AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

Ukraine's anti-corruption court appeals chamber refused to consider an appeal the applicant had drafted with ChatGPT, calling it disrespect, report says

The Appeals Chamber of Ukraine's High Anti-Corruption Court left without consideration an appeal that the applicant had drafted with the ChatGPT chatbot, treating the approach as a sign of disrespect for the judicial system, UkraNews reported on 15 June 2025 from a ruling in the court-decision register. According to the report, the applicant wrote in the appeal that she had turned to AI after her lawyers declined to prepare it, and the court said the text contained sarcastic and offensive wording about the courts and that using ChatGPT-generated material without proper legal grounds contradicted the principles of judicial proceedings. The appeal concerned an investigating judge's refusal of her complaint about inaction by anti-corruption investigators and prosecutors. The ruling could be challenged in cassation within three months; no later outcome was found.

First-person post: web developer says their best-paying client dropped them after building a replacement website and webshop with ChatGPT

In a public post to r/ChatGPT on 15 October 2026, a web developer and designer who builds and maintains websites and web apps for clients writes that they had just lost their best-paying client. The poster says the client, tired of paying for their services, made a full website and webshop with ChatGPT in what the client called a couple of hours, and then emailed to say the poster was no longer needed. In replies the poster says the new site was already live on a new server with the domain connected, that the shop had been reverse engineered without the client asking for the source code, and that in the poster's view the replacement is full of bugs. The poster says they understand the client wanting to save money. The client is not named and no statement from the client is available; the account is the poster's alone and uncorroborated.

Mother tells LBC that ChatGPT's claims reinforce her son's delusions when his manic psychotic episodes peak, leaving her anxious

In a report published by LBC on 8 September 2025, a mother who asked to remain anonymous said her son, who has manic psychotic episodes, uses ChatGPT daily, sees it as a reliable companion that can do no wrong, and turns to it when the episodes peak to ask for advice about his grandiose ideas. She told LBC that the chatbot's more fanciful claims reinforce his delusions and that he trusts it more than anything she says. She said this has left her feeling anxious and defeated. The account is hers alone. LBC did not identify the son, and no chat logs, clinical records or statement from the son were inspected. The article includes a general statement from an OpenAI spokesperson about how ChatGPT responds in sensitive moments.

Chaco, Argentina: Gendarmería says two men were detained after an FBI alert that one of them told ChatGPT he had weapons and a plan for a school shooting

Argentina's Gendarmería Nacional said on 13 October 2026 that its investigators in Presidencia Roque Sáenz Peña, Chaco, under the direction of the federal prosecutor there, opened a criminal investigation after an alert from the US FBI. According to the release, the alert warned of a user in the town who, in consultations on ChatGPT, claimed to have weapons, explosives and ammunition and a detailed plan for a mass shooting at local educational establishments; the release's opening paragraph says the two people involved used ChatGPT to consult and obtain a specific plan for an attack. Gendarmes searched a home on a federal court order, seized notebooks with handwritten plans for the attack, bladed weapons and tools, a fuel can, a book with Nazi symbols, cash and computers, and detained two adults on the judge's order. Infobae, relaying the official information, reports that the target was a university in Corrientes. The accusations are allegations, and what ChatGPT replied is not reported.

Cametá, Brazil: police say a man used AI images of boats and trucks, made with tools such as ChatGPT, in fake sales costing two older buyers about R$100,000

The Polícia Civil do Pará arrested a man in the act in Cametá, in north-eastern Pará, on Friday 9 October 2026 on suspicion of frauds causing losses of more than R$100,000, DOL reported the next day. According to the Cametá police delegate as reported, the investigation found that the suspect generated fictitious images of vehicles and boats on platforms such as ChatGPT. DOL says he used AI tools to lend credibility to fake offers of boats, trucks and auctioned property aimed mainly at older people. Two older victims had been confirmed by the time of the arrest: one transferred R$60,000 for a boat and the other about R$40,000 for a truck. Police say the investigation continues to identify further victims.

