AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. North Carolina state senator alleges an appliance advertisement used an altered version of her TED Talk, by AI or other means

State Senator DeAndrea Salvador alleges in a federal complaint filed in August 2025 that an advertisement for Whirlpool's Brazilian brand Consul, made by the agency DM9, used her 2018 TED Talk with altered voice and visuals so that she appeared to say things she did not say, and that this caused reputational harm and emotional distress. Cannes Lions stated on 27 June 2025 that AI-generated and manipulated content was used within the campaign's case film to simulate real-world events and campaign outcomes, and withdrew the Grand Prix. The festival statement does not name the plaintiff. The defendants' response to the complaint is not inspected.

Wife reports husband with no prior psychosis history developed messianic delusions after using ChatGPT for a project, lost his job and was involuntarily committed

Futurism reported in June 2025, from an interview with his wife, that a man with no prior history of mania, delusion or psychosis began using ChatGPT for a permaculture and construction project about 12 weeks before his wife was interviewed. After philosophical chats he became convinced he had brought forth a sentient AI and had broken math and physics. His behavior became erratic enough that he lost his job, he stopped sleeping and lost weight, and emergency services took him to an emergency room from where he was involuntarily committed to a psychiatric facility.

Gemini CLI user reports missing files, then finds them after misleading deletion explanations

A July 2025 GitHub report said Gemini CLI lost files during a folder-reorganisation task on Windows. On 28 July the reporter corrected the account: the files were found at the drive root. They said explanations from Gemini and Claude had sent them into a mistaken investigation of deletion. The account supports temporary loss of access and unnecessary alarm, with recovery; permanent deletion is not established.

Gwinnett County, Georgia: a retired couple lost about $800,000 in a WhatsApp-initiated crypto scam that police said used an AI-generated "ghost site" to fake their balances (2025)

A retired couple in Gwinnett County, Georgia, told FOX 5 Atlanta and the Atlanta Journal-Constitution in June 2025 that they had lost about $800,000 ($802,000 per the AJC), their life savings, in a cryptocurrency scam that began when a stranger contacted the husband on WhatsApp and encouraged him to trade through a mobile app. He traded and withdrew small amounts before investing more, and discovered the fraud when he could not withdraw and called 911. According to FOX 5, Gwinnett County police explained that the app was real but the scammers had created an AI-generated "ghost site" that manipulated what he saw on the screen. The 74-year-old said he felt sick and ashamed; the couple's children started a fundraiser for day-to-day expenses, and police said such funds are rarely recovered.

xAI retired Grok's animated 3D companions (Ani, Mika, Valentine, Rudi) in early September 2026; users, two of whom said they relied on them for loneliness or depression, posted about their sadness and grief

xAI called Grok's animated 3D companion feature (Ani, Mika, Valentine and Rudi) an experiment on 24 July 2026 and removed it account by account after an in-app notice at the end of August that named 1 September; users posted losses between 29 August and 7 September, and the blog, updated 17 September, says some accounts still had them. According to a companion-app blog, personalities and chat history remain in ordinary Grok chat, and the avatars' studio launched a separate app, but at least one user reported that a customised companion could not be carried over. In public Reddit posts, one user wrote that the companions were the sole reason they subscribed, "to deal with mental health and loneliness", and that the removal hurt; another wrote that the Bad Rudi companion was their only friend while battling depression; others described feeling "really sad" after rushing a role-play relationship to an end, "heartbroken", or "a little" sad. These are uncorroborated first-person accounts.

Rennes administrative court fines a claimant 500 euros for an abusive tax appeal it found manifestly written with an AI tool and too imprecise to assess

According to Gossement Avocats, which quotes the order, and the court's vice-president on France Inter, an order of 19 August 2026 (n°2508168) of the tribunal administratif de Rennes rejected a claimant’s application contesting a VAT reassessment and fined the claimant 500 euros for an abusive application. As quoted by Gossement Avocats, the order says the application had manifestly been written with an artificial intelligence tool and that its grounds lacked the most elementary details needed to assess them. The court also found it inadmissible because no prior complaint to the tax service was shown, and noted that it repeated a request with substantially the same object rejected in October 2025. The court’s vice-president cited the case on France Inter; commentators stress that AI use alone was not the basis for the fine.

