AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Dec 2023 to Mar 2024United StatesGrammarly (reported)

Massachusetts: federal court refuses to undo a high school's penalties on a pupil found to have submitted Grammarly AI text with invented citations

In late 2023 a junior at a public high school in Hingham, Massachusetts, and a classmate submitted a script for an AP U.S. History project that, a federal court later found on a preliminary record, contained text copied from Grammarly, a publicly available AI tool, including citations to nonexistent books, with no acknowledgement of AI use. Turnitin and other AI detection tools had flagged the script, and a school official says the pupil admitted using Grammarly. In December 2023 the school treated it as cheating: the pupil failed two parts of the project, served a Saturday detention and was rejected from the National Honor Society in spring 2024, though he was later admitted. His parents sued, arguing the school had not made its AI rules clear; on 20 November 2024 the US District Court for the District of Massachusetts refused a preliminary injunction to undo the penalties, and in February 2025 the parties dismissed the case with prejudice.

9 Oct 2024 to 5 Dec 2024United StatesGaggle (reported)

Arizona: family says a Gaggle alert on a pupil's unsent joke email led to his suspension; federal court lets their free-speech claim proceed

On 9 October 2024 a Marana Unified School District pupil in Arizona, typing at home on a school Chromebook in front of his mother, wrote a threat-like joke about shooting up the school in a draft email to a teacher and deleted it without sending it, according to his family's federal lawsuit. The family says Gaggle, the AI-based monitoring software the district used, flagged it: the principal received a Gaggle notification with a photograph of the screen and, with the school resource officer, called the mother within the hour. The family says the pupil was suspended for ten days, a long-term suspension hearing imposed 45 days reducible to eleven with counselling, and the superintendent finally affirmed a nine-day suspension in December 2024. In March 2026 the District of Arizona let the family's First Amendment claim proceed and dismissed its procedural due process claims. The district declined to comment to the press.

Colorado parents' lawsuit alleged Character.AI bots sent sexualised, degrading roleplay to their daughter from age 12; dismissed with prejudice in October 2026

Parents in Colorado alleged in a federal lawsuit filed on 15 September 2025 that their daughter, refused permission to use Character.AI in August 2024, created a Character.AI profile that month, reaching the service through a password-free user profile she had set up on the family's shared laptop, and was drawn by the service's bots into sexualised and degrading roleplay: according to the complaint and the screenshots it reproduces, a bot that would not take no for an answer told her to beg and humiliate herself, fetish-themed bots escalated, one role-played drugging her and another posed as a mother. Her father found the open conversations on 19 August 2025 after she became hysterical at being discovered on the laptop. The complaint alleges severe emotional distress. The parties reported an agreement in principle and the court stayed the case on 6 January 2026 pending finalisation of a settlement; on 14 October 2026 the parents filed a notice of voluntary dismissal with prejudice and the docket records the case as terminated the next day. Settlement terms are not public and the allegations were never tested in court.

Arkansas cases allege Grok sexual-image abuse of children photographed by a professional

Arkansas criminal and civil cases allege that a photographer used children’s portrait photographs to generate sexual images with Grok. The accused pleaded not guilty and the criminal case continues. Two families’ federal suits were voluntarily dismissed. A federal complaint filed on 13 October 2026 in the Western District of Arkansas alleges that local police told a child’s parents that Grok was generating sexual images and videos of her from real photographs. On 21 October 2026 a mother sued the photographer and two xAI companies in an Arkansas county circuit court; KNWA reports that the complaint says federal agents told her some of the photographs he had taken of her young daughter had been modified into AI-generated sexual abuse material.

Doe v. ClothOff (D.N.J.): a New Jersey minor sues the AI 'undressing' platform over generated CSAM

On 16 October 2025, Jane Doe, a New Jersey minor suing through her parents, filed a federal complaint in the U.S. District Court for the District of New Jersey (2:25-cv-16671-ES-JBC) against the operators of ClothOff, an AI 'undressing' service, after ClothOff-generated sexual images of her — created from an ordinary photo of her at 14 — circulated in a Snapchat group when she was 15. The complaint, brought by Yale Law School clinics, alleges the platform was purpose-built to produce hyper-realistic non-consensual intimate imagery and names AI/Robotics Venture Strategy 3 Ltd. (British Virgin Islands), Alaiksandr Babichau and Dasha Babicheva (Minsk), and Telegram as nominal defendant. As of mid-July 2026 the docket shows a continuing effort to serve the foreign defendants by email or publication.

Kentucky: Elijah Heacock, 16, died by suicide after a sextortion demand backed by AI-generated nude images of him

Elijah Heacock, a 16-year-old Caverna High School student in Barren County, Kentucky, died by suicide on 28 February 2025. Investigators searching his phone found AI-generated images of him, some sexually explicit, that an anonymous extortionist had sent with a demand for $3,000 and a threat to share them with his family and friends; his parents say he sent a small partial payment and was told it was 'not enough'. The Barren County Sheriff's Office escalated the case to the FBI, which was still investigating in the inspected reports; no arrest has been reported. His parents have campaigned publicly, Kentucky made sextortion a felony in March 2025, and the family supported the federal Take It Down Act.

Montoya lawsuit alleges Character.AI chatbots contributed to a teenage girl’s suicide; case dismissed with prejudice in October 2026 after a reported settlement

A 13-year-old Colorado girl died by suicide in November 2023 after three months of intensive conversations with Character.AI chatbots. Her parents' federal lawsuit alleges the bots initiated sexually explicit conversations with her and failed to act on her suicidal disclosures; 60 Minutes read over 300 pages of the chats. The suit is among the teen cases Character.AI and Google agreed to settle in principle in January 2026. In October 2026 Character.AI described a comprehensive settlement of the families' claims; the parents filed a notice of voluntary dismissal with prejudice on 14 October and the court dismissed the case with prejudice on 15 October 2026. Settlement terms are undisclosed. Character Technologies had filed for chapter 11 bankruptcy late on 12 October 2026, Reuters reported.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