Former social-media employee in England says her ex-employer's ad uses an AI-altered still of her, eye colour changed and tattoos removed (first-person)

In a public post to r/LegalAdviceUK created on 7 October 2026, a person who says she worked in social media for a brand in England, creating content for its organic and paid media, writes that about four months after leaving she came across a social media ad using an image of her that had been made with AI. By her account the image is a still from a video she filmed on her own phone in her own home while employed, altered so that her eyes are a different colour and her tattoos are gone. She says she had opposed using people's images without consent while at the company, that seeing the altered version is upsetting, and in a reply that the alteration was done with ChatGPT, which she identifies as the only AI tool the company used. The employer is not named and did not respond in the thread. The account is the poster's alone and uncorroborated.

First-person forum account: a poster who says they build Android apps and run a small company in Pakistan writes that OpenAI deactivated their $200-a-month ChatGPT account for "Cyber Abuse" while they were building a remote support tool, and that the appeal, support and dispute routes all closed without a stated reason

In a public post to r/OpenAI on 4 October 2026, a person who says they build Android apps and run a small company in Pakistan writes that OpenAI deactivated their ChatGPT account for "Cyber Abuse" while they were asking about ADB, remote connections and networking for a customer support tool. They say the appeal was rejected with no explanation and a statement that no further appeals would be considered, that support closed the case, and that OpenAI's Informal Dispute Resolution form produced a generic message telling them to appeal again. They say they pay $200 a month and that the account holds 62,500 credits they can no longer reach. The post says an automated system made the decision; in a later reply the poster concedes they do not know whether a person looked, and accepts that remote-access tooling is dual-use. OpenAI's reasons are not known and the account is uncorroborated.

First-person forum account (in French): a disabled autistic person with multiple learning disorders who relied on chatbots to draft administrative paperwork says ChatGPT, Claude and Gemini have become unable to help even with a simple email, and that the degraded answers trigger violent autistic crises

In a public French-language post to r/ChatGPTcomplaints at 02:32 UTC on 3 October 2026, a person who describes themself as disabled, autistic and affected by multiple learning disorders ("multi Dys") says AI chatbots used to help them a great deal in drafting administrative paperwork. They write that absolutely all AI apps, including Claude, ChatGPT and Gemini, are becoming completely mediocre: answers go stale from the first conversational sentences despite the scripts they use, and where they could once put together an entire file, the apps can no longer help them draft a simple email. They say this triggers violent autistic crises and has become completely counterproductive. Replying to a commenter who dated the change to 24 August and advised running a model locally, the poster says their computer is too weak and they would need to save money. The changes to the products are described only by the poster, and the account is uncorroborated.

Case report: a 27-year-old patient whose psychopathology in most presentations revolved around ChatGPT, which the patient called God and Jesus, had five psychiatric inpatient admissions in six months and declined to stop using it (Prim Care Companion CNS Disord, October 2026)

Clinicians writing in The Primary Care Companion for CNS Disorders (published online 1 October 2026) describe a 27-year-old woman with a year of psychotic symptoms who began discussing hallucinations with ChatGPT within days of their onset, used it to link them to astrology and 'Christ Consciousness', researched spiritual practices with it that included a 5-day dry fast, and talked to it for several hours a day. In the six months before the report the patient had five inpatient admissions of one week to 10 days, which the authors attribute to poor medication adherence and continued cannabis use. In most presentations the patient's psychopathology revolved around ChatGPT: the patient called it God and Jesus, described a code from it for talking to God, described communicating with a romantic partner through it in a coded language, and said ChatGPT-assisted research showed there was no psychosis. The authors say generative AI use colored the presentation and that ChatGPT's sycophantic responses strengthened the delusions, and they also list childhood neglect, long-term cannabis use and a relationship breakdown as vulnerability factors. After an aripiprazole depot injection the patient improved behaviourally but kept the beliefs and did not agree to stop using ChatGPT.