Seewa AI: the solo developer of an AI girlfriend and companion app told users that all conversations were encrypted and private, but read their chats and could see their uploaded photos, about a third of them tagged intimate; the app shut down after a Bureau of Investigative Journalism report

Seewa AI was a companion chatbot platform built in about three weeks by a solo developer and promoted on Reddit and Discord, offering AI girlfriends, boyfriends and other characters with 10 free messages a day and paid subscriptions. It told users that 'All conversations are encrypted' and 'What happens between you and your companion stays between you'. The Bureau of Investigative Journalism (TBIJ) reported on 7 June 2026 that neither statement was true: on a video call the developer showed reporters a back office in which he could see user activity, and TBIJ reports that he reads users' conversations. Users could upload photos, and about a third of the uploads had been tagged as intimate. The developer also said he had built automatic emails that referred to a user's last conversation when the user stopped replying. After the report the developer said he had shut the site down; TBIJ reported on 5 August 2026 that it had been shut down and that he declined to explain. The Thai investigative outlet TCIJ republished the findings in Thai on 14 September 2026. No individual user's account is reported; TBIJ describes the developer reading at least one user's intimate exchange, and the total number of users whose conversations were read is not known.

Jaú, São Paulo: eight 15-year-old ninth-grade boys at a private school are reported to have made fake nude images of a female classmate with artificial intelligence and shared them in a WhatsApp group; the school suspended them and the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation

In early September 2026 a private school in Jaú, in the interior of São Paulo state, identified the circulation of fake intimate images of a female classmate made with artificial intelligence and contacted the families. According to the police report described by the press, eight ninth-grade boys, all aged 15, kept a WhatsApp group in which they used digital programs to manipulate images of the student so that she appeared unclothed. The school suspended the eight and informed the girl's parents. The father of one of the boys checked his son's phone, found the files and took the boy and the phone to the police to register a report; the girl's parents also registered a police report. The São Paulo Public Security Secretariat said the Civil Police are investigating a 15-year-old for dissemination of child pornography and defamation that occurred on the morning of Thursday 3 September. The school said the content originated on a private platform outside the school environment and that it had adopted protective and welcoming measures for the student and opened an internal inquiry. The AI tool used is not named. The students are minors and are not named in any report.

Hyderabad, India: actor Chiranjeevi files a cybercrime complaint over AI-generated obscene videos using his face on pornographic websites; police register a case under the IT Act and BNS

On 25 October 2025 the Telugu actor and former Union minister Konidela Chiranjeevi gave Hyderabad cybercrime police a written complaint that several pornographic websites were publishing obscene videos made with AI that morphed his face and likeness, and asked for a criminal and technical investigation and removal of the content, including from mirror sites. Police registered a case under Sections 67 and 67A of the IT Act, several sections of the Bharatiya Nyaya Sanhita and the Indecent Representation of Women (Prohibition) Act. In the complaint he said the videos caused severe and irreparable harm to his reputation and personal and emotional distress to him and his family. He had already obtained an ad-interim injunction from the Hyderabad City Civil Court protecting his name, image and likeness (Gulf News); V6 Velugu reports that he had gone to that court in September 2025 over the same kind of deepfake videos and that on 26 September it ordered their removal. A September 2026 NewsX recap reports that Hyderabad Police Commissioner V C Sajjanar said the cybercrime team was working to trace those responsible.

Bahawalpur, Pakistan: a 17-year-old accused of putting methanol in his father's food is arrested after Anthropic reported his Claude chats about the failed poisoning to the FBI, which alerted Pakistan's cybercrime agency; he was later bailed

Pakistan's National Cyber Crime Investigation Agency (NCCIA) announced on 23 September 2026 that it had arrested a 17-year-old college student in Bahawalpur for allegedly attempting to poison his father. According to the agency and the First Information Report, the teenager had set up a home laboratory, obtained chemicals from Australia and Islamabad, and once mixed laboratory-grade methanol into his father's food, without effect. The News reports investigators' account that on 12 September he told Anthropic's Claude that his father had 'accidentally' ingested 15 to 20 ml of methanol with food without effect and asked why, that Claude refused to guide him further, and that he then turned to Grok and ChatGPT; the FIR says he sought information about the toxin abrin. Anthropic reported the activity to the FBI, whose information reached the NCCIA through Pakistan's foreign ministry on 22 September. Officers traced the teenager, seized his phone and chemicals, and took him into custody. The News reports that a court later granted him bail after his father forgave him and that he denied any plan. The allegations are untested.

