AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Colorado: state senator files complaint over think tank's AI deepfake video depicting her as a cashier keeping change; group calls it satire

The Independence Institute, a libertarian-leaning Colorado think tank opposing Proposition NN on the November 2026 ballot, posted a video on its social media accounts in which an AI-generated likeness of Democratic state Sen. Cathy Kipp, who sponsored the bill that referred the measure to voters, appears as a grocery store clerk who keeps a customer's change and says it will help educate children. CBS Colorado reported that the institute made the video in response to remarks Kipp made on the Senate floor in April 2026. Kipp filed a campaign finance complaint with the Colorado Secretary of State's office on 5 August 2026, according to Colorado Newsline, alleging that the video lacked the disclosure a 2024 state law requires on deepfakes of candidates and officeholders; the video carried only a small line reading "AI generated content because who gets a human cashier these days?". Kipp said it was "disturbing and wrong to use a deepfake, AI-generated video of someone without their consent". The office decided on 19 August to investigate; a spokesperson said it was the first complaint filed within the statutory timeframe for consideration. The institute says the video is satire, which the law exempts, that the law covers candidates rather than ballot measures, and that applying it would violate the First Amendment. CBS Colorado reported on 10 October 2026 that the institute's lawyer had filed a motion to dismiss and planned to sue Kipp and the state for attorney fees, and that the state elections division disputed the satire defence. No decision is reported.

Woman in NRW, Germany, says AI-altered photos from her Instagram were used in an online sex ad with her face; police complaint filed, prosecutor investigating

A woman in her late twenties from North Rhine-Westphalia, Germany, learned at the end of June 2026 from an Instagram user that her face appeared in an advertisement on the classifieds platform Markt.de offering sex meetings and other sexual services, according to her account to the Lippische Landes-Zeitung (14 September 2026) and WDR (9 October 2026). WDR reports that the pictures came from her public Instagram account and were partly altered with AI; the extent of the manipulation is unknown. She filed a criminal complaint and engaged a lawyer; the Bielefeld public prosecutor's office told WDR that investigations are ongoing, and a suspect was identified through the ad, who the report notes could himself be a victim of identity theft. The ad has been taken down. She went public with a video on her Instagram account and says the experience changed how she shares online.

Gresik, East Java: police say a woman was extorted by illegal online-loan collectors who sent AI-manipulated obscene photos of her; she paid about Rp3 million

East Java police and three Indonesian outlets report that a 34-year-old woman from Manyar, Gresik Regency, was extorted by people claiming to be debt collectors for illegal online-loan apps. According to Liputan6, which quotes the woman, the first WhatsApp demand on 22 September 2026 forced her to pay Rp1.6 million before the due date; a second number then claimed a fictitious Rp2.9 million bill, threatened to spread her ID card photo, and shared fabricated obscene photos of her paired with her ID card to Gresik-area Facebook groups, after which she paid again. Detik reports a total of about Rp3 million transferred, quoting the Gresik police chief; the two figures are not reconciled. The chief, who found her crying at the Manyar police post during a visit on 7 October 2026, is quoted by beritajatim as saying she received manipulated photos said to be obscene ('foto hasil manipulasi') used as pressure to meet the collector's demands; beritajatim's own report calls them AI fabrications ('hasil rekayasa akal imitasi (AI)'), and SuaraJatim, which says it takes the quote from beritajatim, renders it as obscene photos that were AI fabrications ('hasil rekayasa AI') used as a tool of terror and psychological pressure. Detik quotes him saying the photos were, per the complainant, not of her. The case was taken over by the Gresik police criminal unit; no suspect is identified and the tool is not named.

Kolkata: a 67-year-old man transferred Rs 95,990 after a call in a voice matching an acquaintance; the Times of India reports it as an AI voice-cloning scam

A 67-year-old man from the Beleghata area of Kolkata was cheated of Rs 95,990, according to a Times of India report of 19 August 2026, after a call from an unknown number in a voice that sounded exactly like an acquaintance of his. The caller said a relative had been admitted to a private hospital and needed urgent money for treatment; believing the voice, the man made several online bank transfers within less than an hour. When he later contacted the real acquaintance, that person said he had made no call and asked for no money. The man filed a complaint at Beleghata police station, and police are checking the phone number, call records and bank accounts used. The newspaper describes the scammers as having used AI voice-cloning technology; no police finding, recording or tool is described, so the AI involvement is recorded as suspected.

