AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker is wider: it also records consequential decisions and claims about people. Each account is reviewed for publication; claims may remain uncorroborated or disputed. These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

San Diego: three men are arrested after police treat a Flock license-plate-reader photo of a red Alfa Romeo, taken five miles from where officers had just tried to stop the suspect car, as a hit; one spends nearly a month in jail before charges are dropped

Times of San Diego reported on 7 June 2026, from police reports and tort claims, that on 26 November 2025 San Diego police pursued a red Alfa Romeo after an attempted carjacking in Golden Hill but did not get its plate. A Flock automated license plate reader in Old Town, five miles away, captured a photo of a red Alfa Romeo 23 seconds after the stop attempt, and a pursuing detective recognized it by its red paint and tinted windows. Officers arrested three men leaving that car at an Old Town cigar lounge; the victim identified the passenger in a curbside lineup, and a search found no weapons. The driver bailed out the next day; the passenger spent nearly a month in jail, missing Thanksgiving, before the assault with a firearm and evasion charges were dropped. The two men's tort claims say police misread their own surveillance system; their attorney said the city had denied the claims and that a lawsuit would follow.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