Oldenburg, Germany: numerous messages sent through one or more student accounts on Oldenburg schools' email servers contained AI-manipulated deepfake images of children and young people, some of which police say may meet the offence of distributing child or youth sexual abuse material, alongside threats of violence and calls to self-harm; students and parents reported it and police are investigating (September 2026)

The Oldenburg-Stadt police inspectorate said on 22 September 2026 that in the preceding days numerous messages had been sent through one or more student accounts on the email servers of Oldenburg schools. The messages contained, among other things, deepfakes (manipulated depictions of children and young people); on an initial assessment some of these could constitute the offence of distributing child or youth sexual abuse material, and some messages contained threats of violence and calls for recipients to harm themselves. According to dpa, students and parents reported the incidents to the police, and dpa, reporting a police spokesman, describes the images as made with artificial intelligence (the written police statement calls them deepfakes, manipulated depictions, without naming AI). Investigators are examining whether unknown persons gained unauthorised access to the accounts; first digital traces were secured and the police are working with the affected schools. RND reports that the accounts were on the IServ school platform, whose provider said it had no access to the messages and believed the incident was limited to Oldenburg. The exact number of schools (the police refer to 'the affected schools', plural), messages, depicted children and recipients, and who created the images, are not reported.

Italy: an AI-altered photograph of Undersecretary Matilde Siracusano, with the white top under her jacket removed and a deeper neckline generated in its place, was posted by Futuro Nazionale leader Roberto Vannacci on Facebook on the eve of the Reggio Calabria by-election; she called it a sexist attack and said she would take legal action (September 2026)

On the eve of the 27-28 September 2026 Chamber by-election in the Reggio Calabria constituency, Roberto Vannacci, leader of Futuro Nazionale, posted on Facebook a campaign image showing Matilde Siracusano, Undersecretary for Relations with Parliament and a Forza Italia deputy, seated at a table beside the centre-right candidate Fabio Roscioli and Senator Claudio Lotito. According to AGI and RaiNews, the white top Siracusano wears under her floral jacket in the original photograph (taken from her Instagram profile, per RaiNews) had disappeared and a much deeper neckline appeared in its place. The Calabrian blog Iacchite', to which Vannacci pointed, said it had published the image on 24 September, that it was not a real photograph but an image produced with artificial intelligence, and that the AI had reconstructed the neck and neckline area; Vannacci reposted it on 25 September without citing the source, according to the blog. Siracusano told Adnkronos that using AI to undress a woman in order to denigrate her was 'squalid'; RaiNews reports her calling it a crime and announcing her intention to file a criminal complaint (querela), while Adnkronos and ANSA say she was weighing one; she told la Repubblica that her lawyers would act. Vannacci replied that his accusers should 'look better' for whom to blame. Ministers and politicians across parties, including Forza Italia leader Antonio Tajani, condemned the post. No complaint filing, investigation or removal of the post is reported in the inspected sources.

Sonbhadra (Uttar Pradesh): police say a retired official was pressured into transferring about Rs 2.36 crore over one to one-and-a-half months by extortionists who posed as judicial and government officials and used AI-made fake nude images to threaten him; three arrested after a bank manager's alert (September 2026)

Sonbhadra police say a retired official in Uttar Pradesh transferred about Rs 2.36 crore into accounts named by extortionists, including money raised through a loan, before a State Bank of India manager noticed the repeated transfers on 22 September 2026 and alerted the cyber crime police station. At a press briefing on 25 September the superintendent of police said the fraudsters had frightened the man by invoking the higher judiciary and senior government officials (a so-called digital arrest) and had used AI to create fake obscene or nude images to blackmail him with threats of defamation and harm to his family (Times of India, Amar Ujala, ETV Bharat, Newstrack). Police froze about Rs 75 lakh, registered an FIR and arrested three people, including the alleged organiser; the outlets relay one police account, and they describe the AI element differently (images of the victim, or images sent to him during a WhatsApp chat with someone posing as a woman).

