AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Finnish municipality abolished two director posts and its mayor says their work is now done with AI; a Deputy Ombudsman opened an inquiry

Humppila, a municipality of about 2,000 residents in Kanta-Häme, Finland, abolished its administrative director and technical director posts on the new mayor's proposal, which, according to Helsingin Sanomat as relayed by Maaseudun Tulevaisuus, covered 150,000 euros of the municipality's savings need; Yle reports that the two office holders were dismissed in spring 2026 and that the abolitions followed the municipality's need to improve its weak finances. Maaseudun Tulevaisuus, relaying an interview first published by Helsingin Sanomat, reports that according to the mayor both directors' tasks are now done with the help of AI, and quotes the mayor as saying the writing of new appendices to the administrative regulations was done with AI in a day. A municipal statement said the main aim of its reforms was not to increase AI use or shrink the administration but to secure services. The former administrative director told Iltalehti the abolition of the post was unlawful, saying the tasks have not ceased and that AI cannot bear official responsibility or form a ground for dismissal; the mayor says the process was lawful. According to the former director, the abolition was brought into the change negotiations as the only option without the five days' written advance notice the law requires, and the lawyers' union Juristiliitto assessed the negotiations as at least careless and presented the municipality with a claim for full compensation. The former director appealed to the Hämeenlinna Administrative Court in spring 2026 and reports filing a criminal complaint with Häme police on 9 September 2026, which police confirmed receiving without deciding whether to investigate. Finland's Deputy Parliamentary Ombudsman opened an own-initiative inquiry into the use of AI in the municipality's official work, asking what tasks were given to AI and how lawful decision-making and official responsibility are ensured. No tool is named, and no court or Ombudsman finding is reported.

Systems administrator says a small firm terminated them and its lead developer, replacing both with an Anthropic AI subscription (first-person account)

In a public post to r/jobs titled 'We were replaced with AI', created on 24 October 2026, a server and network administrator writes that they and the company's lead developer were terminated that day: their access to various systems was revoked and the chief executive then called them, with major budget cuts as the theme of the call. The poster writes that both were replaced with 'the Anthropic subscription' and, in a reply, that it is an Anthropic subscription of 180 dollars a month that the chief executive mentioned, and that staff had seen him using it to scan the company's infrastructure days earlier. In replies the poster says the company is small, located in Australia, and that they worked for it remotely, that finance staff had calls with the chief executive scheduled for the following Monday, and that they feel their years of loyalty were thrown away. The company is not named. The account is the poster's alone and uncorroborated; no employer statement was retrieved.

Australia: Commonwealth Bank announced 45 call centre job cuts it attributed to an AI voice-bot, then reversed them as an error, ABC reports

In late July 2025 the Commonwealth Bank of Australia confirmed to the ABC that it was cutting 45 call centre jobs after rolling out an AI chatbot, and ABC reports the bank had briefed the Finance Sector Union that the cuts were due to AI. The union raised a dispute at the Fair Work Commission and said members found call volumes rose after the bot was introduced. On 21 August 2025 ABC reported that the bank called the decision an error, apologised to the affected employees, said it had not adequately considered all relevant business considerations, and offered them the choice to keep their roles, be redeployed or leave. One employee told Yahoo Finance she had helped train a messaging bot known as Bumblebee, was told in late July her role was redundant, judged the redeployment offer unworkable and accepted voluntary redundancy. The bank's quoted statements say AI made customer help faster; the union calls AI a cover for cost cutting. At the bank's annual general meeting in October 2025, AAP reports, that employee confronted executives in front of shareholders and the chairman said the bank had made a mistake. In October 2026 The Sydney Morning Herald reported that, pressed by a senator at hearings of a federal parliamentary committee on AI, chief executive Matt Comyn owned the error in her case and said he had apologised for it in the past.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