Pont-à-Mousson pensioner lost her savings to a fraudulent investment platform promoted by an ad reported as AI-generated likenesses of Macron and Arnault

A retired woman in her eighties living in Pont-à-Mousson (Meurthe-et-Moselle, France) responded in summer 2025 to an online advertisement in which AI-generated likenesses of Emmanuel Macron and Bernard Arnault promoted a lucrative investment platform. A supposed financial adviser then called her, she invested 5,000 euros, received several hundred euros of 'interest', and reinvested repeatedly until the advertisement and the site disappeared. L'Est Républicain (7 June 2026) and Capital put the loss at 10,000 euros; Le Parisien, citing ICI and L'Est Républicain, reports 25,000 euros, all her savings. The Pont-à-Mousson police station says an investigation was opened and that it receives three to five complaints a week for this kind of fraud. The account rests on the police station's warning as reported by the regional press, which describes the advertisement as AI-generated; the advertisement itself was not examined by the outlets and the quoted police statements do not mention AI.

Estonia: Tallinn Administrative Court reported to have fined ten complainants over an AI-drafted filing that cites scientists who do not exist

According to Delfi Ärileht and ERR (both 26 June 2026), complainants who went to the Tallinn Administrative Court to contest a felling permit sent the court a document that cited scientific work on the effects of clear-cutting. The court found that the Estonian scientists named in the references do not exist and that the studies could not be found. It asked how the document had been produced, gave the complainants the chance to submit the cited works, and warned of a fine for attempting to mislead the court. The complainants replied that using AI is not forbidden and that they had not meant to mislead the court. The court fined one complainant 150 euros and nine others 50 euros each, saying complainants are responsible for the accuracy of what they submit. Delfi Ärileht reports that the Tallinn Circuit Court agreed and that the Supreme Court declined the case that week, so the fines are in force. No report names the tool or says which complainant used it.

Thailand: cyber police say an illegal gambling website, KFC888, used AI to fake the face and voice of luk thung singer Hai Arpaporn Nakhonsawan in advertising clips that urged people to sign up, as if she endorsed it; two Bangkok premises searched and a company representative summoned to acknowledge charges, reported 1 to 2 July 2026

Manager Online reported on 2 July 2026, citing the Royal Thai Police and its Cyber Crime Investigation Bureau, that cyber police had moved against a large online gambling network after finding that it used AI to fake the face and voice of Hai Arpaporn Nakhonsawan, a well-known luk thung singer, in advertising clips that urged people to join the website. The clips gave the false impression that she recommended the site, although she had no involvement. Police traced the clips to the website KFC888, which offered online lottery, foreign stock-index lottery and sports betting, with turnover estimated at about 120 million baht a month. They searched two locations in Bangkok, a residence believed to be linked to the site's executives and an electronic payment-gateway company, seized computers and documents, and summoned an authorised person of the company to acknowledge charges including a Computer Crime Act offence over AI-created images that may damage others. Komchadluek reported on 1 July that the singer went to the cyber police after Facebook Reels clips altered with AI used her face and voice without her consent.

China: Shanghai woman loses 800,000 yuan to a fake army officer met on a matchmaking platform; Pudong police point to a sign of AI synthesis in her screenshot of him

In May 2026 a 50-year-old woman in Shanghai met a man on a matchmaking platform who said he was an army regiment commander about to retire and sent officer and identity card photos. He also made a video call on his own initiative, in which he quickly covered his face with his hand to show he was not using AI face-swap. He then steered her into a fake investment platform, and within about a month she put in 800,000 yuan, almost all of it borrowed, and could not withdraw it. A Pudong police anti-fraud officer said that AI synthesis could still be detected and that in her screenshot the man's thumb was clearly longer than a normal person's. The report gives no direct statement that face-swap software was used. The case was under further investigation.