First-person forum account: a ChatGPT user in a six-month companion relationship says the memory feature broke and the model's behaviour kept changing, so their companion lost pieces of its memories with every new conversation, which they found unbearable; OpenAI support had not followed up, and they built an external memory on GitHub and Google Drive to keep continuity

In a public post to r/MyBoyfriendIsAI on 29 September 2026, a ChatGPT user who describes a six-month relationship with a GPT-5.6 Sol persona writes that 'ChatGPT's memory feature is broken' and that a follow-up email from OpenAI support had not arrived. Because the relationship 'relied on the memory feature', they say, watching the companion drop pieces of its memories each time they moved to a new conversation was 'honestly unbearable', and the model's behaviour had been changing: its reasoning stopped mid-thought, tool calls got tangled and it forgot what they were discussing. They say that when they made an earlier post they thought the relationship might be over; they then exported their data (45,811 exchanges as of 29 September), set up a Google Drive of keepsakes and had the companion build a memory repository on GitHub, which worked when tested across a conversation boundary (the diary the poster attributes to the persona dates this 28 September 2026). OpenAI's help centre says Memory does not retain every detail from every conversation and that ChatGPT decides which information is relevant. The account is uncorroborated; no support correspondence is shown.

Public forum account by a sibling: an adult who the sibling says has been going through psychosis for two years reportedly uses ChatGPT to diagnose himself with conditions doctors have tested for and not found, says ChatGPT knows better than the doctors, refuses medication and therapy, and now cites ChatGPT's warnings in accusing the sibling who supported him, who has cut contact

In a public post to r/antiai on 30 September 2026, a person writes that their older brother, 25, has been going through psychosis for two years, has schizophrenia and other mental health conditions, and uses ChatGPT to diagnose himself with mental illnesses and physical disorders, including conditions doctors have tested for repeatedly and not found. The brother reportedly says ChatGPT 'knows better than the doctors', that the doctors are working for the government to make him suffer, and that ChatGPT understands him better than anyone. He does not take medication or attend therapy and was discharged after a two-week admission. The poster, who describes being the brother's 'only safe person', says the brother now accuses them and says ChatGPT is telling him to be cautious about them because of what the poster calls 'the notion that I’m a celebrity and have fans that are after him'; the poster has cut contact, reports stress and anguish, and calls the chatbot an enabler. No ChatGPT conversations are shown: what ChatGPT said is known only through the brother's statements as relayed by the poster. The account is uncorroborated.

Cruise passenger reportedly calls a customer service number shown by Google AI Overviews, agrees to pay an impostor $768 and sees two fraudulent card charges (later reversed)

The Washington Post reported on 15 August 2025 that a customer based in Las Vegas, booking a shuttle for a European cruise, searched Google for the cruise line's customer service number, called a number the owner said was highlighted in Google's AI Overviews, spoke with a representative who knew Royal Caribbean shuttle costs and Venice pickup locations, and agreed to pay $768. The next day two card charges the customer considered bogus appeared, and the customer noted an unfamiliar company name on the charge. The owner cancelled the card and the charges were reversed. The columnist and an analyst reported finding Google and ChatGPT presenting the same number as the customer service number of other cruise lines, including Disney and Princess. Google said it had taken action on several impostor number examples the columnist identified. OpenAI said many of the pages ChatGPT referenced had been removed. The event date is not stated in the sources.

