AI incidents

Reports of AI-related harm and adverse experiences affecting people’s safety, wellbeing, rights and livelihoods. Explore what happened and the evidence available.

NOPE’s core concern is when AI communicates with a person, acts on their behalf, or depicts or impersonates them. The tracker also records consequential decisions, claims and privacy harms involving AI. Each case needs a described connection between AI use and the harm, including private information recorded into or disclosed to an AI service. Each account is reviewed for publication; claims may remain uncorroborated or disputed. How we review and count cases

These figures describe the cases collected by NOPE. Coverage varies with discovery, reporting and available evidence. They do not estimate how often AI-related harm occurs.

Response counts currently use each case’s principal recorded outcome. Further proceedings may be described in its account.

Cases in this selection, counted once in their first known event year. A series may continue beyond that year. Reporting and collection dates are excluded. NOPE has searched recent events more thoroughly than earlier years, so bar heights also reflect collection effort.

Buyer says Gemini judged a marketplace gamepad genuine from listing photos, but the delivered one was counterfeit (first-person)

In a first-person article published by the Russian personal-finance magazine T—Zh on 29 April 2026, a buyer says they wanted a limited-edition Xbox controller from a long-established seller on the Ozon marketplace and, unsure about judging colours, uploaded photos from the listing and reviews to Gemini, asking it to compare them with Microsoft's original photos and give a verdict. The buyer says Gemini was completely certain the controller was original, which strengthened their own assumption, and they bought it. The delivered controller's left stick drifted from the first use; a repairer's programmer would not connect to it, showing it was not made by Microsoft. The seller first refused a return without a service-centre report; the buyer paid 500 roubles for one, then received a refund, but the marketplace did not reimburse the 500 roubles. The buyer acknowledges the listing photos may have shown a genuine unit and that the counterfeit may have been sent only to them.

Ahmedabad: police say two men used ChatGPT to make fake Navratri garba passes and sold 686 through Instagram ads for about Rs 8 lakh

The Ahmedabad Cyber Crime Branch said in October 2026 that it had arrested two men from Rajasthan who sold 686 counterfeit passes for Navratri garba events in Ahmedabad through discounted Instagram advertisements, collecting about Rs 8 lakh. According to the police account relayed by PTI, IANS, Ahmedabad Mirror and Gujarat Samachar, the accused prepared the passes and promotional material with ChatGPT, adding barcodes and QR codes so that they looked genuine, and sent them to buyers from email addresses resembling the organisers'. IANS reports that the case came to light when customers found their passes invalid at the venues. PTI and Ahmedabad Mirror say it came to light through a complaint filed on 16 October by a garba organiser whose event name and logo were used. How many buyers lost money is not reported; 686 is the number of passes.

Cases may have several effects and sources. Mixed accounts qualify when they include a reported harm or adverse experience. People are counted within individual cases where sources support a number; we do not publish a collection-wide total of distinct people.

A source’s existence, the experience it reports and AI’s causal role are separate questions. A lawsuit records allegations unless a subsequent finding establishes them.