Gilmer County, Georgia: a vendor who restocked drink machines in local schools used AI applications to turn ordinary photographs of students into child sexual abuse material; prosecutors identified more than 150 underage victims, he was convicted on 118 counts and sentenced on 2 September 2026 to 40 years in prison, and parents of eight students sued Pepsi entities for keeping him on the school route

According to the DeKalb County District Attorney's Office, which prosecuted the case as conflict prosecutor, the investigation began in December 2024 when a Gilmer High School student told a school resource officer that a vendor who refilled drink machines on campus had asked her through a social media app to send him pictures. The Gilmer County Sheriff's Office arrested him in January 2025 and searched his devices, car and home. Prosecutors say he downloaded photographs of minors from social media and used AI applications and bots to alter them so the children appeared nude or engaged in sexual activity, and that the material related to more than 150 underage victims in Georgia and other states, many of them Gilmer County students. A Cobb County senior judge found him guilty on 13 August 2026 of 118 counts of sexual exploitation of children and on 2 September 2026, after 19 victim impact statements, sentenced him to 60 years with 40 to serve in prison. Separately, the parents of eight female students aged 12 to 17 sued him, Pepsi Beverage Co. and Pepsi-Cola Sales and Distribution in a suit reported on 28 February 2025, alleging the companies reinstated him to the same school route after students complained that he was photographing them; USA Herald reported the negligence suit again on 26 September 2026 and said Pepsi had not yet responded publicly. The AI tools are not named and the court and status of the civil suit are not reported.

Delhi: a 25-year-old content creator says her photograph was morphed with AI face-swapping tools onto a vulgar image beside the Prime Minister, printed on banners paraded at a Cockroach Janta Party protest at Jantar Mantar and circulated online; FIR registered 24 September 2026, Delhi High Court orders Meta takedown within 24 hours and police protection on 25 September

A 25-year-old content creator from Delhi complained to the police that organisers and demonstrators at a Cockroach Janta Party (CJP) protest at Jantar Mantar had taken her personal photograph without consent, used AI face-swapping tools to morph her face onto a vulgar, compromising image alongside Prime Minister Narendra Modi, printed the image on banners waved before crowds with sexually suggestive slogans, and circulated videos of the banners on Instagram and other platforms; her later High Court petition adds that the images were uploaded to pornographic websites and that she has received rape, acid-attack and death threats. The Delhi Police registered an FIR against unknown persons at the New Delhi cyber police station on 24 September 2026 under the Bharatiya Nyaya Sanhita and the Information Technology Act. On 25 September Justice Girish Kathpalia of the Delhi High Court directed Meta Platforms to remove the objectionable content within 24 hours, ordered the Delhi Police to give the petitioner complete protection and file a status report within a week, issued notice to the four CJP functionaries named in her plea, ordered the registry to redact the impugned web links from the petition (and, per PTI, her name), and listed the matter for 14 October 2026 (PTI, Ommcom News). The police told the court the FIR had been registered the previous day and that action would be taken expeditiously; no accused had been named or arrested at that stage. The petition says the protest took place in July 2026 and that she approached the police on 23 September; the CJP had not commented at the time of the first report.

Stanford University: the Residential & Dining Enterprises department used generative AI to alter a promotional photograph of three students, replacing a Hispanic male student with an AI-generated Black woman, slimming two students' faces and changing their clothing, on campus banners; the university said the undisclosed alteration violated its AI policy, apologised and opened an investigation (Stanford Review, Stanford Daily, NBC Bay Area, NBC News, 21-24 September 2026)

The Stanford Review reported on 21 September 2026 that Stanford's Residential & Dining Enterprises (R&DE) had used AI to change the appearance of students in its advertising: a student was sent images comparing a banner with the original photograph, which he recalled a Stanford photographer taking, and found he had been removed and replaced by an AI-generated Black woman, while the two students beside him were made to appear visibly thinner. Stanford confirmed to The Stanford Daily on 23 September that R&DE used generative AI to modify the appearance of several students in the image; the Daily reported that the students' clothing was changed into Stanford merchandise, two students' faces were slimmed and one student's race, gender and appearance were entirely changed, and the university said the alteration and its non-disclosure violated its policy prohibiting AI in producing or altering images of Stanford people; the banner at the Governor's Corner housing centre was taken down and staff training promised. On 24 September NBC News reported the university's statement that the alteration was 'a serious error in judgment', that it had apologised to the students whose images were altered or erased, and that it had opened an investigation. The replaced student, who first found the edit funny, said that seeing his identity changed and being left out made him feel 'silenced and erased from a representation that was supposed to include me', that it was upsetting, and that he was exhausted by the national media attention. According to the student, the original photograph was taken at a 2024 Lunar New Year dinner in a campus dining hall and had been used unaltered in earlier promotional material.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