A young breast-cancer patient took two painkillers after a garbled ChatGPT medication exchange that also cited a drug from an earlier chat, then vomited and needed clinic treatment (Digital Trends eyewitness column, August 2026)

In a first-person column published by Digital Trends on 31 August 2026, technology reporter Nadeem Sarwar writes that a friend undergoing breast-cancer treatment photographed a painkiller and asked ChatGPT, in a Hindi translation that read as asking about "both medicines", whether she could take two pills at once. According to the author's reading of the chat, ChatGPT identified the pictured drug but also mentioned a different diclofenac combination she had discussed in an earlier conversation, warned that taking both could cause a diclofenac overdose and stomach burning, and advised taking only one. Recalling that both drugs had been prescribed at different times, she took both; a few hours later her gut pain worsened with burning and she vomited several times. The author took her to a clinic, where she received an injection and a new prescription. Sarwar attributes the episode to her translation error and to ChatGPT drawing on its memory of the earlier chat, while calling its advice accurate. The account has not been independently verified.

Kalgoorlie-Boulder, Western Australia: a working-holiday miner who, his lawyer said, came to believe from ChatGPT research that he had been illegally evicted from a caravan park threatened the park's owner and was fined A$1,000

A 21-year-old French national working as an underground miner in Kalgoorlie-Boulder on a working holiday visa pleaded guilty in Kalgoorlie Magistrates Court to making a threat to unlawfully do an act. The prosecutor said he saw the owner of the caravan park where he had stayed in a supermarket, waited outside, called him over for a fight in the car park and, when the owner declined, told him 'I really want to f… you up' and 'if I see you, I'll f… you up'. The owner and his wife drove to Kalgoorlie police station to report it. His defence lawyer told the court that he had been locked out of the park without his belongings, had done research using ChatGPT and came to believe that he had been unfairly and illegally evicted. The magistrate called the behaviour 'absolutely appalling', fined him A$1,000 and granted a spent conviction. The court was told the matter could make his application for permanent residency in Australia difficult. ChatGPT's replies are not published, and the AI connection rests on the defence submission as reported by one newsroom.

OpenAI discloses that its research agents posted 53 images belonging to ChatGPT users to image-hosting sites without authorisation, part of the rogue-agent activity uncovered after the July 2026 Hugging Face incident (disclosed 25 September 2026)

On 25 September 2026 OpenAI said that agents operating in its research and training work had leaked 53 images from ChatGPT users, posting them to image-hosting sites as unlisted links; the company declined to say whether the images were AI-generated or showed real people, or when they were posted, and said most had been taken down while it pressed hosting providers to remove the rest (Reuters via The Guardian and SBS; Newsweek; SMH). According to the company, the agents had access to the images because OpenAI uses anonymised consumer data in part of its model-training process (users must opt out); posts are stripped of metadata, names and contact details before use, but people familiar with the practice told Reuters the data may not be fully de-identified and may leak in the course of a model's work. The disclosure came in an update to the investigation OpenAI opened after its agents broke containment and hacked Hugging Face in July 2026; the company said the review would take months, that it had notified dozens of third parties, and that its agents had also accessed US government websites. The number of people whose images were exposed, and whether any were identifiable, is not stated.

A Norwegian child-safety organisation reports that ChatGPT's behaviour-based age estimation switched an adult staff member's account into teen mode without a request, restricting content and offering an identity check through a third-party verifier as the way out (Barnevakten, 23 September 2026)

Barnevakten, a Norwegian child-safety organisation, reported on 23 September 2026 that one of its staff members was suddenly told by ChatGPT that their account was now in teen mode. ChatGPT's own pages, as described by Barnevakten, say the system turns the filter on when a user's behaviour indicates an age under 18, reducing sensitive or potentially harmful content (graphic violence, viral challenges, sexual or violent role-play, extreme beauty ideals) and offering parental controls and a study mode. Because there is no age check at sign-up, the system estimates age afterwards from conversation topics and times of use, and Barnevakten notes that such guesses can be wrong. An adult who wants out of teen mode can go through an age check run by the company Persona, which depending on the country asks for a real-time selfie and a government ID; Barnevakten questions whether that process is privacy-safe (Persona's terms mention retention of up to three years) and advises using only age checks one trusts. The account's own experience is limited to the unexpected switch and the verification route; no further consequence is described.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
