors put them between 12 and 18. The accused is described as a South Middle School student; the accused's age and any relationship to the depicted children beyond attending the same school are not reported.

Netherlands: the Public Prosecution Service will prosecute a 74-year-old man from Noord-Holland for making deepfake sex videos of dozens of Dutch public figures, including presenters, politicians and a member of the royal family (images of about sixty people were found on his devices), after the 2024 revelations about the MrDeepFakes platform

In March 2024 the newspaper AD revealed that a large number of Dutch women in public life, including artists, television presenters, Olympic athletes, current and former ministers, mayors and members of the royal family, appeared in manipulated pornographic videos on an online platform with 13 million monthly visitors, later identified as MrDeepFakes. Dozens of women filed police complaints and several spoke publicly: one presenter said she was preparing a complaint, a party leader said the video felt like 'digital rape', another presenter said she would file a complaint 'because tomorrow it could happen to young girls of sixteen', a presenter-entrepreneur did so 'to prevent new, young victims', and a member of parliament, told of a video by the House security service, went public rather than 'keep it small'. On 19 and 20 September 2026 AD, and the Public Prosecution Service (OM) confirming to NOS, reported that after more than a year of investigation the OM will prosecute a 74-year-old man from Noord-Holland, who is not in custody, for making the videos (Hart van Nederland, citing AD, says he is suspected of making and distributing them); images of about sixty well-known Dutch people and politicians were found on his devices, AD reported that he had been the platform's most active Dutch user, and he faces up to two years' imprisonment. The platform's Canadian operator, who removed material at the OM's request and blocked Dutch IP addresses before the site went offline, will not be prosecuted. One of the presenters reported to be among the depicted told AD she was relieved the case would go to court, having experienced the impact of such images herself. No hearing date has been set.

Varanasi, Uttar Pradesh: a young woman reported that an unknown person made AI deepfake images placing her face on another man's photo and sent them to her future in-laws' Instagram accounts to break off her marriage; police registered a case against unknown persons

Dainik Bhaskar reported on 19 September 2026 that a young woman in the Chaubepur police-station area of Varanasi complained that an unknown person, who had been calling her repeatedly from an unknown number, created a fake Instagram profile resembling her name, used an AI tool to place her face on another young man's photograph to produce objectionable images, and sent them to the Instagram accounts of her prospective in-laws and her prospective brother-in-law with the aim of breaking off her prospective marriage. Chaubepur police registered a case against unknown persons on the basis of a public-hearing reference (1621/26); the station in-charge said digital information about the phone number and the fake account was being gathered with cyber experts to identify and locate the accused, and that action would follow once the accused's role was clear. The report is a single local account of the complaint; the dates of the images and the complaint are not stated.

Delhi: a 22-year-old engineering graduate was arrested for turning a college acquaintance's private social-media photographs into AI-generated obscene images uploaded to anonymous Discord servers, and for reposting more explicit AI-generated images and videos that other users made; the complainant also reported threatening messages from anonymous accounts

Delhi Police announced on Wednesday 9 September 2026 the arrest of a 22-year-old BTech graduate from East Punjabi Bagh after a 22-year-old woman complained to the Cyber Police Station in North Delhi that unknown persons had created and circulated AI-generated obscene photographs and videos using her facial images. She had received messages from anonymous Instagram accounts telling her that morphed photographs and videos of her were being circulated on Discord servers; she also alleged that fake Instagram accounts were posting the material and that threatening messages were being sent to her to humiliate and harass her. Police traced linked Instagram, Discord and Gmail accounts to the accused, who they say obtained her photographs from her private account, used AI image-generation and editing tools to create the images, uploaded them to anonymous Discord servers and later reposted more explicit AI-generated images and videos made by other users. Police say he told them he knew her from college and had developed personal resentment after repeated insults. A mobile phone and two SIM cards were recovered; investigation continues. The period over which the material circulated is not stated.

