AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Utah homeowner says home warranty firm's AI chatbot agreed a $3,000 air-conditioner payout, the firm refused it, then paid after a TV inquiry

KSL TV reported on 29 September 2024 that a homeowner in Draper, Utah, asked Choice Home Warranty through its online chat to replace a failing air conditioner. He declined an offer that would have cost him $1,800, asked for the maximum $3,000 payout, and the chat replied that the company would proceed with the $3,000 payout. When the money did not arrive he called, and a company representative told him, by his account, that the firm had had 'this AI' up for about a week and it was miscommunicating to people; the firm refused to pay. After KSL contacted the company, the homeowner said it agreed to honour the offer, and he received the check. The company did not comment to KSL.

Karnataka police say an AI-generated Trump video promoted a fake 'Trump Hotel Rental' investment app; a lawyer lost about Rs 5.9 lakh

Police in Karnataka, India, said in May 2025 that cybercriminals used an AI-generated video of US President Donald Trump to draw people into a fake 'Trump Hotel Rentals' investment scheme run through a mobile app. A lawyer told police and the Indian Express that in January 2025 he saw short YouTube videos offering investment in Donald Trump hotels, downloaded the app, and deposited Rs 5,93,240 between 25 January and 4 April 2025 after small early payouts, then could not withdraw anything. Police registered a case under the IT Act and the cheating provision of the Bharatiya Nyaya Sanhita and froze Rs 1.5 lakh in a recipient account. Reports describe more than 200 complaints across Karnataka districts with losses near Rs 2 crore; how many of those people saw the video is not established. The technical basis for the police description of the video as AI-generated is not disclosed.

Rhein-Pfalz district police say an older man sent about 27,000 euros after a talk-show video, presumably generated by AI, promised crypto gains

The Ludwigshafen police directorate reported on 3 April 2025 that an older man from the Rhein-Pfalz district had seen a video online of a talk show on the German broadcaster ZDF in which large stock-market gains through a cryptocurrency were promised. The police say the video was presumably generated by AI. He then phoned a supposed personal financial adviser and over six months transferred about 27,000 euros to foreign accounts. He became suspicious when the promised gains were not paid out despite repeated requests and further fees were demanded instead. The police do not say who appeared in the video or how they reached their view.

Lippe district police say a man lost his savings and took out a loan after investment adverts with AI-generated deepfake videos of supposed TV shows

The Lippe police reported on 20 February 2026 that a man from the district had recently been defrauded through a supposed online trading platform, with a five-figure loss. According to the police, the fraudsters used adverts with AI-generated deepfake videos from supposed television programmes, repeatedly demanded fees after he registered and steered him to transfers to crypto exchanges. He was talked into taking out a loan to make further deposits, never received a payout, lost his savings and got into financial difficulty because of the loan. The police do not say whom the videos showed or how they established that they were AI-generated.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
