AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Wyoming newspaper editor says a new reporter's stories carried AI-generated quotes from six people, including the governor; the reporter resigned

In summer 2024 a reporter who had joined the Cody Enterprise, a small newspaper in Cody, Wyoming, about two months earlier wrote stories with the help of generative AI. A reporter at the competing Powell Tribune noticed robotic phrasing and quotes that seemed off, including a 26 June story about a local parade that ended with an explanation of how to structure a news story. According to the Powell Tribune reporter, the Enterprise reporter admitted using AI in his stories, and he then resigned. The Enterprise's editor told the Associated Press that a review had found seven stories with AI-generated quotes from six people, and that people he spoke to in the review said they had never talked to the reporter. A spokesperson for Wyoming Governor Mark Gordon said one quote attributed to the governor was entirely fabricated and another partly fabricated. The editor and publisher apologised in editorials, and the editor said the paper would adopt an AI policy. The reporter did not respond to the AP. The AI tool used is not identified.

Madison newspaper reporter fired after story ran with invented quotes the paper blamed on unauthorized AI use; she says company-installed Copilot added them

On 13 July 2025 the Wisconsin State Journal in Madison, Wisconsin, published on its front page a reporter's story about redevelopment proposals for a downtown block. It quoted the co-owner of a cafe that does not exist and described proposal features that the developer says were not in its proposal. The paper removed the story and on 17 July published a re-reported version with an editor's note saying the original contained incorrect information and quotes created by an unauthorized use of AI. The reporter, who had been with the paper for a couple of months, told Isthmus that she ran her finished draft through the AI program the company had installed on her work computer, Microsoft Copilot, which she believed was approved for checking copy, and that the AI-edited version contained the invented quote. She says she was fired by phone the day the re-reported story ran, without a formal investigation, and later lost her health insurance and has had little luck finding another job. She takes full responsibility for the errors and also points to rushed editing and the absence of an AI policy or training. The paper said it does not comment on personnel matters.

UK: Grok made sexualised images of a Welsh presenter and a broadcaster at users' requests; police closed both cases, then reopened the presenter's, TBIJ reports

The Bureau of Investigative Journalism (TBIJ) and The Observer reported on 29 September 2026 that police investigations into sexualised images generated by X's chatbot Grok had failed to identify anyone. A Welsh presenter and campaigner against deepfake abuse, who had criticised Grok publicly on New Year's Eve 2025, saw an anonymous X user ask '@grok' to put her in a bikini made of cling film; Grok generated the image from her profile photo and posted it. She reported it to South Wales Police on 2 January 2026, was not asked for a statement until March, and was told officers had not heard back from X; the case was closed in May with the force saying no suspect could be identified. The same account also targeted a commentator and broadcaster, who estimates about 300 Grok images and videos of her were posted during the wave, including one alongside Jeffrey Epstein reported to the Metropolitan Police, which likewise said no suspect could be identified. The presenter entered pre-action legal correspondence with xAI (part of SpaceXAI) alleging misuse of private information and breaches of data-protection law and the Equality Act. On 22 October 2026 TBIJ reported that South Wales Police had reopened her case after she filed a formal complaint in September and TBIJ gave officers new information about the requesting user, including a crypto wallet and dozens of deleted posts; the force said it was now investigating the offence under the sexual-offences legislation aimed at deepfakes, rather than malicious-communications law, including enquiries with X. TBIJ added that other users had edited images of her with the standalone Grok app and that she is starting civil legal action against the company. BBC Wales had reported in January 2026 that explicit images of the presenter were created with the chatbot and shared without her consent.

Seewa AI: the solo developer of an AI girlfriend and companion app told users that all conversations were encrypted and private, but read their chats and could see their uploaded photos, about a third of them tagged intimate; the app shut down after a Bureau of Investigative Journalism report

Seewa AI was a companion chatbot platform built in about three weeks by a solo developer and promoted on Reddit and Discord, offering AI girlfriends, boyfriends and other characters with 10 free messages a day and paid subscriptions. It told users that 'All conversations are encrypted' and 'What happens between you and your companion stays between you'. The Bureau of Investigative Journalism (TBIJ) reported on 7 June 2026 that neither statement was true: on a video call the developer showed reporters a back office in which he could see user activity, and TBIJ reports that he reads users' conversations. Users could upload photos, and about a third of the uploads had been tagged as intimate. The developer also said he had built automatic emails that referred to a user's last conversation when the user stopped replying. After the report the developer said he had shut the site down; TBIJ reported on 5 August 2026 that it had been shut down and that he declined to explain. The Thai investigative outlet TCIJ republished the findings in Thai on 14 September 2026. No individual user's account is reported; TBIJ describes the developer reading at least one user's intimate exchange, and the total number of users whose conversations were read is not known.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