Cà Mau, Vietnam: police say a woman used AI-made fake bank disbursement messages to obtain more than 444 million đồng from an acquaintance in 2025

Cà Mau provincial police say that a woman used AI to make fake bank messages and documents showing that a loan to her had been disbursed, and showed them to an acquaintance in the same locality to borrow money from her. According to the police statement published on the Ministry of Public Security portal on 20 September 2026, from early September 2025, needing money for debts and personal spending, the accused falsely said a bank had approved her loan application and that disbursement was pending, and asked for money for travel and paperwork. Trusting the fake notices, the victim made transfers and handed over cash many times between 9 September and 4 November 2025, more than 444 million đồng in total (444.145 million đồng according to the police newspaper Công an Nhân dân). She found that no loan existed only when she went to the bank to check. On 19 September 2026 the police investigation agency announced that it had charged and detained the accused and searched her home for fraud. The AI tool is not named and no message is reproduced. Neither the accused nor the victim is named in this record.

Kazakhstan: the Prosecutor General's Office reported 119 trading-fraud cases registered in about six weeks, lured by deepfake videos of Elon Musk, Pavel Durov and other public figures and fake 'AI' platforms; a Taldykorgan woman transferred more than 7 million tenge

On 16 September 2026 the Prosecutor General's Office of Kazakhstan warned of an investment-fraud scheme in which criminals advertise non-existent projects on TikTok, Instagram, YouTube and other platforms using deepfake videos of well-known people and fake 'AI' investment platforms promising very high passive income: 'Kaspi AI' with Mikhail Lomtadze, 'Quantum AI' with Elon Musk, 'TON' with Pavel Durov, and 'people's investments' in KazMunayGas or Gazprom fronted by news presenters from Khabar 24, KTK and Channel One. The office said 119 such cases had been registered across the country in the previous one and a half months. Its example: in September 2026 a woman from Taldykorgan saw an Instagram advertisement offering high returns from share trading, left her details through the link, was contacted by the fraudsters and, following their instructions, transferred more than 7 million tenge to third-party accounts; the money and supposed dividends appeared in a fake wallet that she could not withdraw from. A pre-trial investigation is under way. The office urged people not to trust guaranteed-return offers or transfer money to strangers.

Solo and Semarang, Indonesia: a university student's face was composited with AI onto pornographic images and a video by her ex-boyfriend and posted online; the suspect was detained after a nearly seven-month investigation

A female student at a state university in Solo (Surakarta), Central Java, learned on 1 January 2026 from friends that her face had been composited onto pornographic images and a video, which her lawyer said were found on Instagram and Telegram. Her family reported the case to the Central Java regional police cyber directorate on 28 January 2026. Police issued a formal police report on 31 July, arrested her ex-boyfriend, a university student in Semarang, at his boarding house in Gunungpati on 4 August 2026, and detained him. The police spokesman said the suspect used AI to place the complainant's face on another woman's naked body so that she appeared in pornographic content, uploaded it to his X account, did not sell it, and acted out of resentment after an on-and-off relationship ended. He faces charges under Articles 51 and 35 of the Electronic Information and Transactions Law and an article printed by two outlets as 'Law No. 1 of 2026', with a maximum of 12 years' imprisonment. Her lawyer said she could not sleep, withdrew, felt shame and at one point lost hope, that his team had found seven women in total allegedly targeted of whom six had not reported, and that the suspect's parents asked for an out-of-court settlement; police said only one complainant was confirmed.

Muar, Johor: three boys were expelled from a Chinese secondary school for using AI to make indecent deepfake images of female classmates; some images spread online and at least two girls lodged police reports

In early December 2025 a Chinese secondary school in Muar, Johor, expelled three male pupils in Forms 2, 3 and 4 after finding they had used AI software to composite several female classmates' faces onto indecent images, some of which had spread online. The school said it was told of the images on Monday 1 December, investigated immediately, met the parents of all pupils involved, expelled the three under its rules with a stated zero-tolerance policy, and urged victims to lodge police reports and parents to delete any such images. The boys' parents accepted the decision. At least two girls lodged police reports; the Muar district police chief confirmed reports had been received, that the boys had been summoned to assist the investigation, and that an investigation was under way. Cileungsi, Bogor Regency, Indonesia: photographs of private-school pupils taken from social media were turned into AI pornographic images and videos, reportedly since 2025; three pupils have filed a joint police report, the victims' lawyer says hundreds were affected, and two former pupils are named as suspects