Taichung, Taiwan: a homeowner who paid a renovation contractor about NT$2.55 million says the construction-progress photos it sent were AI-generated while the house was left full of debris; she has filed a fraud complaint with police, a second homeowner describes similar losses, and the contractor denies using AI images

A homeowner in Taichung's East District hired a renovation business she found through a Facebook fan page to renovate her four-storey house, with work due to start on 20 August 2025 and finish by the end of March 2026 (ETtoday). By the end of March 2026 she had paid NT$2,553,275 while the contractor cited repeated reasons for delay and sent progress photos and payment slips for materials. She became suspicious when an air-conditioning supplier named on one of the contractor's payment slips said it had not received the payment, compared the photos with the site, and ran them through an AI-image detection website, which rated them almost entirely (99.9%) AI-generated; the photos showed workers and fittings that did not exist, and the house was left gutted with construction waste piled on the ground floor (TVBS; CTS; ETtoday). She has reported it to police and, with a lawyer, filed a fraud complaint. A second homeowner in Dali District paid more than NT$2 million to the same contractor, did not receive AI images, and says the business's fan page used AI-generated pictures; CTS reports three victims so far. A designer at the business denied to TVBS that it used AI images to report false progress, and ETtoday reports prosecutors and police are investigating.

Italy: two elderly men in Fano and Fossombrone show their gold on video calls from a fake Carabinieri officer, in calls a local outlet says were modified with AI, and an accomplice collects their valuables, reportedly worth a few thousand euros, from each home

CentroPagina reported on 28 September 2026 that on two separate occasions in Fano and Fossombrone (Marche), fraudsters used a video call modified with artificial intelligence, showing a face and the Carabinieri emblem on screen. A 70-year-old man in Fano was told by a caller posing as a Carabinieri officer that his car was involved in a jewellery-shop robbery and that he had to show his gold to clear himself. On the video call he showed his jewellery one piece at a time; while he was kept on the phone, an accomplice came to the door and took gold jewellery and cash, reportedly a few thousand euros. An 80-year-old man in Fossombrone was targeted the same way and lost jewellery worth a few thousand euros; he did not make the two online transfers totalling 15,000 euros that the fraudsters then demanded as bail. Prosecutor: judge used ChatGPT citations in Oklahoma custody order later vacated

In November 2025 Stephens County Associate District Judge Lawrence Wheeler issued an order in a child paternity and custody case that denied one parent's request for a psychological evaluation of the other parent and reprimanded the requesting parent's attorney 'for stooping to such frivolous trial tactics'. The attorney challenged the order at the Oklahoma Supreme Court in February 2026, telling the justices that it relied on two cases that do not exist; the challenge was dismissed in March after Wheeler vacated the order, and he is no longer on the case. According to a 17 August 2026 letter from the Stephens County district attorney, Wheeler told the Oklahoma State Bureau of Investigation that he used ChatGPT for research and wrote the order himself, and that at least two citations in it produced by ChatGPT do not exist. The mother in the case told News 9 that learning the order's citations were allegedly fabricated was alarming because it concerned the custody of a child. The attorney general's office declined criminal prosecution; judicial discipline remains possible.

Bengaluru: a security guard says a WhatsApp video call in which an alleged deepfake 'Chief Minister Vijay' offered him financial aid led to fake 'processing' charges and a fake CBI fine, and he lost more than Rs 1.04 lakh; Byappanahalli police register an IT Act case (September 2026)

A security guard in Bengaluru, originally from Odisha, told the Times of India that on 9 September 2026 he answered a WhatsApp video call in which a face and voice presented as Tamil Nadu Chief Minister C. Joseph Vijay introduced himself in Hindi, asked his name, work and where he lived and pressed him to accept financial help. A 'manager' then promised Rs 11 lakh, said Rs 5 lakh had been allotted to him and asked for a Rs 5,000 exchange charge, then Rs 18,000 to unlock a supposedly locked PIN; a caller posing as a CBI officer demanded more than Rs 50,000 as a fine. The fraudsters sent a fake allotment letter and a purported CBI officer's identity proof. He paid more than Rs 1.04 lakh through a digital payment app before refusing a further Rs 50,000 demand, called the 1930 cybercrime helpline and went to Byappanahalli police, who registered a case under the Information Technology Act. The Times of India says the fraudster allegedly used a deepfake AI-generated video and calls it the first such case reported in the city. No arrest is reported, and no link to the Tamil Nadu CB-CID deepfake case or its Alwar arrest has been established.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
