AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Jerusalem police say three boys aged 13 to 15 used AI to turn social-media photos of schoolgirls into nude images and spread them; at least 30 girls harmed

Israel Police said in February 2025 that three boys aged 13 to 15 from Jerusalem were suspected of taking photos of girls from social media, editing them with artificial intelligence software into nude images and distributing them without consent. According to Ynet, at least 30 girls were harmed; police said investigators identified dozens of victims and that a search of the suspects' homes found edited nude images of several girls. The head of the Jerusalem district youth investigations unit described dozens of cases of girls' privacy being violated through nude images edited with AI technology. The three were questioned over publishing a sexual image without consent, and the case was passed to prosecutors to decide on charges.

Mother tells Ynet she waited two days after ChatGPT, while advising a check, said it was relatively rare; her teenage son lost a testicle

Ynetnews reported on 12 April 2026 that doctors at Meir Medical Center had seen three teenage boys lose a testicle in the past month after arriving too late for treatment of testicular torsion. The mother of one of them told Ynet that when her son complained of lower abdominal pain she thought it might be constipation and consulted ChatGPT, which gave several options, said it could be something serious and that they should get it checked, but also said it was relatively rare, so they thought they could wait. After two days without improvement the boy's testicle was swollen and he could hardly walk; an urgent care clinic sent them to hospital and ChatGPT, consulted again, told them to go to the emergency room immediately. At the hospital, late at night, doctors diagnosed severe torsion, operated and removed the testicle; the boy is recovering at home. The doctors attribute the month's unusually high rate of complications mainly to delayed arrival at the emergency room, which they say the war has partly influenced.

Israeli court strikes a defendant's expert report it found rested on AI-hallucinated case law and orders her to pay the banks' costs

On 8 October 2026 the Magistrates' Court in Nof HaGalil-Nazareth, Israel, struck from the file a banking expert's opinion that a defendant in a civil negligence suit had filed, finding that the heart of the opinion and its annexes rested on clear 'AI hallucinations': a Supreme Court ruling the court could not locate, a book it could not find and statutory sections quoted with text the statute does not contain. The court also refused the defendant's request to amend her third-party notice against two banks, partly because the amendment relied on the struck opinion, and ordered her to pay costs of 2,500 shekels to each bank. No AI tool is named, and the decision records no response from the expert.

Israel: indictment and police say a 20-year-old used AI-animated ID photos as fake verification videos to open credit cards in at least 33 people's names

According to an indictment filed on 16 July 2026 (date per Maariv), as reported by i24NEWS, a 20-year-old man impersonated at least 33 Israelis over about half a year, obtained credit cards in their names and used them for purchases of about 67,000 shekels in Israel and abroad, with attempted charges of more than 56,000 shekels more. Investigators collected fake identification videos of the victims that, according to the indictment as reported, he created with artificial intelligence and used to pass the card issuers' remote identification. Maariv reports, citing police, that the suspect cut the face photos from identity cards and animated them with AI software so that the card holder appeared to speak the required declarations. A police investigator told Maariv that investigators had to locate more than 50 people whose identities were used and that some victims were frightened when shown the videos of themselves in interrogation rooms. The victims who were questioned denied any connection to the cards. The investigator said most of them did not know their identities were being used and that the credit companies bore the direct financial loss. He was arrested on landing in Israel, did not cooperate with the investigation, and the prosecution asked for his detention until the end of proceedings.

23 Feb 2026 to 25 Feb 2026ItalyUnidentified voice-cloning tool

Italy: Fideuram chairman Paolo Molesini is deceived by a WhatsApp impersonation of Intesa Sanpaolo's CEO and a phone call in the reportedly AI-cloned voice of a lawyer he knew into ordering about 95 million euros of transfers; at least 36 million remain missing and he resigned on 12 March 2026, weeks later

On 23 February 2026 Paolo Molesini, then chairman of Fideuram (the private-banking subsidiary of Intesa Sanpaolo), received a WhatsApp message from an unknown number from someone claiming to be Intesa's chief executive Carlo Messina, announcing a confidential acquisition that had to be executed through Fideuram. The same day a caller presenting as a lawyer of A&O Shearman whom Molesini knew, whose voice ANSA, Il Fatto Quotidiano and Corriere della Sera, reporting from the Milan court papers, describe as created with artificial intelligence (today.it, which also cites the seizure decree, writes that the court papers do not yet answer whether the voice was imitated), had him sign a confidentiality agreement and sent eleven payment instructions; Fideuram's treasury head was separately contacted by someone posing as Fideuram's CEO. Between 23 and 25 February eleven transfers totalling about 95 million euros went to accounts in Portugal and at Bank of China; the bank's alarm systems and the Milan prosecutors recovered about 40-42 million from China and 13 million seized in Portugal, leaving at least 36 million (39.5 million per the court papers) missing after conversion into cryptocurrency. Molesini, who Corriere writes is not under investigation, resigned as chairman on 12 March 2026, which Fideuram announced as being for personal reasons; a 48-year-old Israeli citizen is under investigation as a member of the gang.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