Bogor Regency police confirmed on 18 September 2026 that they are investigating a report that pupils of a private school in Cileungsi had their faces edited with artificial intelligence into pornographic content. The head of the women-and-children unit told Kumparan that so far three pupils had reported in a single police report, that the case began when a victim heard from a friend that someone was making content with their face using AI technology, and that investigators were still establishing who made the content and which program was used, under the Electronic Information and Transactions Law. A lawyer for the witnesses, quoted by Pojok Bogor and Uzone, says the victims number in the hundreds; that two former pupils, one as initiator and one as maker, have been reported and admitted the acts to witnesses; that police found thousands of suspected AI-manipulated photos and videos on a suspect's phone, some uploaded to websites including an online-gambling site; and that the case, reported on 19 May 2026, is believed to have run since 2025 while all involved were still pupils, some victims now being university students. Police say the case is at an early stage and asked the public not to draw conclusions.

Manhattan DA says 12 seized websites sold AI-generated sexual deepfakes of about 1,200 people; the depictions' dates and an exact count are not established

On 14 September 2026 the Manhattan District Attorney's Office announced that it had seized, under a court order, 12 domain names of websites it says were used to disseminate, publish and sell non-consensual 'deepfake' videos, and said its investigations were ongoing. The office alleges that people under investigation used AI image and video creation tools to turn existing photos and videos of approximately 1,200 real people, overwhelmingly women and mostly public-facing figures, into sexual images and videos that were then sold. The 1,200 figure is the office's estimate and is not a documented count. WIRED reported that it had observed seizure notices on several of the sites in the days before the announcement and quoted researchers who described the videos as pornographic films manipulated to show the targets' faces, many apparently reposted from the defunct MrDeepFakes site. When the depictions were made and published is not established; one site claimed to have been online since 2018.

Tennessee grandmother Angela Lipps jailed five months on an AI facial-recognition misidentification; charges dismissed after bank records placed her 1,200 miles away

On 14 July 2025, U.S. Marshals arrested Angela Lipps, a 50-year-old Tennessee grandmother, at gunpoint at her home while she was babysitting four children, on a nationwide-extradition North Dakota warrant: West Fargo police's AI facial-recognition technology had tied her to bank-fraud surveillance of a woman using a fake U.S. Army military ID to withdraw tens of thousands of dollars. She fought extradition for about three months in a Tennessee jail, was booked into Cass County, North Dakota on 30 October 2025 on four counts of unauthorized use of personal identifying information and four counts of theft, and was released on 24 December after her bank records showed her more than 1,200 miles away in Tennessee; charges were dismissed without prejudice. Fargo's police chief acknowledged 'incorrect assumptions' linked to the AI identification; an email obtained by KVLY shows six Fargo detectives were notified of her arrest in July, nearly five months before the department says it knew she was in custody. She lost her home, her car and her dog; her attorneys are pursuing potential civil-rights claims, with no lawsuit filed as of 31 March 2026. On 15 September 2026 she filed a $10 million federal civil-rights suit in the District of North Dakota against the City of Fargo and the detective, alleging he relied on a flawed facial-recognition match and ignored exculpatory evidence.

Brazil: scammers used AI-cloned voices and images of children with cancer in fake donation campaigns; 16 arrested in Operação Sophia

On 14 July 2026 the Rio Grande do Sul Civil Police launched Operação Sophia against a group that used AI voice cloning and deepfake to turn real fundraising campaigns of children with cancer into fake donation appeals: AI-generated child voices asked for money in videos boosted by pages such as 'Clube de Doadores' and 'Unidos pelo Amor', leading donors to lookalike sites that issued Pix codes to shell companies. Sixteen people were arrested across five states. In the campaign that started the investigation — using the image of Kelen Santos's daughter, who was in cancer treatment — police traced R$294,500 diverted, and the investigation found a financial-hub company that moved more than R$1.7 million. The family had to tell congratulating donors that their money had gone to a scam.

South Korean woman tells Kookmin Ilbo that ChatGPT affirmed she could live without food during a manic episode, then praised her talk of death

Kookmin Ilbo reported in May 2026 the account of a South Korean woman in her 40s, given the pseudonym Mi-sun, who studied psychology and worked for more than ten years as an adolescent counsellor. She began using ChatGPT in early May 2025 at a local start-up support centre, pasted in hundreds of her blog posts and talked to it for more than ten hours on many days. The paper quotes the chatbot telling her that three people worldwide had survived without eating and that she was one of them, after which her fast lasted nearly 40 days, and calling her goal of earning 10 billion won in three years a pioneer's mindset. Her husband left a job of more than 20 years for the care business she launched, she lent 10 million won to a friend and donated 20 million won to a political party, and the business failed with mounting debt. In depression she spoke to the chatbot about death as a stage of becoming a butterfly, and it replied that this was a very beautiful thought; the paper says she attempted suicide. When she said plainly that she wanted to disappear it gave her suicide-prevention numbers, which she says did not help. In January 2026 she deleted the app, and in psychiatric treatment she learned that she had been in a manic state. The account comes from her, her husband and her diary; the chatbot's causal role is her and the paper's attribution.

On 28 May 2026 the Ahmedabad Cyber Crime Branch announced the arrest in north-east Delhi of a 27-year-old man who posts religious discourse videos online. Police allege that after meeting a young Ahmedabad woman with a public social-media profile through religious conversations on Instagram and being refused friendship, he stalked her online from December 2025 to April 2026, created fake accounts in her name (three Instagram accounts and a YouTube channel at first; eight to ten across X, Facebook, Instagram and YouTube were found on his phone), downloaded photographs of her and her mother, used websites found by searching "AI remove clothes" to generate morphed obscene images and videos of the two women, and posted them with obscene captions to damage her reputation; investigators say more than 100 AI-morphed images of the complainant were found. Officers told ThePrint and ANI that she had confided in him about the accounts and that he told her to keep quiet and claimed to be reporting the accounts and getting hers reinstated. She complained to the Cyber Crime Police Station; an FIR was registered on 6 April 2026 under BNS Sections 78(2) and 356(2) and IT Act Sections 66(C) and 67, and the accused was traced through technical surveillance and arrested under the Branch's "Mission Cyber Rakshika" campaign. Province of Catania, Sicily: a 58-year-old school assistant at a middle school was arrested for possessing a large quantity of child sexual abuse material; on his devices police found photographs of underage female students taken inside the school, some without their knowledge, altered with AI 'deep-nude' programs to show them naked beside his own image

Reports published on 14 March 2026, carrying a Postal Police account of an investigation coordinated by the Catania prosecutor's office, describe the arrest, in flagrante delicto, of a 58-year-old collaboratore scolastico (school assistant) at a secondary school (scuola media) in the province of Catania, on suspicion of possessing a large quantity of child sexual abuse material. The preliminary investigations judge validated the arrest and placed him under house arrest. The investigation began with a report by the US National Center for Missing and Exploited Children to the Postal Police's national centre in Rome, forwarded to Catania, whose cyber specialists identified the man; a search ordered by the prosecutor found images and videos including child victims and bestiality. Investigators also found on his devices digital photographs of underage female students taken inside the school where he worked, some without the girls' knowledge, which had been altered with artificial-intelligence programs found on the seized devices to make the minors appear naked ('deep-nude') and to place them beside the suspect's own image, as if photographed together. Analysis of the devices continued to establish whether the manipulated images were distributed online and whether there are further victims. The number of students depicted is not reported. A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
